THE PREVALENCE OF MOLAR INCISOR HYPOMINERALIZATION AND STATUS OF FIRST MOLARS IN PRIMARY SCHOOL CHILDREN (초등학생의 Molar Incisor Hypomineralization 유병률과 제1대구치 치아우식증 및 수복상태 조사)

  • Molar Incisor Hypomineralization(MIH) have recently gained intensive research interests and various clinical trials. Most prevalence, etiology, treatment studies in MIH were carried out in the European countries, and data from the Korean were seldom. This study aimed to investigate the prevalence of MIH and the status of dental caries and treatment on the first permanent molars in primary school children. For this study, 1,344 primary school students in Busan and Ulsan cities were examined directly the permanent incisors and first molars. The results of the survey were as follows: 81 MIH cases were identified among 1,344 cases. The prevalence of MIH in this group of children was 6.0%. Children with MIH showed a significantly higher DMFT value for permanent teeth than children without MIH. The mean number of decayed, missed, and filled in 1st permanent molars(DMFT index) was 1.17. The rate of children with restoration on 1st permanent molar was 13.6% and the sequence of restoration materials was as follows : composite resin, amalgam, gold inlay.

  • Hidden caries is a subtype of the occlusal pit and fissure caries type and is defined as a dentinal caries lesion near the occlusal surface of the tooth seen on a radiograph. In visual examination, the occlusal enamel is seen intact or is minimally perforated. Covert caries, Occlult caries or Fluoride syndrome are used as synonym. The percentage of occlusal dentin lesions that are clinically undetected ranges from 1.4-50%. Little is known about the mechanisms involved in the development of hidden caries. But it is thought that extensive use of topical fluoride or the special bacteriological profile has been a major factor. This case report is about detection and treatment of hidden caries of two children who visited the department of pediatric dentistry, Yonsei University Dental Hospital. The color of caries dentin found in hidden caries lesion is lighter than cavity forming caries, which makes it more difficult to detect caries by visual examination. Therefore diagnosis of hidden caries is often accomplished after clinical sign is recognized by patients. The use of advanced caries detection aids such as Diagnodent.. with periodic radiographic examination is seemed to be helpful for early detection of hidden caries.

  • In this study, the characteristics and oral health status of manufacturing workers were investigated to confirm the factors affecting their risk of developing dental caries and to prepare basic data for developing oral health promotion programs for such workers. A self-administered questionnaire survey and saliva microorganism examinations (S. mutans and Lactobacillus tests) were conducted from July 1 to August 1, 2014 targeting the manufacturing workers in the Busan and Gyungnam areas, to confirm the factors affecting dental caries development among manufacturing workers. As a result, the risk of dental caries was found to be higher in the workers who worked for 10 hours or longer a day than in those who worked for less than 10 hours a day, and in the smokers than in the non-smokers. A 1-point increase in the brushing pattern score resulted in 0.044 times less risk of dental caries development in the S. mutans test and 0.004 times less risk in the Lactobacillus test Therefore, systematic support is needed to improve the work environment of manufacturing workers, along with continuous oral health education in workplaces.

ANTICARIOGENIC EFFECT OF FLUORIDE VARNISHES AND CHLORHEXIDINE VARNISHES (불소 바니쉬와 클로르헥시딘 바니쉬의 항우식 효과)

  • Dental caries which is one of the most common chronic disease complexly developed by the action of oral bacteria, diet, and host factor. Various prevention program enhance resistance of demineralization and reduce the acidogenecity of oral bacteria have been introduced, representative material is fluoride and chlorhexidine. The purpose of the study was to evaluate and compare effectiveness of fluoride varnish and chlorhexidine varnish in vivo. Bovine tooth specimens were implanted in the lower space maintainers and applied with fluoride varnish and chlorhexidine varnish. After seven days in oral environment, metal mesh was covered to make similar condition of plaque accumulation and induce caries. All specimens were analysed by EPMA to evaluate quantitative change of Ca, P and by polarized microscope to identify histological changes. The results were as follows : After initial artificial caries induction in the mouth, there were remarkable enamel caries lesion in the control group under polarized light microscopy. The highest amount of mineral decrease were showed in control group. No statistically significant mineral decrease were showed in fluoride varnish group, while chlorhexidine varnish group showed only significant decrease of P(P<0.05). In conclusion both fluoride varnish and chlorhexidine varnish seemed to be effective for protecting enamel surface from caries activity, although fluoride varnish has more anticariogenic effect than chlorhexidine varnish.

REMINERALIZATION OF INCIPIENT ENAMEL CARIES LESION BY CPP-ACP PASTE (CPP-ACP제제를 이용한 법랑질 초기 우식증의 재광화 치험례)

  • The dental caries can be recovered or worse depending on the velocity of de- and remineralization of tooth. It is possible to remineralize the lesions by fluoride agent, but the results can be distinguished according to one's salivary flow rate, quantity of ion contents, and pH of the saliva. This article presents good results after applying the CPP-ACP paste for the patients who have incipient enamel caries. We instructed the patients to apply the paste everyday in the customed tray. We observed the white spot lesions without getting worse and reducing the size of lesions. After applying the CPP-ACP paste for 6 weeks, we concluded that; 1. It was possible to remineralize the incipient caries without preparation of the teeth. 2. CPP-ACP paste was successful for recovery of the demineralized lesions, especially for mild and moderate caries lesions, not for the severe developemental defects or chronic lesions. 3. CPP-ACP paste was efficient for pediatric patients, and the custumed tray was very useful for patients to apply the paste. 4. The ability of the patients for caring their oral hygiene was improved by routine check up and instructions.

  • As the minimally invasive approach against white spot like early caries lesions in proximal surfaces of the teeth, therapeutic sealing has been introduced and studied for effective materials and methods to arrest the early caries lesion effectively, which is still going on. This study was performed for the purpose of evaluating its validity for the non-cavitated lesions according to the depth from surface using therapeutic sealing followed by artificial caries induction and evaluation with micro-CT, and we obtained the results as follows. 1. It was revealed that the deeper the caries lesions are, the lower radiation intensity at lesion body areas in pre-treatment specimen. 2. In the sealed groups, there were no differences in radiation intensity between pre- and post-treatment, whereas there were significant decreases in unsealed groups(p<0.05). 3. Even in the specimens with the lesions reaching deeply into DEJ, the effect of sealing was significant(p<0.05). Conclusively, it was thought therapeutic sealing can be an effective tool against the early caries lesions, regardless of their depth into tooth, unless cavitated.

Association between Parenting Styles and Dental Caries in Preschool Children (미취학아동에서의 부모의 양육방식과 치아 우식 간의 상관 관계)

  • This study aimed to evaluate the relationship between parenting styles and childhood dental caries using a sample of 3 to 6 years old children in Korea. The subjects were 158 children aged 3 to 6 years old and their parents in Korea. The parenting styles were divided into three groups (authoritative, authoritarian, and permissive) using a translated version of the Parenting Styles and Dimensions Questionnaire (PSDQ). Among the 353 parents/child dyads, 158 questionnaires were returned. Authoritative parenting style was the majority (95.6%), followed by authoritarian (3.8%), and permissive (0.6%). There were no statistically significant differences between dental caries and parenting styles. The mean of dft index in authoritative group was lower than others. In the authoritative domain, the higher the authoritative tendency, the lower the dft index. Overall, authoritative parenting styles resulted in low rates of dental caries for the children. The stronger the authoritative tendency of the parents, the lower the experience of dental caries in the children. Therefore, parenting styles were likely to affect the oral health of a child, but it seemed necessary to supplement the evaluation tool to evaluate the parenting styles.

Dental Health Capacity of The First Permanent Molars among Children in a Primary School in Suncheon City (순천시 일개 초등학교의 제일대구치 건강도 조사)

  • The purpose of this study was analyzing the first permanent molars condition that was how catch on general oral health of the children in primary school, to make use of the basic data on dental health policy project for the school oral health. The sampling was done at the primary school in Suncheon. It made a survey on the first permanent molars of 823 person who had attended in March, 2009. By using the WHO standards and Clune's dental health capacity, the conditions of first permanent molars calculated. DMFT index and it's condition of first molars statistically are analyzed by Pearson's correlation. 1. The dental health capacity of the first permanent molars was 92.2%. 2. The DMFS index of first permanent molars was 3.97. 3. The dental health capacity of first permanent molars have related with DMFT index(r= -0.895). 4. DMFS index of the first molars have positive relation with DMFT index(r= 0.902). In this repory, I would like to suggest that the evaluation of dental health project for the school aged children should be considered including the healthy status of the first molars.

Relation between Maternal Eating Behavior and Dental Caries Experience in Children (어머니의 식행동과 아동의 치아우식경험도와의 관련성)

  • This study aims to provide basic information for conducting reducing children's dental caries, and proper eating habits and ineffective oral health education activities by survey of children's oral examination and their mothers based on 9 elementary school at a local area in Gyeonggi Province from May, 4 until July, 20 2009, and the results were as follows: 1. The children's DMF rate by gender was higher in men than women, the children's dental caries by grades was higher in higher grades than lower grades, so there were statistically significant differences between children's dental caries experiences in gender and grades(p<0.05). 2. Mother's socio-economic characteristics, mother's detergent food intake frequency, the mother's protective food intake frequency and the snack consumption etc. had not great effects on the children's dental caries(p>0.05). In the dental caries experience in children according to the maternal eating behavior, there were the significant differences(p<0.05) between the degree of unbalanced diet in mother and dental caries experience df index in children. Proper eating behavior of balanced diet in mother must take precedence for children's oral health, and required for their children to have interests about proper eating habits and oral health.

Antimicrobial Activity of Extracts from Some Traditional Oriental Medicinal Plants against Dental Caries Bacteria (치아우식 원인균에 대한 한약재 추출물의 항균효과)

  • Antimicrobial activity of extract from some traditional oriental medicinal plants were evaluated for their antimicrobial activity against six oral pathogens, Streptococcus salivarius, Streptococcus oralis, Streptococcus mutans, Streptococcus sanguinis, Lactobacillus acidophilus and Lactobacillus casei, which are associated with caries disease. The antimicrobial activity was examined by determining the inhibition zone using the disc diffusion assay. In antibacterial activity test, extracts of Scutellaria baicalensis, Chrysanthemum indicum, Kochia scoparia, Hydnocarpus anthelmintica and Caesalpinia sappan showed inhibitory effects (40 mg/ml) against tested caries bacteria. Especially, the C. sappan extract showed the strongest activity on S. oralis (40 mm), L. casei (35 mm) and S. mutans (28 mm). Thus, this result suggests that C. sappan may be applicable to preventing dental caries.
