The Effect of Transverse Abdominal Exercise for 3Weeks for Lumbar Muscle Strength and Pain Relief on Chronic Low Back Pain Patients (3주간의 복횡근 강화운동이 만성요통 환자의 요부근력과 통증완화에 미치는 영향)

  • Purpose : The purpose of this study was to analyze the effects of transverse abdominal exercise on the change of chronic low back pain and lumbar muscle strength. Method : 18 chronic lumbago patients were transverse abdominal exercise for 3weeks. Result : 1. The strength of the lumbar extensor and flexor of the male subjects was increased significantly after abdominal exercise(p<0.05). 2. The strength of the lumbar extensor and flexor of the female subjects was increased significantly after abdominal exercise(p<0.05). 3. The study can confirm significant relationship between the lumbar muscle strength and lumbar pain before and after the exercise Conclusion : The study of could find the increase of the ability of the lumbar extensor and flexor of both male and female subjects suffering from chronic low back and pain using three-week transverse abdominal exercise. The study confirmed the general decrease of pain after the experiment.

The Influence in Lumbosacral Angle, Lumbar Lordosis, Pelvic Level and Symptoms by Standing Lumbar Traction on HIVD Patients (HIVD 환자의 선자세 요부견인이 Spine Angle에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the influences of lumbosacral angle, lumbar lordosis, pelvic level and symptoms after standing lumbar traction on HIVD patients. METHOD: For this investigation standing lumbar traction was administered to 22 patient who were diagnosed of HIVD. Standing lumbar traction was given to the subject patients for 3 weeks, times a week and each standng lumbar traction lasted 25 minutes. RESULT: For lumbosacral angle statistically significant different was not found although the lumbosacral angle was normalized. For lumbar lordosis statistically significant different was not found although the lumbar lordosis angle was decreased. For pelvic level statistically significant different was not found although the pelvic level was equalized. Statistically significant improvement in symptoms was found after standing lumbar traction. There was significant correlation between lumbar lordosis and lumbosacral angle. CONCLUSION: This study was found that the influences of standing lumbar traction was to decrease symptoms than lumbosacral angle of patients with HIVD. Therefore, it is necessary that to treat the patients with HIVD applied the method to correct spine angle and pelvic level with standing lumbar traction.

  • The pelvic girdle function as an integrated unit with all three bones moving at all three joints, are influenced by the lower extremities below and vertebral column and trunk above sacroiliac movements are caused by spinal motion, whereas iliosacral movements are caused by movements of the lower limbs. Concept of normal functional integration among the lumbar spine, pelvic and hip joint is basic to the understanding of dysfunction in this region and also functional movement of the lumbo-pelvic-hip region are part of the clinical examination, consequently the integrated biomechanics of these region need to be understood. The purpose of this review is to ascertain the integrated biomechanics among the lumbo-pelvic-hip complex by consideration of literature and to give sufficient information to be able to render accurate assessment and treatment for the syndromes described.

The Difference of Success Rate between the Midline Approach and the Paramedian Approach of Spinal Anesthesia in each of Flexed Patients and Straightened Patients (한국 성인에서 정중접근법과 측면접근법에 의한 요부 척추천자시 전굴 자세에 따른 성공률의 비교)

  • Background: Advantages of paramedian approach over midline approach include less tissue trauma and less dependence on patient's ability to assume a fetal position. When midline approach fails in patients who are difficult to take a position with lumbar spine flexed, paramedian approach to interlaminar space may succeed. Methods: Success rates of spinal anesthesia were measured for 47 flexed patients by midline approach, 48 flexed patients by paramedian approach, 23 straightened patients receiving spinal anesthesia by midline approach, and 23 straightened patients by paramedian approach. Results: Success rates in flexed patients were 100% by midline approach and 100% by paramedian approach. Success rates for straightened patients were 13.0% by midline approach and 78.3% by paramedian approach. For straightened patients there were no correlations between success fate and age, weight, Ponderal Index but there were correlations between height and midline approach of spinal anesthesia. Conclusion: Pertaining to adult Koreans, success rates of spinal anesthesia for strainghtened patients were less than those for flexed patients. Success rate of paramedian approach of spinal anesthesia among strainghtened patients were better than those of midline approach.

Comparison Study on Lumbar Strength and Lumbar Flexor/Extensor Ratio of Spondylolisthesis Patients and Herniated Disc Patients (척추전방전위증 환자와 추간판탈출증 환자의 요부근 기능과 굴곡/신전근 비율에 관한 비교연구)

  • The Purpose of this study was to compare the lumbar strength and lumbar flexor/extensor ratio between spondylolisthesis and herniated disc patients. The patients who had a subacute low back pain have been proved to each disease through MRI and we measured the maximal isometric strength of all patients(28) through MedX lumbar extension machine(Ocala, FL). In all patients, males had higher lumbar extensor strength than that of females. Especially, the spondylolisthesis patients had lower lumbar extensor strength than that of herniated disc patients. The statistical significant(p<.05, p<.01) differences were manifested in $48^{\circ},\;60^{\circ}\;and\;72^{\circ}$ between male groups. Also the statistical significant (p<.05) differences were manifested in $60^{\circ}\;and\;72^{\circ}$between female groups. In the lumbar flexor/ extensor ratio, the males of spondylolisthesis groups represented the functional weakness in the flexed portion of the range of motion, and the females of spondylolisthesis and the all patients of herniated disc represented the functional weakness in the extended portion of the range of motion. In conclusion, we may propose the program such that the spondylolisthesis males must increase the ratio of extended portion exercise, and the spondylolisthesis females and herniated disc patients must increase the ratio of flexed portion exercise.

The Effects of Yoga Low Back Pain Exercise and Lumbar Extensor Muscle Endurance Exercise on Chronic Low Back Pain Patients (요가 요통체조와 요부 신전근 지구력 운동이 만성요통 환자의 통증에 미치는 영향)

  • Purpose: In the present study, the degree of subjective pain in chronic low back pain patients through the course of six week of yoga low back pain exercise and lumbar extensor muscle strengthening training was analyzed, and a systematic and effective home exercise therapy program was proposed. Methods: For this study, we sampled 21 random patients with chronic low back pain who were receiving outpatient treatment at G Hospital Pusan, and conducted experiment for six weeks by applying yoga low back pain exercise+modalities (7) lumbar extensor muscle endurance exercise+ modalities (7) and modalities (7). To measure the degree of pain, the visual analogue scale (VAS) and Oswestry Low Back Pain Questionnaire were used. Pain was measured before the experiment and after six weeks. Results: Significant differences were observed between yoga, extensor muscle endurance exercise and modalities (VAS=0.00 Oswestry=0.00). The yoga and extensor endurance muscle exercise reduced low back pain by more than the modalities. Conclusion: This study will be used to develop a preventive and reduction method of LBP. Therefore, Yoga and extensor muscle endurance exercise are appropriate for use as a home program exercise to reduce low back pain in patients.

The effect of sling exercise on lumbar stabilization and muscle strength (슬링운동이 요부안정화와 근력에 미치는 영향)

  • The purpose of Lumbar Stabilization is to recover the ability of regulating movement of muscular strength, which currently becomes an essential approach to the treatment for lumbago. Sling exercise is a dynamic exercise and a method of active exercise for the patients to take part in their own treatment. This research is to develop the correlation between Lumbar Stabilization and muscular strength as well as Sling exercise. The subjects of this experiment are 12 healthy and normal male and female lasting for 4 weeks. EMG and Postural Med were used as measuring apparatuses. After experiment of 4 weeks exercises, there was not a meaningful result in the measured result of EMG(P>0.05), But there was a significant increase in the result of Postural Med after the experiment(P<0.05). According to this result, we can find out that there is a significant correlation between Sling exercise and muscular strength as well as Lumbar Stabilization. The increase in the lumbar region augments Lumbar Stabilization and the reaction speed of muscle power. So it recovers the stabilization of spine. This Sling exercise program is efficient for the treatment and prevention of back pain.

The Effects on 3-Dimensional Exercise of Lumbar Stabilization for Chronic Low Back Pain (3차원 요부 안정화 운동이 만성요통에 미치는 효과)

  • Purpose: This study is intended to evaluate the effects of lumbar stability after 3-dimensional exercises of lumbar stabilization. Methods: Total subjects with chronic low back pain(age ranged from 20 to 60) were recruited. All subjects received 3-dimensional exercise of trunk stabilization during 4 weeks in 00 orthopedic clinic, from May 15 to October 15 in 2006. All measurements of each patients were measured before and after lumbar stabilization exercise. Results: After lumbar stabilization exercise by CENTAUR(R), the muscle power was increased from 57.99kNm to 72.01kNm, there were statistically remarkable differences(p<0.05). VAS was lessened from 6.35 to 3.26, there were statistically remarkable differences(p<0.05). After lumbar stabilization exercise by CENTAUR(R), the temperature was increased from $27.68^{\circ}C$ to $28.26^{\circ}C$, there were remarkable differences statistically(p<0.05). Conclusions: It has been turned out that lumbar stabilization exercise has positive effect on the muscle strengthening, pain index and thermal change.

Effects of Mulligan's Mobilization with Sustained Natural Apophyseal Glides on the Paraspinal Muscle Activity of Subjects with Chronic Low Back Pain (Mulligan의 Sustained Natural Apophyseal Glides Mobilization이 만성 요통 환자의 요부근 활성도에 미치는 효과)

  • Purpose: Low back pain occurs and frequently recurs in many people. An imbalance of paraspinal muscle activity can cause low back pain. Mulligan's concept of mobilizations with sustained natural apophyseal glides (SNAG) is a common method for treating low back pain. The purpose of this study was to investigate the effects of this method on paraspinal muscle activity in patients with chronic low back pain. Methods: Twenty-one patients with low back pain participated in this study. Patients were classified using the Oswestry disability index. The subjects' paraspinal muscles were measured by surface electromyography both before and after the SNAG mobilization. We measured the root mean square value of the paraspinal muscles during lumbar flexion and extension. Results: Paraspinal muscle activity in patients with chronic low back pain was different between the right and left sides. Importantly, paraspinal muscle activity significantly decreased after using the SNAG mobilization method. Conclusion: This study shows a difference between paraspinal muscle activity in chronic low back pain patients and finds that the Mulligan's concept of SNAG mobilization is effective at reducing imbalances in paraspinal muscle activity in low back pain patients.

The Effects of Spinal Stabilization Exercise using Gravity on patients with Degenerative Disc Disease (중력을 이용한 요부안정화 운동이 만성요통을 가진 노인환자에게 미치는 영향)

  • Purpose: The purpose of this study was finding out the effects of spinal stabilization exercise using Centaur which is a 3D spinal stabilization sports implement on Chronic low back pain patients over 8 weeks. Methods: 30 patients with DDD were observed during the study. Their average age was 66.88years, height was 152.12cm and average weight was 58.91kg, 4 males and 26 females were involved. 8 various investigations were performed and varied values were compared with reinvestigation done after having exercised 8 weeks using 3-D CENTAUR We used VAS(visual analog scale) in order to see the variation of pain intensity, MOI(modified oswestry index) in order to see limitation of daily life. Results: VAS was lessened from 7.57 to 2.63, limitation of routine life(MOS) from 23.48 to 11.30, there were remarkable differences statistically(p<0.05). As a result of muscular investigation for static spinal stabilization by 8 variations of body deflection, muscular strength were all increased and there were signigicant differences statistically(p<0.05). Conclusion: It has turned out that pain and limitation of routine life was lessened, as a result of 8 weeks exercise using CENTAUR, and deep muscular power was increased. Thus it has turned out that 3-D spinal stabilization exercise has an effect on the strengthening spinal muscles and alleviation of their pain for old patients with DDD.
