Factors influencing oral health status of immigrant women in multi-cultural families (다문화 이주여성의 구강건강상태에 영향을 미치는 요인)

  • Objectives: This paper aimed to contribute to better oral disease prevention and practice of health behavior for immigrant women in multi-cultural families, to define missing and filled permanent teeth index of immigrant women, data from the 6th Korea National Health and Nutrition Examination Survey from the Korea Centers for Disease Control and Prevention was used. Methods: For the immigrant women to be subjects, they needed to be born overseas, had acquired Korean citizenship as a married immigrant women, and the estimate of the number of subjects was 133,093 women. For analyzing data, SPSS 21statistical program was used. We used covariance analysis (ANCOVA) andgeneral linear models for finding the relation with the missing and filled permanent teeth index. The significance level was 0.05. Results: DMFT-index of immigrant women was 7.33 points. $R^2$ was 0.416; and increased with age, and $R^2$ was 0.126 points higher (p<0.01). In household income, 'lower' was 5.933 points lower than 'upper' (p<0.05), and in toothbrushing after lunch, 'yes' was 3.598 points lower than 'no' (p<0.01). In preventive treatment, 'yes' was 4.301 points lower than 'no' (p<0.05). Conclusions: The result of this paper is as follows: for maintaining oral health of immigrant women, we think that the government needs to develop an oral health policy and a customized education system suited to immigrant women for preventive management of dental disease in immigrant women. In addition, basic data will be provided for public dental health programs based on the result of the study.

DENTAL TREATMENT OF A PATIENT WITH HADDAD SYNDROME UNDER GENERAL ANESTHESIA: CASE REPORT (Haddad syndrome 환아의 전신마취 하 치아우식 치료: 증례보고)

  • Haddad syndrome, the association of Congenital Central Hypoventilation Syndrome (CCHS) and Hirschsprung's disease (HD), is a disease caused by mutation of PHOX2B gene. This disease, which occurs very rarely worldwide, is characterized by adequate ventilation during wakefulness and hypoventilation during sleep due to decreased ventilatory sensitivity. In this case report, we report a case of dental treatment under general anesthesia in a child with Haddad syndrome. A 3-year-old child with Haddad syndrome visited Seoul National University Dental Hospital for dental treatment. Dental treatment was planned under general anesthesia because of his medical condition. Treatment was successfully done without any postoperative complications. This case suggests that general anesthesia may be advantageous for dental treatment due to the risk of respiratory depression in a child with Haddad syndrome.

  • X-linked dominant hypophosphataemic rickets(XLHR) is a hereditary disease generally transmitted as an X-linked dominant trait. Oral manifestations include poorly mineralized dentin, enlarged pulp chambers and root canals, early tooth loss. Also, spontaneous dental abscesses could occur in the absence of a history of trauma or caries. This report describes the clinical finding and dental treatment of a child, who came to the hospital for treatment caries but was refered to pediatrics because the child showed clinically short of status, bow-leg and radiographically enlarged pulp chamber and canal, there as diagnosed as XLHR. After the diagnosis, 13 years follow-up and treatment were performed.

  • McCune-Albright syndrome (MAS) is a rare disease characterized by fibrous dysplasia (FD), Cafe-au-lait spots, and endocrine disorder. A 4-year-old girl with MAS visited the clinic with a chief complaint of facial asymmetry and bruxism without any pain. Facial asymmetry and many dental problems such as midline deviation, "ground glass appearance" on the entire jaw, thinned cortical bone, loss of lamina dura and ectopic germs were found. Because of severely displaced tooth germs and FD affected jaw, there is a high possibility of malocclusion during mixed/permanent dentition. It is necessary to observe the eruption pattern periodically. If there are clinical symptoms like an abnormal eruption pattern, facial asymmetry or high caries susceptibility, appropriate interventions of dentist are required.

  • Robinow syndrome or fetal face syndrome is a rare inherited disorder characterized by short stature, mesomelic brachymelia, hypoplastic genitalia, and a typical facial appearence ("fetal face") with frontal bossing, hypertelorism, ear abnormalities, a short upturned nose, long philtrum, micrognathia, and macrocephaly. Intraoral features have included quite a few dental cavities, crowding, hypoplastic uvula, cleft lip or/and cleft palate, gingival hyperplasia, alveolar hyperplasia, enamel hypoplasia, delayed eruption, and congenital missing of the permanent teeth. We report on a 10 years old girl with Robinow syndrome. The patient had most of the typical anomalies of the syndrome and negative family history but, in addition, had mental retardation, hearing loss, and serous otitis media. Intraoral findings included dental cavities, crowding, hypoplastic uvula, repaired cleft palate, and mouth breathing. Dental treatment and V-tube insertion(by dept. of ENT) were performed under general anesthesia. In all cases of Robinow syndrome, thorough evaluation and united treatments with medical specialists should be performed.

SEROTYPES AND ANTIBIOTIC SUSCEPTIBILITIES OF STREPTOCOCCUS MUTANS ISOLATED FROM DENTAL PLAQUES OF CHILDREN (치아우식아동과 그의 모친(母親)의 치태(齒苔)에서 분리(分離)한 Streptococcus mutans의 혈청형(血淸型) 분포(分布) 및 약제내성(藥劑耐性)에 관(關)한 연구(硏究))

  • A total of 141 strains of Streptococcus mutans were isolated from dental plaques of 153 subjects. Eighty-two children with caries-experience, 18 caries-free children and their mothers were participated. All isolates were examined for their serotypes by immunodiffusion method with 7 reference sera and their antibiotic susceptibilities to 7 antibiotics by agar dilution method using 7 kinds of antibiotics, and were compared their results by caries experience (DMFT ratio) and by intrafamilial levels. 1. Isolation rate of Streptococcus mutans were greater in samples of carious teeth than those of sound teeth, and in mothers than in children. 2. Multiple serotypes of Streptococcus mutans were occasionally found in a single samples. 3. Of the total 141 isolates (83 isolates from children, and 58 from their mothers), type c isolates were most prevalent (63.8%). Type d,e and f were found, comprizing 14.9%, 10.6% and 6.4% respectively. Serotype g, a and untypable strain were also found but far lower frequencies (2.8-0.7%), and type b was detected. 4. These results suggest that there are no significant correlation among the distribution of serotypes, antibiotic susceptibilities, caries experience and intrafamilial relationships. 5. Most of isolates were susceptible to chloramphenicol (100%), penicillin (95.7%), ampicillin (94.3%), and gentamicin (92.2%), but about one-third isolates were resistant to cephaloridine (27%), streptomycin (33.3%) and kanamycin (47.6%), resulting that 91 strains (64.5%) among 141 isolates were resistant to one or more drugs used. 6. Of the 91 resistant strains, 20 different resistant patterns were observed, and the most frequently encountered patterns were KM, SM and CE.

THE PRACTICE PATTERN OF PEDIATRIC DENTISTS IN KOREA (소아치과 개원의 진료 현황 분석)

  • Pediatric dentistry is differentiated from other fields of dentistry in that it provides comprehensive dental care for children and adolescents. In early days, pediatric dentistry used to be confined to the caries treatment and extraction of primary teeth However, the practice spectrum of pediatric dentistry has broadened to the orthodontic, surgical, esthetic and preventive treatments A survey that contained questions about practice patterns were mailed to 50 pediatric dentists, and 21 surveys were returned. Results were as follows: 1. The average number of patients per week was 82.4, and the average number of treated teeth per patient was 2.35. 2. Preventive treatments comprised 15.7%, restorative treatments 55.7%, endodontic treatments 15.6%, surgical treatments 10.5%, and orthodontic treatments 2.4%. 3. In restorative treatments, amalgam restoration comprised 3.8%, glass ionomer 5.5%, composite resin 63.0%, and stainless steel crown 27.7%.

A study on the correlations between salivary levels of lysozyme, lactoferrin and secretory Immunoglobulin A to Streptococcus mutans and caries susceptibility (치아우식감수성과 타액내 Lysozyme, Lactoferrin 및 Streptococcus mutans에 대한 secretory IgA 수준과의 상관관계에 관한 연구)

  • Saliva plays an important role in modulating the oral microbial ecology. And it is suggested to influence the initiation and progression of the dental caries. To evaluate the correlations between the salivary antimicrobial agents and the caries susceptibility, the 51 subjects were divided into 3 groups according to caries experience ; caries resistant group, medium caries susceptible group, and high caries susceptible group. Stimulated whole saliva was collected, and the salivary levels were measured for lysozyme, lactoferrin, and secretory-IgA to Streptococcus mutans. The lysozyme level was estimated using Micrococcus diffusion plate, lactoferrin level was determined with a non-competitive avidin-biotin enzyme immunoassay, and the titer of secretory IgA to Streptococcus mutans was assayed with ELISA. The results were as follows: 1. Lysozyme levels of each group showed no significant difference statistically (p>0.05). 2. The caries resistant group and the medium caries susceptible group had significantly higher levels of lactoferrin than the high caries susceptible group (p<0.05). But no clear difference was observed between the caries resistant group and the medium caries susceptible group(p>0.05). 3. The caries resistant group and the medium caries susceptible group showed relatively higher levels of the secretory IgA to Streptococcus mutans than the pigh caries susceptible group, but no significant difference was observed statistically (p>0.05).

State of dental treatment among disabled patients at K university hospital pediatric dentistry (K 대학 소아치과에 내원한 장애인의 구강진료 실태)

  • The purpose of this study was to examine the state of dental treatment among disabled patients by the type of disability. After the medical records of 531 disabled patients who received treatment at the pediatric dentistry in K university hospital, the following findings were given: 1. As for age distribution by year, the rate of patients aged 10 or down rose to 42.5 from 5.1 percent, and the 16-20 age group increased from 16.7 to 24.8 percent. But the rates of patients aged between 11 and 15 and aged 21 and up were on the rise(p<.05). 2. Concerning the type of disability by year, there was an increase in the number of patients with brain lesions, mental retardation, developmental disorder and Down's syndrome(p<.05). As to the number of dental caries by the type of disability, the patients with heart diseases had the most dental caries that numbered 8.49, followed by Down's syndrome, metal retardation, brain lesions, the other disabilities and developmental disorder. 3. In relation to dental treatment experiences by the type of disability, the patients with developmental disorder(57.5%) received the most dental treatment, followed by mental retardation, the other disabilities, brain lesions, Down's syndrome and heart diseases(p<.05). 4. Regarding general anesthesia experience by the type of disability, the patients with mental retardation(31.6%) were put under general anesthesia the most, followed by developmental disorder, brain lesions, the other disabilities, heart diseases and Down's syndrome(p<.05). In conclusion, nationwide efforts to nurture separate dental personnels responsible for the disabled, to expand relevant facilities and to improve the health care insurance are required to promote the oral health of disabled children.

The effect of calcium and magnesium concentration in saliva on dental caries activity after consuming calcium (칼슘 섭취 후 타액 내 칼슘 및 마그네슘 농도가 치아우식활성도에 미치는 영향)

  • Objectives: The purpose of the study was to investigate the effect of calcium concentration in saliva on dental caries activity after consuming calcium. Methods: A total of 59 adult women aged 20 to 40 years were surveyed for calcium intake. The daily average calcium intake was analyzed through dietary records of the subjects. The subjects were divided into two groups based on daily average calcium intake. Salivary pH and concentrations of minerals in the saliva were obtained from A group and B group. Calcium ($Ca^{2+}$) and magnesium ($Mg^{2+}$) concentrations in saliva were measured by HPLC-Ion chromatography using 15 mM sulfuric acid. The dental caries activity test was quantified by salivary buffer capacity test and plaque pH test. Results: The mean $Ca^{2+}$ concentrations of A group was $12.75{\mu}g/m$, the mean $Ca^{2+}$ concentrations in the B group was $16.30{\mu}g/mL$ (p<0.05) and respectively, $Mg^{2+}$ concentrations were found to be $0.48{\mu}g/mL$ and $0.51{\mu}g/mL$. Calcium intake and calcium concentration in saliva showed a significant correlation (r=0.380). Conclusions: The mean $Ca^{2+}$ concentrations in saliva was higher in the high calcium intake group. Therefore, calcium intake in saliva was correlated with dental caries.
