Effects of Shoulder Rehabilitation Exercise Convergence Program on Pain, Range of Motion and Exercise Self-efficacy After Shoulder Surgery (어깨재활운동 융합프로그램이 어깨수술 후 통증, 관절운동범위 및 운동자기효능감에 미치는 효과)

  • The purpose of this study was to examine the effects of shoulder rehabilitation exercise convergence program. This study used a nonequivalent control group pretest-posttest design. A sample of 58 patients undergoing shoulder surgery, was included. The experimental group was given shoulder rehabilitation exercise convergence program including audiovisual education, shoulder rehabilitation exercise and exercise monitoring. The data were collected using a structured questionnaire and measured range of motion(ROM) of shoulder joint and were analyzed using descriptive statistics, ${\chi}^2-test$, t-test and ANCOVA. The experimental group reported significantly lower shoulder pain than control group(p<.001). Shoulder joint ROM and exercise self-efficacy were significantly higher in the experimental group than in the control group(p=.038; p<.001). This findings indicate that shoulder rehabilitation exercise convergence program can be an effective intervention for shoulder surgery patients. Continued development of convergence interventions for patients undergoing various conditions in practice, is suggested.

Stabilization of Recurrent Medial Shoulder Luxation by Use of Three Bone Anchors in a Dog (재발성 내측 어깨 탈구를 보이는 개에서 본 앵커를 이용한 외과적 치료 증례)

  • A 12-year-old male poodle weighing 2.0 kg presented for evaluation of right thoracic limb lameness. The owner reported that the dog showed recurrent bilateral shoulder joint luxation after a car accident for a year. Recently, the left shoulder joint appeared well maintained, but right shoulder joint luxation was exacerbated. On physical examination, the dog showed non-weight bearing lameness on the right thoracic limb. Craniocaudal radiographic views revealed medial displacement of the right humerus. Mediolateral radiographic views revealed overlap of the glenoid cavity and humeral head. Muscle atrophy of the right thoracic limb, reduced biceps brachii muscle tendon tone, a tear of the medial glenohumeral ligament, and a rupture of the subscapularis tendon were identified intraoperatively. Transposition of the biceps muscle tendon was performed. However, at 7 days, there was evidence of right shoulder reluxation on radiographs. The second surgery was performed with two $2mm{\times}6mm$ cortical bone anchors and a $4mm{\times}6mm$ cancellous bone anchor placed in the cortical bone of the distal scapula and the cancellous bone of the proximal humerus respectively. Two scapular bone anchors were then connected with a humeral bone anchor using heavy nylon suture to minimize shoulder abduction range of motion. On radiographs right after surgery and 6 weeks after surgery, the affected limb revealed no evidence of medial shoulder luxation. At 6 months, no evidence of lameness was noted on the right thoracic limb.

Effect of surgical site instillation after laparoscopic urologic surgery (비뇨기과 복강경 수술 후 국소마취제 점적의 효과)

  • The aim of this study was to evaluate the effect of Ropivacaine on pain following laparoscopic surgery. Between March 2015 and June 2017, all 97 patients with renal mass who elected to receive laparoscopic nephrectomy were reviewed, retrospectively. Normal saline (0.9%) was used as a control in 45 patients while Ropivacaine was used for local anesthesia in the instillation group (n=52). Pain score (visual analog scale) of the shoulder and upper abdominal area between the two groups was significantly different between recovery time and 24 h (p<0.05), with no significant differences between 48 and 72 h (both p>0.05). In the instillation group, pain score of shoulder and abdominal pain in the two groups according to the amount of CO2 used was not significantly different between recovery time and 24, 48, and 72 h (all p>0.05). Ropivacaine reduced shoulder and upper abdominal pain that occurred within 24 h after surgery. Effective pain control with Ropivacaine is needed in the early postoperative period when a large amount of CO2 is used in laparoscopic surgery.

The Effect of Tai-Chi Exercise on the Function of Shoulder Joint, Heart Rate in Breast Cancer Patients (타이치(Tai-Chi) 운동이 유방암 수술환자의 어깨관절 가동범위와 심박동수 변화에 미치는 영향)

  • Good health-related quality of life is regarded as the goal of elderly residential care. Treatment for breast cancer produces side effects that diminish functional capacity and quality of life among survivors. Tai-Chi exercise offers the benefits of fitness and health. Tai-chi has been claimed to generate beneficial effects with respect to a wide range of diseases. The object of this research is to evaluate the effects of tai chi exercise on shoulder joint movement and cardiovascular function in elderly breast cancer patients. The subjects were 40 patients residing in B metropolitan city, who had undergone surgery for breast cancer within 2 months. They underwent post-operative radiotherapy and an anti-cancer chemical treatment. They practiced tai-chi exercise for 60 min a day, four times a week for 12 weeks under the direction of a special tai-chi instructor. Shoulder joint movement and heart rate, were measured before and after the experiment. The results were as follows; Shoulder joint movement, including flexion, extention, adduction and abduction, significantly increased (p<0.05) after treatment, and heart rates significantly decreased after treatment (p<0.05). From the results, Tai-Chi exercise was shown to be extremely effective in improving shoulder joint movement and heart rate in elderly breast cancer patients. Further study is needed in this area.

Arm Morbidity after Breast Cancer Treatments and Analysis of Related Factors (유방암 환자의 상지 부작용과 관련 요인)

  • Purpose : To evaluate the incidence of arm morbidity following breast cancer surgery including axillary dissection and to identify related factors. Materials and Methods : One hundred and fifty nine patients were studied using a self-report questionnaire and a clinical examination. Lymphedema, reduction of range of motion in shoulder joint and subjective symptoms (pain, impaired arm movement, numbness, stiffness) were evaluated. As related factors, demographic, oncologic characteristics and types of treatment were analysed. Results : The incidence of lymphedema ($\geqq$2 cm difference comparing to unaffected arm) was $6.3\%$, $10.7\%$, $22.5\%$ and $23.3\%$ at each 10 cm, 20 cm, 30 cm, and 40 cm from wrist. Reduction of range of motion in shoulder joint ($\geqq$ 20 degree difference comparing to unaffected arm) was noted In more than 1/3 patients for flexion, abduction and internal rotation. Especially the reduction of range of motion in internal rotation was severe ($>50\%$ reduction) in 1/3 patients. Approximately 50 to $60\%$ of patients complained impaired arm movement, numbness, stiffness and pain. Body mass index (BMI) was the significant risk factor for lymphedema. Conclusion : Lymphedema was present in 1/3 of patients and the common sites of edema were 30 cm 40 cm proximal from the wrist. Also most severe reduction of range of motion in shoulder joint was with internal rotation. There needs weight control for lymphedema because BMI was the significant risk factor for lymphedema. Also rehabilitation program for range of motion especially internal rotation In shoulder joint should be developed.

  • Recent advances in technology of medical image have made surgical simulation that is helpful to diagnosis, operation plan, or education. Improving and enhancing the medical imaging have led to the availability of high definition images and three-dimensional (3D) visualization, it allows a better understanding in the surgical and educational field. The Real human field of view is stereoscopic. Therefore, with just 2D images, stereoscopic reconstruction process through the surgeon's head, is necessary. To reduce these process, 3D images have been used. 3D images enhanced 3D visualization, it provides significantly shorter time for surgeon for judgment in complex situations. Based on 3D image data set, virtual medical simulations, such as virtual endoscopy, surgical planning, and real-time interaction, have become possible. This article describes principles and recent applications of newer imaging techniques and special attention is directed towards medical 3D reconstruction techniques. Recent advances in technology of CT, MR and other imaging modalities has resulted in exciting new solutions and possibilities of shoulder imaging. Especially, three-dimensional (3D) images derived from medical devices provides advanced information. This presentation describes the principles and potential applications of 3D imaging techniques, simulation and printing in shoulder and elbow practice.

Exploratory Arthroscopic Treatment to Diagnose and Treat Osteochondritis Dissecans of the Shoulder Joint in a Dog (개의 어깨 관절에서 박리성 골연골염의 진단과 치료를 위한 탐색적 관절내시경의 적용)

  • A 6-month-old, intact male Great Pyrenees (35 kg) was referred with 2 weeks continuous left forelimb lameness to the Chonbuk Animal Medical Center, College of Veterinary medicine, Chonbuk National University. The lameness became worse three days before being referred to the hospital. Upon the physical examination, the patient had pain when the left shoulder joint was palpated, and the lameness was visible in the left forelimb during ambulation on gait examination. There were no remarkable findings on radiological and neurological examination. Osteochondritis dissecans (OCD) was suspected based on medical history and gait tests. As a definitive diagnosis could not be made, exploratory arthroscopic surgery was performed to examine the inside of the shoulder joint. During the operation, mild bicipital tenosynovitis, synovitis and OCD which was located on the caudal medial area of humeral head were revealed. Arthroscopic procedures were used for the treating OCD, including the removal of the OCD flap and debriding of the subchondral defect until hemorrhaging by use of an electrical burr. The patient was discharged a day after surgery. After 2 weeks, the patient again presented at the hospital due to complications, including inflammation of the surgical lesion because of licking and seroma within the subcutaneous tissue. Antibiotics were administered and an aseptic bandage was applied. And simple surgical operations were performed for the removal of the cyst and seroma. Eleven weeks following arthroscopy, the lameness was completely resolved. Arthroscopy has the advantage of allowing gross examination inside the joint capsule. Due to this advantage, arthroscopy is one of the best advanced options for diagnosis in dogs with undiagnosed joint pain.

Physical Function and Fatigue in Mastectomy Patients (유방절제술 환자의 신체기능과 피로)

  • Purpose : The purpose of this study was to provide information for developing a rehabilitation intervention that improves adaptation and the quality of life after mastectomy by investigating the level of physical function and fatigue in mastectomy patients. Methods : The subjects were 63 patients selected from St. Mary's Kangnam hospital Data were collected from March to June, 2003. Physical function was measured by the range of motion of the shoulder joint, shoulder function, and physical symptoms. fatigue was measured by using the Brief Fatigue Inventory (BFI). Results : The range of motion on the affected side was significantly lower than that in the healthy side. Shoulder function score was highest in the item of 'pull on pants', and lowest in the item of 'back zipper'. Physical symptom score was highest in the item of 'numbness', and lowest in the item of 'Itching sensation'. The highest item interfered by fatigue was 'normal work (includes both work outside the home and daily chores)'. Conclusion : The result of this study suggests that effective rehabilitation intervention for mastectomy patients should be needed to improve physical function, and reduce fatigue.

Effectiveness of Individualized Upper Extremity Exercise Program for Patients after Mastectomy (유방절제술 후 환자를 위한 맞춤형 상지 운동프로그램 효과)

  • This study was conducted to develop an individualized upper extremity exercise program for patients after mastectomy and evaluate its effectiveness. A nonequivalent control-group pretest-posttest design was used for this investigation. Research participants were patients with breast cancer who underwent mastectomy at two university hospitals (experimental group, n=17; control group, n=16). The experimental group received an individualized upper extremity exercise program three times a week for 7 weeks. Data were analyzed by the ${\chi}^2-test$, Fisher's exact test, Mann-Whitney U-test and Wilcoxon Signed Rank test using SPSS WIN 21.0. Inter-group comparison revealed significant differences between pre-test and post-test in hands behind the back, flexion, abduction, external rotation, internal rotation, and grip strength. Moreover, the experimental group showed significant improvements in hands behind the back, flexion, abduction, external rotation, internal rotation, and grip strength relative to the control. In conclusion, the individualized upper extremity exercise program was shown to be an effective intervention to improve physical functions in upper limbs and shoulders. Therefore, this program should be applied to patients after mastectomy in clinics since many patients have suffered from decreased physical functions in upper extremities.

A Study on Musculoskeletal Symptom and Pain related to Working Posture of Operation-Room Workers. (수술실 근무자의 업무수행 관련 근골격계 부담정도와 통증에 대한 연구)

  • Purpose : The purpose of this study was to identify musculoskeletal symptom and pain related working posture with operation-room workers. Methods: The subjects of this study were 73 people who were working from hospital operating-room. A questionnaire consisting of general characteristics, pain symptoms, and pain frequency was given. Objective working posture analysis was done by using REBA after recording moving images in hospital operating-rooms. Results: The result were as follows. The risk level of musculoskeletal symptom related working posture was high, and the management level was at the least 2; 'some management was required for musculoskeletal symptom'. The frequency of pain was 3~4 times/week and the severe pain were experienced from legs, neck, and shoulder. The result of relation of general characteristics and pain were significant variable with sex, age, marital status, weight, past working history, average working time(hr/day), working style, and working department. Conclusion: These results contribute to a better understanding of operation-room workers have high possibility of musculoskeletal disease and are severe musculoskeltal pain related working posture.
