Implementation of curved type a metallic plate system at the Bone contact (골 접촉 곡선형 금속 고정 시스템 구현)

  • This study was developed the metallic plate for fixation in the femur fracture for the orthopedic region and rigid fixation with plates has a firm place in fracture treatment. Most plates can be used for rigid as well as biological and dynamical fracture fixation. The device's designation and sizing has a specific with bending structural stiffness and strength, known meaning that is reliable regardless of the plate by the short type and long type. Short plate have a wrapping of femur and long plate have to preserve a pole of femur. The bending strength of the curved metallic long plate has to evaluate a 11,000N and The bending strength of the curved metallic short plate has to evaluate a 6,525N. The tensile stress through to press a plate is $1573N/m^2\;and\;1539N/m^2$. The device can be used to support Revision case of Hip Implant and to use a case of Hip screw compression of Hip Neck Fracture.

A Biomechanical Study on the Various Factors of Vertebroplasty Using Image Analysis and Finite Element Analysis (의료영상 분석과 유한요소법을 통한 추체 성형술의 다양한 인자들에 대한 생체 역학적 효과 분석)

  • This study investigates the biomechanical efficacies of vertebroplasty which is used to treat vertebral body fracture with bone cement augmentation for osteoporotic patients using image and finite element analysis. Simulated models were divided into two groups: (a) a vertebral body, (b) a functional spinal unit(FSU). For a vertebral body model, the maximum axial displacement was investigated under axial compression to evaluate the effect of structural integrity. The stiffness of each FE model simulated was normalized by the stiffness of intact model. In the case of FSU model, 3 types of compression fractures were formulated to assess the influence on spinal curvature changes. The FSU models were loaded under compressive pressure to calculate the change of spinal curvature. The results according to the various factors suggest that vertebroplasty has the biomechanical efficacy of the increment of structural reinforcement in a patient who has relatively high level of BMD and a patient with the amount of 15%, PMMA injection of the cancellous bone volume. The spinal curvatures after compression fracture simulation vary from 9$^{\circ}$ to 17$^{\circ}$ of kyphosis compared to that the spinal curvature of normal model was -2.8$^{\circ}$ of lordosis. These spinal curvature changes cause the severe spinal deformity under the same loading. As the degree of compressive fracture increases the spinal deformity also increases. The results indicate that vertebroplasty has the increasing effect of the structural integrity regardless of the amount of PMMA or BMD and the restoration of decreased vertebral body height may be an important factor when the compressive fracture caused the significant height loss of vertebral body.

Effect of Bee Venom Acupuncture Complex Therapy of Thoracolumbar Compression Fracture (흉(胸).요추압박골절(腰椎壓迫骨折)에 대한 봉약침(蜂藥鍼) 복합치료(複合治療) 효과(效果))

  • Methods : We investigated 28 cases of patients with thoracolumbar compression fracture. We divided patients into two groups : We treated one group with complex oriental medical treatment focusing on Bee Venom Acupuncture therapy, and did the other group with complex oriental medical treatment focusing on acupuncture therapy. To investigate the effectiveness of treatment applied for two groups, we used Visual Analog Scale(VAS) and Oswestry Low-back Pain Disability Index(ODI). We compared the VAS score and ODI score of two groups statistically. Results : 1. Female is prominent in distribution of sex, the average age bracket is 70's, and the largest group was Faccidental fall in the distribution of cause. 2. In regard to level of injury, T12 was found to be most prominent, followed in turn by L1, L2. In regard to grade of clinical symptoms, Grade III was most prominent, followed by Grade IV, Grade II. 3. As a result of evaluation by using VAS score and ODI score, treatment score at final was significantly different from that at the baseline in each group. 4. After treatment, Bee Venom acupuncture therapy group showed significant difference on VAS score and ODI score compared with acupuncture therapy group. Conclusions : Bee Venom acupuncture therapy can be available for relieving symptoms related with thoracolumbar compression fracture.

Effects of Korean Medicine Treatments on Pain Reduction of Thoracolumbar Compression Fracture Patients : A Restrospective Observational Study (흉추 및 요추 압박골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of the study was to investigate the effect of Korean medicine treatments on pain reduction of thoracolumbar compression fracture patients. We analysed 48 patients who have been diagnosed as thoracolumbar compression fracture on Computed Tomography(CT) or Magnetic Resonance Imaging(MRI) scan and had admitted to Dunsan Korean Medical hospital from January 1,2014 to April 4, 2017. The analysis was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 24 program. We used Visual Analog Scale(VAS) and pain reduction scale to evaluate pain reduction of patients. The result showed that there were statistically significant pain reduction on thoracolumbar compression fracture patients treated with korean medicine treatments. In conclusion, we found that the korean medicine treatments showed positive effect on pain reduction of thoracolumbar compression fracture patients.

  • There are little report on treatment of compression fracture with Traditional Korean Acupuncture Therapy. We suggest to treat Lumbar compression fracture with the SAAM(사암) Acunpture, Carthami-Flos Herbal Acupuncture Therapy. The Treatment methods of the SAAM(사암). Acnpucture, Eliminating extravasated Blood, was apllied at SP3, LU9, LI11 and Carthami-Flos Herbal Acupuncture at the waist. We treated the patient who was diagnosed as lumbar compression fracture by MRI. After 15 days of Treatment, SAAM Acupuncture and Carthami-Flos Herbal Acupuncture, a remarkable improvement was made for Lumbar compression fracture. There was reports about SAAM Acupuncture and Carthami-Flos Herbal Acupuncture of Patient with Lumbar Compression Fracture. It is very effective to reduce the pain and shortening the period of therapy.

Pulmonary Bone Cement Embolism Following Percutaneous Vertebroplasty (요추 압박 골절의 골 시멘트를 이용한 척추성형술 치료 후 발생한 폐동맥 시멘트 혈전증: 증례보고)

  • Purpose: Pulmonary cement embolization after vertebroplasty is a well-known complication. The reported incidence of pulmonary cement emboli after vertebroplasty ranges frome 2.1% to 26% with much of this variation resulting from which radiographic technique is used to detect embolization. Onset and severity of symptoms are variable. Case description: We present the case of a 83-year-old women who underwent fourth lumbar vertebroplasty and subsequently had dyspnea several days later. Posteroanterior chest radiography showed multiple linear densities. Computed tomography of thorax revealed also multiple bilateral, linear hyperdensities within the lobar pulmonary artery branches are detected in axial and coronal views. Literature Reviews: Operative management of vertebral compression fractures has included percutaneous vetebroplasty for the past 25 years. Symptoms of pulmonary cement embolism can occur during procedure, but more commonly begin days to weeks, even months, after vertebroplsty. Most cases of pulmonary cement emboli with cardiovascular and pulmonary complications are treated nonoperatively with anticoagulation. Endovascular removal of large cement emboli from the pulmonary arteries is not without risk and sometimes requires open surgery for complete removal of cement pieces. Conclusion: Pulmonary cement embolism is a potentially serious complication of vertebroplasty. If a patient has chest pain or respiratory difficulty after the procedure, chest radiography and possibly advanced chest imaging studies should be performed immediately.

A Bile Duct Cancer Patient Whose Stent Shifted Significantly Over the Course of External Beam Radiotherapy (외부조사 방사선치료 기간 중 총담도 스텐트의 위치 변화가 컸던 사례)

  • The author reports a bile duct cancer patient whose stent shifted significantly from right to left over the course of radiotherapy. The 80-year-old female patient had a short stature with thoracic kyphosis and mutiple spinal compression fractures. She was also emaciated and very lean. By comparing the weekly scanned computed tomography images, the author found her stent to have shifted by more than 4 cm from right to left over the course of external beam radiotherapy. The results of this case study suggest that for a very lean and emaciated kyphotic bile duct cancer patient, the possibility of large interfractional movement of the bile duct or stent during radiotherapy should be considered.

  • Vertebral compression fractures commonly afflict the elderly. Some patients suffer from severe mechanical pain in spite of treatments with strong analgesics and bracing. Vertebroplasty, which was originally used for vertebral hemangioma, is effective for patients who do not respond to these more conservative treatments. However, the procedure has some risk. Leaks of bone cement into perineural tissues can be a serious complication. In contrast to the lumbar vertebrae, the outer margin of the pedicle of the thoracic vertebrae is almost in line with the outer margin of the body. This, combined with the thinner pedicle of the thoracic vertebrae, makes proper needle placement difficult. The posterolateral approach is preferred to the transpedicular approach in order to avoid the danger of destroying the inner cortex of the pedicle. But there can be a problems with the standard posterolateral approach. The rib can be broken, the pleura can be punctured. A new and safer approach is possible. Before penetrating the bone, the needle is positioned at the upper margin of the transverse process, 5 mm away from the pedicle. To achieve this positioning, the needle must puncture the skin 1~1.5 cm laterally and 3~5 mm cranially to the target point on the bone. This approach was used for 10 patients and we achieved good results with no serious complication.

Percutaneous Sacroplasty for the Sacral Insufficiency Fracture Caused by Metastasis (척추 전이암에 의한 천추 압박골절의 경피적 천추성형술 -증례보고-)

  • Insufficiency fractures of the sacrum are relatively common and cause severe low back and buttock pain. Percutaneous vertebroplasty is effective for treating vertebral compression fractures. We present a case of percutaneous sacroplasty for the treatment of low back and buttock pain in a sacral insufficiency fracture. A 79-year-old male with non-small lung cancer presented with severe low back and buttock pain after series of radiation treatments. Preoperative MRI showed both a sacral ala and S2 metastatic insufficiency fracture. An epidural port was inserted for continuous morphine infusion and sacral nerve root blocks were performed. However, his pain did not diminish and we attempted percutaneous sacroplasty. Both sides of the sacroplasty were done with a fluoroscopy-guided technique with 1.7 ml and 2.3 ml of bone cement injected into the right and left sacral ala. Pain relief was significant and the patient was able to sit down 1 day after the procedure.

  • The purpose of this article is review about the use of ultrasonography in the evaluation of the elbow joint. Ultrasonography has several key advantages in elbow joint, including easy assessment of target structure in elbow joint and ability to perform real time dynamic examination and to compare with opposite site. Ultrasonography is easy available to visualize abnormalities affecting tendons, muscles, ligaments, bursae, and occult fractures around the elbow joint and also allows accurate assessment of ulnar nerve in cubital tunnel. The role of ultrasonography will increase further with regards to evaluation of soft-tissue abnormalities of the elbow joint.
