Overdenture with magnetic attachments for a patient with Parkinson's disease: a case report (파킨슨병 환자의 자성 피개의치 수복 증례)

  • It is very hard for edentulous patients to satisfy retention and stability with their complete denture. Especially, for patients with systemic diseases such as Parkinson's disease causing involuntary and limited movements, fabricating complete dentures may be challenging to both dentists and patients due to functional limitations. To overcome this problem, overdentures can be used through conserving retained roots. In this case, a 45-year-old male patient had to extract numerous teeth due to dental caries, and at the time of visit he was taking drugs for diabetes and Parkinson's disease for a period of long time. A complete denture was applied to maxilla, and for mandible, an overdenture on two preserved retained roots was applied where its retention and stability were obtained by magnetic attachments. This case reports that overdenture with retained roots improved overall functional limitations.

  • Caries remains prevalent throughout modern society and is the main disease in the field of dentistry. Although studies of this disease have used diverse methodology, recently, X-ray microtomography has gained popularity as a non-destructive, 3-dimensional (3D) analytical technique, and has several advantages over the conventional methods. According to X-ray source, it is classified as monochromatic or polychromatic with the latter being more widely used due to the high cost of the monochromatic source despite some advantages. The determination of mineral density profiles based on changes in X-ray attenuation is the principle of this method and calibration and image processing procedures are needed for the better image and reproducible measurements. Using this tool, 3D reconstruction is also possible and it enables to visualize the internal structures of dental caries. With the advances in the computer technology, more diverse applications are being studied, such automated caries assessment algorithms.

EFFECT OF THE ADDITIONAL ETCHING PROCEDURE ON PUSH-OUT BOND STRENGTH OF ONE-STEP RESIN CEMENT (부가적 부식 과정이 단일 접착 과정 레진 시멘트의 접착 강도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of additional etching procedure prior to Maxcem resin cement application in indirect restoration cementation using push-out bonding strength. One hundred and two extracted human molars were used to make indirect resin restorations of gold inlay and Synfony. These restorations were cemented using Maxcem and Variolink II. Additional etching procedures were done for one group with Maxcem. Three groups have 17 specimens in both restoration types. Push-out bond strength was measured using multi-purpose tester and calculated for bonding strength per sqaure-millimeter area. The mean bonding strength values were compared using SPSS 12.0K program for one-way ANOVA and Scheffe's Test with 95% significance. Under the condition of this study, the additional etching procedure prior to usage of Maxcem resulted in reduced bond strength for both of restoration types.

Full mouth rehabilitation of edentulous patient with fixed implant prosthesis (고정성 임플란트 보철물을 이용한 완전 무치악 환자의 구강회복 증례)

  • There are various treatment options such as conventional complete denture, hybrid prosthesis and implant-supported fixed prosthesis for fully edentulous patients. In case of implant-supported fixed prosthesis, compared to removable prosthesis, it is difficult to place the implant in the correct position considering the anatomical contours of the final prosthesis. In this case, a full mouth rehabilitation with implant-supported fixed prosthesis was performed for a patient who required extraction of all remaining teeth due to dental caries and chronic periodontitis. In the implant placement stage, the implant was placed in the desired position using a surgical guide fabricated considering the anatomical contours of the final prosthesis, and the function and esthetics were evaluated through correction and re-fabrication of the fixed provisional restoration. A final restoration of porcelain fused to gold prosthesis was delivered to the patient based on the provisional restoration. To cope with complications such as loosening of screws and fracture of porcelain, a screw-retained type prosthesis was fabricated for the posterior part and a screw-cement-retained type prosthesis for the anterior part. As a result, the patient showed an improved prognosis in terms of functional and esthetics after the final prosthesis was delivered.

  • Since January 2019, insurance coverage for caries treatment of permanent teeth using composite resin in children aged 12 and under has started in South Korea. The purpose of this study was to compare the pattern of permanent molar composite resin restoration aged 12 and under before and after the insurance coverage from January 2009 to March 2020 in the Department of Pediatric Dentistry at Kyung Hee University Dental Hospital at Gangdong. Since the insurance coverage was started in 2019, the frequency of permanent molar composite resin restoration aged 12 and under has increased more than twice. There was a significant change in frequency of resin restoration by shape of cavity comparing before and after the insurance coverage, whereas there was no significant difference in changes by patient age, gender and position of tooth. As accessibility to the permanent tooth composite resin restoration is increased by the insurance coverage, which shows a fair survival rate, it is expected that it could be able to secure oral health in the early permanent dentition aged 12 and under.

Effectiveness of Oral Health Education Program using Home-using Portable Device for Children (정량광형광기를 이용한 어린이의 구강건강관리교육 효과)

  • This study was performed to determine the effectiveness of oral health education program with a home-using portable device according to the individual oral health status in children. 58 children who were 6 - 12 years old were included in this study. All subjects were affiliated to moderate or high caries risk group based on caries risk test. They were divided into 2 groups: (I) home-using portable device group (II) control group. Both groups were evaluated with simple plaque score (SPS) using camera type quantitative light-induced fluorescence device and educated with identical oral health education methods. Subjects in group I were demanded to use a home-using portable device. After 1 month, both groups were re-evaluated. Cariview score that can reflect the acidogenic potential of plaque bacteria was statistically reduced in both groups (p < 0.001). There was a statistically significant difference between two groups in the change of Cariview score (p = 0.022). In group I, the decrease was larger than that in group II. There was no statistically significant difference in the change of SPS (p = 0.937). Oral health education improved oral hygiene status in children. However, this study confirmed that it was much more effective to improve oral health status in children with a home-using portable device in their daily oral hygiene care.

Comparison of Prevention Methods against Enamel Demineralization adjacent to Orthodontic Bracket Using Fluoride (교정용 브라켓 주위의 불소를 이용한 법랑질 탈회 예방 방법 비교)

  • As a common side effect of fixed orthodontic treatment, demineralization of the enamel adjacent to the bracket and band occurs in patients with poor oral hygiene. The purpose of this study was to investigate what is the most effective method to prevent demineralization around the fixed orthodontic appliance among various methods using fluoride. 80 extracted bovine incisors with a healthy surface were classified into four groups as experimental materials: (Group I) Control group, (Group II) V $varnish^{TM}$, (Group III) Tooth Mousse $Plus^{(R)}$, (Group IV) $Vanish^{TM}$ XT. After treatment for each group, mineral loss and Vickers surface microhardness were measured at 0, 30, 60 and 90 days after demineralization in artificial carious solution. Mineral loss was the lowest in group IV, followed by group II and group III, which showed a significant difference. The surface microhardness was the lowest in group IV, followed by group II and group III, which showed a significant difference. Through this study, group IV showed the best effect to prevent enamel demineralization around the bracket. Group III showed significant prevention of enamel demineralization compared with the control group, but the effect was less than that of the other groups.

Factors related to the difference in the incidence of caries between children in fluoridated and non-fluoridated areas (수돗물불소농도조정사업 지역과 비사업지역 아동 간의 치아우식 발생 격차 관련요인)

  • Objectives: The aim of this study is to analyze the differences in factors related to the incidence of dental caries between children in fluoridated and non-fluoridated areas and compared the DMFT and DMFS scores to confirm the effect of water fluoridation program (WFP) in Geoje and Changwon. Methods: The oral health examination and questionnaire survey were conducted in fluoridated and non-fluoridated areas. The number of surveyed children aged 8, 10, and 12 years in the fluoridated and non-fluoridated area of two cities was 1,524 and 1,383, respectively. Self-recorded questionnaires included self-perception of their own dental health, daily toothbrushing frequency, intake frequency of cariogenic sweet snacks and beverages, experience of gingival bleeding, experience of unmet dental treatment, and use of oral hygiene device except for toothbrush and toothpaste. The prevalence of caries and fissure-sealant status were surveyed. The logistic regression analysis was used to analyze the difference in children's self-rated oral health status between the fluoridated and non-fluoridated area. The mean number of sealed teeth and surfaces, marginal means of the DMFT and DMFS scores adjusted for the difference in the samples' sex distribution, and region were compared between the fluoridated and non-fluoridated areas. Results: In the fluoridated area, the experience rate of unmet dental treatments was higher among children aged 10 years, intake frequency of cariogenic sweet snacks and beverages were higher among the whole sample, and experience rate of professional fluoride application were lower than in the non-fluoridated area. The DMFT score for permanent dentition adjusted for differences in sex, region, and mean number of fissure-sealed teeth in the fluoridated area was significantly lower among whole samples and 8-, 10-, and 12-year-olds alone than in the non-fluoridated area. Conclusions: WFP can alleviate oral health inequality because it is effective in reducing the incidence of caries among children is disadvantaged demographic, socioeconomic, and cultural contexts.

Full mouth rehabilitation of patient with severe dental caries with implant fixed prosthesis fabricated with milling and 3D printing method: A case report (밀링 및 3D 프린팅 방법으로 제작된 임플란트 보철물을 이용한 심한 우식 환자의 완전 구강 회복 증례)

  • Passive fit of prosthesis is an essential property of implant supported prosthesis for long term success and minimization of complications. And the property is determined mostly by fabrication procedure. There were limitations of extensive implant prosthesis because conventional casting method generate contraction error of long span prosthesis. However, Computer-aided design/Computer-aided manufacturing (CAD/CAM) technology of 3D printing and milling metal framework can overcome those limitations. This case is a full mouth rehabilitation using extensive implant fixed prosthesis. Removable interim prosthesis was made for esthetic, functional evaluation and a guide for implant insertion. After the insertion, implant fixed interim prosthesis was delivered. After additional evaluation and adjustment, final prosthesis was designed with CAD, the fabricated with CAM. Milling technique was used for anterior screw type implant superstructure and 3D printing technique was used for the anterior and posterior implant copings. Fit of the final restoration was favorable. The practitioner and patient were both esthetically and functionally satisfied with the final result.

DENTAL TREATMENT OF A PATIENT WITH BARTTER SYNDROME: CASE REPORT (Bartter 증후군 환아의 치아우식 치료: 증례보고)

  • Bartter syndrome is an inherited renal tubular disorder characterized by hypokalemia, hypochloremic metabolic alkalosis, hyperreninemia, hyperprostaglandinism, and normal blood pressure. Bartter syndrome is classified by neonatal and classic type. Clinical manifestation of Bartter syndrome considered in dental treatment include tendency to dehydration, vomiting and erosion, attrition and abrasion of the teeth. A 2-year-4 month old boy with Bartter syndrome type III was referred to the Seoul National University Dental Hospital for dental evaluation and treatment. He showed hypokalemic hypochloremic metabolic alkalosis and had been treated with indomethacin and potassium chloride. He had hypocalcified teeth with or without multisurface caries lesions in all dentition and the erosion of maxillary teeth was detected. Dental procedure under general anesthesia was scheduled due to multiple caries and his medical condition. The dental procedure was successfully performed. This case suggests that electrolyte imbalances need to be treated prior to dental treatment and complete coverage restoration is necessary to protect the eroded teeth. An appropriate management plan for the patients with Bartter syndrome should include considerations of the need for close interaction with the pediatrician for pre- and post-operative care. General anesthesia may be recommendable to manage the patients having multiple caries with Bartter syndrome.
