해양원격의료 도입 및 운영 방안에 관한 기초연구

  • 육상과 떨어져 근무하고 있는 해양근로자들은 의료접근도의 한계성으로 인해 건강관리에 많은 어려움을 호소하고 있다. 이와 같은 해양근로자들에게 U- health기반의 해양원격의료시스템 도입을 한다면, 질병예방 및 건강관리, 나아가 건강증진에 이르기까지 매우 효과적일 것이다. 따라서 이 연구는 향후 해양근로자의 원격 건강관리 서비스를 위한 원격의료 도입 및 운영의 기본 방향을 제공하는 것을 목적으로 한다.

  • 이 논문은 코로나 팬데믹 시대에 원격 의료 서비스의 중요성이 부상함에 따라, LLM(Large Language Model)과 웨어러블 기기를 활용한 의료 기술의 발전과 이를 통한 의료 서비스의 혁신에 대해 다루고 있다. 코로나 19 대응을 위해 원격 의료에 대한 법적 제한이 완화되며, 이에 따른 원격 의료 시스템의 확대를 언급하고 있다. LLM 을 활용한 의료 정보 관리와 웨어러블을 통한 건강 모니터링을 소개하며, 대화형 AI 를 통한 문의사항 처리와 2 차 처방, 실시간 번역 AI 기술 등의 기술적 혁신을 언급하고 있다. 이러한 기술들이 의료 서비스의 혁신과 개인 건강 관리에 새로운 차원을 열어주지만, 보안 문제와 디지털 격차 등의 문제가 동반될 수 있다고 경고하며, 이를 극복하기 위한 대책과 지속적인 개선이 필요하다고 강조하고 있다.

  • 최근 건강에 대한 관심이 늘면서 의료 환경도 진료 중심 의료에서 예방 중심 의료로, 질병관리 중심에서 건강관리 중심으로 패러다임이 바뀌고 있다. 또한 국민 경제에서 의료비 비중은 점차 증가하고 있는 추세이다. 이러한 이유로 자신의 라이프 스타일 및 건강을 직접 관리하려는 인구가 증가하고 있다. 이를 IT 기술에 접목하여 가정 내에서 질병관리뿐만 아니라 병원 간의 연결을 통한 원격진료 서비스 제공 등을 목표로 하는 웨어러블 헬스케어 분야 동향을 살펴보고 이에 대응하기 위한 DSP 설계 동향에 대해서 살펴보겠다.

U-health Service Model for Managing Health of Chronic Patients in Multi-platform Environment (멀티플랫폼 환경의 만성 질환자 건강관리를 위한 유헬스 서비스 모델)

  • U-health services have been progressed as treatment and management for specific diseases and prevention services for providing the behavior management to customers according to the increase in chronic patients. The conventional U-health services provide required services and bio-information monitoring only through remote diagnoses and counsels and that represent limitations in preventing and managing metabolic syndrome patients like chronic patients. Thus, in this study a multi platform based U-health service model for managing the health of chronic patients is proposed. The multi-platform based U-health service model can provide continuous health information, diet, and exercise services regardless of the location of customers through PCs and smart phones. In addition, it is able to provide prescription services to doctors and nurses using a CDS (Clinical Decision Support) module based on clinical information. Doctors can identify the life pattern of patients through a behavior modification program and provide customized services to patients. The U-health service model provides effective services in multi-platform environments to customers and that will improve the health of chronic patients.

  • Recently, the need for telemedicine services and user's service requirements are increasing, the research for more convenient telemed service is underway in various fields. In this paper, we propose that Integrated telemedicine system possible to regular health care. unlike the existing telemedicine system, we combine the voice communications systems and biometric information based on CPS (Cyber Physical System) using wireless communication sensor and elderly people who disabilities are transmit the information comfortably, Also doctors in correspondent site are able to improves the accuracy of the medical treatment by received information about biometric and audio communication. In addition Health status information is stored selectively and then doctor can check the elders status everytime and everywhere. In this paper, we designed the telemedicine system suitable for korea aging society situation, therefore we could offer the Elderly-friendly medical service for seniors who live in silver town.

  • u-Health refers to "Always, ubiquitously" prevention against disease, diagnosis, medical treatment service that can receive treatment even if patient does not visit hospital as abbreviated word of ubiquitous and health. U-Health transmits in individual's living body sign and measuring of health information and health information system that is consisted of process of analysis and feedback transmiting and measure individual's living body signal and health information and health proprietary company or medical institution operates. If analyze pattern to information that health information system is transmited, health care officer or chief physician means that offer healthcare and medical treatment service to remote about target customer. Wish to present authoritativeness model for u-Health's inside and outside of the country same native place and u-Health's BMT in this research.

Characteristics of the Requirement for U-healthcare Service Providing Contents for Dietary Life of the Old People (고령자의 식생활 관리 콘텐츠 제공형 U-Healthcare 서비스 요구 특성)

  • Along with the social interests and needs for health, the U-Healthcare Service as a new model to control individual dietary life and to prevent chronic diseases is now drawing the public attention. In this study, the characteristics of the requirement of U-Healthcare Service were investigated from subjects aged over 50 years old. As parts of the expectation to the U-Healthcare service, the customized clinical care & control (37.7%) and the systematic understanding of individual health state (21.4%) appeared significantly. And for the application of the U-Healthcare Service to the field of healthcare, the control of dietotherapy (43.9%) appeared higher than anything else and thus the customized service for individual dietotherapy seemed to be needed. Regarding the field of searching for health information, the disease control (35.6%), food materials (28.6%), and recipe for health foods (22.7%) appeared in sequence and revealed that the majority over 50% of subjects were interested in foods. The requirements for the information of dietary formula were also investigated through 5 points scaled questionnaires on respective topics of nutritive components, menus customized for corresponding health states (diseases), and the results obtained from surveys on respective topics all showed the needs for all information by representing each point value exceeded 3.9. Through the results obtained from this study, the provision of customized information enabling the nutritional control in accordance with individual states of health or disease was estimated to be needed for the popularization of U-Healthcare Service providing contents for dietary life of clients especially those of aged over 50 years.

  • Depression and mental illness across all age groups, and steady increasing in suicide rate are our major social problems which cause dramatically decreasing government competitiveness. While Korean government has implemented various policies to improve mental health and prevent suicide, it faces revealing issues in progress on implementation and referral management systems. The current research examined domestic and foreign cases to understand the actual status and directivity of ICT convergence plan in terms of mental health. The United States and the United Kingdom have provided convergent ICT service, telepsychiatric care to senior citizens in rural areas, children and adolescents, socially disadvantaged class, etc. In Korea, emergency-notice devices are installed to prevent suicide for single elderly people, and diverse mobile health devices are developing to cure depression and Alzheimer's disease. ICT convergent mental health care service is needed to save socioeconomic costs and realize universal welfare for an aging society. Social consensus on sharply conflict issues, medical law and medical devices about telepsychiatry service, effectiveness studies on users' unique psychiatric characteristics by life cycle, and region-specialized strategies for the least densely populated rural regions with lower access to medical care should be considered.

  • u-Healthcare는 유비쿼터스와 원격의료 기술을 활용한 건강관리 서비스를 말한다. 목표는 삶의 질 향상 추에 있다. 경제수준이 향상되면서 삶의 질에 대한 중요성이 대두되고 있으며, 만성질환자 및 인성 질환의 예방 및 효율적 관리는 개인 및 사회적으로도 중요한 의의를 가진다고 할 수 있다. u-Healthcare 서비스를 제공하기 위해서는 언제 어디서나 이용사의 건강상태를 진단할 수 있는 생체계측기술이 필요하기 때문에 u-Healthcare 측정 모듈의 개발은 매우 중요하다. 본 논문에서는 개발한 모바일 연동 다기능 u-Healthcare 측정 모듈을 이응하여 심전도, 체지발, 호흡수, 체온 및 운동량 측정 등의 신호를 측정하였다. 측정된 신호를 Bluetooth 기반의 transceiver를 통해 u-Healthcare Center로 전송하며, Diagnostic Monitor를 통해 신호를 분석하여 건강상태를 모니터링 할 수 있는 검증모니터를 구현하였다.

  • IT 기술의 발전으로 'U-Health'를 기반으로 하는 건강관리 앱의 필요성이 증가하고 있다. 'U-Health'는 무선 인터넷 기술을 이용하여 질병의 예방과 치료를 위한 원격진료 서비스로서 다양한 형태로 사용자들에게 제공된다. 본 논문에서는 안드로이드 플랫폼을 활용하여 근골격계 환자가 의사의 도움 없이 스마트기기를 이용하여 텍스트 문진과 이미지 문진을 통해 의사의 진단과 동일한 결과를 얻을 수 있는 건강관리 앱을 소개한다. 그리고 예약 및 진료 정보 등 병원에서 제공하는 서비스를 스마트기기 상에서 이용할 수 있도록 병원 시스템과 연동하였다.
