Acromioclavicular Separation with the Fracture of the Coracoid Process - 2 Cases report - (견봉쇄골 탈구와 동반된 오구돌기 골절 - 2예 보고 -)

  • The acromioclavicular separation (AC separation) is a common injury, which is often accompanied by the rupture of the coracoclavicular ligament (CC ligament) in severe occasions. In rare forms of AC separation, the fracture of the coracoid process would occur rather than the rupture of the CC ligament. Only 31 cases of such injury have been reported in the English literature. We present 2 additional cases with literature review. The fracture of the coracoid process is not readily seen on anteroposterior shoulder radiograms. Severe AC separation without widening of CC distance on anteroposterior shoulder radiogram heralds the fracture of the coracoid process.

  • We presented two cases of unstable fracture of the distal clavicle in children associating superior displacement of the ipsilateral distal clavicle and widening of the coracoclavicular (CC) interval. They obtained good osteosynthesis and reduction of the CC interval by conservative treatment. The primary conservative treatment should be considered for the treatment of the unstable distal clavicular fracture with widening of the CC interval in children.

Treatment of Acromioclavicular Dislocation by Modified Phemister Operation Augmented with Coracoclavicular Sling (오구 쇄골간 슬링으로 보강된 변형 Phemister 술식을 이용한 견봉 쇄골 관절 탈구의 치료)

  • Purpose: The purpose of this study was to evaluate the clinical and radiological results of a modified Phemister method reinforcing the 4 strands of an Ethibond sling for acromioclavicular joint dislocation. Materials and Methods: Between September 1999 and May 2007, 30 acromioclavicular joint dislocation cases underwent a modified Phemister method reinforcing the 4 strands of an Ethibond sling. The average follow-up period was 28.2 months (range: 24~33 months). Clinical outcomes were evaluated using the Weitzman classification; the state of coracoclavicular space reduction was done using radiologic findings. Results: According to the Weitzman classification, there were 24 excellent, 4 good and 2 fair case outcomes. The average coracoclavicular distance improved from 16.9 mm to 7.3 mm immediately after surgery. The average ratio of coracoclavicular distance comparing to the contralateral side at the final follow-up was 1.24 (range: 0.68~1.71). Complications included retraction of K-wires in 5 cases and restriction of joint motion in 4 cases. Conclusion: The modified Phemister operation using augmentation of the coracoclavicular ligament by 4 strands of Ethibond is an effective treatment modality in acromioclavicular joint dislocation.

견봉쇄골 관절 탈구의 수술적 치료

  • 저자들은 1990년 1월부터 1997년 1월까지 을지의과대학병원 정형외과에서 제3형 급성 견봉쇄골 관절 탈구로 진단받고 Phemister 방법, Bosworth 방법, Weaver and Dunn 방법등으로 수술 받은 70례의 환자에 대하여 이중 1년 이상 추시 가능하였던 60례를 대상으로 하여 임상적 및 방사선학적 분석을 시행하여 다음과 같은 결과를 얻었다. 수상후 평균 오구쇄골간격은 건측과 비교시 평균 7.1mm의 차이를 보였는데 수술 방법에 상관없이 추시시에는 평균 2mm의 차이를 보였다. 임상적 평가에서는 우수 23례$(38\%)$, 양호 31례$(52\%)$, 보통이 6례$(10\%)$를 보여 대부분 좋은 결과를 얻어 오구쇄골 간격 증가가 임상적 결과와 반드시 일치하지는 않는 결과를 가져왔다

Coracoclavicular Ligament Augmentation Using Endobutton for Unstable Distal Clavicle Fractures - Preliminary Report - (불안정성 쇄골 원위부 골절에서의 Endobutton을 이용한 오구 쇄골 인대 보강술 - 예비 보고 -)

  • Purpose: The purpose of this study was to evaluate the radiologic and clinical outcomes after operative treatment using endobuttons for unstable distal clavicle fractures. Materials and Methods: Between October 2007 and September 2009, 9 consecutive patients who were followed up for at least more than 12 months after operative treatment using a TightRope$^{(R)}$ were studied. The radiologic results on the serial plain radiographs and the clinical results according to the American Shoulder Elbow Surgeons (ASES) score were analyzed. Result: Bony union was shown in 8 cases (88.9%) and the average time to union was 12.9 (range: 9~16) weeks. The average coracoclavicular distances at the postoperative and final follow-up were 5.6 mm and 6.2 mm, respectively, with no statistically significant difference (p>0.05). The average ASES score was 90.3 (range: 78~96) and the clinical outcomes were 6 excellent, 2 good and one fair. There were no complications such as implant failure or infection except for one case of nonunion due to loss of the initial reduction. Conclusion: A major advantage of TightRope$^{(R)}$ fixation for unstable distal clavicle fractures is that no further surgery is needed to remove the implant. We suggest that this technique provides an alternative for fracture with a distal fragment, which is difficult to fix.

Surgical treatment of the Acute Acromioclavicular Dislocation (견봉 쇄골 관절의 급성 완전탈구에 대한 수술적 치료)

  • The acromioclavicular joint is commonly affected by traumatic and degenerative conditions. Most injuries are due to direct trauma, such as a fall on the shoulder. Although there is general agreement on treatment of type I, Ⅱ, Ⅳ, V and VI acromioclavicular injuries, the treatment of type Ⅲ injuries remains controversial. Sixty patients, ranging in age from 19 to 57 years(average, 32), were evaluated an average of 57.5 (range, 13 to 96) months after surgical reconstruction for Rockwood type Ⅲ Ⅳ, V acromioclavicular dislocation. Phemister method (47 cases), Bosworth (3 cases), Weaver and Dunn method (10 cases) were used to correct displacement. An increase of the coracoclavicular distance of the injured shoulder over the normal shoulder was average 7.1㎜ at initial, average l㎜ on postoperatively, and average 2㎜ at follow-up. Overall, 54 of 60(90%) patients achieved satisfactory results. Degree of increase of the coracoclavicular distance has no inliluence to clinical results.

Evaluation of the Surgical Treatment for Chronic Acromioclavicular Joint Injury; Weaver and Dunn Method Versus Acromial Bone Block Transfer (오구견봉인대만을 이용한 술식과 견봉골편을 포함한 오구견봉인대를 이용한 변형 Weaver and Dunn 술식에서의 만성 견봉쇄골관절 손상 치료결과 추시)

  • Purpose: The purpose of our study was to compare treatment results of two different surgical techniques for chronic acromioclavicular joint dislocations. Materials and Methods: Fifty consecutive patients diagnosed as chronic acromioclavicular joint dislocations between January 1997 and June 2009 were included in the study. Patients were randomized into two different groups. Patients in the first group (n=20) were treated using a modified Weaver and Dunn method using a simple coracoacromial ligament transfer method. Patients in the other group (n=30) were treated with acromial bone-block transfer containing coracoacromial ligament. Mean follow-up times for the two groups were 13.1 and 14.9 months, respectively. Results: At 1 year postoperatively, mean coracoclavicular distance, the VAS score and the ASES score for the Weaver-Dunn method group; for the bone block transfer method group were 3.8 mm (-3 to 6 mm), 3.5 (1.0 to 7.0) and 91.1 (81.66 to 95); 3 mm (-2 to 6 mm), 4.2 (1.0 to 7.5) and 79.6 (31.66 to 95). There were no significant differences in radiologic (p=0.377) and functional (p=0.093) results between the two groups. Failures in the former and latter group were, respectively, two and one. Conclusion: The bone block transfer method shows a tendency to maintain coracoclavicular distance and appeared to yield similar results as the modified Weaver Dunn method.

Treatment of the Acromioclavicular Joint Dislocation Using a AO Hook Plate (AO Hook 금속판을 이용한 견봉쇄골 관절 탈구의 치료)

  • Purpose: We wanted to evaluate the results of treating acromioclavicular joint dislocation using an AO hook plate. Materials and Methods: Between February 2008 and September 2009, 10 patients underwent implant removal after surgical treatment of acromioclavicular joint dislocation using a AO hook plate. The Constant-Murley scoring system was administered postoperatively for evaluating the clinical results, and simple X-ray was taken for evaluating the state of reduction. Results: All cases showed satisfactory results on the clinical and radiological study. The mean Constant-Murley score at follow-up was 90.5 (range: 84~95). Three patients had some degree of discomfort with the hook plate, but these symptoms were relieved on removal of the plate. The radiological evaluation showed restoration of the vertical displacement of the clavicle in all the patients. No complications occurred such as infection, plate failure or redislocation after removal of the plate. Conclusion: The short term follow-up results of treating acromioclvicular joint dislocation using an AO hook plate were satisfactory both clinically and radiologically.

  • A triple disruption of the SSSC, an extremely rare injury, has been reported by only a few authors. We present a patient who had sustained a triple disruption of the SSSC: coracoid and scapular spine fractures, and an acromioclavicular joint separation. Treatment consisted of an anatomical restoration of the SSSC, with maintenance of the acromiohumeral and coracohumeral distances; this was achieved by open reduction and internal fixation of the fractures and of the separation. Six months after surgery, the injured shoulder was asymptomatic, with full range of motion. Five years after surgery, at the final follow-up, the function of the SSSC had been restored to the patient's complete satisfaction.

Treatment of Acromioclavicular Joint Injuries Using Clavicle Hook Plates (Clavicle Hook Plate를 이용한 견봉 쇄골 관절 손상의 치료)

  • Purpose: To analyze results of treating acromioclavicular injuries using clavicle hook plates. Materials and Methods: Between February 2008 and April 2010, 18 patients with acromioclavicular joint injury treated by clavicle hook plates were reviewed. Using the Rockwood classification, 7 dislocations were classified as type III injuries and 11 as type V injuries. Implant removal was done at least 3 months from initial fixation. Simple X-rays were taken for comparing the state of reduction, and clinical outcomes were evaluated by the Korean Shoulder Scoring System and the American Shoulder Elbow Society score. The average follow-up period was 9 months. Results: On weight bearing X-rays, the average coracoclavicular distance showed a 114% increase compared to the contralateral side at the time of injury. It decreased to 23% by the last follow-up. The mean Korean Shoulder Scoring System and the American Shoulder Elbow Society score were 80 and 74 at the last follow-up. Conclusion: The immediate stability these plates provide allows rapid rehabilitation. Clavicle hook plate fixation for acromioclavicular joint dislocation is considered an effective method for ensuring satisfactory results both radiologically and clinically.
