The Diagnostic Usefulness of Endoscopic Findings and Detection Rates of CLO and HpKit Test for Helicobacter pylori Infection in Children (소아의 Helicobacter pylori 감염에 있어서 위내시경 소견의 진단적 의의와 CLO 및 HpKit 검사의 검출률)

  • Purpose : We performed this study to evaluate the diagnostic usefulness of endoscopic finding of nodular gastritis, CLO and HpKit test for H. pylori infection in children. Methods : Gastroduodenal endoscopy and mucosal biopsy were performed on 212 children who visited our hospital between Jul. 1999 and May 2000 due to abdominal pain. We performed CLO and HpKit test for H. pylori with the time interval of 15, 30 minutes, 1, 2, 3, 24, 48, 72, 96, 120 and 144 hours. Histological examination of H. pylori was made by H-E or Alcian yellow stain with biopsy specimens. Sensitivity, specificity, positive predictive and negative predictive value of nodular gastritis, CLO and HpKit test were calculated from the analysis of above data. Results : Sensitivity and specificity of 3 hour-CLO test was 68.4% and 100% respectively. Sensitivity and specificity of 3 hour-HpKit test was 65.8% and 100% respectively. No significant difference in sensitivity and specificity was found between in 3 hour-CLO and HpKit test(P>0.05). Sensitivity of CLO test increased as time lapsed, but corresponding specificity did not decrease as time lapsed(sensitivity and specificity at 144 hours : 89.5% and 94.8% respectively). However, sensitivity of HpKit test increased as time lapsed, but specificity markedly decreased. Sensitivity and specificity of the nodular gastritis was 78.9% and 93.7% respectively. Conclusion : Both CLO and HpKit test have relatively low sensitivity and specificity for the detection of H. pylori in 3 hours of testing in children. The endoscopic finding of nodular gastritis is another good standard in the diagnosis of H. pylori infection in children.

원자력용 316LN 스테인레스강의 예민화 특성에 미치는 미량원소의 영향

  • 316LN 스테인레스강의 예민화 특성에 미치는 합금성분의 영향을 분석하기 위해 4종류의 다른 조성을 갖는 합금을 제조하여 Oxalic 시험, Modified Strauss 시험 그리고 DL-EPR 시험을 수행하여 각 시험조건에서의 예민화 특성을 관찰하였다. 실험결과, 짙소 첨가는 예민화 저항성을 증가시키나 그 정도는 미약하여, 0.16 % 질소 첨가강의 예민화 곡선에서 꼭지점은 75$0^{\circ}C$, 3 시간에 위치하였다. 반면, Ti과 B 첨가는 316LN강의 예민화 거동에 매우 큰 영향을 미치며, 0.03 % Ti 및 0.0036 % B 첨가강의 예민화 곡선에서 꼭지점은 14 시간까지 나타나지 않았다. Effective Cr량과 예민화 저항성과의 관계를 경험적으로 표현한 여러가지 관계식에 4종류의 스테인레스 강을 적용시켜 본 결과 기존의 식으로는 Ti 및 B 첨가강에서 발생한 현저한 예민화 저항성 증가를 선명할 수가 없었다. Ti과 B의 성분을 고려한 새로운 관계식이 제안되어야 할 것으로 사료된다.

  • 수중에서 어류들이 색이시 생물적인 목표물이나 어로작업시 어구 등의 시각자극과 최대탐지거리 등을 예측하기 위해서 어류의 주된 시각 예민도 요소인 명암대비 역치와 최소 분해각을 수치모델링 하였다. 어류시각의 명반응과 순반응에 따라 명암대비 역치와 최소분해각은 배경휘도와 체장에 따른 자연대수의 함수로 표현하였다. 이 때 관련된 수식의 계수들은 기존의 실험 결과나 어종간의 시각 예민도 등에서 추정할 수 있었으며 또한 생리 생태나 개체차 등도 조정할 수 있다. 본 모델은 먹이생물이나 어구 등의 시각자극에 의한 시인정도와 시정 등의 추산에 이용될 수 있으며, 아울러 전반적인 수중 광학적인 조건하에서 시각자극의 세기에 따른 시각 예민도와 그 반응에 의한 어류행동 모델에 응용될 수 있을 것이다.

Antinociceptive Effect of Testosterone in Androgenized Female Mice (남성화된 암컷 생쥐에서 Testosterone이 통각예민도에 미치는 영향)

  • Objects : Aimed to test the hypothesis that neonatal testosterone exposure in female mice influences the development of testosterone-related pain inhibitory system and that testosterone administered in adulthood decreases the pain sensitivity. Methods : Thirty androgenized(testosterone propionate $100{\mu}g$ ip within 24 hrs after birth) adult female and twenty five control(normal saline $100{\mu}g$ ip within 24 hrs after birth) adult female mice were injected with testosterone propionate 1mg/kg/day for 3 consecutive days from 84th experimental days. Nociceptive sensitivity was measured before and after treatment of testosterone by tail flick latency on 84th and 86th experimental days. Results : 1) On the 84th experimental day, basal nociceptive sensitivity was significantly higher in the androgenized group($2.7{\pm}0.4$ sec) as compared to the control group($3.3{\pm}1.1$ sec). 2) Testosterone treatment on the 84th experimental day significantly lowered nociceptive sensitivity in both androgenized($5.2{\pm}0.9$ sec) and control groups($4.6{\pm}1.8$ sec). However the effect was significantly greater in the androgenized group. 3) Nociceptive sensitivity on 86th experimental day before administration of testosterone was significantly lower in the androgenized group($4.8{\pm}1.9$ sec) as compared to the control group($3.9{\pm}1.2$ sec). 4) Testosterone treatment on the 86th experimental day significantly lowered the nociceptive sensitivity in both groups, but the androgenized group($5.9{\pm}0.9$ sec) showed significantly lower post-treatment nociceptive sensitivity as compared to the control group($4.9{\pm}1.5$ sec). 5) Nociceptive sensitivity was decreased significantly after injection of testosterone once a day for two consecutive days in the androgenized group(${\Delta}2.1{\pm}1.0$ sec), but not in the control group(${\Delta}0.5{\pm}1.3$ sec). Conclusions : There may be a testosterone-related pain inhibitory system, the development of which is enhanced by exposure to testosterone in the neonatal period, and the activity of which is also mediated by testosterone in the later life.

Modelling Relative Water Flow and its Sensitivity of Fish in a Towed Fishing Gear (예망어구내의 상대유속과 어류의 유수감각 예민도에 대한 모델링)

  • 예망어구의 유체역학적인 특성과 예망어구내에서의 유속 측정 자료를 기초로 어류의 측선 유수감각에 작용하는 유효한 유수자극량을 추산하고 정량화하기 위하여 예망어구내의 3차원적인 상대유속의 선형적인 분포정도를 모델링하였다. 예망어구내의 상대유속은 망지의 제반규격과 예망어구의 수중 전개형상에서의 각부 거리비에 따라 상대유속비를 선형적으로 수식화하여 본문의 식(3)과 (8)로 표현하였다. 이러한 상대유속비는 어류의 측선 유수감각기관에 의하여 탐지될 수 있는 와동 유수자극량의 지표로써 사용될 수 있다. 어류의 유수감각 예민도는 어류의 전장과 어체주위 상대유속에 의하여 선형적으로 변화되는 것으로 가정하고 본문의 식(9)와 같이 탐지가능한 상대유속차의 최소값으로 나타내었다. 예망어구내의 상대유속 분포에 관한 본 모델의 계산 결과는 실물예망어구에서 실측된 상대유속의 범위내에서 거의 근사하게 나타났으며 본 모델은 어류의 대망행동 모델링에서 예망어구의 유수자극과 어류의 유수감각 예민도를 상대적으로 정량화하여 어류행동의 반응요인으로 적용할 수 있을 것이다.

A Study on Availability about Positron Emission Mammography of Breast Cancer (유방암 환자의 Positron Emission Mammography에서의 유용성에 관한 연구)

  • Purpose: Until now the general study for breast cancer patient has been mammography, breast sonography for anatomic diagnostics and $^{18}F$-FDG whole body PET for functional one. But the PEM (Positron Emission Mammography) was developed to increase sensitivity, specificity and accuracy to improve the disadvantage of each study. Therefore this present study aims to describe availability of PEM for improving diagnosis of breast cancer. Materials and Methods: During 3 months from January in 2010, PEM was performed on 100 patients who had history of breast cancer. Using Naviscan's PEM Flex Solo II scanner, PEM images of breast were acquired. And then we evaluated sensitivity, specificity and accuracy of PEM data by comparing results of PEM images with postoperative pathologic finding. Results: On the 100 patients, it could reveal 89 true positive, 9 false positive, 7 false negative and 87 true negative. Thus the results of sensitivity, specificity and accuracy for PEM was evaluated 92.7%, 90.6% and 91.7%. Conclusion: The sensitivity, specificity and accuracy have been reported 83.7%, 68.5% and 77.1% in mammography, and that was 89.1%, 79.1% and 83.4% in breast sonography. However the results of PEM were more increased than others. Therefore the results of this study will be available that PEM used diagnosis with breast cancer, used preoperative operation plan and helps to discovery of a part recurrence.

  • 여러 니켈합금 중 인코넬 600은 원전 가압경수로(PWR)의 튜브 재료로 널리 사용되는 재료이다. 스테인리스강과 비교하여 우수한 내식성과 기계적 특성으로 인해 화학설비, 발전설비, 그리고 해양설비 등과 같은 여러 산업분야에서도 널리 사용되고 있다. 그러나 가압경수로용 증기 발생기의 튜브 재료와 같은 특수 환경에서 예민화에 따른 입계부식 및 입계응력부식 등의 문제가 흔히 보고되고 있다. 이러한 내구성 문제는 설비 및 장비의 수명, 내구성 그리고 안정성 등의 치명적인 영향을 미친다. 따라서 용접, 열처리, 그리고 가공과정에서 발생하는 Inconel 600의 예민화 및 입계부식에 관한 연구가 활발히 진행되고 있다. 그러나 100시간 이상 장시간 열화된 시편에 대한 예민화 연구는 전무한 실정이다. 본 연구에서는 장시간 열화된 Inconel 600의 예민화를 평가하기 위해 최대 1,000시간까지 열화를 실시하였으며, 이에 대한 평가방법으로 Double-Loop Electrochemical Potentiokinetic Reactivation(DL-EPR) 시험법을 적용하였다. 본 실험에서 사용된 인코넬 600의 화학성분(wt, %)은 0.01 C, 0.05 Si, 0.14 Mn, 15.3 Cr, 0.5 Cu, 0.015 S, 그리고 나머지는 Ni 이다. 예민화 평가를 위한 등온 열화는 전기열처리로를 이용하여, $550^{\circ}C$$650^{\circ}C$에서 최대 1000시간까지 실시하였다. 열화에 따른 미세조직 변화는 scanning electron microscope와 energy dispersive x-ray spectroscopy를 이용하여 실시하였다. DL-EPR 실험은 $25^{\circ}C$의 0.1M $H_2SO_4$ + 0.001M KSCN(potasium thiocyanate) 수용액 하에서 실시하였으며, 분극은 OCP로부터 600 mV(SSE vs.)까지 1.67mV/s 주사속도로 forward scan을 실시 후, 동일한 속도로 OCP까지 reverse scan을 실시하였다. Degree Of Sensitization(DOS)값은 anodic scan peak($I_a$) 값과 reverse scan peak($I_r$) 값의 비로 산출하였다. $$i.e.\;DOS=I_r/I_a{\times}100$$. 그 결과, 온도 변수에 따른 Inconel 600강의 예민화 거동은 서로 상이한 경향을 나타내었다. $550^{\circ}C$의 경우, 열화가 진행됨에 따라 DOS값은 급격하게 증가하는 경향을 나타냈다. 반면, $650^{\circ}C$에서는 일정시간 이후부터 Cr 확산 현상에 의한 탈 예민화 현상이 관찰되었다.

Findings of F-18 FDG Whole Body PET in Patients with Stomach Cancer (위암 환자에서 F-18 FDG 전신 PET의 소견)

  • Purpose: Stomach cancer is one of the most common malignancies in Korea, but there is no report on FDG PET in patients with stomach cancer. We observed findings of FDG PET in patients with stomach cancer. Materials and Methods: In 13 patients with pre-operative stomach cancer, PET and CT were performed. Primary lesion and regional lymph nodes detection were aualyzed. Correlation between FDG uptake ratio and each prognostic factor of primary lesion was analyzed. In 19 patients diagnosed as recurrence or displaying suspicious symptoms, conventional work up including tumor marker and PET were performed. Recurrence detection of anastomotic site, distant metastasis, and tumor marker elevation were analyzed. Results: Sensitivity for primary lesion detection was 83.3% (CT 71.4%) and two submucosal lesions were undetected. FDG uptake ratio was variable and had no correlation with invasion-depth, size, Borrmann type, staging and differentiation. Sensitivity for regional lymph node detection was 58.3% (CT 58.3%) and the lesions less than 1cm were undetected. Sensitivity for recurrence detection was 100% but there were three false positives. Sensitivity for distant metastasis detection was 64.3% and significantly higher than that of conventional work-up (21.4%). Average of tumor marker level in patients who were confirmed as recurrence was higher than false positive. Conclusion: PET is more useful than conventional work up in distant metastasis detection when recurrence is suspected. In pre-operative stomach cancer, PET is comparable to CT for detection of primary lesion and regional lymph node metastasis and detection of distant metastasis requires further study.

Role of $^{18}F$-fluorodeoxyglucose PET/CT in Recurrent Ovary Cancer (재발 난소암의 진단에서의 $^{18}F$-fluorodeoxyglucose PET/CT의 유용성: Enhanced CT와 Tumor Marker CA 125와의 비교)

  • Purpose: To date, anatomical imaging modalities of the pelvis and tumor markers have been the mainstay of surveillance for recurrent ovary cancer. This study aimed to assess the role of $^{18}F$-FDG PET/CT in evaluation of ovary cancer recurrences, especially in comparison with enhanced a and tumor marker CA 125. Materials and methods: 73 patients who had PET/CT scan for restaging of confirmed ovary cancer, and additional imaging with enhanced a of the pelvis within one month were included. CA 125 level was available in all patients. From the PET/CT images, maximum standard uptake values (SUVmax) of suspected recurrence sites were recorded. Confirmation was available through re-operation or biopsy in 26 cases, and clinical assessment with series of follow-up images in 47. Results: PET/CT had 93% sensitivity and 88% specificity for detecting recurrent ovary cancer. Enhanced a of pelvis had sensitivity and specificity of 83% and 88%, and CA 125 50% and 95%. Conclusion: PET/CT has higher sensitivity for detecting recurrent ovary cancer compared to enhanced a though the differences were not significant. PET/CT has significantly higher sensitivity than CA 125. However, the three tests all agreed in only 43% of the recurrence cases, and recurrence should be suspected when any of the tests, especially PET/CT, show positive findings.

Mediating Effect of Social Support in the Influence of Interpersonal Sensitivity on College Life Satisfaction (대인예민성이 대학생활만족도에 미치는 영향에서 사회적지지의 매개효과)

  • This study was conducted to find a method of improving social support in college students by analyzing the effects of interpersonal sensitivity and social support on college life satisfaction. To achieve the purpose of the study, questionnaires were distributed to 475 students in six colleges located in Busan Metropolitan City and the collected data were used for analysis. Then, hierarchical regression analysis was conducted using the SPSS statistic program. The major study findings are as follows. First, interpersonal sensitivity negatively influenced social support. Second, interpersonal sensitivity negatively influenced college life satisfaction. Third, social support positively influenced college life satisfaction. Fourth, social support had a mediating effect in the influence of interpersonal sensitivity on college life satisfaction. These study findings demonstrate that there is a need to develop a psychological counseling program for reducing interpersonal sensitivity of college students and improving social support and expand an interpersonal relationship and family relationship improvement program.
