The Usefulness of Fluorescein Angiography and Alternative Tests for Assessment of Fundus Hemorrhage (안저 출혈 시 형광안저혈관조영술 및 대체 검사의 유용성)

  • Fundus hemorrhage refers to abnormalities in the retinal tissue and blood vessels. Therefore, when a hemorrhagic change in the fundus occurs, the ophthalmologist orders various ophthalmic tests to evaluate the degree of hemorrhage and determine the progress of the lesion before, during, and after treatment to accordingly establish a treatment plan. Currently, the most useful and universal fundus examination includes optical coherence tomography (OCT), fundus photography (FP), and fluorescein angiography (FAG). Existing fluorescein angiography test methods for establishing a treatment plan for severe fundus bleeding have limitations. The authors propose that peripheral pupil and the 5-quadrant method should be performed using ultra-wide-angle fluorescence fundus angiography (UWFFA). Using this method, it is possible to quickly determine the area to be described, avoid the radius of bleeding as much as possible, and provide the ophthalmologist with a range of damaged tissue and abnormal blood vessels. Nevertheless, there are cases in which ophthalmologists judge that fundus bleeding is so severe that ultra-wide-angle fluorescence fundus angiography is meaningless. In such cases, ophthalmic ultrasound and electroretinogram may be used in that order as alternative methods of examination. Therefore, some clinical situations require the use of ophthalmic ultrasound and electroretinogram and should be performed accurately.

Ultra-wide Field Fundus Photography Using Eye Steering Technique in Patients with Symptomatic Posterior Vitreous Detachment (후유리체박리 환자에서 눈방향전환 초광각안저촬영술의 유용성)

  • Purpose: To evaluate the availability of ultra-wide field fundus photography based on eye steering technique to diagnose retinal breaks in patients with symptomatic posterior vitreous detachment (PVD). Methods: The medical records of patients with symptomatic PVD were reviewed. Retinal breaks were independently identified using four eye steering capture images of ultra-wide field fundus photographs. The sensitivity and specificity of eye steering capture imaging for diagnosing retinal breaks were calculated. Results: A total of 94 eyes of 94 patients were included. Using fundus examination after pupil dilatation, retinal breaks were diagnosed in 42 (45%) eyes. The sensitivity of the eye steering capture imaging was 98% (95% confidence interval [CI]: 88-100%), and the specificity was 98% (95% CI: 90-100%). Of the 58 retinal tears, 28 (97%) involving the superior quadrant, 10 (100%) involving the inferior quadrant, 6 (100%) involving the nasal quadrant, and 13 (100%) involving the temporal quadrant were identified using eye steering capture images. Conclusions: Ultra-wide field fundus photography based on eye steering technique was useful for diagnosing retinal breaks in patients with symptomatic PVD. However, eye steering photography could not adequately replace the fundus examination after pupil dilatation in all cases.

Factors Influencing Fundus Examination in Patients with Diabetes (당뇨병 환자의 안저검사 수검 영향요인)

  • Purpose: This study was conducted to identify factors influencing fundus examination to prevent diabetic retinopathy in diabetes patients to provide basic data to improve screening rates of fundus examinations. Methods: Raw data from the 6th Korea National Health and Nutrition Examination Survey, which is a cross-sectional and nationally representative survey, were used in this study. The subjects of the study were 1,029 adult diabetes patients over the age of 19 years who had been diagnosed with diabetes. The demographic characteristics, diabetes and disease-related characteristics and health behavior characteristics according to the fundus examinations were analyzed by the chi-squared test and logistic regression analyses were used to examine the factors influencing fundus examination. Results: A total of 333 patients underwent fundus examination at a screening rate of 32.2%. We identified factors influencing fundus examination in patients with diabetes as level of education, type of diabetes care, period of diabetes, and smoking. Conclusion: A multiple approach is required to raise the low screening rate of fundus examination, including specialized education for low-education groups. Moreover, nursing intervention should focus on subjects who do not engage in insulin and oral hypoglycemic agents and with diabetes for a long period.

Ocular manifestations in Leigh syndrome (Leigh 증후군 환아에서의 안과적 이상 소견)

  • Purpose : Leigh syndrome is a typical type of mitochondrial disease. This study was conducted to analyze the types of ophthalmologic symptoms and results of funduscopy conducted in the ophthalmologic examination of patients with Leigh syndrome. Methods : Funduscopy was conducted on 24 subjects, who were chosen among those diagnosed as having mitochondrial respiratory chain complex defect and who were clinically suitable for the criteria of Leigh syndrome. Their clinical features, ophthalmologic symptoms, and ophthalmologic examination results were retrospectively analyzed. Results : Of the 24 patients with Leigh syndrome, 11 developed ophthalmologic symptoms and no abnormal finding was observed in 13. The most frequent abnormal finding was visual disturbance in 5 patients. Funduscopy revealed abnormal findings in 17 patients; retinal pigmentation was the most frequent abnormality and was seen in 9 patients. Conclusion : Funduscopy can be an important screening test to find ophthalmologic abnormalities among patients with mitochondrial disease (MD), including those patients whose ophthalmologic symptoms are inconspicuous. It is predicted that an improved screening test can be made in the future that will identify risk factors related to ophthalmologic symptoms.

Smartphone Fundus Photography in an Infant with Abusive Head Trauma (학대뇌손상 영아에서 스마트폰으로 촬영한 안저소견)

  • Purpose: To report fundus photography using a smartphone in an infant with abusive head trauma. Case summary: An 8-month-old male infant presented to the emergency room with decreased consciousness and epileptic seizures that the parents attributed to a fall from a chair. He had no external wounds or fractures to the skull or elsewhere. However, computerized tomography of the brain revealed an acute subdural hematoma in the right cranial convexity and diffuse cerebral edema, leading to a midline shift to the left and effacement of the right lateral ventricle and basal cistern. The attending neurosurgeon promptly administered a decompressive craniectomy. Immediately after the emergency surgery, a fundus examination revealed numerous multi-layered retinal hemorrhages in the posterior pole extending to the periphery in each eye. He also had white retinal ridges with cherry hemorrhages in both eyes. We acquired retinal photographs using the native camera of a smartphone in video mode. The photographer held the smartphone with one hand, facing the patient's eye at 15-20 cm, and held a 20 diopter condensing lens at 5 cm from the eye in the other hand. Our documentation using a smartphone led to a diagnosis of abusive head trauma and to obtain the criminal's confession, because the findings were specific for repetitive acceleration-deceleration forces to an infant's eye with a strong vitreoretinal attachment. Conclusions: This ophthalmic finding had a key role in the diagnosis of abusive head trauma. This case presented the diagnostic use of a smartphone for fundus photography in this important medicolegal case.

A Hospital-based Case-control Study on the Risk Factors of Cerebrovascular Disease (뇌혈관질환의 위험요인에 대한 환자 - 대조군 연구)

  • A hospital-based, matched case-control study was carried out to evaluate the relation ship of various suspected risk factors including snoring and serum level of cholesterol to cerebrovascular disease in Korea. A total of 127 incident cases of cerebrovascular disease(74 cases of cerebral infarction and 53 cases of intracerebral hemorrhage) admitted to. the department of neurology in a university hospital from December, 1993 to March, 1995 were compared with 127 matched controls admitted to same hospital in same period. A multivariate analysis suggested that ECG abnormality(left ventricular hypertrophy and atrial fibrillation), family history of cerebrovascular disease, fundoscopic abnormality, previous history of transient ischemic attack and hypercholesterolemia were risk factors of cerebrovascular disease, ECG abnormality, fundoscopic abnormality, smoking and hypercholesterolemia were also suggested as risk factors of cerebral infarction.

Acute Phase Retinal Phototoxicity Induced by Blue Fluorescent Light (토끼에서의 청형광 광선에 의한 실험적 급성 망막독성)

  • Even though phototherapy is an effective means of treating hyperbilirubinemia, adequate binocular shields are essential to prevent light hazard of retina during phototherapy. The phototherapy to young albino rabbits was performed under various conditions; exposured for 12, 24, and 48 hours, continuity or intermittence of irradiation, and use of shields. Although there was no definite pathologic change in indirect ophthalmoscopic examination, severe damage was seen at electron microscopic level. In all experimental groups, swelling and edema of photoreceptor were seen at right after phototherapy. Futhermore, more severe damage was seen in case of intermittent irradiated group and prolong exposed group. In mydriatic eyes, severity of damage was similiar to non mydriatic eyes, but area of retinal damage was more extensive than non-mydriatic eyes. In control group which was protected by binocular shields during phototherapy, no significant abnormality was found.

A Case of Chronic Monocular Solar Retinopathy (만성 단안 일광망막병증 1 예)

  • Solar retinopathy is a rare but well-recognized clinical entity of macular damage, caused by viewing a solar eclipse or direct sun gazing. A 21-year-old man gazed at the sun for approximately thirty seconds at noon using a monocular telescope with his left eye. Forty-eight hours after sun gazing, the patient experienced symptoms of blurred vision and central scotoma in the left eye. Eight months after sun gazing, the visual acuity decreased from 1.0 to 0.1 in the left eye and the fundus examination showed a round, yellowish-white discoid lesion at the left fovea. Fluorescein angiography showed an early window defect in the fovea of the left eye, that persisted without size change during the late phase resulting from atrophy of the retinal pigment epithelium. A small, central scotoma of the left eye was also found in the visual field test. The visual acuity was unchanged at the end of a one-year follow-up period.
