Occlusal rehabilitation of post-traumatic malocclusion patient after reduction of panfacial fracture, using selective occlusal adjustment and implant prostheses on centric relation: a case report (다발성 안면 골절의 정복 후 발생한 부정교합 상태의 환자를 중심위에서 선택적 교합 조정 및 임플란트 보철수복으로 교합관계를 회복시킨 증례)

  • Invasive or non-invasive reduction of fractures could be conducted as treatments of traumatic maxillofacial bone fractures. But when suboptimal reduction or malunion of maxillofacial bone fracture occurs, malocclusion could occur as a result of the lost relationship of the mandible and midface. This malocclusion is called post-traumatic malocclusion and orthognathic surgery, orthodontic treatment, selective grinding and prosthetic reconstruction are suggested as treatments for post-traumatic malocclusion after securement of stable TMJ. Stable TMJ is essential for occlusal rehabilitation to prevent occlusal change and relapse of malocclusion. Centric relation and adapted centric posture are suggested as start points of occlusal rehabilitation because they are most stable TMJ position. This case report presents a case in which post-traumatic malocclusion occurred after reduction of panfacial fracture. To rehabilitate full mouth occlusion, selective grinding and prosthetic reconstruction of implant supported fixed prostheses were conducted in centric relation and showed satisfying results in functional and occlusal aspects.

  • Destruction of normal temporomandibular joint architechture may produce serious functional and cosmetic deficiencies. The literature is well documented as to the etiology and pathogenesis of temporomandibular joints. Numorous surgical procedure have been advocated for temporomandibular joint ankylosis from condylectomy to arthroplasty, cartilage transplant, metallic prosthesis, interpositional implant. These were to able reconstruct the normal mandibular function, and any even procedure could obtain the satisfactory results. In this paper, we reviewed young adult patient with TMJ ankylosis and facial asymmetry who was treated with metallic condylar prosthesis and orthognathic surgery.

Skeletal Factors Related to Open Lock of the Temporomandibular Joint (측두하악관절의 개구성 과두걸림 환자의 골격적 특성에 관한 연구)

  • This study was performed to investigate the skeletal factors related to open lock of the temporomandibular joint(TMJ). We compared the skeletal measurements on the cephalogram and transcranial radiograph among 3 groups, open lock group consisting of consecutively filed 50 patients with at least one open lock episode within recent 1 year, temporomandibular disorder(TMD) group of 50 TMD patients without open lock diagnosed by Research Diagnostic Criteria for TMD (RDC/TMD) Axis I, and normal group of 50 patients without TMD or open lock. The patients of TMD and normal group were randomly selected in an age-and-gender-matched way with ones of open lock group. Open lock group showed smaller saddle angle than normal group on cephalograms and steeper inclination of the articular eminence than TMD and normal groups on transcranial radiographs. These results imply that the patients with the joint located more anterior and the articular eminence with steeper inclination might be riskier to TMJ open lock.

  • Traumatic injuries in the young permanent dentition are common, but root fractures, defined as fractures involving dentin, cementum and pulp, are relatively uncommon. Appropriate management of root fracture involves repositioning the coronal portion of the tooth fragment and firm immobilization with a splint for 2 to 3 month. Root canal treatment should not be initiated until the sign of necrosis or resorption are apparent because in most cases, the apical fragments maintain their vitality. The following case report describes a patient with root fractures injured three times over the period of 7 years. The results, clinically and radiographically, were acceptable, but long term periodic evaluation is required.

  • Mandibular trauma is developed due to traffic accident, fall down, industrial injury, and others. TMJ disorder is usually also developed after facial traumatic injury. Many authors suggested that disc displacement or tearing, acute synovitis, TMJ ankylosis, traumatic arthritis, or effusion are developed after facial trauma. It is still very controversible what is the best treatment of TMJ injury such as condylar fracture and meniscal injury. In TMJ injury, synovial inflammation is developed and pain mediators such as prostaglandin E2 or leukotriene B4 are released from the synovial membrane. This can be a cause of TMJ disorder. I present a variety of experimental study about the condylar fracture and meniscal injury and enzyme-immunoassay of synovial fluid after mandibular trauma that have been studied since 1992 and establish the treatment criteria of traumatic TMJ injury. I think that the treatment option of condylar fracture depends upon the surgeon's criteria exclusively. There are no significant differences between conservative and surgical treatment. If the aggressive functional physical therapy and long-term followup be performed, the favorable functional recovery of TMJ can be obtained. And I think that the initial surgical management of meniscus of TMJ is unnecessary in condylar fracture. And also arthrocentesis can be available to release the patient's subjective symptoms and improve the healing of injured TMJ.

  • Miniplate의 사용은 Champy등의 보고 이후 구강외과 영역에 있어서 외상 혹은 악교정수술등 많은 분야에 이용되기 시작했다. 전통적인 악간고정 방법에 비해 miniplate는 사용하기 쉽고, 정확한 해부학적 정복 및 고정을 가능케 하며 결과적으로 빠른 치유를 가능케 한다는 장점들을 갖고 있다. 그러나 감염, wound dehiscence, 부식, 나사의 헐거워짐, 주위조직의 변색 그리고 stress shielding effect등의 후유증들이 보고되어 왔으며 가장 무해한 금속이라고 알려진 titanium 또한 주위조직 및 lung, spleen, liver 그리고 kidney 등과 같은 조직으로 metal을 전이시키는 것으로 보고되고 있다. 특히 안와골 주변과 같은 상부 연조직이 얇은 곳에 miniplate를 사용할 경우 miniplate가 만져지거나 보일수도 있다는 점이 보고되었으며 따라서 술후 사용되었던 miniplate를 제거 해야만 할 것인가가 또한 주된 관심사로 보고되고 있다. 이에 저자는 이러한 금속성 miniplate의 단점을 개선하고자 자가 두개골을 이용한 플레이트를 고안하였다. '자가 두개골 플레이트'의 적응증은, enophthalmos 개선의 목적으로 두개골을 채취해 임플란트로 사용하는, enophthalmos를 동반한 관골 및 안와골의 복합골절환자에 있어 두개골의 추가채취가 가능한 경우가 될 수 있겠다. 자가 두개골을 이용해 플레이트를 고안하려고 본 증례에서의 시도는 구강외과 영역에 있어서 자가 두개골이 골편이 고정재료로도 쓰일 수 있다는 가능성을 보여주었다는 점에서 의미가 있다고 저자는 생각한다.

  • With the exception of gun shot wound, the incidence of penetrating injury of face and neck areas nonorganic foreign bodies is relative low. But the diagnostic evaluation and therapeutic management of penetrating facial wounds need careful decision, when the anatomic proximity of the major vessels and nerve is considered. Penetrating facial trauma with concomitant vascular injury present challenging problems, the immediate complication of this vascular injury are severe bleeding, hematoma formation, shock, obstruction of airway. The vascular injury is conformed by angiography. In this report, a industrial tool(long tack) fired by explosive air is penetrated into face and to neck. In angiograms penetrating injury of the vertebral artery is detected. We performed the embolization of the vertebral artery with coils and manual removal of the foreign body without any complication was followed.

  • The traffic accident was one of most common cause for the facial bone fracture. When it involved the midfacial structures, the nasal bone fracture was usually shown. If the reduction was not done in time, it would result in facial deformity. Simple case could be corrected by simple rhinoplasty. However, severe cases would require more invasive technique. We used triangular osteotomy included the nasal bones, the vomer, and the medial wall of maxilla for the correction of post-traumatic nasal deformity and reported the result with the review of literatures.

  • This is a case report of correction of posttraumatic diplopia and enophthalmos using autogenous composite conchal cartilage and temporal fascia. The results are as follows. 1. Diplopia and enophthalmos may be persistent complications in zygomatic complex fracture. 2. The concha is a reservior of autogenous cartilage which leaves no cosmetic and functional deformity in reconstruction of orbital floor. 3. Warpping the cartilage in temporal fascia may serve to increase implant stability and get graft with smooth edge. 4. Donor incision lines are well hidden within the scalp and in auriculocephalic fold.

  • There is always a risk of injury in sports. With the recent development of the economy, there have been more opportunities to be exposed to risks, and safety issues to prevent accidents have emerged as a central task. However, intensive research on trauma is lacking in the oral & maxillofacial area associated with sports. It has only been partially introduced, with a comprehensive survey of whole-body trauma or mixed with various trauma in the oral & maxillofacial area. In this paper, the overall content of trauma in the oral & maxillofacial area caused by sports activities will be reviewed for clinicians.
