Sling Exercise Therapy (S-E-T) : A total concept for exercise and active treatment of musculoskeletal disorders (슬링운동치료(Sling Exercise Therapy) : 근골격계 질환에 대한 능동적 운동과 치료의 총체적 개념)

  • 슬링 운동 치료(Sling Exercise Therapy)는 근골격계 질환 환자의 영구적인 치유를 목적으로 슬링을 이용한 능동적인 치료와 운동법을 체계화한 접근법이다. 이 개념은 10 여년 전부터 노르웨이의 물리치료사들을 중심으로 발전되어 왔으며, 현재 뇌졸중 환자와 신경학적 질환자 그리고 소아 환자와 건강 증진 프로그램에서도 이용되는 방법들이 개발되어지고 있다. 현재 정형물리치료 분야에 대한 세계적인 흐름은 전기치료 기구나 물리치료사의 손에 의해 수동적으로 이루어지는 치료 접근에서 점차 환자가 스스로 물리치료사에 의해 특별히 고안된 운동방법에 따라 능동적인 치료적 운동을 통해 손상 치료와 건강 관리를 하는 방향으로 발전되어지고 있으면 많은 치료 분야에서 이러한 접근법을 채택하고 있다. 슬링운동치료 개념은 이러한 능동적인 운동법을 주된 목적으로 이용하고 있다. 이 논문은 근골격계 질환의 치료를 중심으로 쓰여졌다. SET 개념에는 체계적인 진단과 치료의 요소들이 포함되어져 있다. 진단 체계에는 열린 사슬과 닫힌 사슬을 이용하여 단계적으로 부하를 가해 줌으로서 근육의 상태를 검사하는 방식을 가지고 있다. 치료적 체계를 구성하는 요소에는 근육의 안정화, 감각운동 훈련, 열린 사슬과 닫힌 사슬을 이용한 훈련법, 가동성있는 연부 조직의 역동적 훈련, 심폐 강화 훈련, 단체 훈련, 가정에서의 개별적 훈련과 추후 관리 그리고 운동 프로그램을 제공하기 위한 컴퓨터 소프트웨어가 포함되어 있다.

Impact of Awareness and Educational Experiences on Cardiopulmonary Resuscitation in the Ability to Execute of Cardiopulmonary Resuscitation among Korean Adults (한국 성인에서 심폐소생술에 대한 인지, 교육경험이 그 시행능력에 미치는 영향)

  • This study was performed to identify the impact of awareness and educational experiences on cardiopulmonary resuscitation in the ability to execute of cardiopulmonary resuscitation among Korean adults. This study used original data of 2014 Community Health Data Survey. 228,712 participants in this survey were resident in South Korea who is aged 19 or older on July 2014. Participants in this survey were sampled an average of 900 residents(target error ${\pm}3percent$) per community health center of Korea. Data were analyzed by using R 3.1.3 employing chi-squared test, fisher's exact analysis, and logistic regression analysis. Ability to execute CPR was significantly higher in males(3.34 time), higher the education level (1.61 times), the white color occupation (1.14 times), the higher the income level (1.07 times), the higher the education level (0.91 times), non-hypertensive patients (1.12 times), non-diabetic patients (1.16 times), non-dyslipidemic patients (0.86 times), non-stroke patients (0.30 times), CPR education experience group (3.25 times), CPR experience group with manikin-based training (4.30 times), higher subjective health status (1.08 times, 1.16 times) respectively. This study identified that awareness, educational experience, and mannequin-based learning experience of CPR impacted on the ability to execute CPR. Responding to education-related factors could contribute to reducing the rate of out-of-hospital acute cardiac arrest by improving the ability to execute CPR of the general public.

  • Chest X-ray images is the most common and widely used in clinical findings for a wide range of anatomical information about the prognosis of the disease in patients with cardiopulmonary rehabilitation. Many analysis algorithm was developed by a number of studies regarding the region segmentation and image analysis, there are specific differences due to the complexity and diversity of the image. In this paper, a diagnosis support system of the chest X-ray image based on image processing and analysis methods to detect the cardiopulmonary disease. The threshold value and morphological method was applied to segment the pulmonary region in a chest X-ray image. Anatomical measurements and texture analysis was performed on the segmented regions. The effectiveness of the proposed method is shown through experiments and comparison with diagnosis results by clinical experts to show that the proposed method can be used for decision support system.

  • Cross contamination between a patient and rescuer or CPR trainees can occur when performing mouth to mouth ventilation during cardiopulmonary resuscitation (CPR). On the other hand, there has been a lack of research on the filtration efficacy of face shields that are designed to protect people from cross-contamination. This study aims to secure the safety of rescuers from communicable diseases in pre-hospital emergency settings and CPR trainees by verifying the protective effects of face shields. The FA shield and CM Shield were used to verify the safety. The bacteria collected from filters used by CPR trainees were incubated. These incubated bacteria were smeared onto the new filters, and were then blown out through the filters using a Bag Valve Mask (BVM) and the pathogens at the front and the back of the filters were checked. While the FA shield was effective in preventing the transmission of pathogens, the CM shield did not prevent the transmission of pathogens. Therefore, some of face shields that received national certification are ineffective in preventing cross-contamination. Accordingly, it is necessary to verify the safety of other face shields used domestically.

  • Heart-lung transplantation is a widely accepted treatment for Eisenmenger'syndrome. The patient is a 41-years-old male diagnosed with Eisenmenger'syndrome due to patent ductus arteriosus. The pressures were checked as follows: aorta 130/80 mean 100 mmHg, pulmonary artery 130/80 mean 109 mmHg, and right ventricle 130/20 mmHg, right atrium mean 20 mmHg. The patient needed heart-lung transplantation due to enlarged right pulmonary artery (diameter 7.5 cm). The donor was a 24 years-old male diagnosed as brain death due to subdural hematoma. Ligation of patent ductus arteriosus was performed under the cardiopulmonary bypass followed by heart-lung transplantation. Patient was extubated on postoperative day one, transferred to the general ward on day 3, and was discharged on postoperative day 33. Cardiac and lung biopsy was performed on postoperative day 41 with no signs of rejection.

  • Since the first report of coronary artery bypass grafting (CABG) in patients with end-stage renal disease in 1974, numerous reports have documented the feasibility of CABG in patients with chronic renal diseases. Patients with chronic renal failure often have comorbid disorders such as hypertension, and diabetes mellitus, each with their own complications and associated impact on both short and long-term survivals. In addition, infection and sepsis have been identified as significant causes of morbidity and mortality in most series of patients with end-stage renal disease undergoing cardiac surgical procedure. As a result of these and other factors such as perioperative volume and electrolyte disturbances, patients with chronic renal failures are at an increased risk of complication and mortality after CABG. We report 3 cases of "Off-pump" CABG in the chronic renal failure patients. patients.

Reversals in Decisions about Life-Sustaining Treatment and Associated Factors among Older Patients with Terminal Stage of Cardiopulmonary Disease (만성 심폐질환을 가진 말기 노인 환자의 연명의료 의사결정의 번복 및 관련 요인)

  • Purpose: The purpose of this study was to investigate the frequency, patterns, and factors of reversals in decisions about life-sustaining treatment (LST) among older patients with terminal-stage chronic cardiopulmonary disease. Methods: This was a retrospective correlational descriptive study based on medical chart review. De-identified patient electronic medical record data were collected from 124 deceased older patients with terminal-stage cardiopulmonary disease who had made reversals of LST decisions in an academic tertiary hospital in 2015. Data were extracted about the reversed LST decisions, LST treatments applied before death, and patients' demographic and clinical factors. Multivariate logistic regression analysis was used to identify the factors associated with the reversal to higher intensity of LST treatment. Results: The use of inotropic agents was the most frequently reversed LST treatment, followed by cardiopulmonary resuscitation, intubation, ventilator therapy, and hemodialysis. Inconsistency between the last LST decisions and actual treatments occurred most often in hemodialysis. One-third of the reversals in LST decisions were made toward higher intensity of LST treatment. Patients who had lung diseases (vs. heart diseases); were single, divorced, or bereaved (vs. married); and had an acquaintance as a primary decision maker (vs. the patients themselves) were significantly more likely to reverse the LST decisions to higher intensity of LST treatment. Conclusion: This study demonstrated the complex and turmoil situation of the LST decision-making process among older patients with terminal-stage cardiopulmonary disease and suggests the importance of support for patients and families in their LST decision-making process.

  • Cardiac arrest is a series of conditions that occur when the heart is stopped, regardless of the cause. Cardiac arrest due to heart disease is included in the cause of death in korea every year and is unpredictable. One of the only ways to save a patient's life when a cardiac arrest is cardiopulmonary resuscitation is to maintain circulation through this procedure. Therefore compared the quality of chest compressions with visual information using mirrors. There was a significant difference in the mean depth of chest compressions(48.93±6.76, 53.86±4.56, <0.001), and there was also a difference in compression to relaxation ratio(0.87±0.13, 0.96±0.10, <0.002). There was also a significant difference in attitude awareness(4.93±0.85, 8.14±1.38, <0.001).

The Clinical Experience of The Descending Thoracic and Thoracoabdominal Aortic Surgery (하행대동맥 및 흉복부 대동맥 수술의 임상적 경험)

  • Background : The thoracic and thoracoabdominal aortic surgery is a complicated procedure that has various method of approach and protection. The authors have performed several methods to treat these diseases. Therefore, we attempt to analyze their results and risks. Material and Method: From June of 1992 to August of 2001, we performed 26 cases of thoracic aortic surgery and 10 cases of thoracoabdominal aortic surgery. There were 17 aortic dissections, 17 aortic aneurysms, one coarctation of aorta and one traumatic aortic aneurysm. The thoracic aortic replacement was performed under a femorofemoral bypass, an LA to femoral bypass, or a deep hypothermic circulatory arrest. The thoracoabdominal aortic replacement was performed under a femorofemoral bypass or a pump assisted rapid infusion. Result: There were 7 renal failures, 11 hepatopathies, 7 cerebral vascular accidents, 2 heart failures, 5 respiratory insufficiencies, and 2 sepsis in postoperative period. There were 9 hospital mortalities which were from 2 bleedings, 2 heart failures, 2 renal failures, a sepsis, a respiratory failure, and a cerebral infarction. There were 3 late deaths which were from ruptured distal anastomosis, cerebral infarction, and pneumonia. Conclusion: Deep hypothermic circulatory arrest was not good supportive methods for thoracic aortic replacement. Total thoracoabdominal aortic replacement was a high risk operation.

The Comparison of Clinical Study of Off Pump and On Pump CABG (On Pump-CABG와 Off Pump-CABG의 임상적 고찰에 관한 비교연구)

  • In an attempt to avoid the adverse effects of the cardiopulmonary bypass, off pump coronary artery bypass grafting(Off pump CABG) that has recently been rediscovered and refined. We compared the preoperative risk factors and in-hospital outcomes of patients done Off pump with those done On pump CABG. Material na Method: One hundred seventy eight patients was underwent CABG between January 2001 and July 2001 12 patients whom underwent associated valvular or left ventricular volume reduction surgery were excluded in this study Data were collected for 52 Off pump CABG and 114 On pump CABG for patient and disease risk factors, extent of coronary disease, and in-hospital outcomes. Result: Off pump CABG and On pump CABG groups did not show any differences in their patient and disease risk factors, and extent of coronary disease. Off pump CABG group had significantly lower mean operation time(234 $\pm$ 37 min vs 290 $\pm$ 48 min, p<0.001), lower mean CK-MB level(10.1 $\pm$ 13.5 IU/L vs 33.1 $\pm$ 18.2 IU/L, p<0.001) and mean ventilation time(14.8 $\pm$ 3.5 hours vs 16.2 $\pm$ 4.9 hours, p=0.048) than On pump CABG groups. On pump CABG group had significantly more distal grafts(3.4 $\pm$ 0.9 vs 2.6 $\pm$ 0.8, p<0.001) than Off pump CABG groups. There were no operative mortality in two groups. Off pump) CABG and On pump CABG groups did not show any differences in their postoperative complications and outcomes including perioperative myocardial infarction, stroke, respiratory failure, renal failure, reoperation, the amount of bleeding, the need of intraaortic balloon pump, the need of inotropics, and the stay of intensive care unit and hospital. Two patients were converted to On pump CABG. Conclusion: This study showed that patients having Off pump CABG are not exposed to a greater risks of adverse outcomes and also provided evidence that patients having Off pump CABG have significantly lower operation time, CK-MB, ventilation time and less distal grafts. Although there may be potential benefits to Off pump CABG, further studies must be directed to determine those patients who would benefit from Off pump CABG.
