Prosthetic rehabilitation of an edentulous patient who has a maxillary defect is difficult to achieve for many clinicians. The maxillary defect causes leakage of air during pronunciation and compromises denture stability, support and retention by reducing denture-bearing area and breaking peripheral seal. In these patients, the sizes and shapes of defects are very important factors which attribute to prognosis of maxillary obturators. This case report shows the prosthetic rehabilitation of the patient who had maxillectomy on the right maxillary sinus because of squamous cell carcinoma. The patient had a stepwise treatment protocol which started with pre-operative dental examination and followed by surgical, interim and definitive obturation phase. In this case, an acceptable level of retention could be obtained due to well-defined static defects and the preserved premaxillae and the patient was satisfied with the result of the treatment in the aspect of function and esthetics.

Pancreatic cancer is commonly presented with distant metastasis. However metastasis to central nervous system (CNS) of pancreatic cancer was rarely reported. 79-years-old man was hospitalized with sudden onset right arm dysesthesia and weakness. In brain magnetic resonance imaging, multifocal high signal intensity lesions in cerebral and cerebellar cortices were observed. Leptomeningeal and parenchymal enhanced lesions were also noted in contrast-enhanced T1 images suggesting a metastasis from the pancreatic cancer. Stroke like manifestation of CNS metastasis of pancreatic cancer is extremely rare. Careful history taking and evaluation should be performed to find the origin of the sudden neurologic deficit.
Non-small cell lung cancer (NSCLC) with invasion of mediastinal structures is classified as stage IIIB, and has been considered surgically unresectable However, in a selected group of these patients, better results after surgical resection compared to non-surgical group have been reported. The aim of this study is to evaluate the role of surgical resection in treatment of mediastinal T4 NSCLC. Material and Method: Among 1067 patients who underwent surgical intervention for non-small cell lung cancer from Aug 1987 to Dec 2001 in Korea cancer center hospital, 82 patients had an invasion of T4 mediastinal structures (7.7%). Resection was possible in 63patients (63/82 resectability 76.8%). Their medical records in Data Base were reviewed, and they were followed up completely until Jun 2002. Surgical results and prognostic factors of NSCLC invading mediastinal structures were evaluated retrospectively. Result Lung cancer was resected completely in 52 patients (63.4%, 52/82). Lung resection was lobectomy (or more) in 14, pneumonectomy in 49. The mediastinal structures invaded by primary tumor were great vessel (61.9%), heart (19%), vagus nerve (9.5%), esophagus (7.9%), and vertebral body (7.9%). Nodal status was N0 in 11, N1 in 24, and N2 in 28 (44.4%). Neoadjuvant therapy was done in 6 (9.5%, 5 chemotherapy, 1 radiotherapy), and adjuvant therapy was added in 44 (69.8%, 15 chemotherapy, 29 radiotherapy) in resection group (n=63). Complication was occurred in 23 (31.7%), and operative mortality was 9.5% in resection group. Median and 5 year overall survival including operative mortality was 18.1 months and 21.7% in resection group (n=63), 6.2months and 0% in exploration only group (n=19, p=.001), 39 months and 32.9% in N2 (-) resection group (n=35), and 8.8 months and 8.6% in N2 (+) resection group (n=28, p=.007). The difference of overall survival by mediastinal structure was not significant. Conclusion: The operative risk of NSCLC invading mediastinal structures was high but acceptable, and long-term result of resection was favorable in selected group. Aggressive resection is recommended in well selected pateints with good performace and especially N2 (-) NSCLC with mediastinal invasion.
Purpose: This study explored the anal sphincter-saving rate and down-staging rate after preoperative chemoradiotherapy for treating lower rectal cancer. We also explored the prognosis of the patients who refused surgery after preoperative chemoradiotherapy. Materials and Methods: Thirty seven patients with histologically proven lower rectal cancer who underwent preoperative chemoradiotherapy were retrospectively analyzed. In each case, the tumor location was 0 to 5 em from the anal verge, and curative resection of the cancer with performing a sphincter-saving procedure was not feasible before chemoradiotherapy. In each case, the staging examinations, including biopsy, were done before starting radiotherapy and this was repeated at 1 month after radiation therapy. Results: After chemoradiotherapy, among the 37 included patients, 56.8% and 32.4% were downstaged to the T stage and N stage, respectively, when comparing the postradiotherapy stage with pre-radiotherapy stage. Twenty five patients underwent complete resection of cancer at 6 weeks after radiotherapy: eleven, eight and six patients underwent abdominoperineal resection, low anterior resection and local excision, respectively. The sphincter-saving rate among the 24 completely resected cases was 54.2%. Twelve patients refused surgery after radiotherapy. Among 6 patients who refused surgery with biopsy-proven complete remission after chemoradiotherapy, 5 patients were alive without disease at a median follow up period of 31 months, and only 1 patient had local failure. Conclusion: For lower rectal cancer, a high sphincter-saving rate was accomplished with preoperative chemoradiotherapy. The prognosis of the patients who refused surgery with biopsy proven complete remission after chemoradiotherapy was good and these patients need to be kept under close surveillance.
From October, 1987 to December, 1995, nine patients underwent total correction of tetralogy of Fallot in adults over 20 years of age. There were 5 male and 4 female patients aged from 22 to 42 years(mean, 29.6 years). Three patients were in New York Heart Association(NYHA) functional class II, and 6 patients in class III. The hemoglobin values ranged from 10.8 to 20.7 gm/㎗ (average, l5.6gm/㎗). The preoperative clinical features were as follows: cyanosis, 8 patients; dyspnea on exertion, 6 patients; clubbing of fingers, 5 patients; frequent upper respiratory infection, 3 patients. At the operation both infundibular and valvular stenosis were present in all patients. Reconstruction of right ventricular outflow tract(RVOT) using Goretex was required in 7 patients, and transannular patching with Goretex in 2 pateints. Left pulmonary angioplasty with pericardium was done in 2 patients. No hospital deaths occurred. Four of 9 patients(44.4%) had postoperative low cardiac output syndrome, and postoperative bleeding in 5. One patient required reoperation due to residual ventricular septal defect and tricuspid regurgitation 3 months after the first operation. The mean follow-up period was 25 months, range 11 to 77 months. All was asymptomatic and in NYHA class I. We suggest that advanced age is not contraindication to surgery in tetralogy of Fallot, and tetralogy of Fallot in adults could be operated on due to low mortality.

An 11-month old girl with a feeding difficulty and recurrent aspiration pneumonia received surgical correction of complete vascular ring, which was formed by right aortic arch, aberrant left subclavian artery(LSCA) originating from Kommerell's diverticulum(KD) and ligamentum arteriosum. Through left posterolateral thoracotomy, the ligamentum arteriosum was divided to relieve the tracheo-esophageal bundle. KD was separated from the right descending aorta, and the left subclavian artery was severed from KD at its origin and trasfered to the side wall of left common carotid artery. Postoperative course was uneventful, and the patient has been followed up with a clinical improvement.
본 연구의 목적은 관통된 뼈의 결손부위를 각기 다른 membrane(Gore-tex, Vicryl, Guidor)을 사용하여 보다 나은 골 재생을 얻을 수 있는 가를 평가하는 데 있다. 여섯 마리 흰 족제비의 12개 하악 소구치를 근관치료한 후, 치근단 절제술을 실시하여 $3mm\times5mm$ 크기의 관통된 뼈의 결손을 소구치의 근첨부에 형성하였다. 전부 12개의 결손부위가 형성되었고, 이를 3개의 군으로 나누었다. 대조군으로 결손부위를 membrane barrier없이 점막골막피판으로 덮었다. 다른 각 군은 결손부위를 각 Gore-Tex, Vicryl, Guidor membrane을 사용하여 설측과 협측 모두 덮었다. 각 군을 치유기간 6주와 12주로 두 아군으로 분리하였다. 방사선학적 소견으로 6주군에서 대조군은 $65\%$, Gore-Tex군은 $90\%$, Vicryl군은 $95\%$, Guidor군은 $65\%$의 결손부위의 치유를 보였다. 12주 군에서 대조군은 $80\%$, Gore-Tex군은 $95\%$, Vicryl군은 $95\%$, Guidor군은 $90\%$의 치유를 보였다. 조직학적 소견으로 대조군에서는 완전한 골 재생이 일어나지 않았으며, 결손부위로 결합조직이 자라들어온 것이 관찰되었다. Gore-Tex 6 주군에서는 대부분 fibrillar bone이 관찰되었고, 12 주군에서는 부분적으로 lamellar bone이 형성되었다. Vicryl군에서는 거의 완전한 골의 재생이 관찰되었다. 6주군에서는 재생된 뼈는 fibrillar bone이고 부분적으로 lamellar bone을 관찰되었고, 12주 군에서는 주로 lamellar bone으로 구성되었다. Guidor군에서는 제한적인 골 재생과 함께 심한 염증이 관찰되었다. 본 연구의 결과에서 조직재생유도술은 일반적으로 골 재생을 증진시킴을 볼 수 있었고, Vicryl membrane이 가장 뛰어난 골 재생유도 가능성을 보였다.
Infantile hepatic hemangioendotheliomas (IHHEs) are benign vascular tumors, but can be associated with the life-threatening complications, such as congestive heart failure, disseminated intravascular coagulation, and massive bleeding. Various therapeutic options have been developed and the treatment response depends on the patient's clinical status and the nature of the lesion. In the case of a symptomatic IHHE, a non-invasive and precise diagnosis should be performed promptly before the therapeutic method is chosen. Additionally, it should be kept in mind that the residual lesions have malignant potential. We report a case of a congenital giant IHHE that was successfully reduced in size by interferon-$\alpha$ and completely removed by surgical tumor resection with a hepatic lobectomy.

Hypertrophic osteoarthropathy(HOA) is a systemic disorder characterized by proliferative periostitis of long bone and polysynovitis. Clubbing of the fingers and toes usually accompanies HOA. This syndrome is most frequently with lung cancer. Although disappearance of HOA with removal of the tumor is well known, very few typical cases are reported in Korea We report case of lung cancer with HOA, which was dramatically relieved after the surgical resection of the tumor.
This study was conducted to investigate the effect of infusion of sterile boar and their semen as pregnancy antigen factor for improvement of production in gilts. We observed 160 gilts between 7 to 8 months of age that were divided into 4 treatment groups vie. A group-epidimetomized boar, B group-no sperm semen infusion, C group-vasectomized boar, and D group-control in a completely randomized design. The number of total pigs born(NT) of A, B, C, and D groups were 10.05, 10.44, 11.63, and 9.97 pigs, respectively(P<0.05). And the NT of C g.cup was the highest among treatment. The number of live pigs born(NB) was similar to NT The NB of C group (10.70) was higher than that of A(9.12) and control(9.11)(P<0.05). However, there was not significant between B and C groups. The progesterone concentration of C group was the highest compare to other group at the 6th day after breeding. However, the progesterone concentration of C was lower than other groups after 8th day. There were not significant among cortisol of A, B, C and D groups. According of the results of this study, the infusion with vasectomized boar and semen at estrus before gestation can be improved reproductive efficiency because of more litter size in gilts.
