A Case of Cardiac Lymphoma Developed in Right Atrium (우심방에 발생한 심장 임파종 -1례 보고-)

  • 원발성 심장 림프종은 드문 질환으로 원발성 심장종양의 1.3%를 차지하며 절외성 림프종의 0.5%에서 발생한다. 그러나 악성 림프종에 의한 이차성 심장 전이는 비교적 빈번해서 약 8.7-27.2%로 보고 되고 있다. 66세남자 환자는 호흡곤란을 주소로 내원하였다. 경흉부 심초음파에서 우심방내에 종괴가 관찰되었다. 종양의 수술적 제거를 시행하였고 조직학적 검사에서 우심방을 침범한 악성 임파종으로 진단되었다. 환자는 급성 종격동염이 발생하였고 술후 9일째 패혈증과 수술부위 출혈로 사망하였다.

  • Primary malignant lymphoma of lung is a very rare disease, only 0.34% of all malignant lymphomas. In our case, a 76 year old male patient had a solitary mass without lympha- denopathy at chest CT scan. He underwent right middle lobectomy through a posterolateral thoracotomy incision. Pathologic study confirmed a diagnosis of malignant lymphoma and chemotherapy was started by an oncologist.

  • The Iymphoproliferative disease after the organ transplantation is more commonly seen with the increase according to the increasing number of the organ transplantations and it occurs more frequently in the cases of heart and lung transplantations that needs more aggressive immunosuppression. It demands urgent evaluation and management because of poor prognosis. We transplanted left lung of a man to the woman who suffered from severe dyspnea due to terminal pulmonary emphysema in discrepancy of ABO blood type. Postoperatively, We used triple regimen immunotherapy(cyclosporin, azathioprine, prednisolone) and followed up in the out patient clinic. During the follow up, we found abnormal mass lesion on the transplanted lung and performed gun biopsy. We confirmed malignant lymphoma on the pathollgic examination and two cycled chemotherapy was given after reducing dose of immunosupression. The patient died of sudden onset of pulmonary edema of the transplanted lung.

  • Malignant lymphoma in chest wall is a rare form of extranodal variety as occasionally localized tumor. The primary site is suggested from rib or soft tissue, but the incidence in rib is higher than soft tissue. The patient, a male, aged 32, noted a swelling and pain over the left lower posterior chest from 3 months prior to admission. Roentgenograms revealed bony destruction in left 9, & 19th ribs posteriorly and associated chest wall mass which was ill defined uncleared margination. Under diagnosis of malignant tumor, the operation was performed and the tumor mass resected widely with en bloc technique than the fascia lata was grafted at chest wall defect. The histology of specimen was disclosed as malignant lymphoma, non-Hodgkin`s diffuse lymphocytic poorly differentiated form. The radio-and chemotherapy were combined post-operatively and the complete remission is acquired.

Lymphangioma of the Chest Wall (흉벽의 임파관종)

  • Lymphangioma(or cystic hygroma) of the chest wall is rare case. We experienced one such case in a 16-year-old girl who complained of a large recurrent mass on her right upper post erolateral chest wall which had developed several years ago. The diagnosis was made following a physical examination, chest magnetic resonance imaging(MRI), and radio isotope (RI) lymphangiogrphy and was confirmed by a histopathological examination. We performed total excision of the lesion followed by a repeated sclerosing therapy with intralesional injection of Vibramycin.

Disseminated Intravascular Coagulation in Experimental Fowl Cholera of Chickens (닭의 가금(家禽) 콜레라 감염시(感染時)의 파종성(播種性) 혈관내(血管內) 응고증(凝固症))

  • Chickens from 10 to 32 weeks of age were inoculated with P. multocida via seven routs(intravenous, intramuscular, intraperitoneal, subcutaneous, into ear, intranasal, per oral). The development or distribution of disseminated intravascular coagulation (DIC) in multiple organs and the role of P. multocida endotoxins in disease process of fowl cholera were studied. The histological diagnosis of DIC was made by demonstration of fibrinous in arterioles, capillaries, venules and medium-sized blood vessels. The presence of fibrinous thrombi in blood vessels of multiple organs was observed in chickens which died within approximately 3 days post inoculation. Fibrinous thrombi were observed most frequently in the lung(90% of all cases with DIC) followed by liver (70%), kidney (60%), heart(20%), spleen, brain, pancreas, thymus and thyroid gland. The density of fibrinous thrombi (i.e. the number of thrombi per section) was greatest in the lung, followed by spleen, kidney, liver and heart. It is thought that the widespread hemorrhage of acute fowl cholera is also caused by P. multocida endotoxin which initiates DIC in variety of organs. The cause of death for the chickens after infection with acute fowl cholera is probably due to an endotoxin (septic) shock accompanied with DIC in multiple organs.

Clinical Evaluation of the Primary Mediastinal Tumors and Cysts (원발성 종격동 종양 및 낭종의 임상적 고찰)

  • This report is an analysis of 43 cases of primary mediastinal tumors and cysts which were treated in the Department of Thoracic and Cardiovascular Surgery, Tae Jeon Eul li General Hospital from January, 1986 to April, 1995. There were 19 males and 24 females, and their ages ranged from 4 years to 68 years, with mean age of 31.3 years. The most common symptoms were dyspnea 48.8%, chest disconyort 39.5%, cough )4.9%, and chest pain 23.3%, and the most frequently encountered location of tumor was anterior mediastinum followed by posterior, and middle mediastinum. From the pathologic viewpoint, teratoma was the most frequent type followed by neurogenic tumors, cysts, thymoma, and Iymphoma. The most useful diagnostic method was plain X-ray examination of the chest, and final diagnosis could usually be made only after an operation. All of benign tumors were completely removed and malignant tumors were treated with chemotherapy and radiotherapy after operation There were 7 (16.3 %) cases of postoperative complications, and there was no postoperative mortality.
