Anomalous Insertion of the Anterior horn of the Discoid Lateral Meniscus into the Anterior ACL - A Case Report - (외측 원판형 연골 전각부의 전방십자인대 전방부로의 부착변형에 관한 증례보고)

  • The lateral meniscus is morphologically more variable than the medial meniscus. An abnormal lateral meniscus also varies with respect to its size, shape and stability, and such variations can occur in any patient of any age. The most common variant is a discoid lateral meniscus. We recently encountered a patient with a discoid lateral meniscus for which the anterior horn of the meniscus was anomalously inserted into the anterior ACL. The patient was forty five years old women who had persistent pain for three to four years without any specific trauma history. She was preoperatively diagnosed as having discoid lateral meniscus by MRI, and was confirmed the presence of the complex tear and anomalous insertion of the anterior horn of the discoid lateral meniscus into the anterior ACL by arthroscopic examination. Arthroscopic subtotal meniscectomy was performed and the symptoms were improved after surgery.

  • Although the lateral discoid meniscus of the knee has been observed frequently, the discoid medial meniscus has rarely been reported in the literature. A discoid medial meniscus with the horizontal tear and the peripheral tear of upper lip were observed. This tear pattern is not usual in the lateral discoid meniscus as well as in the normal meniscus. A plausible mechanism of the tear pattern compared with the normal shape and the discoid lateral meniscus was discussed in the following.

  • Most of discoid menisci are lateral and medial discoid meniscus is very rare. There is no report about simultaneous medial and lateral discoid meniscus in one knee joint. A 15-year-old male patient was diagnosed as having a complete medial discoid meniscus with horizontal tear and intact incomplete lateral discoid meniscus by means of magnetic resonance image and arthroscopy. The patient was treated by arthroscopic partial meniscectomy for both discoid menisci.

  • Although the lateral discoid meniscus has been observed frequently, the medial discoid meniscus is an extremely rare anomaly. The incidence of lateral discoid menisci ranges from 1.4% to 15.5% and the incidence of medial discoid menisci ranges from 0.06% to 0.3%. We present 1 patient of bilateral medial discoid menisci with tear on left side and 1 patient of unilateral medial discoid meniscus with tear, that were diagnosed by magnetic resonance imaging (MRI) and arthroscopy. We performed partial meniscectomy on 2 cases of symptomatic medial discoid menisci under arthroscopy and meniscus-related symptoms were relieved. We report 3 cases of 2 patients with medial discoid meniscus.

Analysis of Lateral Meniscus Injury - Non-discoid versus Discoid - (외측 반월상 연골 손상의 분석 - 비원판형과 원판형의 비교 -)

  • Purpose: To know about the clinical characteristics such as causes, types, associated injuries and treatment of the lateral meniscus injuries. Materials and Methods: Retrospective analysis was performed for 129 lateral meniscus tears which were divided into 2 groups, Non-discoid and Discoid. Analysis included age and sex distribution, causes of injury, types and location of tear, associated injuries, and methods of treatment. Results: Both Non-discoid and Discoid were prevalent in man and Discoid group was equally distributed through 2nd to 5th decades and Non-discoid group was more prevalent in 4th to 5th decades. Sports injury was the most common cause in both groups and the Nondiscoid group tended to sustain minor injuries compared to Discoid group. Complex tear and body was most common type and site of tear in both groups. Associated injuries were common in Non-discoid group and isolated injuries were common in Discoid group. Partial meniscectomy was commonly done in Non-discoid group and subtotal meniscectomy in Discoid group based on the types of tear. Conclusion: Non-discoid and Discoid lateral meniscus injuries are similar in types and sites of injuries but different in causes of injury, associated injuries and methods of treatment. Therefore, proper diagnosis and treatment would be necessary according to the characteristics.

  • Purpose : To analyze the clinical results of isolated lateral meniscus injury according to discoid versus non-discoid, athletes versus non-athletes and repaired cases versus cases treated with meniscectomy. Materials and Methods : Between January 1997 and June 2000, arthroscopic lateral meniscus surgery was performed in 329 cases. We reviewed 80 cases of isolated lateral meniscus injury without associated ligament injury or other pathologic condition retrospectively. The study population was composed of 54 males and 26 females with average age of 30.2 years(range, $17\~40$ years). Average follow up period was 15 months(range, $12\~39$ months). Eighty cases were classified into three categories; discoid group versus non-discoid group, athletes versus non-athletes, repaired cases versus cases treated with meniscectomy. Clinical evaluation was performed using Lysholm knee score and Tegner activity. Results : Non-discoid group had higher incidence of longitudinal tear than discoid group. Athletes group had higher incidence of repaired cases than non-athletes group. The repaired group had better clinical result than patient treated with meniscectomy group. Conclusion : Although lateral meniscus has some special features including anatomic mobility which can cause instability after meniscectomy and technical difficulties in repairing, it is recommended repairing meniscus tear not only longitudinal but also horizontal and complex tear to obtain better clinical results.

Arthroscopic treatment for symptomatic lateral discoid meniscus in a 26-month-old girl -A case report- (26개월 여아에서 발생한 슬관절 외측 원판형 연골 파열의관절경적 치험례 -증례보고-)

  • In children, the snapping-knee syndrome is strongly suggestive of a diagnosis of discoid menisci. Furthermore the loss of extension of the knee suggests a diagnosis of torn lateral discoid meniscus. Symptomatic lateral discoid meniscus tear in a very young child, especially under the 3-year-old, was very rarely reported and the treatment protocol has not been established. Some therapeutic advancement was achieved in symptomatic lateral discoid meniscus but still technical difficulties exist. We undergone arthroscopic meniscal reshaping in a 26-month-old girl. At the time of postoperatively 2-year 9-month follow up, excellent result has achieved in clinical and functional outcome according to the scale of Ikeuchi without recurrence of symptom or postoperative complication. We reported out experience and results that arthroscopic reshaping procedure was done for management of symptomatic lateral discoid meniscus in a 26-month-old girl.

Discoid Medial Meniscus - A Case Report - (내측 원판형 연골 - 증례 보고 -)

  • The most of discoid meniscus is located in lateral side. Medial discoid meniscus is rare. The medial discoid meniscus was reported by Cave and Staples at first in 1941 but rarely reported since then. A 8 year's old boy was diagnosed as having a complete medial discoid meniscus by means of MRI and arthroscopy. Authors performed the arthroscopic partial menisectomy for medial discoid meniscus. For confirmation of the bilaterality, arthroscopic examination on contralateral knee was performed but we did not find a medial discoid meniscus. We report 1 case of a medial discoid meniscus with a brief review of literatures.
