Imaging Findings in Pediatric Musculoskeletal Infection and Inflammation (소아 근골격계 감염성 질환 및 염증성 질환의 영상 소견)

  • Infections and inflammatory conditions of immature musculoskeletal systems in pediatric patients also affect the adjacent muscles, connective tissues, and joints. Rapid diagnosis leading to appropriate treatment can significantly impact the occurrence of complications and mortality rates due to these conditions. When a radiologist becomes familiar with the imaging findings of pediatric musculoskeletal infections and inflammatory diseases, rapid differential diagnoses and more timely and appropirate treatment could be possible. In this paper, we introduce the imaging findings of infectious and inflammatory diseases affecting the immature musculoskeletal system, such as osteomyelitis, pyogenic arthritis, juvenile idiopathic arthritis, and hemophilic arthritis, based on the anatomical and pathophysiological characteristics of the immature musculoskeletal system in children.

  • 낭은 상피세포로 싸여있는 병적인 공간으로 내부가 액체나 반액체로 채워져 있다. 악골에서 낭은 다른 골 부위에서보다 더 흔하게 발생하는데 이는 대부분의 낭이 치아 형성 후에 악골에 남아있는 치성잔류물로부터 발생하는 경우가 많기 때문이다. 구강악안면부에서 발생하는 낭은 연조직에서 발생할 수도 있지만 대부분 골내에서 발생하고, 법랑질 상피나 치배와 관련해 악골내에 발생하는 치낭성이다. 치낭성은 WHO분류에서 발생근원에 따라 크게 염증성과 발육성 기원의 두 가지 군으로 분류되고, 발육성 낭은 치성과 비치성으로 세분할 수 있다. 악골에 발생하는 낭은 대부분이 치낭성이며 가장 흔한 악골 낭종은 치근단낭으로 염증성이고, 발육성인 함치성낭과 치성각화낭도 다음으로 발생률이 높은 편이다. 소아환자에서 함치성낭과 치성각화낭의 발병률이 성인보다 더 높았다. 비치성낭 중에서 비구개관낭이 가장 많이 발생하며, 연조직에서 발생하는 낭과 상피세포 경계가 없는 낭성 병소인 가성낭(pseudocyst)은 드물게 관찰된다. WHO에서 치성각화낭과 서회화치성낭은 파괴적 성장을 하고 재발 경향이 높으므로 양성 낭성 종양으로 재분류하고 있다.

  • implant라는 것은 생리학적으로 생체에 있어서 이물이라는것을 인식해야 한다. 또 그 일부를 상부구조물을 제작하기 위하여 구강내에 노출시킴으로써 연조직과의 접촉부위가 감염으로 염증상태를 지속하고 있다. 그뿐아니라 저작시에 교합압에 의한 위격으로 골내에 여러 가지 변화를 일으킨다. 이와같이 생체내에 이물를 implant 하였을 때 생기는 반응은 생체의 방어기전으로서 염증을 일으키는데 이 염증의 진도에 따라 동화, 흡수, 배제중의 어느 하나로 결말짓게 된다. 따라서 implant를 시술하였을때 생기는 조직의 반응을 최소한으로 감소하기 위한 생물학적 적응조건으로서 재료, 약물, 기구, 시술방법 등을 잘 선택하는 것이 implant의 임상응용에 요절이 되는것이다. implant의 종류는 실로 다양다종이다. 그러나 어떤 implant와 시술방법을 선택하든지 악골에 접착, 또는 식위하여 견고하게 장착되어 그 implant된 자체를 통하여 가장 천연치에 가까운 기능을 얻는것이 implant의 시술목적이다. 임상의가 implant를 시술하면서 이것이 과연 성공할것인가? 즉 저작효율과 내구연한이 얼마나 갈것인가 우려하지 않을수 없다. 그러므로 그 우려를 덜고 성공으로 이끄는 임상상의 문제점을 찾아 논하기로 한다.

Sonographic Examination of the Soft Tissue Using Artifacts (인공물을 이용한 연조직의 초음파 검사)

  • The artifacts in sonography is not always harmful. Sometimes it is beneficial for the evaluation of the water contents of the soft tissue and estimating the degree of inflammation and character of the regenerated tissue indirectly using artifacts such as acoustic shadowing and the enhanced transmission. It can also be useful to evaluate the possibility of aspiration of the calcifies masses by knowing of the contents of the water among them. Unlike the MRI it is useful to get real time informations with low cost in diagnosis and treatment of the soft tissue disease usinf artifacts in sonography.

Identification of bacteria from the peri-implant sulcus of orthodontic mini-implants using 16S rDNA clone library (16S rDNA 클론 library 제작 및 핵산염기서열 결정을 통한 교정용 미니임플랜트 주위 열구의 세균 동정)

  • Objective: The purpose of this study was to compare the bacterial flora at the peri-implant sulcus of the orthodontic mini-implant placed in the alveolar mucosa with the bacterial flora at the adjacent healthy gingival sulcus. Methods: Two plaque samples from 7 patients were collected by inserting paper points into the sulcus between the mini-implant and ligature wire connected to the mini-implant head and inflamed alveolar mucosa, and from the gingival sulcus of a healthy tooth adjacent to the mini-implant. Results: Using 16S rDNA clone library, the 24 kinds of bacteria including Haemophilus aphrophilus, Sphingomonas species, Capnocytophaga species, Prevotella melaninogenica, Lachnospiraceae species, Porphyromonas species, Neisseria flava were identified only from the sulcus around the mini-implant. These bacteria constituted only 9.2% of total clones, and the bacteria identified from both the sulcus around mini-implants and the gingival sulcus constituted 80.4% of total clones. Of these bacteria, clones of Prevotella species, Atopobium rimae, Veillonella species, Streptococcus intermedius/constellatus, Streptococcus salivarius were more frequently isolated from the peri-implant sulcus. Conclusion: This study suggests that a broad epidemiological study is needed to find causative bacteria which induce inflammation from the peri-implant sulcus.

Diffuse Large B-Cell Lymphoma Associated with a Chronic Inflammatory Condition Induced by Metallic Implants: A Case Report (금속성 임플란트로 인한 만성 염증 상태와 연관된 미만성 거대 B세포 림프종: 증례 보고)

  • Chronic inflammatory condition associated with metallic implant insertion is a risk factor for diffuse large B-cell lymphoma (DLBCL). Metal ions play a role in the pathogenesis of lymphoma. We report a rare case of DLBCL in a patient who had a metallic implant in the proximal tibia for 15 months. Radiologic studies, including US and MRI, showed disproportionately large extraosseous soft-tissue mass and bone marrow involvement without prominent bone destruction. Multiple complications are associated with metallic implants, and misdiagnosis may lead to inappropriate treatment. Therefore, distinguishing lymphomas caused by a metallic implant-induced chronic inflammatory condition from other periprosthetic benign lesions and malignant soft tissue masses is challenging, but it is critical.

  • Immunoglobulin G4 (IgG4)-related disease is a rare systemic fibroinflammatory condition characterized by organomegaly or tumefactive lesions associated with lymphoplasmacytic infiltration rich in IgG4 plasma cells. We report a case of IgG4-related disease involving the subcutaneous layer of the left upper arm in a 48-year-old female presenting with an unusual soft tissue mass. US and MRI showed an irregular infiltrative soft tissue mass, indicating possible malignancy or inflammation. We discuss the diagnostic criteria, histopathologic features, radiological features, and treatment of IgG4-related disease.

  • Periodontal tissue destroyed by inflammation is difficult to achieve regeneration of the tissue and esthetic restorations only by surgical methods. In particular, improvement of esthetics is more difficult if the problem is related to the implant. A 23 year old woman suffered from unesthetic anterior implant prosthesis. According to her dental history, a repeated bone graft and soft tissue graft failed at a local dental clinic. It was needed to resolve the inflammation and to improve the esthetics. A free gingival graft and ridge augmentation accompanied by guided bone regeneration and a vascularized interpositional periosteal connective tissue graft was performed. Instead of implant prosthesis, a conventional fixed bridge was adopted for better esthetic result. The patient was satisfied with the esthetic conventional fixed prosthesis. This case report introduces esthetic rehabilitation of unesthetic implant prosthetics in the maxillary anterior dentition by a combination of surgical and prosthetic approaches.

  • After the resection of oral tumor, defected maxillofacial structure caused functional difficulties including phonetics, mastication and esthetic aspects. In this cases, implant retained prosthesis can contribute to the functional enhancement. Regardless of the success rate in grafted bone, however, the soft tissue usually had a shape which was susceptible to inflammation. Moreover, infected graft bone presented rapid destruction. For success of the prosthetic treatment, adequate soft tissue treatment and frequent recall check are the essential factors to the successful implant prognosis.

  • Angiomatoid fibrous histiocytoma is a rare soft tissue tumor that generally affects children and young adults. We report a case of angiomatoid fibrous histiocytoma in an 11-year-old boy who complained of a back mass for 3 years. Surgical excision was performed. The excised specimen showed a $4.0{\times}3.6{\times}3.0cm$, well circumscribed, grayish white tumor, with multicystic changes. Histological examination showed proliferation of spindle or round shaped tumor cells. There was a dense fibrous pseudocapsule with prominent chronic inflammatory cell infiltrates.
