Chondrosarcoma is one of the most common types of primary bone sarcoma. With the exception of the mesenchymal subtype, chondrosarcomas are usually low-grade lesions and rarely show multicentricity or distant metastasis. Only rare cases of multicentric chondrosarcomas have been reported in association with Ollier's disease and Maffucci's syndrome. To our knowledge, no report has been issued of a synchronous multicentric chondrosarcoma occurrence across a joint. We experienced a 30-year-old man with a synchronous monomelic juxta-articular multicentric chondrosarcoma across a shoulder joint in the absence of pulmonary and visceral metastases. He was treated by curettage and cement filling with allograft in the acromion and wedge resection with cement filling in the proximal humerus. At the 18-month follow-up, there was no evidence of recurrence, and the patient had full range of motion without pain.

Either absorbable or nonabsorbable sutures can be used in meniscal repair. Some surgeons prefer nonabsorbable sutures because they believe that nonabsorbable sutures improve meniscal healing as compared with absorbable sutures. But, permanent existence of sutures in the lesion could make problems. We report a case of Inside-Out meniscus repair that nonabsorbable sutures came out of the joint capsule tearing the substance of the meniscus.

Purpose: We report mid to long - term results of meniscal transplantation and evaluate the important factors for successful outcomes. Materials and Methods: Between December 1999 and September 2002, 25 meniscal transplantations were performed using fresh frozen allograft. The lateral meniscus was transplanted in 19 cases and medial meniscus in 6 cases. The mean age was 34 years (range, 17~50 years) and the mean follow up was 54.8 months (range, 6~85 months). Preoperative measurements were made using a ruler graded in millimeters. Lateral meniscus was fixed by keyhole technique and medial meniscus was fixed by double bone plug technique with suturing the periphery of the meniscal transplant. All patients were evaluated with Knee Assessment Scoring System (KASS), Lysholm knee score, and Tegner activity scale for daily activity. Results: Symptoms improved in all cases. The average KASS score increased from 61.7 preoperatively to 83.8 postoperatively. The average Lysholm knee score increased from 77.7 preoperatively to 87.7 postoperatively (excellent in 3 cases, good in 17 cases, fair 4 cases, poor 1 case). But painful swellings were 3 cases, numbness in 1 case, and granuloma due to non-absorbable suture material in 1 case. Peroneal nerve palsy in 1 case was recovered after 6 weeks postoperatively. Conclusion: Meniscal allograft transplantation after subtotal or total menisectomy can significantly relieve pain and improve function of the knee joint. The exact preoperative sizing and secure fixation are essential for successful outcomes.

Purpose: We analyzed the oncological outcome and prognostic factor of the chondrosarcoma arising from benign bone tumor due to malignant transformation. Materials and Methods: From April 1986 to April 2009, 18 cases were considered eligible. We analyzed retrospectively the patient's characteristics and prognostic factors that affect to the local recurrence and distant metastasis. Results: As classified by primary benign bone tumor, 4 cases were solitary osteochondroma, 11 cases were multiple osteochondromatosis and 3 cases were multiple enchondromatosis. The mean follow-up period was 85 months. The 5-year disease free survival rate of 18 patients was 85.9%. Their overall MSTS score was 25.2 (84%). There were local recurrence in 3 cases and no distant metastasis. We found that tumor location and surgical margin affected to the prognosis significantly. Conclusion: In secondary chondrosarcoma patients, the prognosis was good relatively and tumor location and surgical margin are important prognosis factor.

Purpose: To evaluate the results of meniscal repair and partial meniscectomy of lateral meniscus injury associated with tibial plateau fracture. Materials and Methods: Between February 1993 and August 2004, 24 cases (23 patients) of lateral meniscus tear with tibial plateau fracture were evaluated retrospectively. The most frequent type of tear was the longitudinal tear of the meniscocapsular junction (14 cases, 66.7%). All fractures were reduced under the control of arthroscopic and image intensifier. Arthroscopic repair of the longitudinal tear of the lateral meniscus or arthroscopic partial meniscectomy was performed. The final results were evaluated with the Ikeuchi criteria and Lysholm score. Results: At the final follow-up, the outcome was excellent in 12 cases (85.7%), good in 1 case (7.1%) and fair in 1 case (7.1%) among 14 cases of meniscal repair, and the excellent in 4 cases (66.7%) and good in 2 cases (33.3%) among 6 cases of partial meniscectomy according to the Ikeuchi criteria, There was a significant improvement of Lysholm score after surgery, 92.3 postoperatively compared with 56.6 preoperatively (p<0.0001, paired t-test). Conclusion: The fracture of the lateral tibial plateau did not seem to affect on the healing of the meniscus repair and partial meniscectomy.

Neurilemmoma is a benign tumor which arises generally from the Schwann's cells. It is a slowly growing tumor and treated as surgically. We experienced a case of neurilemmoma of the oropharynx in a 28-year-old female who had noticed a gradual swelling of the left side of the oropharynx with swallowing difficulty for 6 months. The tumor was removed successfully intra-orally under the general endotracheal anesthesia and verified pathologically as neurilemmoma.

Extraskeletal osteochondroma, a variant of chondroma, typically arises in the para-articular location of hands and feet. It is a rare disease and is particularly uncommon when joint components are not involved or localized away from joints. Herein, we report a case of extraskeletal osteochondroma in the posterior neck of a 66-year-old female. The characteristic radiologic finding of our case is presented, along with the typical findings of the disease and review of related literature reports.

The products of c-fos and c-jun proto-oncogenes form the heterodimeric complex activator protein 1 (AP-1), which plays an important part in the control of bone cell proliferation and differentiation, as well as in the development of bone tumors. The expression of c-fos protein was examined in 35 cases of human osteosarcomas as formalin-fixed paraffin-embedded tissue sections using a monoclonal antibody. The expression of c-fos was restricted to bone-forming lesions, while low grade cartilaginous tumors were devoid of immunoreactivity. The highest levels of c-fos expression were detected in osteoblastic osteosarcoma (13 of 17 cases with grade one on two) while two chondroblastic osteosarcomas, one fibroblastic osteosarcoma, and two parosteal osteosarcomas were negative. Two cases of telangiectatic osteosarcomas were positive for c-fos protein. However, since there is a tendency of high c-fos protein expression at the higher histological grade, significant differences were not present in the expression of c-fos protein. Thus c-fos expression may be implicated in the development of osteosarcomas, but they appear to have little or no relevance in the development of low grade cartilaginous neoplasms.

Malignant transformation of a solitary osteochondroma is rare, and usually takes form of a chondrosarcoma. We present a case of low grade osteosarcoma arising in a solitary osteochondroma of the right femur in a 30-year-old woman. As the lesion was initially continuous with corresponding components of the parent bone, so the possibilities of other diagnoses were excluded. After the initial excision, there were 3 times of recurrences during the follow up period of 3 years. The histologic subtypes of the recurred osteosarcomas were different each other, which were high grade chondroblastic, osteoblastic and fibroblastic respectively.
