Benign Schwannoma of the Esophagus-Surgical experience of two cases- (식도에서 발생한 양성 신경초종-수술치험 2예-)

  • Esophageal schwannoma is very rare and almost of all cases are diagnosed as esophageal submucosal tumor preoperatively. Final diagnosis is made by postoperative immunohistochemical (IHC) staining of the surgical specimen. We experienced two cases of esophageal submucosal tumor, one was 63 year old female suffering from three months of dysphagia and another was 39 year old female complaining of two months of intermittent dysphagia. Two esophageal tumors were completely removed by esophagectomy and enucleation through right thoracotomy respectively. Postoperative IHC staining demonstrated S-100 positive without mitotic figures and confirmative diagnosed was made as benign esopphageal schwannoma.

IMPACTED PREMOLARS AND MOLARS ASSOCIATED WITH DENTIGEROUS CYSTS IN CHILDREN (어린이에서 함치성 낭과 연관된 매복 소구치와 대구치의 치료)

  • Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features, unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatment required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth, complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise, preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement, osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.

  • Cyst is a cavity filled with fluids and semi-fluids that is lined with epithelial cells. Odontogenic cysts are those that form within the jaw which origin from dental follicles, enamel epithelium remnants of the crown, Malassez epithelial cell rest and basal cell layer of the oral epithelium. In such cases, treatment methods such as enucleation, marsupialization, decompression, surgical excision etc. can be used according to the lesion's characteristics, size, relationship with the surrounding tissue, patient's age and developmental status. This case was to report an odontogenic cyst caused by an impacted immature permannent tooth and its treatment. The cyst was removed by decompression. Cystic cavity was healed with bone tissue and the impacted permanent tooth erupted without any recurred cystic lesion.

  • 결핵성 임파선염은 아직도 한국에서 많이보는 질환이며 젊은여자 성인 특히 아시아인이나 흑인에서 가장 흔하다. 이 질환은 두경부에 흔히 무통성으로 서서히 림프절이 커지는 양상을 보이나 인체내 결절이 있는 어디나 오며 종격동 결절에도 온다. scrofula(선통(腺病))는 다양한 종양, 비종양성 종괴, 감염질환 특히 경부임파선염 풍과 구별되야만 한다. 비록 병력, 역학, 임상양상 흉부 엑스선 그리고 결핵반응검사로써 만족할만한 진단을 내리지만 절제생검으로 조직 및 배양이 필요하기도 하다. 궤양이나 만성 누공 형성을 막기위해 부분생검이나 절개 및 배농은 반드시 피해야 한다. 치료는 항결핵화학요법이며 가끔 외과적 적출술이 약물요법과 더불어 요구되기도 한다.

Clinical Experience of Three-dimensional Reconstruction Using Free TRAM Flap after Total Maxillectomy with Orbital Exenteration (상악골 전절제술 및 안와내용물 적출술 후 횡복직근 유리피판을 이용한 3차원적 재건에 대한 치험례)

  • Purpose: Extensive midface defect following total maxillectomy with orbital exenteration and cheek skin resection should be three dimensionally reconstructed with a large flap that have a sufficient volume of tissue and multiple skin islands. We describe our transverse rectus abdominis myocutaneous(TRAM) free flap with three skin islands which was successfully used in this situation. Methods: A 58-year-old man was performed enbloc total maxillectomy including orbital contents and wide cheek skin because of invasive maxillary squamous cell carcinoma. He was immediately reconstructed with TRAM flap that was designed not vertical but transverse fashion for providing sufficient skin area. Also, deepithelialization procedure making for multiple skin islands was done in flap insetting period when appropriate modification according to the intraoperative situation was possible. Dead space was completely obliterated by bulky muscular tissue, and three skin islands were used for lining of lateral nasal wall, palatal surface, and cheek skin restoration. Results: Postoperative course was satisfying. Maintaining of proper ipsilateral nasal airway, loss of rhinolalia and oronasal regurgitation of food particles, and restoration of cheek contour were successfully obtained. Conclusion: We report clinical experience of threedimensional reconstruction using free TRAM flap after total maxillectomy with orbital exenteration.

CASE OF DENTIGEROUS CYSTS TREATED BY MARSUPIALIZATION (조대술을 이용한 함치성 낭종의 치료)

  • Dentigerous cyst is a cyst arising by tissue fluid accumulation between the enamel and the residual enamel epithelium. It includes the crown of the impacted tooth and excludes the root. The treatments of the dentigerous cyst are enucleation, marsupialization, decompression, surgical excision and etc. In these cases children were evaluated for the chief complaints of the remained mandibular primary second molar. Each cases showed swelling on the buccal side of the primary secondary premolar. In the radiologic evaluation a radiolucent lesion including the crown of the mandibular secondary premolar was found, and it was diagnosed as dentigerous cyst. And the histopathologic examination showed the same result. Marsupialization was operated through alveolar socket and the alveolar socket was protected with vaseline gauze, the obturator combined with space maintainer was followed by the operation. After the operation, the impacted permanent tooth was showing faster eruption speed than the other normal teeth.

  • After clinical and laboratory tests of a 12-year-old warmblood horse (castrated male, 540 kg) displaying symptom of pain, a diagnosis of intestinal obstruction due to foreign substances like sand was made and an abdominal section and enterotomy were conducted to confirm and extract the intestinal stone (enterolith) on the right dorsal part of the ascending colon. The extracted intestinal stone was around 2.9 kg and greenish brown rugby ball sized ($22cm{\times}10cm$) stone. Analysis of the element components (EDX) using a scanning electron microscope revealed percentages of C (30.08%), O (39.85%), Mg (3.89%), P (11.15%), and Ca (11.16%) at a magnification of 400. This was the first example of a successful extraction and treatment of intestinal stone in a horse domestically and thereby has its significance.

  • A clinical study was performed on 152 cases of surgical esophageal disease treated by the Department of Thoracic & Cardiovascular Surgery of Korea University Hospital from Jan. 1989 through July 1994. The most common esophageal disease was cancer which was seen in 73 cases (48%) among 152 cases. All were treated surgically' 52 patients (71%) were managed by curative or palliative resection with reconstruction and feeding gastrostomy or jejunostomy, otherwise Celestine tube insertion was performed on the remaining 21 patients for palliatio'n. Esophageal leiomyoma occurred in 6 cases(3.9%), among them 1 case was performed with trio recoscopic enucleation . Achalasia were in 7 cases (4.6%) and was treated with modified Heller's m otomy and with Belsey Mark IV operation. Diverticulum were in 11 cases (7.2%). Esophageal stricture occurred in 20 cases (14.1 %) and 17 of 20 cases were managed with bypass surgery. Esophageal perforation was seen in 20 cases, its cause was instrumental trauma in 7 cases, stab wound in 4 cases, foreign body in 4 cases, spontaneous perforation in 3 cases, and others 1 case Other disease including congenital lesion was seen In 1 Scases.

Surgical treatment of Metastatic Lung Cancer (전이성 폐암에 대한 외과적 치험)

  • Background: In spite of the development of chemotherapy, prognosis of metastatic lung cancer was poor. On the other hand, surgical intervention has proven itself to bring out superior results, therefore more operations are being praticed based on this superiority against chemotherapy and other modalities on metastatic lung cancer. Material and Method : We analyzed the surgical cases performed from 1983 to 1997 on 17 cases and estimated 5 year survival rate using Kaplan-Meier method. Result: Average age was 42.8, ratio between male and female was 10:7. We had 8 single lobe resections,3 pneumonectomies,1 wedge resection,2 bilobectomy and 3 cases of lobar resection with wedge resection. 5 deaths have occured and among the 5, 3 patients were caused by recurrence of ca. The remaining 12 patients are being followed up in OPD basis among these, 3 recurrence were observed and 9 are still free of cancer. The average survival time was 40.5 months and 5 year survival rate obtained through the Kaplan-Meier method was 60.4%. Conclusion: Even though we are a bit short of cases, we recommend that a thoracic surgeon approach this disease through a surgical method as possibly as he could.

A Case of Neurofibroma on the Tongue (설첨부에 발생한 Neurofibroma의 1례)

  • Neurofibroma are slow growing benign tumors arising from the neurilemal sheath (Schwann cells) and fibroblasts of the peripheral nerve. This benign tumor may occur as a solitary lesion or as a part of the syndrome of neurofibromatosis. The neurofibroma of oral tissues is rare in otolaryngologic field and usually presents as a firm, elevated, nonpainful lesion that shows the usual histologic findings of a neurofibroma or neurilemmoma. Recently, we have experienced a neurofibroma arising from the tongue in a 54 year old female and removed successfully under local anesthesia. No evidence of recurrence has been observed up to date.
