The Effects of Regular Walking Exercise on Metabolic Syndrome, Cardiovascular Risk Factors, and Depressive Symptoms in the Elderly with Diabetic Mellitus (노인 당뇨병 환자의 규칙적 걷기운동 프로그램이 대사증후군, 심혈관 위험도 및 우울정도에 미치는 효과)

  • Purpose: This study aimed to estimate the effects of a regular walking exercise program on metabolic syndrome, cardiovascular risk factors, and depressive symptoms among the elderly with diabetic mellitus (DM) based on the Theory of Reasoned Action (TRA). Methods: This study has randomized and stratified experimental design with experimental and control groups. We developed a regular walking exercise program suitable for the elderly with DM based on the guidance of AAHPERD. The experimental group participated in the regular walking exercise program, which contains walking exercise 3 times a week and 50 minutes each time for 3 months and education on controlling diet and preventing complications once a week and 20 minutes each time for 4 weeks. Post-test was conducted after 3 months to estimate metabolic syndrome, cardiovascular risk factors, and depressive symptoms. Results: The regular walking exercise program was effective for decreasing the waist size, the level of fasting blood glucose (FBG) and triglyceride (TG), cardiovascular risk factors and the severity of depressive symptoms among the elderly with DM. Conclusion: The incidence of complications would be decreased by applying a regular walking exercise program.

  • Although left Left ventricular hypertrophy (LVH) is not only an adaptive response of the heart to increased cardiac workload in hypertension, it surelybut also is the most potent risk factor of overt cardiovascular complications such as coronary heart disease, heart failure, arrhythmia and stroke in the hypertensive population. Also it has become generally accepted that subclinical cardiovascular disease begins in childhood and LVH is the most readily assessed marker for that. As LVH can be seen in children and adolescents with even mild blood pressure elevation with the reported prevalence of 10 to 47%, aggressive antihypertensive treatment is critical in preventing the development of hypertensive heart disease in that those cases.

Association of Coronary Artery Calcium Scores with Cadiovascular Disease Risk Factors in an Asymptomatic Adults (무증상 성인에서 심혈관질환 위험요소와 관상동맥 석회 수치와의 관계)

  • Coronary artery calcium scores(CACS) has been used as surrogate marker for coronary atherosclerosis. We evaluated 1042 patients who visited the Department of Health Promotion Center in Chonnam National University Hospital and had a test of the CACS from January 2006, to December 2008. This study was performed to evaluate the relation of the CACS with Cadiovascular disease(CVD) risk factors and FRS. CACS and FRS was a significant difference between the group whose calcium score was 0 and the group whose calcium scores were 1 in case of men 2.38(95% CI, 1.83-3.11), women 2.12(95% CI, 1.03-4.35). The age-and sex-adjusted odds ratios for predictor of CVD risk factors to women with age was 1.10(95% CI, 1.06-1.15), HDL-cholesterol was 2.38(95% CI, 1.04-5.44), Fasting plasma glucose was 2.89(95% CI, 1.16-7.21), to men with age was 1.11(95% CI, 1.08-1.14), LDL-cholesterol was 2.12(95% CI, 1.28-3.50), gamma-GTP was 1.73(95% CI, 1.17-2.55), Diabetes mellitus medication was 3.92(95% CI, 1.73-8.89). The CACS seems to be a siginificant factor to evaluate the CVD risk factors.

  • 요즈음은 인터넷이나 대중매체를 통해 일반인들도 다양한 질병에 대한 많은 정보를 얻을 수 있다. 이에 속하는 것으로 당뇨병은 대표적 만성질환이며 누구나 한번쯤은 들어 본 적이 있는 질환일 것이다. 현대와 같이 고도로 의학이 발달한 상황에서도 만성질환은 완치되는 것이 아니고 평생 관리를 해야하는 질병이기 때문에 많은 환자들이 살아가는 동안 서서히 지치게 되고 관리를 소홀히 하게 됨으로써 당뇨병성 신경병증, 망막증, 신증, 대혈관병증(뇌졸중, 심혈관질환, 말초혈관질환) 등과 같은 합병증의 발생으로 일상생활에 지장을 받게 된다. 따라서 당뇨병환자는 일생 주기적으로 검사와 함께 자신에게 맞는 약을 복용하고 경우에 따라서는 인슐린 주사까지 맞음으로써 지속적인 혈당관리를 하여 합병증의 발생을 최소화 하는 것이 필요하다. 이상에 언급한 다양한 합병증 중 가장 중요한 사망원인은 심혈관질환으로 40세 이후에 발생한 당뇨병환자에서 전체 사망 원인의 50% 이상을 차지한다. 또한 대부분의 당뇨병성 만성합병증이 적극적인 혈당관리로 효과적인 예방이 가능한 반면 심혈관질환과 같은 대혈관합병증은 혈당조절이 잘 되고 있는 환자에서도 당뇨병 발생 기간과 무관하게 발생하고 또 여러 혈관에 걸쳐 다발적이고 광범위하게 발생하므로 원인이 되는 다른 위험요소들에 대한 관리가 함께 이루어져야 한다.

Cost-Sensitive Learning for Cardio-Cerebrovascular Disease Risk Prediction (심혈관질환 위험 예측을 위한 비용민감 학습 모델)

  • In this study, we propose a cardiovascular disease prediction model using machine learning. First, a multidimensional analysis of various differences between the two groups is performed and the results are visualized. In particular, we propose a predictive model using cost-sensitive learning that can improve the sensitivity for cases where there is a high class imbalance between the normal and patient groups, such as diseases. In this study, a predictive model is developed using CART and XGBoost, which are representative machine learning technologies, and prediction and performance are compared for cardiovascular disease patient data. According to the study results, CART showed higher accuracy and specificity than XGBoost, and the accuracy was about 70% to 74%.

Effects of Blood Factors on Coronary Artery Calcification Scores (혈액인자가 관상동맥 석회화 수치에 미치는 영향)

  • Coronary artery calcification is associated with cardiovascular risk factors and metabolic syndrome, and several studies have already reported that coronary artery calcification score are closely related to the amount of atherosclerotic plaques. This study was conducted on 109 patients who underwent coronary calcium CT who visited the comprehensive health examination center in Daegu city during the period from December 2020 to February 2021. we would like to investigate the relationship between coronary artery calcification score and blood factors. As a result of the study, the abnormal group increased the risk of calcification by 1.113 times compared to the normal group in the waist circumference factor. In the fasting glucose factor, the abnormal group increased the risk of calcification by 1.036 times compared to the normal group, and in the triglyceride factor, the abnormal group was normal. As the risk of calcification increased 1.008 times compared to the group, the waist circumference factor, fasting glucose factor, and triglyceride factor were found to be factors affecting coronary artery calcification score. The risk of developing calcification is primarily associated with waist circumference, anemia and triglycerides, and health care and health checks are expected to help reduce the incidence of cardiovascular disease and reduce medical costs.
