The Structure of the Ventricle in the Heart of the Pond Smelt, Hypomesus nipponensis (Osmeridae) (바다빙어과 빙어 (Hypomesus nipponensis)의 심실 구조)

  • The structure of the ventricle in the heart of Hypomesus nipponensis was investigated by light and scanning electron microscope. The heart consisted of four consecutive chambers, the sinus venosus, atrium, ventricle and bulbus arteriosus. The wall of the ventricle was divided into endocardium, myocardium, subepicardium and epicardium. The valves were observed in the artrioventricular and bulboventricular junctions. The ventricular myocardium was an entirely spongy without coronary vessels. The trabecular network was formed with lumina included a central lumen and the trabecula was cylindrical shape. Collagen distribution was apparent in the subepicardium, artrioventricular valve and bulboventricular valve. But in the trabeculae, collagen distribution was observed partly in the base of the ventricle. Especially, the endocardial bridges were observed between trabeculae. These results might be considered that the structure of the ventricle in the heart of pond smelt is adapted to sedentary habit associated with its habitat and lifestyle.

  • A ventricular assist device (VAD) is used for bridge to heart transplantation and heart diseases. Knowing the status of a pneumatic pulsatile VAD when implanting it into the body is important: when the velocity of blood flow through the VAD is slow, a thrombus may occur, and thrombosis can be fatal to a patient. In order to determine the state of a VAD, various sensors need to be implanted. Because this introduces the risk of infection and difficulties with sensor management, we developed a method for estimating the state of a VAD indirectly via the pressure in an air tube that can be measured in vitro. We compared the measured values to in vitro experimental results. The estimated and measured values showed some errors, but the accuracy can be improved by refining the estimation process to minimize the risk of infection.

Isolated Right Ventricular Hypoplasia -A case report- (독립된 우심실 형성부전 - 1예 보고 -)

  • Isolated right ventricular hypoplasia is a rare clinical entity. We describe a case of right ventricular hypoplasia, single atrium and spongy myocardium of left ventricle. The volume of right ventricle was half the volume of left ventricle and z-value of tricuspid valve was -4 preoperatively The patient, 6-year-old boy, underwent atrial partitioning with 3 mm fenestration, Postoperative course was smooth and he tolerated the biventricular state well during follow-up. Follow-up catherterization was done 27 months later The tricuspid valve grew well (z-value= -0.4) and atrial septal fenestration is closed spontaneously. This article reports a case of successful biventricular repair in a patient with isolated right ventricular hypoplasia.

Reoperation of Postinfarction VSD (심근경색증의 합병증으로 발생한 심실중격 파열의 재수술)

  • After early surgical repair of ventricular septal rupture complicated by myocardial infarction, newly developed ventricular septal defects were discovered on the 7th and the 40th postoperative day in 2 cases. We reoperated these patients for closure of newly developed ventricular septal defects with Dacron patch successfully on the 77th and the loth day after discovery of those defects. From these results we conclude that early surgical rep ir for ruptured ventricular septum following myocardial infarction is an appropriate approach with low risk and that remnant or recurred ventricular septal defect can be corrected rather safely with proper myocardial preservation and unrestricted application of intraaortic balloon pump or other ventricular assist device.

Surgical and Long Term Results for Double Outlet Right Ventricle by the Type of Ventricular Septal Defect (심실중격결손의 형태에 따른 양대혈관 우심실기시증의 수술 및 장기 결과)

  • The results of biventricular repair for double outlet right ventricle have been improved in recent series. We studied the surgical and long term results for total correction of double outlet right ventricle by the type of ventricular septal defect. Material and Method: Between November 1979 and December 2003, 126 patients had biventricular repair for double outlet right ventricle. The mean age was 1.8 years (range 1$\~$44) and 86 patients ($68.3\%$) were male. We classified and studied this disease by the type of VSD. Result: The locations of VSD were subaortic in 79 ($62.7\%$), subpulmonary in 17 ($13.5\%$), doubly committed in 16 ($12.7\%$) and noncommitted in 14 ($11.1\%$). 28 patients had palliative operation before total correction and the mean interval to total correction was 41.0$\pm$45.1 months. The methods of total correction were intraventricular baffling in 37 ($29.4\%$), intraventricular baffling with patch enlargement of right ventricular outflow tract in 49 ($38.9\%$), intraventricular baffling with Rastelli procedure in 15 ($11.9\%$), arterial switch operation in 8 ($6.3\%$) and REV procedure in 4 ($3.2\%$), etc. Hospital mortality rate was $10.3\%$ (13 patients) and 25 reoperations were performed in 24 patients ($19.0\%$). The risk factors for hospital mortality and reoperation were cardiopulmonary bypass time (p=0.020) and previous palliative operation (p=0.013), respectively. Follow up was possible in 98 patients and mean follow up period was 118.9$\pm$70.7 months. The percent survival and survival for freedom from reoperation at 15 years were $82.5\%$ and $66.7\%$, respectively. The survival rate was significantly lower (p=0.003) in transposition of great artery type and remote type than in simple ventricular septal defect type and tetralogy of Fallot type, but there was no statistical differences in survival rate for freedom from reoperation. Conclusion: It is thought to be that acceptible surgical and long term results can be obtained with application of appropriate methods of repair for double outlet right ventricle.

인공심장의 연구개발 현황

  • 국내에서 개발중인 인공심장으로는 서울대학교 공과대학 기계설계학과와 서울대학교병원 의공학과에서 공동으로 개발중인 전기-기계식 인공심장이 있다. 모우터가 내장된 실린더가 모우터의 정.역전에 따라 통 안에서 좌.우로 움직이면서 좌.우심실을 교대로 압축한다. 모우터가 들어 있는 실린더의 위치가 통 안에서 고정되어 있지 않고 움직이므로, 한쪽 심실이 팽창할 때 실린더가 다른 한쪽 심실 공간을 차지하여 결국 전체 체적은 하나의 심실 공간과 모우터 공간만 있으면 되므로 모우터의 위치가 고정된 형식에 비해 체적 감소가 가능하다. 에너지 변환기를 구성하는 기계요소로는 2단의 유성기어열과 랙(rack)뿐으로서 비교적 구조가 간단하여 고장의 가능성이 낮다. 따라서 기존 모델들이 지니고 있는 단점들이 거의 극복 되었다. 현재 모의 순환실험에 성공하고 동물 실험을 준비 하고 있다.

Postcardiotomy Ventricular Assist Device (개심술후 심실 보조장치)

  • 의료기술의 발달로 오늘날의 개심술은 대부분 성공적으로 시행되고 있지만 극히 일부분은 아직도 개심술후 심한 심실기능의 저하로 사망하는 겨우가 있다. 1960년대 중반 Spencer와 DeBakey에 의해 개심술후 심한 심실기능이 저하된 환자에게 심실보조장치를 이용하여 성공적으로 치험한 이후로 많은 발전을 거듭해 왔다. 저자들은 4개월 (6.5Kg)된 남아에서 개심술후 심한 저심박출증으로 인공심폐기의 제거를 못하여 좌심실 보조장치(Centrifugal Biopump)를 사용하여 성공적으로 치험하였기에 보고하는 바이다.

Effect of Simple VSD Repair on Doppler-Derived Right Ventricular Systolic Time Interval (심실중격결손 봉합이 우심실 수축기 시간 간격에 미치는 영향)

  • Background: Ventricular septal defect(VSD) that causes pulmonary hypertension increase right ventricular workload. Echocardiographic assessment of right ventricular systolic time interval (RVSTI) has been used to predict pulmonary artery pressure in various cardiopulmonary diseases. This study was undertaken in infants with simple VSD to observe the alteration of the right ventricular workload through the changes of RVSTI after repair of VSD. Material and Method: We evaluated heart rate, the ratio of the left atrium/aortic root diameter (LA/Ao), right ventricular pre-ejection period(RVPEP), right ventricular ejection time(RVET), and its ratio(RVPEP/RVET) as a predictor of right ventricular workload in 12 children with simple VSD. These were measured three times at the preoperative period, at the 3 month and between 6 month and 1 year(average 9.5${\pm}$1.8month) after repair of VSD by M-mode & Doppler echocardiograph from the pulmonic valve echogram. Result: Heart rate was decreased significantly after repair(137.1${\pm}$13.7 vs 114.4${\pm}$21.1 and 104.1${\pm}$10.2, p<0.01). LA/Ao ratio was decreased significantly after repair(1.71${\pm}$0.32 vs 1.47${\pm}$0.33 and 1.39${\pm}$0.23, p<0.05). RVPEP/RVET were decreased after repair (0.38${\pm}$0.09 vs 0.32${\pm}$0.08 and 0.29${\pm}$0.09, p<0.01). Heart rate corrected RVPEP/RVET were significantly decreased only after 6 months(0.32${\pm}$0.03 vs 0.30${\pm}$0.05 and 0.28${\pm}$0.06, p<0.05). Conclusion: We found elevated right ventricular workload was progressively decreased until more than 6 months after repair and the RVSTI may serve a useful guide in postoperative care for children with VSD.

Development and Animal Tests of Pneumatic Ventricular Assist Device (공압식 심실보조장치의 개발 및 동물실험)

  • Ten acute and chronic in vivo studies were utilized to develop a pneumatic ventricular assist device (VAD) as a bridge to heart transplantation or a circulatory assist device for patients with end-stage heart disease or poor myocardial funct on after cardiac surgery. Two sizes of blood pump of 70cc for adult patients and 34cc for pediatric patients were implanted in the animals. Ventricle of the blood pump was made from the polyurethaae to enhance antithrombogenecity. The VAD was implanted between the left atrium and the descending aorta. Average flow rate was 2.38 L/min for adult and 0.41 L/min for pediatric VAD at the rate of 60 bum. The duration of support ranged from 1 to 26 hours. The most frequent complication was bleeding. Main causes of death were heart failure and respiratory failure. The device function was good for short term use. Studies to date suggest that, with further refinement, a reliable long term VAD that will have clinical application can be developed.
