Anatophysiological Changes Related to Myopization Induced by Alcohol Ingestion on Eyes (알코올 섭취 후 발생한 근시화와 관련된 눈의 해부생리학적 변화)

  • 알코올 섭취 후 발생하는 해부생리학적 변화가 눈의 일시적인 근시화에 미치는 영향을 알아보고자 하였다. 평균 24.5${\pm}$1.5세의 남자 8명(16 안)을 대상으로 0.42 g/kg의 알코올을 30분 간에 걸쳐 섭취토록 한 다음, 알코올 섭취 1시간, 4시간, 24시간 후의 교정굴절력, 각막곡률반경과 두께, 동공직경, 안압, 그리고 안구축의 길이변화를 측정하여 알코올 섭취 전과 비교하였다. 알코올 섭취 1시간 후에 알코올 섭취 전과 비교하여 호흡 중 알코올 농도가 가장 높았고(p<0.001), 교정굴절력의 구면 (-)굴절력 증가(p<0.05), 동공 크기의 감소(p<0.05), 안압의 하강(p<0.001), 그리고 안구축의 길이 증가가 나타났다. 알코올 섭취 4시간 후에도 모든 측정값들이 섭취 1시간 후와 동일한 경향이었다. 그러나 알코올 섭취 24시간 후에는 알코올 섭취 전과 비교하여 의의있는 변화를 보이지 않았다. 알코올 섭취로 인한 일시적인 근시화는 안압하강과 안구축의 길이변화와 관련성이 있을 것으로 판단된다.

Relationship of Refractive and Anatomical Changes on Eyes after Alcohol Ingestion (알코올 섭취에 의한 눈의 굴절변화와 해부학적 변화와의 관계)

  • Purpose: This study was to investigate the relevance between refractive and anatomical changes temporarily on the eyes after alcohol ingestion. Methods: Eight subjects (16 eyes) which were $24.5{\pm}1.5$ aged males drunk the alcohol of 0.42 g per kg of body weight within 30 minutes. Refractive errors, the radius of corneal curvature, corneal thickness, pupillary size, intraocular pressure, and the length of the ocular axis at 1 h, 4 h, and 24 h after alcohol ingestion were compared with them of non-alcoholic state. Results: At 1 h after alcohol ingestion, breath alcohol concentration was the highest (p<0.001), more negative spherical power was needed (p<0.05) for correction, pupillary diameter was decreased (p<0.05), intraocular pressure was decreased (p<0.001), and the length of the ocular axis was increased compared with each one of non-alcoholic state. At 4 h after alcohol ingestion, all anatomical changes were the same tendency as at 1 h after alcohol ingestion. But at 24 h after alcohol ingestion, both refractive changes and anatomical changes were not significant compared with them of non-alcoholic state. Conclusions: Temporary changes of refractive error after alcohol ingestion may be related with decrease of intraocular pressure and increase of the length of ocular axis.

Ultrasonographic and Biometric Evaluation of the Eyes of Horses and Cattle (말과 소에서 눈의 초음파측정과 생체측정의 평가)

  • 말과 소의 정상 안구내의 구조와 크기를 알아보기 위하여 10마리의 말과 14두의 Holstein 안구를 적출하여 생리식염수내에서 초음파상으로 안구내의 구조를 확인하였으며 각막의 두께, 전방의 깊이, 수정체의 두께, 초자체의 깊이 및 안구축을 측정하였다. 초음파로 측정한 안구를 $-30{\circ}C$ 로 동결한 후 Diamond cutter로 절단하여 caliper로 측정하여 Student-t test로 처리하여 수치를 비교하였다. 초음파상에서 말은 암컷$(35.99{\pm}1.97)과 숫컷(35.94{\pm} 3.36)$, 좌측(36.26)과 우측$(35.67{\pm}2.65)$눈의 크기가 비슷하였으나, 소에서도 좌측$(29.06{\pm} 3.36)과 우측(28.53{\pm} 3.36)$눈의 크기가 비슷하여 통계학적 유의차는 없었다. 본 연구에서는 7.5MHz 초음파기의 B-Mode 방식을 이용하여 말과 소의 안구의 구조를 확인하고 측정하였는데 임상적으로 매우 유용하여 수의안과학에서 적용할수 있는 가치있는 진단법이다.

Mathematical Expression of Eye Movement Using Rotation Matrix (Rotation Matrix를 이용한 안구운동의 수학적 표현)

  • The eye movement of the eyeball's center of a rotation can represent with the rotation matrix $R_x$, $R_y$, $R_z$ due to a coordinate axis rotation transformation of Cartesian coordinate, and describes of an abduction, an adduction, an elevation, a depression, an intorsion, an extorsion in principle rotation six forms of the eye. The eye movement from primary eye position to tertiary eye position could be composed with the rotation matrix combination, and by the primary rotation of six and the secondary rotation of eight, could be represented with the extrocular muscle of six. The position of the cornea vertex point or pupil point due to the eye movement can describe to transform the rotation matrix of the cartesian coordinate to spherical coordinate$(r,{\theta},{\phi})$.

A Study on Double Angle of Optic Foramen in the Rhese Method (Rhese법 촬영에서 시신경구멍의 이중 각도에 대한 연구)

  • The purpose of this study is to confirm the double of optic foramen in Korean and apply it to the Rhese method. First, the angle between the right optic foramen and the MSP was measured on the axial image using MPR technique of the 3D CT. Second, we measured the angle between the right optic foramen and OML in sagittal of MPR images. As a result, the angle between the optic foramen and the MSP was $39.9{\pm}4.63^{\circ}$ on average, which was different from the $53^{\circ}$ presented by Rhese method(p<0.05). The angle between optic foramen and OML was $40.8{\pm}6.6^{\circ}$. In conclusion, this study confirms that the standard of the Rhese method proposed in current textbook is difficult to apply to Koreans. Therefore, it is necessary to study angle of Korean standard in various general x-ray technique.

The Assessment of Scattered Ray According to the Beam Thickness of Z-axis in MDCT(Multi Detector Computed Tomography) (MDCT(다배열검출기 전산화단층촬영장치)에서 Z-축의 빔 두께에 따른 산란선의 평가)

  • The purpose of this study is to measure scattered ray which is occurred except for Z-axis range of the detector in MDCT's iso-center and present the basic data about the standard for reduction of scattered ray. The development of MDCT brings out the enlargement of beam thickness to the patient's Z-axis, which distributes to the increase in exposure dose according to the rise of scattered ray. Also MDCT brings out the increase of scattered ray about 4times more than SDCT. To evaluate scattered ray according to the change of beam thickness on MDCT, we measured scattered ray of MDCT's Z-axis beam thickness by using one 16-slice CTs and two 64-slice CTs. We used the ionization chamber 60ml 2026C as the equipment of measurement. In our results, we found out that the change of scattered ray according to the beam thickness in the same kVp has increase of scattered ray. Secondly we found out the increase of scattered ray according to the increase of kVp. Lastly we found out the decrease of scattered ray according to the increase of the distance from the ionization chamber.

A Case of an 18-month-old Boy with Type 3 Gaucher Disease Presenting with Hepatosplenomegaly and Growth Retardation: The Clinical Course after Enzyme Replacement Therapy (18개월 남아에서 간비장비대, 성장 부진을 동반한 3형 고셔병 증례: 효소 대체 요법 후 임상 경과)

  • Gaucher disease (GD) is an autosomal recessive lysosomal storage disorder caused by beta-glucosidase deficiency. An 18 month-old male with hepatosplenomegaly, anemia, thrombocytopenia, and growth retardation referred to our hospital. The patient showed neurological symptoms, such as supranuclear gaze palsy and developmental delay. Bone marrow biopsy performed to rule out malignancy and the results revealed no malignant cell; however, abnormal histiocytes suggesting storage disease was noted. Based on hepatosplenomegaly, bicytopenia and unexplained neurologic manifestations, enzyme activity and genetic analysis were conducted emergently with a strong suspicion of GD. Beta-glucosidase activity in leukocyte was decreased. GBA sequencing to confirm the diagnosis revealed compound heterozygous pathogenic variants (i.e., c.754T>A, c.887G>A), both previously reported as the cause of neuronopathic GD. Under the diagnosis of type 3 GD, the patient immediately received enzyme replacement therapy (ERT). After 17 months of ERT, the size of spleen decreased, and hemoglobin and platelet count returned to normal. In addition, the activity of chitotriosidase and angiotensin converting enzyme decreased. However, myoclonic movement and generalized seizure occurred at the age of 19 months and antiepileptic drug was started. Other neurological deterioration including supranuclear gaze palsy and developmental delay also persisted. A new therapy to overcome neurologic problems should be developed for patients with type 3 GD.
