Two Cases of Night Fever Treated with Acupuncture on Young-gol and Dae-back: Considered from Meridian and Collateral Aspects (영골·대백혈 자침을 통한 야간 발열 환자 치험 2례)

  • Objectives : The purpose of this study is to study the effects of acupuncture on Young-gol and Dae-back on night fever and investigate the meridian and collateral aspects of Dong-Si acupuncture through cases of night fever due to Yin deficiency. Methods : Patients have received acupuncture on both sides(Young-gol and Dae-back) at 10 pm. The acupuncture treatment lasted for 15 minutes. Body temperature, pulse, and Numeric Rating Scale(NRS) scores for dryness of mouth were measured for the evaluation. Results : Both patients reported symptomatic reduction including night fever, pulse rate, NRS about dry mouth following the described treatments. The night fever disappeared after acupuncture treatment. The mean of pulse rate changed from 82.6 to 74.8(case 1), from 83.5 to 74.3(case 2). The mean of NRS changed from 4.2 to 1.7(case 1), from 3.3 to 2.4(case 2). Conclusions : In both cases, acupuncture was effective in relieving night fever symptoms. This can be considered from meridian and collateral aspects and the Korean medicine physiological perspective. In the future, systematic and diverse studies related to this will be needed.

  • 본 구명조끼는 해양 사고시 구조되기까지의 시간을 단축시키며 구조되기까지의 생존율을 높여 주기 위해 연구 하였다. 본 구명조끼는 구명조끼와 LED를 결합하여 야간 사고 시 지속적인 구조요청을 가능하게 한다. 또한 구명조끼 내부에 발열체를 내장 시킴 으로서 해양사고 시 사망원인의 1순위인 저체온증이 오기까지의 시간을 연장시켜 구조되기까지의 생존율을 높여 준다.

A Case Report of Systemic Type Juvenile Idiopathic Arthritis with Night Fever (야간 발열을 동반하는 전신형(Systemic type) 소아기 류마티스 관절염(Juvenile Idiopathic Arthritis; JIA) 환아 증례보고)

  • Objectives: This study is to report a case that has an effective result to systemic type juvenile idiopathic arthritis patient with an oriental medicine treatment. Methods: We treated the patient with an oriental medicine and tapered down with the western medicine. We followed up the laboratory blood tests every two or three months and through telephone at least six days in a week. Results: The symptoms of systemic type juvenile idiopathic arthritis were vanished and the patient maintains his condition with oriental medicine treatment after discontinued all Disease-Modifying Antirheumatic Drugs(DMARDs). His ESR, CRP levels and other blood test results were back to normal range. Now he is in clinical remission status. Conclusions: According to the result, the oriental medicine treatment is considered to be effective on the systemic juvenile idiopathic arthritis and further studies will be needed with more cases.

Clinical Investigation of 11 Cases of Chronic Eosinophilic Pneumonia Reported in Korea (국내에서 보고된 만성 호산구성 폐렴 11예에 대한 임상적 고찰)

  • Background: Chronic eosinophilic pneumonia(CEP) presents with profound systemic symptoms, including fever, malaise, night sweats, weight loss, and anorexia together with localized pulmonary manifestations such as cough, wheeze, and sputum. It is an illness occurring predominantly in women. The chest radiogragh shows fluffy opacities that often have a characteristic peripheral configuration. The hallmark of CEP is the peripheral blood eosinophilia and a prompt response to oral corticosteroid therapy. We investigated characteristics of eleven patients of chronic eosinophilic pneumonia, reported in Korea. Method: There were eleven reports of CEP from 1980 to 1996, including three cases experienced in our hospital. The journals were analysed in respects of clinical history, laboratory, and radiographic findings. Results: 1) Male vs. female ratio is 3 : 8. The peak incidence occurred in forty and fifty decades. The atopic diseases were present in 6 cases. Asthma was the commonest manifestation 2) The presenting symptoms were as follows: cough, dyspnea, sputum, weight loss, fever, general weakness, night sweats, urticaria with the descending incidence. 3) Peripheral blood eosinophilia was present in all patients(mean ; 38.4%) and serum IgE level was elevated in nine patients(mean ; 880IU/ml). Conclusion: The diagnosis of chronic eosinophilic pneumonia is based on classic symptoms, including fever, night sweats, weight loss with a typical roentgenogram of peripheral pulmonary infiltrates and peripheral blood eosinophilia, and that is confimed by lung biopsy and/or bronchoalveolar lavage. Chronic eosinophilic pneumonia is responsive to corticosteroid promptly and recommended at least 6 months of therapy to prevent relapse.

Malignancies in children who present with bone pain (뼈의 통증이 있을 때 감별해야 할 악성종양)

  • Bone pain in a child could be associated with cancer as an initial manifestation of the disease. The childhood malignancies that frequently present bone pain are leukemia, neuroblastoma, and primary bone tumors such as osteosarcoma and Ewing sarcoma. Persistent bone or joint pain associated with swelling, mass, or limitation of motion implies underlying serious causes. Systemic manifestations such as lymphadenopathy, hepatosplenomegaly, fever, fatigue, night sweat, and laboratory abnormalities are also suggestive of malignancy. The index of suspicion tends to be low since less than 1% of children who complain of bone pain are diagnosed as cancer. Nonetheless, pediatricians should be alert to the possibilities of cancer since early detection and prompt treatment might reduce mortality.

  • In comparison with some other light sources, LED has merits such as long lifetime, pollution free, and high energy efficiency. Lately, due to development of LED with high brightness and capacity, LED, which has been applied in display system only, has applied in the field of lighting system. As power LED for lighting system can be burned out by heat problem, the driving current of power LED has to be controlled below the designed value. In this paper, power LED photovoltaic lighting system, which has the current limitting function, has been described. After photovoltaic power is generated from PV panel. it is charged into a battery. And then, after the charged power is converted to DC24[V] through a boost DC-DC converter, it is supplied to power LED at night. It has been validated by designing and testing of 72[W] power LED lighting system, which includes a PV charger, a boost DC-DC converter and a current limiter for driving power LED.

A Study on the Heating and Cooling Energy Load Analysis of the KNU Plant Factory (KNU 식물공장의 냉난방 에너지 부하 해석에 관한 연구)

  • The heating and cooling energy load of the KNU plant factory was analyzed using the DesignBuilder. Indoor temperature set-point, LED supplemental lighting schedule, LED heat gain, and type of double skin window were selected as simulation parameters. For the cases without LED supplemental lighting, the proper growth temperature of lettuce $20^{\circ}C$ was selected as indoor temperature set-point together with $15^{\circ}C$ and $25^{\circ}C$. The annual heating and cooling loads which are required to maintain a constant indoor temperature were calculated for all the given temperatures. The cooling load was highest for $15^{\circ}C$ and heating load was highest for $25^{\circ}C$. For the cases with LED supplemental lighting, the heating load was decreased and the cooling load was 6 times higher than the case without LED. In addition, night time lighting schedule gave better result as compared to day time lighting schedule. To investigate the effect of window type on annual energy load, 5 different double skin window types were selected. As the U-value of double skin window decreases, the heating load decreases and the cooling load increases. To optimize the total energy consumption in the plant factory, it is required to set a proper indoor temperature for the selected plantation crop, to select a suitable window type depending on LED heat gain, and to apply passive and active energy saving technology.

  • Purpose: Coronavirus disease 2019 (COVID-19) has posed a significant burden to infant and toddler's care globally, while the disease severity is generally mild in this age group. In this study, we aimed to assess epidemiological and clinical aspects of COVID-19 in infants and toddlers in Seoul, South Korea. Methods: We used Seoul Metropolitan Government's epidemiological investigation database to describe the epidemiological and clinical characteristics of COVID-19 in infants and toddlers, between March 2020 to December 2021. Results: A total of 5,025 infants and toddlers aged <5 years was diagnosed between the observed period. 2,720 (54.1%) had symptoms, and fever was the most common symptom in 1,941 (74.1%). Of the diagnosed cases, 96.4% did not have underlying diseases. In a district level, extended opening of childcare facility was associated with increased risk of COVID-19 in infants and toddlers. Conclusions: An efficient monitoring system, resembling routine clinical care, is crucial, considering the low rates of severe progression and fatality among infants and toddlers. Moreover, a well-grounded intervention based on scientific evidence, rather than unconditional closures, is necessary to establish a suitable childcare policy that ensures safety from infectious diseases while not overlooking the developmental aspects of social skills.

A Study on An Integrated GEO/TES with Geothermal Heat Exchanger and Thermal Ice Storage (지중열 교환기와 빙축열조(Thermal Ice Storage)를 연계시킨 통합 지중열-빙축열조 시스템(Integrated GEO/TES))

  • Peak cooling load of large buildings is generally greater than their peak heating load. Internal and solar heat gains are used fur selection of adquate equipment in large building in cold winter climate like Canada and even Korea. The cost of geothermal heat exchanger to meet the cooling loads can increase the initial cost of ground source heat pump system to the extend less costly conventional system often chosen. Thermal ice storage system has been used for many years in Korea to reduce chiller capacity and shift Peak electrical time and demand. A distribution system designed to take advantage of heat extracted from the ice, and use of geothermal loop (geothermal heat exchanger) to heat as an alternate heat source and sink is well known to provide many benifits. The use of thermal energy storage (TES) reduces the heat pump capacity and peak cooling load needed in large building by as much as 40 to $60\%$ with less mechanical equipment and less space for mechanical room. Additionally TES can reduce the size and cost of the geothermal loop by 1/3 to 1/4 compared to ground coupled heat pump system that is designed to meet the peak heating and cooling load and also can eliminate difficuties of geothermal loop installation such as space requirements and thermal conditions of soil and rock at the urban area.
