We examined 104 patients(primary group, controlled group) who had visited PNUH from 1994 to 2002, having been diagnosed as temporomandibular disorders(TMDs) and treated in conservative ways such as Behavior modification, medications, physical therapies and splint therapies. We also examined 54 patients(recurred group, experimental group) who had visited PNUH from 1991 to 2001, having been diagnosed as TMDs and experienced recurrence after conservatively treated. To find out the symptoms of Recurred TMD patients and their results of conservative treatments, we compared these two groups mentioned above. The obtained results were as follows: 1. Both primary and recurred groups have showed great improvements with conservative treatments. 2. Both primary and recurred groups have showed no differences in pain, LOM, MCO in their first visits but the noise were louder in primary group. 3. Both primary and recurred groups have showed no differences in pain, LOM, MCO when the treatments were over but the noise were louder in recurred group. 4. Treatments modalities, diagnosis, sex, kind of disease had not affected the results of treatment in either of groups. 5. It has come out that much better results were achieved when the patients in primary group had treated for over 6 months and for more than 10 times.
In order to evaluate the prognosis of conservative treatment for Craniomandibular Disorders, 127 patients were subjected at the Dept. of Oral Medicine, Pusan National University Hospital from 1983 through 1991. All the changes of patients' symptoms and related factors were analyzed before treatment, after treatment and at follow-up examination by means of subjective and objective symptom indicies (Ai, AAI, SDS, Di, CDS, MCO). 1. All the indices were reduced and MCO became increased at follow-up examination (p<0.01). 2. As the duration after treatment became longer, all the indices became reduced and MCO became increased. 3. There were no significant differences in index scores according to sex and age. 4. Long-term patients showed higher index scores and lower MCO than short-term patients(p<0.01). 5. Chronic group showed higher index scores and lower MCO than acute group (p<0.05). 6. Macrotrauma group showed higher index scores and lower MCO than microtrauma group (p<0.05) 7. While muscle group showed the lowest MCO, muscle and joint group showed the highest index scores (p<0.01). 8. The long-term success rate of conservative treatment was more than 80% (p<0.01)
The author performed on epidemiological study of the TMD in 189 elderly people and 195 young people with Helkimo index. The clinical dysfunction index was based on data from clinical examination and the anamestic dysfunction index was based on data from the interview with the investigated person. The results were as follows : 1. In the elderly population, 27.5% reported that they had subjectively symptom of TMD but 43.4% had sign and symptoms of TMD in clinical examination. 2. The mean values for maximal opening differed significantly between elderly and young populations, 47.22mm and 51.44mm (p<0.001) and less than a 40mm opening was observed 7.9% in elderly population, 0.5% in young population (p<0.001). 3. The mean values for lateral movement to the right and left did not differ with age, which were 8.56mm and 8.47mm in elderly population, 8.90mm and 8.81mm in young population, but the mean value for protrusion differed significantly between elderly and young populations, 6.89mm and 7.64mm (p<0.01). 4. A higher incidence of TMJ noise was recorded in the elderly and young populations than young population, especially crepitus, but a higher incidence of clicking was recorded in young population (p<0.05).
The purpose of this study was to evaluate the therapeutic effect of occlusal stabilization splint on the clinical symptoms and the condylar movement in patients with Temporomandibular Disorders(TMD). For the study, 15 TMD patients treated with stabilization splint and followed up were selected. The age of them was from 18 to 65 years and the mean period of treatment was 2.9 months. The author examined signs and symptoms of TMD according to Dr. Friction's evaluation from and recorded the condylar paths with Denar pantronic before and after splint therapy. The obtained results were as follows : 1. On the first visit, 11 patients(73.3%) showed muscle tenderness on palpation and the frequency was lateral pterygoid, masseter, medial pterygoid, temporalis, sternocleidomastoideus in the order named. 2. Occlusal stabilization splint was more effective in pain relief(100%) than in other dysfunction improvement(85.7%) 3. The amount of maximum opening increased from 37.1㎜ to 42.2㎜, but those of protrusion and laterotrusion changed little. 4. Pan. PRI scores decreased from 32.9 to 21.8, which meant improved reproducibility of mandibular border movements, and the group with sever dysfunction category showed more decrease in score than the group with moderate or slight dysfunction category.
The author studied the changes of subjective, objective symptoms and muscle activities with EM2 (myo-tronics Co., Seattle, USA) before and after MRA therapy. The 11 patients were treated with MRA and active range of motion, pain and mouth opening limitation were checked at each visit for 6-8 weeks. Electromyographic activities were measured in both anterior lobe of temporalis and middle fibers of masseter at the position in rest, clenching and mastication. The obtained results were as follows : 1. There were significant decrease in pain and mouth opening limitation and significant increase in active range of motion after MRA therapy. 2. The muscle activities tended to decrease in general, especially in the temporal and masseter muscles on clenching and in the masseter on mastication after MRA therapy. 3. There were no significant differences in muscle activities between affected and unaffected side, but there was significant differences in temporal muscle on clenching side after therapy. 4. There were no significant differences in active range of motion, pain and mouth opening limitation between acute and chronic groups. 5. There was more significant decrease in muscle activities of the affected side in acute group than those in chronic group.
The life changes of TMJ patients were evaluated through the Social Readjustment Rating Scale (SRRS) questionnaire. 61 female TMJ patients and 80 dental new female outpatients were studied at the TMJ Clinics, Department of Oral Diagnosis, PNUH from February to September 1987. The obtained results were as follows; 1. The life change unit (LCU) totals and number of life events in the TMJ patients were significantly higher than those in the control subjects, especially during the 1-6 months before presentation for the hospital. 2. There was no significant difference in the number of the high-scored (over 150 LCU totals) between the TMJ patients and the control subjects. 3. There was a significant difference in LCU totals and life events between the TMJ patients and the control subjects by age, despite no difference in LCU totals and life events between the young and the advanced subgroups. 4. The LCU totals and the number of life events in the TMJ patients over 13-year-school age were significantly higher than those in the control subjects over 13-year-school age and the TMJ patients under 12-year-school age. 5. The married subgroup in the TMJ patients showed no significant difference in LCU totals and life events as compared with the unmarried one. 6. Categorizing life events into 6 items (marital life, health, occupation, family, finances and social), the TMJ patients had the higher frequencies in marital life and occupation than the control.
To evaluate the correlationship between sign/symptoms of craniomandibular disorder(CMD) and possible contributing occlusal factors, the author analyzed craniomandibular index(CMI), clinical dysfunction index(CDI) and occlusal index(Oi) obtained from 88 subjects (32 males, 56 females, mean age 28.7) consisted of 49 CMD patients(15 males, 34 females, mean age 28.7) and 39 non-CMD patients (17 males, 22 females, mean age 24.5). The obtained results were as follows : 1. There was very high significant correlation bebween CMI and CDI in total subjects (r=0.83, p<0.01) 2. There was very low correlation between working-side interference and TMJ noise which was observed as a statistically significant value(p<0.05). And also there ws very low correlation between nonworking side interference and muscle tenderness, CDI, dysfunction index(DI), palpation index(PI) which were observed as statistically significant value (p<0.01) 3. There was very low correlation between as ymmetry of tetruded contact position(RCP)-intercuspal position(ICP) slide and muscle tenderness, TMJ tenderness, TMJ noise, CMI, DI, PI which were observed as statistically significant values(p<0.05) 4. In general there was low correlation between Oi and muscle tenderness, CDI, CMI, DI, PI which were observed as statistically significant values (p,0.05). But Oi had a relatively higher correlation with muscle tenderness, PI than whith CDI, DI.
A prevalence study was carried out on 847 CMD patients who had visited the Department of Oral Medicine in Pusan National University from 1990 to 1993. To obtain the same type of information, all subjects were interviewed and examined clinically using a standardized examination form, The ratio of women to men was about 3:1 and all subjects were divided into acute and chronic groups on the basis of 6 months of duration. Diagnostic groups consisted of muscle disorder, joint disorder and muscle-joint disorder. As related to gender, duration and diagnosis subjective and objective symptoms in CMD were studied. The obtained results were as follows : 1. Muscle-joint disorder had the highest percent, followed by muscle disorder and joint disorder. 2. The most common reasons for CMD treatment were pain, joint noise and limited opening, while headache and neckache were relatively often reported as associated symptoms and dizziness, ringing in the ears also reported as secondary CNS excitatory effects. 3. Pain was more ofter seen in women, acute group and muscle-related disorder groups (p<0.05, p<0.01). Noise was significantly frequent in chronic group and joint-related groups (p<0.01). 4. Analysis of contributing factors presented that macrotrauma was found frequently in men (p<0.05), and that muscle-related groups were more related to stress than joint disorder grop (p<0.05). 5. Hard end feel was seen significantly often in joint-related disorder group (p<0.05). On the other hand, soft end feel was frequent in muscle disorder. 6. Reciprocal clicks and crepitation increased with chronicity. Subjects with joint-related disorder groups significantly often reported all kinds of noises (p<0.01). 7. Tender muscles and joints were more often reported in women and chronic group. Whereas muscle-related disorder groups revealed significantly more tender muscles (p<0.01). joint-related disorder groups presented significantly more tender joints (p<0.01).
The present study explored the influence of temporomandibular disorders(TMD) with anterior open bite on swallowing. Fifteen subjects with anterior open bite(group A), 9 subjects with anterior open bite and TMD(group B), and 24 subjects without malocclusion or TMD symptoms (group C) were included. BioPAK system(Bioresearch Inc., Milwaukee, WI, USA) was used to record the muscle activities of anterior temporal, masseter, sternocleidomastoid(SCM) and anterior digastric muscles during maximum clenching and swallowing. Positional change of the mandible during swallowing was also recorded using the same system. The obtained results were as follows: 1. Group A, B, and C did not show significant differences each other in the muscle activity of resting position in most of head and neck muscles. 2. Group B showed significantly lower muscle activity in maximum clenching than group C in anterior temporal(p<0.01), masseter(p<0.05), SCM(p<0.05) and digastric muscles(p<0.05). 3. Group A showed significantly lower muscle activity during swallowing than group C in anterior temporal and masseter muscles(p<0.01). Group B showed significantly lower muscle activity during swallowing than group C in anterior temporal(p<0.01), masseter(p<0.01), and SCM muscles(p<0.05). 4. Group A and B showed increased positional change of the mandible during swallowing measured from vertical, anteroposterior and lateral aspects, and prolonged swallowing(p<0.05). 5. After given instructions for normal swallowing pattern, group A and B showed increased muscle activity during swallowing in anterior temporal, masseter and SCM muscles(p<0.01). Positional change of the mandible and time elapsed for swallowing also decreased after the instruction(p<0.01). 6. Occlusal splint did not change the muscle activity during swallowing. However, vertical change(p<0.01) and swallowing time(p<0.05) were decreased with splint.
