We experienced a case of hemoptysis caused by bilateral upper lobe aspergilloma. He was 66 years old and had a history of old unclear pulmonary tuberculosis with irregular medication 20 years ago and intermittent hemoptysis for several years. In x-ray study, there was a bilateral upper lobe aspergilloma with cavity. He received bilateral wedge resection through thoracotomy with some interval to reduce postoperative complications. We selected the priorty of operation through the bronchoscope in the operation room. Both sides had the same pathology of aspergilloma and he was discharged after an uneventful postoperative course.

This is a case report of surgical management of a bilateral intralobar pulmonary sequestration with horseshoe lung presenting with frequent U I with productive sputum. Simple chest X-ray showed pneumonic consolidation and infiltration on both lower lobes, and chest CT revealed multiple cystic lesions compatible with pulmonary sequestration. The aortography demonstrated two anomalous systemic arteries arising from the thoracic aorta just above the diaphragm to both sequestrums. Left lower lobectomy was performed through the left thoracotomy with ligations and divisions of the both systemic feeding arteries to the left and right sequestrum, and division of the isthmic portion of horseshoe lung without removal of right sequestrum. The patient was discharged on the postoperative loth day and followed-up till now without any sequelae and symptoms of residual right sequestration. The recent follow-up chest CT 5 months after the operation revealed spontaneous regression of the residual right sequestrum. Authors would suggested that only division of aberrant artery to sequestrum without lobectomy may be applied in uncomplicated ca e of intrapulmonary seqilestration.

Objectives : To report the patient bilateral facial palsy, because it is unusual. Methods : One patient with bilateral facial palsy was selected. Herbal medicine(Cheunggansoyosan, bubidongyeom-2) was administered and acupuncture was applied for twice a day in 73 days. Facial function was evaluated by House-Brackmann scale, Yanagihara's scale and Standardization of muscular paralysis. Results : Cheunggansoyosan, bubidongyeom-2 and acupuncture treatment improved facial function after 73 days of treatment. Adverse effects were not reported. Conclusions : This study shows that the Korean medicine treatment was effective in improving bilateral facial palsy.

Wilms' tumor, aniridia, genitourinary anomalies, and mental retardation (WAGR) syndrome is caused by deletion of chromosome 11p13, including the Wilms' tumor (WT1) and aniridia gene (PAX6) loci. Here, we report the first case of WAGR syndrome in Korea; the patient was a 2-year-old girl with bilateral aniridia from birth who presented with abdominal distention and mental retardation. Cytogenetically, she had deletion of chromosome 11p11.2-13. Bilateral Wilms' tumors were successfully treated by chemotherapy and surgery. She has been tumor-free for 19 months off chemotherapy with preserved renal function.

Atherosclerosis has more than 60% of the causes of arterial occlusive diseases. The abdominal aorta and lower extremity arteries are the most common sites of occlusion. We have treated surgically 2 cases who had intermittent claudication and were diagnosed as simultaneous aortobifemoral and bilateral femoropopliteal obstruction by angiography, but had ineffective results from medical treatment or angioplasty. Simultaneously aortobifemoral bypass using Hemashield Y graft and bilateral femoropopliteal bypass using autologous greater saphenous vein were done. After operations, the symptom disappeared and there were no specific post-operative complications except abdominal wound dehiscence. In postoperative angiography, we had obtained good patency of bypass graft. We are following up patients through the out patient department without recurrence up to 16 months.

An orchiopexy was performed in an 18-month old adult English bulldog with bilateral cryptorchidism. One month postoperatively, the dog was twice-treated with GnRH (50 ug/kg) at an interval of 2 weeks. Semen was collected and evaluated before and after surgery. Fertility was determined by artificial insemination. No spermatozoa were observed before orchiopexy and 2 months postoperatively. However, 6 live sperm were detected 4 months postoperatively and normal sperm characteristics (except sperm concentration) were present 7 months postoperatively. A female bulldog, inseminated with the semen from the bulldog 8 months postoperatively, delivered 6 offsprings. Spermatogenesis and spermatozoa with fertilizing capacity were recovered by postpubertal orchiopexy and GnRH therapy in a bilateral cryptorchid dog.

A study was performed comparing dosimetric characteristics of volumetric modulated arc and intensity modulated radiatio therapy on patients with bilateral breast cancer. For 5 patients, 3 plans were made for each patient; IMRT beams 8 and 12 of the beam intensity modulated radiation therapy, volumetric modulated arc therapy plan. The average PTVs volumes and $D_{98}$ for 12-IMRT were $51.04{\pm}0.57$ Gy (right), $50.80{\pm}1.07$ Gy (left), $42.94{\pm}16.16$ Gy (right), $42.56{\pm}2.09$ Gy (left). HI ($D_5{\sim}D_{95}$) and $CI_{90,95}$, even 12-IMRT has shown excellent results. In OAR, 3 plans showed excellent results. But the lowest dose of 12-IMRT. 12-IMRT achieved similar PTV coverage and sparing of organs at risk than 8-IMRT and VMAT.

Our patient is the first reported case of Prader-Willi syndrome(PWS) with bilateral Hutch's diverticula. We believe that hypotonia in PWS is associated with multiple bladder diverticula and vesicoureteral reflux. We conclude that PWS can be considered a possible associated syndrome in patients with multiple bladder diverticula.

Ectopic eruption of the first permanent molar means the first permanent molar assumes an atypical path of eruption resulting in premature atypical resorption of the second primary molar. If the reversible eruption does not occur, early loss of the second primary molars results in space loss, mesial tipping of the first permanent molar, impaction of the second premolar, buccal segment crowding and overeruption of opposing tooth. The main objectives of treatment are (1) to prevent loss of the second deciduous molars so it can continue to serve as a space maintainer and (2) to regain lost arch length, allowing the second premolar to erupt into normal position. The optimal treatment approach depends on a number of factors including the clinical eruption status of /6/, the change in position of /6/, the amount of enamel ledge of /E/ entrapping /6/, the mobility of /E/, and the presence of pain or infection. Unilateral appliance to correct the mesial angulation of ectopic permanent first molars, as in the majority of the appliance designs, would produce a resultant force that would further enhance the space loss. A bilateral support similar to the holding arch design is recommended to maximize the anchorage. These case reports present the successful result of preserving space for the second premolar in treatment of ectopic eruption of the first permanent molar using Halterman appliance with bilateral anchorage on patients visiting department of pediatric dentistry in Samsung Medical Center.
