The Influence of Depression Self-stigma, Depression Literacy, and Professional Help-seeking Attitudes on Medication Adherence in Chronically Ill Older Adults Who Have Experienced Depression (우울을 경험한 만성질환 노인의 우울 자기낙인, 우울문해력, 전문적 도움요청태도가 약물복용 이행에 미치는 영향)

  • Purpose: This study aimed to examine the effects of self-stigma related to depression, depression literacy, and professional help-seeking attitudes on medication adherence among older adult patients with chronic diseases who have experienced depression. Methods: A cross-sectional survey was conducted involving a total of 163 patients with chronic diseases who had been taking medication for more than six months. Data were analyzed by independent t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis using SPSS/WIN 27.0 program. Results: The mean score for medication adherence was 14.86±2.27. Medication adherence showed a significant correlation with self-stigma of depression and depression literacy. Factors influencing medication adherence in older adults with chronic diseases who have experienced depression included self-stigma of depression, depression literacy, and age. Specifically, patients under 70 years of age, and patients between 70 and 79 years of age. The explanatory power of these variables was 11%. Conclusion: To improve medication adherence among older adult patients with chronic diseases who have experienced depression, it is essential to reduce self-stigma related to depression. Additionally, because depression self-stigma has a negative impact on proactive depression interventions, it is necessary to implement intervention activities aimed at raising awareness and reducing self-stigma.

Multidimensional Health Trajectories and Their Correlates Among Older Adults (노인의 다중적 건강 변화궤적 유형화 및 관련요인 탐색)

  • The purpose of this study was to provide an understanding of the trajectories of multidimensional health among older adults, including depression, chronic diseases, and cognitive function. Data were drawn from the 1-6 waves of the Korean Longitudinal Study of Ageing(KLoSA), and a sample of 2,059 respondents aged 65 and older at baseline was used for the analyses. Latent growth curve models and growth mixture models were used to explore the changes in depression, chronic diseases, cognitive function, and heterogeneous trajectories among them. One-way ANOVAs with Scheffé post-hoc analysis and chi-square tests were used to find differences in sociodemographic characteristics, health behaviors, and life satisfaction across the latent trajectory classes. Latent growth curve models revealed that depressive symptoms and the number of chronic diseases increased over time, while cognitive function showed gradual decreases. Three heterogeneous patterns of multidimensional health trajectories were identified: normal aging, increase in chronic diseases, and chronic deterioration. Significant differences were observed in sociodemographic characteristics, health behaviors, and life satisfaction across the three latent classes. In particular, low educational attainment, household income, and life satisfaction were associated with the chronic deterioration class. Based on the findings, we discussed suggestions for health promotion education targeting older adults. This study also emphasizes the importance of home economics education in promoting health literacy across the life course.

Understanding Factors Associated with Unmet Need for Outreach Community Health Service among Older Adults in Seoul (노인 방문건강관리 서비스 미충족 영향요인: 서울시 찾아가는 동주민센터 사업을 중심으로)

  • The purpose of this study was to examine various factors influencing the needs of Seoul's newly implemented outreach community services for older adults, and to suggest the direction of the outreach community health services in Seoul. A multi-level regression was conducted using data collected by face-to-face interviews from 1,000 individuals aged 65 and 70 in 17 districts, where participated in the Seoul's outreach community services. The results demonstrated that socioeconomic status (higher income and living alone), health status (having multiple chronic conditions and depression, lower health literacy), limited experience of the outreach community services, and low government trust at the individual level were associated with higher unmet need for the community outreach services. In addition, shorter participation period of the outreach services and financial independency at the district level were associated with higher unmet need for the services. The findings from this study implies the need for improving the quality of services by focusing on vulnerable groups such as individuals with lower income and worse health status. In addition, the outreach community health services may need to target individuals aged 66 to increasing efficiency of the services through utilizing results of life-cycle health checkup by the National Health Insurance Corporation.
