Outcome of Extraskeletal Osteosarcoma; Case-control Study with High Grade Soft Tissue Sarcoma (골외성 골육종의 치료결과; 고악성도 연부조직육종과 환자-대조군 분석)

  • Purpose: Compared to soft tissue sarcoma, the relative risk of extraskeletal osteosarcoma is still not clear. The purpose of this study is to identify the difference in survival and local recurrence rate between two soft tissue sarcomas. Materials and Methods: Twelve patients with pathologically confirmed extraskeletal osteosarcoma were analysed. For retrospective matched case-control study, we selected 72 patients who were confirmed as high grade soft tissue sarcoma and had similar tumor location, tumor size and age to extraskeletal osteosarcoma. Results: Median age was 50 years old. Five cases were located in upper extremity, four in the buttocks, three in the lower extremity. Overall survival rate of extraskeletal osteosarcoma group and high grade soft tissue sarcoma group at 5 years were 52% and 55%. There is no significant difference (p=0.8). Local recurrence rate and metastasis rate were 58%, 67% in extraskeletal osteosarcoma group and 36%, 51% in soft tissue sarcoma group, which were not stastistically significant(p=0.2, p=0.4). Conclusion: Extraskeletal osteosarcoma have similar local recurrence, metastasis and survival rate compare to high grade soft tissue sarcoma. The number of patients of this study were too small to identify outcome of extraskeletal osteosarcoma. Further multi-institutional study should be attempted.

Treatment of soft Tissue Sarcoma by General Orthopedic Surgeon Rather than Orthopedic Oncologist (골연부조직 종양 전공자가 아닌 일반 정형외과 전문의에 의해 치료된 연부조직육종)

  • Purpose: To investigate the quality of training hospital based treatment, we evaluated the soft tissue sarcoma treatments afforded by general orthopedic surgeon rather than orthopedic oncologist. Materials and Methods: We reviewed the details of 25 patients with pathologically confirmed soft tissue sarcoma who registered in our hospital between July 1997 and 2006 September. We evaluated initial diagnoses, the surgical treatment (including adjuvant therapy) and the follow up method used and related these to the principles of soft tissue sarcoma treatment. Results: The study cohort comprised 16 men and 9 women of mean age of 50.2 years. A diagnostic biopsy was performed in 9(36%) cases before definitive surgical treatment. Wide excision was performed in 13(52%) cases. For the 12 cases in which the grade of sarcoma was estimated, adequate surgical treatment with adjuvant therapy was performed only in 4(33.3%) cases. In addition, an adequate follow up schedule was adopted in only 4(16%) of the 25 study subjects. Conclusion: Unexpectedly, many cases of soft tissue sarcoma were treated inadequately even in a training hospital. An intensive education program on the treatment of soft tissue sarcoma is necessary for all orthopedic surgeons.

  • Leiomyosarcoma is an uncommon soft tissue sarcoma of mesenchymal cell origin, which shows smooth muscle differentiation. Leiomyosarcoma is seldom found in the pediatric population, and accounts for fewer than 2% of all soft tissue sarcomas. Leiomyosarcoma of the chest wall is extremely rare in children. We report here a case of an 8-year-old boy with a primary leiomyosarcoma that was incidentally found as a rib mass. The patient underwent a complete resection for a suspected osteochondroma diagnosed by a three-dimensional chest computed tomography examination. Pathological findings of the mass revealed intersecting fascicles of spindle cells showing cigar-shaped nuclei, inconspicuous nuclear pleomorphism and occasional mitotic figures in the background of a suspected osteochondroma of the rib. This report documents the first description of a leiomyosarcoma possibly arising in an osteochondroma of the rib in a child.

Prognostic Significance of $O^6$-MGMT and Promotor Hypermethylation in Patients with Soft Tissue Sarcomas (연부조직육종 환자에서 $O^6$-MGMT 와 촉진자 과메틸화의 예후적 중요성)

  • Purpose: The DNA repair protein, $O^6$-methylguanine-DNA methyltransferase (MGMT), removes alkyl adducts from the $O^6$ position of guanine. Epigenetic inactivation of MGMT has been found in human neoplasia and considered one of the implicated factors in chemoresistance. Materials and Methods: Sixty-two patiensts with soft tissue sarcomas (STS) were analyzed for the status of MGMT protein expression by immunohistochemistry and the promoter hypermethylation of the MGMT gene using methylation-specific PCR. Result: The loss of MGMT expression was found in 20 cases (32.3%) of total 62 STS. MGMT promoter hypermethylation rate was 25.0% (11/44 cases). The loss of MGMT expression showed significant association with high AJCC stage, high FNCLCC grade, and aggressive behavior. However,when the group who received chemotherapy was analyzed (n=27), loss of MGMT expression was correlated with worse survival in multivariate analysis (p=0.024). MGMT promoter hypermethylation is associated with high FNCLCC grade. MGMT promoter hypermethylation status had a strong correlation with loss of MGMT expression (p=0.000). Conclusion: Our results suggest that MGMT promoter hypermethylation and loss of MGMT expression had a tendency to be associated with poor prognosis and that loss of MGMT protein expression is frequently occurs via MGMT promoter hypermethylation.

  • A dosimetric evaluation of volumetric modulated arc therapy, intensity modulated radiation therapy, and three-dimensional conformal radiation therapy for the lower extremity soft tissue sarcoma For the lower extremity soft tissue sarcoma, volumetric modulated arc therapy, intensity modulated radiation therapy, and three-dimensional conformal radiation therapy were evaluated to compare these three treatment planning technique. The mean doses to the planning target volume and the femur were calculated to evaluate target coverage and the risk of bone fracture during radiation therapy. Volumetric modulated arc therapy can reduce the dose to the femur without compromising target coverage and reduce the treatment time compared with intensity modulated radiation therapy.

  • Soft tissue sarcomas of popliteal fossa are rare, accounting for less than 5% of all soft tissue sarcomas of the extremities. In an extracompartmental space such as the popliteal, cubital fossa and inguinal space, where major vessels and nerves traverse, performing resections with wide margin is difficult and sometimes marginal margin is inevitable for limb salvage. For popliteal tumor resection, posterior approach would be a classic method. For tumors with small size and not adherent to surrounding structures, tumor is easily resected by this approach and dissection of nerve sheath or adventitia of vessel. On the contrary, tumors of large size and infiltrating the posterior structure of knee joint may show difficulty in en-bloc resection itself. These cases were candidates for amputation. Furthermore, tumors involving both popliteal fossa and anterior compartment usually had no choice but to have an amputation to prevent local recurrence. We regarded soft tissue sarcoma showing this kind of presentation as bone tumor having extraosseous mass. We performed wide en-bloc resection of proximal tibia and fibula along with sarcoma involving both compartment on liposarcoma of 47-year old man.

Synovial Sarcoma Misunderstanding as a Benign Soft Tissue Tumor Finding on Ultrasonography (초음파에서 양성 연부조직 종양으로 오인된 활막육종)

  • Ultrasonography is an effective first-line examination for soft-tissue masses. We reported a case who presented with superficial soft-tissue mass on the left distal thigh accompanying by pain. On ultrasonography, the lesion was determined as a benign mass. The patient, however, was diagnosed with synovial sarcoma based on the histopathologic result. Herein, we reported the case and reviewed of relevant literature.

A Study on Dose Sensitivity according to Rice Cultivar in Extremities Radiation Therapy with Rice Bolus (사지의 방사선치료에 사용되는 쌀 볼루스의 품종에 따른 방사선량분포의 민감도 연구)

  • To investigate the radiation dose sensitivity in extremity radiatioin therapy depending on rice cultivar which have different size and shape of grains, plan results are compared that used rice bolus Korean and Thai rice. Phantoms that are each no bolus, Korean rice bolus, Thai rice bolus were used and prescribed 100 cGy to isocenter and checked the point dose of 12 points of interest of each phantoms. The meane dose are 103.57±1.98 cGy in Thai rice bolus using, 104.27±2.12 cGy in Korean rice bolus and 104.99±6.40 cGy in phantom without bolus. Dose distribution of Thai and Korean rice bolus differed significantly in Wilcoxon's Signed Rank test (p=.011). It has been confirmed that that the bolus using Thai rice, which has a small grain size, shows a more even dose distribution.

Medical Imaging of Well-differentiated Liposarcoma Arising from the Falciform Fat in a Dog (개에서 겸상인대에서 유래된 분화지방육종의 영상의학증례)

  • A 7-year-old castrated male Cocker spaniel dog was referred to Gyeongsang National University Veterinary Medical Teaching Hospital with a severe abdominal distension. A huge, inhomogeneous fat opacity mass was identified on the abdomen radiographic survey. From the abdominal ultrasonographic examination, the mass was found to have irregular marginated hyperechoic capsule with cellular hypoechoic fluids inside. On the computed tomography, an encapsulated inhomogeneous mass was identified with fat attenuation peripheral and soft tissue attenuation inside. The origin of the mass was confirmed as falciform fat by surgical excision. The resected mass appeared to be a firm reddish-brown color with a smooth surface. The histopathological appearance was a well-differentiated liporsarcoma. The patient has no recurrent signs 1 month after surgery.
