  • Purpose : To evaluate tile frequency of urinary tract anomalies in male neonates less than 3 months old who presented with urinary tract infection(UTI) and to evaluate a appropriate imaging approach after first UTI. Materials and methods : During a period of 5 years from March 1994 to February 1999, 65 male infants less than 3months old(range: 4-92 days, mean: 43 days) with UTI were evaluated. Ultrasound(US) and Voiding cystourethrogram(VCUG) were done in 60 patients. Due to refusal and technological problem, 5 patients were missed. 99mTc-dimercaptosuccinic acid renal scan (99mTc-DMSA renal scan) was recommended to most patients but performed in 40 patients. Renal scan was performed at least 3 months later after urinary tract infection. Results : Urinary tract anomalies were found in 26 of 65 infants. Twenty-six had vesicoureteral reflux(VUR), two had both VUR and double ureter, two had both U and posterior urethral valve. In patients with VUR, eight had renal scar or renal atrophies. In case of renal scar or atrophy, grades of VUR were III or above. Conclusion : We suggest that US and VCUG should be routinely performed in infants(<3months)with first UTI. 99mTc-DMSA renal scan should be performed only when renal parenchymal damage was observed in US and VUR grade III or above in VCUG. • Even though the urinary stones are rare in children, careful observation and monitoring are necessary because the incidence has been increasing. This study is aimed to document the characteristics of urinary stones in children including the symptoms, diagnosis, with or without urinary tract infection, other urinary tract anomaly and treatment. 45 patients under the age of 15 years with urinary stones hospitalized during Jan. 1986 to Jun. 1996 at Severance Hospital were reviewed retrospectively. Patients' mean age was 6.5 years and sex ratio (male : female) was 5.4:1. The most common symptom was gross hematuria. Stones accompanied with urinary tract infection was 46.7%, and stones associated with urinary tract anomaly was 35.6%. Extracorporeal shock wave lithotripsy, percutaneous nephrolithotorny, hydration and diuretics were the treatment modality used. Urinary stone were found to be a significant cause of urinary tract obstruction in children, requiring prompt diagnosis and treatment. Although KUB, IVP, and ultrasonography were commonly used to make the diagnosis, many cases were detected only by ultrasonographic study.

  • There is greater standardization of quality control for microscopic examination of urine than for physicochemical test. In this study, we investigated whether it is possible to control the sediment accuracy by microscopic examination through the real thing by preserving the essential sediment with glutaraldehyde, which is required for the rationality of sediment quality control. A urine specimen was prepared using 2.5% glutaraldehyde as a preservation solution. Samples treated with urine preservatives confirmed the morphological deformation of the cells for four weeks at intervals of one week and confirmed whether they should be preserved for 4 weeks thereafter. After preparing the required sediment slide, two more slides were produced; one was stored in a refrigerator for, and the other was stored at room temperature. The morphological deformation of the specimen was confirmed. Glutaraldehyde has the effect of preserving the refrigerated essential sediments and storing them for up to 8 weeks, refrigerated storage after slide production, stabilized by 3 days. Moreover, after treatment with preservatives, the production of the slide and comparison between the measured values between the laboratories and examiners showed a low consistency. In conclusion, we showed that the urine sediment components can be preserved, and it can be used for quality control and education through real objects.

  • The purpose of this paper is to suggest a plan on succeeding together-Busan New Port & BJFEZ(that is, port hinterland development). This study deals with the terms of three connection growth factors which are a firm's subjective factors, a industrial environment factors, and a governmental policy factors. According to analysis results of the factors, a firm's subjective factors(4.11 score) are scored the most ones of connection growth factors of Busan New Port & BJFEZ, to be compared with a governmental policy factors(4.01 score). with a industrial environment factors(3.92 score). Therefore, first of all, Busan New Port & BJFEZ have to promote connection growth through as follows, a firm's subjective factors : (1) to procure competitiveness through establishment of connection growth model, (2) to promote concentrated marketing strategy & market response ability, (3) speedy meeting to customer's needs, (4) to procure reliability of Busan New Port & BJFEZ. And, the next, Busan New Port & BJFEZ have to promote connection growth through considering a governmental policy factors, a industrial environment factors.

  • The effect of ultrasonic decomposition was introduced to develop a pretreatment method for the analysis of potassium ion in human urine by potentiometry. N-(4’-benzo-15-crown-5)-anthracene-9-imine, which has a good selectivity coefficient for potassium against ammonium, was used as an ion-selective material for the determination of potassium in urine with relatively high concentration of $NH_4{^+}$. Protenis in urine be removed by 85.1% when the sample acidified with 1.0 M $HNO_3$ was preteated for 100 s by sonication. Potential response of the membrane electrode in the pretreated urine had a slope of 54.6(${\pm}0.2,\;n=5$) mV/decade over the linear range of log $[K^+]$=-5~-1(r=0.9997). When an oxidant, $H_2O_2$, was addwd to the urine sonicated with $HNO_3$, the deproteinization increased 10% more than that in case if only $HNO_3$ and then the maximum ratio of ca. 95% was obtained. Moreover, the Nernstian slope for $K^+$ added to the urinary sample increased to 56.7(${\pm}0.1,\;n=3$) mV/decade. When the calibration curves were measured, the slopes did not vary even after the electrode was successively used 20 times with ultrasonic cleaning. The results showed that an ultrasonic pretreatment method provides simplicity in use, reduced treatment time and improved potentiometric characteristics of the membrane as the method effectively removes ca. 95% of proteins in urine.

  • Urinomas can occur after renal trauma or perforation of the collecting system during an endosurgical procedure. However, spontaneous urinomas are very rare. Here we report a case of a spontaneous perinephric urinoma following the removal of a Foley catheter in an 18-year-old girl with acute kidney injury caused by septic shock. The patient had been treated for septic shock, acute kidney injury, and acute respiratory distress syndrome, and had a Foley catheter in place for seven days. After Foley catheter removal, the patient complained of consistent voiding difficulty. An abdominal computed tomography scan showed a large amount of left perinephric fluid, and the aspirated fluid included urothelial cells, confirming the diagnosis of a urinoma. The urinoma was successfully treated by insertion of a double-J stent into the left ureter. This report discusses the available literature on urinomas, and their clinical features, diagnosis, and treatment.

  • To investigate the long-term variation of water qualities, we have daily monitored physio-chemical characteristics of surface water in the Geum River estuary from June 1996 to April 2004. We found that the water qualities were determined by three dominant factors : 1. fluvial input from Geum River ($28.3\%$), 2. chemical processes such as nitrification and phosphate addition originated from sediment resuspension and domestic sew- age input ($18.6\%$), 3. biological processes such as nutrient consumption by primary producers ($13.5\%$). The factor 1 (fluvial input) effectively affected the water quality of the estuary particularly during the normal or low river discharge. The factor 2 (chemical processes) and the factor 3 (biological processes) showed distinct seasonal differences due to their relative strengths of biological activities. The factor 3 was a governing parameter during the period of spring algal bloom in 2004. For the spring period, an empirical equation derived from the multi-regression analyses showed that the in-situ chlorophyll-a distributions in the estuarine water were successfully simulated by the phosphate concentrations and N/P ratios. Therefore we suggest that phosphate functions as a limiting factor for the primary productivity in the Geum River estuary for the dry season, especially during spring.
