Effects of Functional Electrical Stimulation of Rectus Abdominis on Respiratory Capabilities in Children with Spastic Cerebral Palsy (복직근의 기능적 전기자극이 경직형 뇌성마비 아동의 호흡 능력에 미치는 효과)

  • 본 연구는 복직근의 기능적 전기자극이 경직형 뇌성마비 아동의 1회 호흡용적과 폐활량 등의 호흡능력과 복직근의 근육활동에 미치는 영향을 알아보기 위해서 대구 시내 OO특수학교에 재학 중인 14세 이상 16세 미만의 경직형 사지 뇌성마비 아동 6명(남자 3, 여자 3)을 대상으로 하여 복직근에 1회 20분. 주 3회씩 총 6주간 기능적 전기자극을 적용하였다. 기능적 전기자극의 적용 6주 전과, 직전에 두 번의 사전 검사와 적용 후 사후 검사를 실시하였다. MP30(Biopack, USA)을 이용하여 폐활량과 예측치 폐활량 백분율, 1회 호흡용적을 검사하였으며 Table면전극 근전도(Noraxon, USA)를 이용하여 복직근의 근육 활동량을 측정하였다. 분석결과는 다음과 같다. 1. 복직근의 기능적 전기자극 적용 6주 전과 직전의 폐활량은 유의한 차이가 없었으나, 적용 후의 폐활량은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 2. 복직근의 기능적 전기자극 적용 6주 전과 직전의 예측치 폐활량 백분율은 유의한 차이가 없었으나, 적용 후의 예측치 폐활량 백분율은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 3. 복직근의 기능적 전기자극 적용 6주 전과 직전의 1회 호흡용적은 유의한 차이가 없었으나, 적용 후의 1회 호흡용적은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 4. 복직근의 기능적 전기자극 적용 6주 전과 직전의 복직근의 활동 값은 유의한 차이가 없었으나 적용 후의 복직근의 활동 값은 적용 전에 비해 유의한 증가를 나타내었다(p<0.05). 이상의 결과를 종합해 볼 때 6주간의 복직근의 기능적 전기자극이 경직형 뇌성마비 아동에 있어서 복직근의 근력을 강화시킴으로써 폐활량과 1회 호흡 용적 등의 호흡능력을 향상시킴을 확인할 수 있었다.

The Single Lung Transplantation for End-Stage Emphysema by Functional Criteria (말기 폐기종 환자에서 기능적 기준에 의한 일측 폐이식술)

  • Although lung transplantation has been accepted as the most effective treatment for end-stage pulmonary emphysema, it is not only very hard to find a donor but also to obtain a relatively healthy lung. Furthermore, it is more difficult to match the size of the allograft, considering the height, the weight, and the size of the thoracic cage. The single lung transplatations for the end-stage emphysema have been more commonly performed than bilateral lung transplantation due to the shortage of the donors and the long-term survival rate of the single lung transplantations has shown no reasonable difference compared with that of the bilateral lung transplantationh. Recently, the functional criteria based on a comparison of predicted TLCs(Total Lung Capacities) of the donor and recipient according to height, sex and age, have been accepted at a more suitable.

Effects of Obesity on Pulmonary Functions in Children (소아에서 비만이 운동 전후의 폐활량에 미치는 영향)

  • Purpose : The incidence of obesity has increased in Korea recently. Obesity leads to higher risks of hypertension, hyperlipidemia and insulin resistance. It also leads to risks of respiratory complications. This study was performed to see the effects of obesity on children's pulmonary functions and on developed bronchospasm after exercise loading according to their obesity degrees. Methods : 257 obese children and 150 non-obese children were enrolled. Obese children were divided into 3 groups by the obesity degrees. Pre- and post-exercise loading $FEV_1$(forced expiratory volume at one second), FVC(forced vital capacity) and PEFR(peak expiratory flow rate) were checked in all subjects. The percent predicted values of each parameter was compared according to obesity degrees and the differences between pre-exercise and post-exercise values. Results : The percent predicted value of $FEV_1$, FVC decreased only in the severe obesity group compared with those in the control group. However percent predicted PEFR declined according to obesity degrees. The percent predicted value of $FEV_1$, FVC and PEFR after exercise loading were much lower than those before exercise loading in all groups. Conclusion : As the degree of obesity was higher, the percent predicted value of pulmonary function was lower. And after exercise loading, as the degree of obesity was higher, the reduction of percent predicted value of pulmonary function was larger. Therefore the pulmonary function in obese children must be observed carefully. Further studies on the effects of pulmonary functions in obese children are necessary.

Correlation Between NT-proBNP and Pulmonary Arterial Pressure in COPD Patients (만성 폐쇄성 폐질환 환자에서 뇌나트륨이뇨펩티드와 폐동맥압과의 상관관계에 관한 연구)

  • Background: Pulmonary hypertension is one of the cardiovascular complications of in COPD. However, a diagnosis of pulmonary hypertension requires an invasive test, such as right heart catheterization. NT-proBNP is a cardiac hormone that is elevated when a cardiac volume or pressure overload is present. It was hypothesized that NT-proBNP might play a role in detecting of pulmonary hypertension in COPD patients. Method: The 31 COPD patients, who underwent all of NT-proBNP, echocardiography, and spirometry in Seoul National University Bundang Hospital during the period from November 2003 to July 2005, were retrospectively analyzed. Result: Of the 31 COPD patients, 9 patients had pulmonary hypertension. A significant positive correlation was observed between the NT-proBNP and pulmonary arterial pressure (r=0.589, p=0.002). However, there was no significant correlation observed between the $FEV_1$ and NT-proBNP and $FEV_1$ and pulmonary arterial pressure. Conclusion: NT-proBNP might indicate the presence of pulmonary hypertension in COPD patients.

Factors Predicting the Development of Radiation Pneumonitis in the Patients Receiving Radiation Therapy for Lung Cancer (방사선 치료를 시행 받은 폐암 환자에서 방사선 폐렴의 발생에 관한 예측 인자)

  • Background : Radiation pneumonitis(RP) is the major serious complication of thoracic irradiation treatment. In this study, we attempted to retrospectively evaluate the long-term prognosis of patients who experienced acute RP and to identify factor that might allow prediction of RP. Methods : Of the 114 lung cancer patients who underwent thoracic radiotherapy between December 2000 and December 2002, We performed analysis using a database of 90 patients who were capable of being evaluated. Results : Of the 44 patients(48.9%) who experienced clinical RP in this study, the RP was mild in 33(36.6%) and severe in 11(12.3%). All of severe RP were treated with corticosteroids. The median starting corticosteroids dose was 34 mg(30~40) and median treatment duration was 68 days(8~97). The median survival time of the 11 patients who experienced severe RP was significantly poorer than the mild RP group. (p=0.046) The higher total radiation dose(${\geq}60Gy$) was significantly associated with developing in RP.(p=0.001) The incidence of RP did not correlate with any of the ECOG performance, pulmonary function test, age, cell type, history of smoking, radiotherapy combined with chemotherapy, once-daily radiotherapy dose fraction. Also, serum albumin level, uric acid level at onset of RP did not influence the risk of severe RP in our study. Conclusion : Only the higher total radiation dose(${\geq}60Gy$) was a significant risk factor predictive of RP. Also severe RP was an adverse prognostic factor.

The Effect of Pulmonary Rehabilitation in Patients with Chronic Lung Disease (만성 폐질환 환자에서의 호흡재활치료의 효과)

  • Background : It is known that pulmonary rehabilitation improves dyspnea and exercise tolerance in patient with chronic lung disease, although it does not improve pulmonary function. But there is a controversy whether this improvement after pulmonary rehabilitation is due to increased aerobic exercise capacity. We performed this study to evaluate the effect of pulmonary rehabilitation for 6 weeks on the pulmonary function, gas exchange, exercise tolerance and aerobic exercise capacity in patients with chronic lung disease. Methods : Pulmonary rehabilitations including education, muscle strengthening exercise and symptom-Umited aerobic exercise for six weeks, were performed in fourteen patients with chronic lung disease (COPD 11, bronchiectasis 1, IPF 1, sarcoidosis 1 ; mean age $57{\pm}4$ years; male 12, female 2). Pre- and post-rehabilitaion pulmonary function and exercise capacity were compared. Results: 1) Before the rehabilitation, FVC, $FEV_1$ and $FEF_{25-75%}$ of the patients were $71.5{\pm}6.4%$. $40.6{\pm}3.4%$ and $19.3{\pm}3.8%$ of predicted value respectively. TLC, FRC and RV were $130.3{\pm}9.3%$, $157.3{\pm}13.2%$ and $211.1{\pm}23.9%$ predicted respectively. Diffusing capacity and MVV were $59.1{\pm}1.1%$ and $48.6{\pm}6.2%$. These pulmonary functions did not change after pulmonary rehabilitation. 2) In the incremental exercise test using bicycle ergometer, maximum work rale ($57.7{\pm}4.9$) watts vs. $64.8{\pm}6.0$ watts, P=0.036), maximum oxygen consumption ($0.81{\pm}0.07$ L/min vs. $0.96{\mu}0.08$ L/min, P=0.009) and anaerobic threshold ($0.60{\pm}0.06$ L/min vs. $0.76{\mu}0.06$ L/min, P=0.009) were significantly increased after pulmonary rehabilitation. There was no improvement in gas exchange after rehabilitation. 3) Exercise endurances of upper ($4.5{\pm}0.7$ joule vs. $14.8{\pm}2.4$ joule, P<0.001) and lower extremity ($25.4{\pm}5.7$ joule vs. $42.6{\pm}7.7$ joule, P<0.001), and 6 minute walking distance ($392{\pm}35$ meter vs. $459{\pm}33$ meter, P<0.001) were significantly increased after rehabilitation. Maximum inspiratory pressure was also increased after rehabilitation ($68.5{\pm}5.4$ $CmH_2O$ VS. $80.4{\pm}6.4$ $CmH_2O$, P<0.001). Conclusion: The pulmonary rehabilitation for 6 weeks can improve exercise performance in patients with chronic lung disease.
