Evaluation of environmental surface contamination and disinfection effects on multidrug-resistant organism (다제내성균 환경표면 오염도 및 소독 효과 평가)

  • This study was carried out to evaluate the effects of disinfection using environmental disinfectant after having assessed the extent of contamination through microbial culture testing and the Adenosine Triphosphate Bioluminescence method among the environmental management evaluation methods used for the environment in the hospital ward of patients infected by multidrug-resistant organisms. This study was conducted with the patient wards isolated due to multidrug-resistant organisms as the environmental surface. Specimens were collected from five locations including infusion pumps, IV poles, bedside cabinets, bed railings, keyboards, and blood pressure measurement cuffs. ATP and microbial culture testing were executed prior to, immediately after, and five minutes post-disinfection. According to the result contamination of the infusion pumps was statistically significantly reduced after disinfection. In addition, the bacteria before and after disinfection reduced in IV pole, bed railing, and keyboard. That is, regular environmental surface disinfection can provide safer environments to patients against infection. Therefore, it is necessary to establish guidelines including disinfection methods and intervals for environmental surfaces by evaluating the persistence of disinfectants at various institutions in the future.

Factors Affecting Sexual Autonomy among Female Students of Colleges (여자 대학생의 성적자율성에 영향을 미치는 요인)

  • This study confirmed factors affecting sexual autonomy with self-esteem and sexual awareness as variables to lay the foundation for nursing intervention that helps female college students grow into subjects who can promote safe sexual activity and sexual health. A self-administered online survey of 139 students from three Colleges was conducted from February 15 to 21, 2021. The data were analyzed using a t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and hierarchical multiple regression. The significant predictors of sexual autonomy were self-esteem (𝛽=0.393, p<.001), experience of dating (𝛽=0.213, p=.008) and sexual consciousness (𝛽=0.175, p=.028) and their explanatory power was 22.1%. In order for female college students to have autonomy as sexual subjects, they must be able to have a positive sense of self-esteem that values themselves and healthy sexual consciousness. Furthermore, they must be able to have a positive experience of socializing with the opposite sex. Therefore, individual efforts and social support to promote self-esteem are required, and educational mediation is needed to contribute to the promotion of sexual autonomy through efficient communication between men and women with a healthy sexual consciousness

A study on environmental status, FIM and Illness intrusiveness of home stayed stroke patients (재가 노졸중 환자의 환경상태, 기능적 독립, 질병으로 인한 장애영향에 관한 연구)

  • This study was conducted to examine the environmental status, FIM(Function Independence Measure), Illness intrusiveness of home stayed stroke patients. Data were collected on 145 patients living in K city and the neighborhood located in southern part of Korea. The data were analyzed by means of SPSS program. Results : In assessment of environmental safety, 46.2%(67) felt unsafe around their residential area. The most dangerous facilities in their residence was stairs(58.2%), gas/electric facilities(14.9%). For sanitary problems, 28.2%(41) felt they have problems at their home. The most important problems were outdoor polluted toilet(22.0%) and scattered and dirty residential circumstances(22.0%). The average FIM score was 74.52(4.14) of total score 126(7). In motor area FIM was lowest at stair walking(3.01) and bathing(3.16). In cognitive area FIM was lowest at social activities(3.36). The mean score of Illness intrusiveness was 58.86(4.52) of total score 91(7). The major area largely impacted by stroke were health(5.61), working(5.68), active leisure activities(5.56). There was significant reverse correlation between FIM and illness intrusiveness(r=-0.66, p=0.00). Conclusion : It was needed environmental improvement and rehabilitation program for stroke patients, and repeated follow up study.

The Experiences of the Newly Admitted Elderly Residents in Nursing Home (노인요양시설 거주노인의 입소초기 경험)

  • Purpose: This study was conducted to investigate newly admitted nursing home resident's experience and adjustment problems in the early stage of institutionalization. Method: In depth interviews was done on 6 elderly residents who were newly admitted. Data from the interview were analyzed using theme analysis method. Results: Results showed that reasons for Institution admission were decreasing physical health and cognitive functioning. Residents reported that difficulties they experienced in institution life were lack of individualized care, loss of autonomy and privacy associated with the demands of group living, and loss of control over activities of daily living. Conclusion: Efforts need to be made to maintain the residents' decision-making and autonomy. Listening intentionally, offering choices, and promoting personal identity might be utilized to reduce relocation stress and enhance adaptation to nursing home living.

Influencing Factors on Perceived Safety of CCTV among College Students (대학생의 CCTV 체감안전도에 대한 영향 요인)

  • The purpose of this study was to examine the influencing factors of perceived safety for CCTV(Closed Circuit Television), and to confirm the correlation between fear of crime, social disorder, physical disorder in the community, and perceived safety for CCTV. Subjects were 140 students from one college located in H city using survey from June 17, 2022 to August 25, 2022. Data was analyzed by descriptive statistics, Pearson's correlation, and Stepwise multiple regression using the SPSS 23.0 program. As a result of the study, fear of crime was correlated with social disorder, physical disorder among community characteristics, and perceived safety of CCTV. Also, social disorder among community characteristics was correlated with physical disorder, perceived safety of CCTV, and physical disorder was correlated with perceived safety of CCTV. The factors affecting perceived safety for CCTV were fear of crime and social disorder, and the explanatory power was 57.2%(Adjusted R2=.572). In conclusion, it is necessary to improve the perceived safety of CCTV by improving the awareness of fear of crime and social disorder.

Development and Evaluation of Patient Safety Reporting Promoting Education Program (환자안전보고 촉진 교육프로그램의 개발 및 평가)

  • The purpose of this study was to develop and evaluate patient safety reporting promoting strategy for nurses to improve nursing care quality. The process included three phase - construction, implementation and evaluation phase. Before the construction phase has two kinds of subcategories as analysis and verification. Analysis phase was consisted of literature review and field study, and verification was conducted by two specialist. At the construction phase, patient safety reporting system, educational materials, planning for public relation, and reward preparation were developed. After implementation during 12 weeks, we evaluated pre-post scores of satisfaction, stress of conscience, job performances. The participants were 51 nurses working for a hospitals. The program was developed and then administered to the experimental group for 12 weeks. One group pretest-posttest design was used for this study. Data were analyzed using descriptive analysis, paired t-test with the SPSS WIN 18.0 program. After the intervention, job performance scores improved significantly from 3.62 to 3.75(t=2.653, p=.009). But job stress didn't changed significantly(t=.785, p=.434). These strategy can be applied to many clinical setting that will be helpful to promote patient safety reporting for nurses.

Change in patient safety, quality of medical care, organization culture and hospital management performance after healthcare accreditation system for convalescent hospital (요양병원 의료기관인증제 시행 후 환자안전, 의료의 질, 조직문화 및 병원경영성과의 변화)

  • This study was conducted to investigate the effects of implementation of the healthcare accreditation system on patient safety, quality of medical care, organization culture and management performance of convalescent hospitals. Ten hospitals were selected at random from among 36 convalescent hospitals in Daegu that had obtained accreditation until September 2015. To accomplish this, hospital staff were surveyed from 15 February to 25 March, 2016 using a questionnaire with 253 objects. We investigated how the healthcare accreditation system has influenced patient safety, service quality, organization culture and performance of hospital management. The change in the treatment support system showed the highest score of 3.93 among quality change and medical service items, while that of personnel source culture showed the highest score of 3.78 points among organization culture items, and that of internal process view, study and growth view showed the highest score of 3.71 among management performance items. Multiple regression analysis revealed that factors positively changing hospital management performance were nurses, employees and managers, basic value systems, organization culture, personnel source culture, open system culture, hierarchical culture, and production oriented culture. The quality of patient safety and medical care was positively influenced by changes in the quality of convalescent hospitals after implementation of the healthcare accreditation system. However, among the four types of organizational culture, the financial perspective was relatively low compared to other perspectives.

Mediating effects of Critical Thinking Propensity between Perception of Patient Safety Culture and Practice of Standard Precaution of Nurses in Small and Medium Hospitals (중소병원 간호사의 환자안전문화 인식과 표준주의지침 수행도의 관계에서 비판적 사고성향의 매개효과)

  • This study was conducted to confirm the relationship between perception of patient safety culture, critical thinking propensity, and practice of standard precaution among nurses in small and medium-sized hospitals. The research subjects were nurses from three small and medium-sized hospitals located in S City and C City who understood the purpose of the study and agreed to participate. The final study subjects were 158 nurses. The data were collected from January 5, 2020 to March 31, 2020, and descriptive statistics, t-test, one-way ANOVA, correlation, regression analysis, and Sobel test of the collected data were performed using SPSS/WIN26.0 Program. As a result of the study, nurses' practice of standard precaution showed a significant positive correlation with perception of patient safety culture (r=.524, p<.001) and critical thinking disposition (r=.471, p<.001). Critical thinking propensity was found to have a partial mediating effect in the relationship between perception of patient safety culture and practice of standard precaution. Based on the results of this study, repeated research can be conducted in the future, and can be used to develop and apply educational programs that can improve nurses' practice of standard precaution.

A Study about Discharge Criteria to Determine Patients' Readiness for Safe Return to Home after Ambulatory Surgery (통원 수술환자의 안전한 귀가 결정을 위한 퇴실기준에 관한 연구)

  • Purpose: This study was conducted to compare three discharge criteria; 1) discharge criteria of S Hospital determined by nurses, 2) discharge readiness determined by patients, and 3) the Modified Post-Anesthetic Discharge Scoring System (MPADSS). The usefulness of MPADSS as a discharge criteria for the patients'safe return to home after ambulatory surgery was also evaluated. Methods: A total of 370 day surgery cases were investigated. The MPADSS was employed in every 30 min. in parallel with discharge readiness assessment by nurses and patients. The percentage of the patients who were categorized as being ready to discharge were compared according to three discharge criteria. Results: The percentage of patients scored to be as MPADSS > 9 in 30 min, 60 min, 90 min were 96.5%, 99.5%, 100%respectively. Whereas 11.1%, 44.3%, 71.1%of patients rated themselves as being ready to discharge and 2.7%, 23.5%, 54.3% of patients actually discharged by nurses according to discharge criteria of S Hospital. Conclusion: Nurses tend to keep patients longer in the hospital when compared to the patient's own assessment about their readiness to home and to that of MPADSS. Faster discharge in the evening than day time suggests patient discharge can be influenced by nursing factors. This brings out the importance of scoring system to determine the safe discharge. The MPADSS could be a useful tool in evaluating patients for safe discharge.
