Optimum Parameter Design for Defibrillator (제세동기 최적 파라미터 설계)

  • In designing defibrillator, several parameters such as patient's transthoracic impedance, output energy level, peak current, and time duration of current waveform must be considered to generate optimum electrical shocks on the heart. Patient's transthoracic impendence depends on the physical and health condition of patient. In this study, before the development of a defibrillator, the range of above parameters value as circuit elements was determined to derive optimal waveform by predicting and analyzing the performance of designed circuit by means of simulation with the software, P-Spice. The efficiency of parameter design was verified through the performance test with the developed defibrillator.

  • A 56-year-old female with a permanent unipolar pacing(VVI) system underwent elective coronary bypass surgery and mitral valve replacement. Because the unipolar sensing is subject to considerable nonmyocardial electrical noise, the pacing function may be disturbed by the use of electrocautery. Temporary atrial and ventricular bipolar epicardial leads and external generator were used for maintaining A-V sequential pacing during the use of electrocautery. Before aortic cross-clamp was released after cardiac operation, regular cardiac rhythm could spontaneously be resumed with an integrated cardioplegic strategy, avoiding the use of defibrillator

  • In this paper, an automatic external biphasic defibrillator that removes ventricular fibrillation efficiently with a low discharging energy has been developed. The system is composed of software including a fibrillation detection algorithm and a system control algorithm, and hardware including a high voltage charging/discharging part and a signal processing part. The stability of the developed system has been confirmed through continuous charging/discharging test of 160 times and the detection capability of the real-time fibrillation detection algorithm has been estimated by applying a total of 30 various fibrillation signals. In order to verify the clinical efficiency and safety, the system has been applied to five pigs before and after fibrillation inductions. Also, we have investigated the system efficiency in removing fibrillation by applying two different discharging waveforms, which have the same energy but different voltage levels.

  • 이 연구는 하트세이버를 배지를 수여 받은 구급대원과 심정지 환자 중 심폐소생술로 자발순환이 회복된 후 환자를 대상으로 심정지 생존의 요인을 파악하고 분석한 결과 심정지에서 소생한 환자의 75%가 가정에서 발생되었으며, 출동에서 현장까지 도착 소요시간이 6.1분, 현장심폐소생술 소요시간 6.2분, 출동에서 병원 응급실 도착까지 소요시간 23.0분이 소요되었다. 또한 목격자에 의해 시행된 심폐소생술은 50.0%, 발견 당시 심장리듬은 심실세동이 87.5%, 무수축 12.5%였고, 심폐소생술과 동시에 자동제세동기를 사용한 환자는 87.5%, 심폐소생술만 시행한 환자 12.5%, 기관내삽관 시행이 12.5%였다. 자발순환이 회복된 환자에게 시행된 심폐소생술 소요시간은 10.9분 이었으며, 책임구급대원은 1급응급구조사가 75.0%, 출동 구급인원은 3명 출동이 87.5%였다.

  • In August 2000, the American Heart Association and the European Resuscitation Council published the conclusions of tile International Guidelines 2000 Conference on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care which contains both the new recommendations and an in-depth review. The most important changes in the recommendations according to the authors are discontinuation of the pulse-check for lay people, 500 ml instead of 800∼1,200 ml tidal volume during bag-valve-mask ventilation (FiO2 > 0.4) of a patient with an unprotected airway, unifying correct endotracheal intubation size as 8.0 mm, vasopressin (40 units) and epinephrine (1 mg) as comparable drugs to treat patients with ventricular fibrillation, early prehospital survey and intravenous lysis for patients who have suffered coronary artery syndrome and stroke.

Clinical Characteristics and Prehospital care in Prehospital Cardiac Arrest Patients by Paramedic's Reports (구급일지를 통한 병원전 심정지 환자의 임상적 특성과 병원전 응급처치)

  • In order to report characteristics of out-of-hospital cardiac arrest patient in whom 119 rescuers used prehospital care by Paramedic's Reports. 1,016 out-of-hospital cardiac arrest patients were transferred to hospitals by 119 rescuers between January 1st and December 31st, 2008. Prehospital reports of 983 cardiac arrest patients by 119 were analyzed. Shockable rhythm with AED use was 20.3%(VF 18.4%, VT 1.9%), then 66.5% of shockable cardiac arrest patients was resuscitated by AED. Bystander basic life support was 14.8%. There were significant differences in the recurrent survival rates between shockable rhythm and non-shockable rhythm(13.0% vs 2.0%,7.4%, p=0.000). There was also significant differences in the recurrent survival rates between adequacy rate of AED(21.6% vs 2.4%, p=0.000). But there was no significant differences in the recurrent survival rates between done bystander CPR and none(9.0% vs 5.5%, p=0.10). The performance of bystander CPR and usage of AED, and appropriate CPR done by 119 rescuers were unsatisfactory by paramedic's reports. To improve the adequacy of Basic life Support and to increase the performance on Advanced Life Suppport, we must challenge to develop the emergency medical systems.

  • Cardiopulmonary resuscitation (CPR) is an emergency treatment that stimulates blood circulation and breathing when the function of the heart stops or stops breathing. CPR can be divided by basic life support (BLS) and advanced life support (ALS). BLS involves the use of chest compression to force the blood flow to the main organs, rescue breathing to improve the breathing to the respiratory failure patient and the use automated external defibrillator (AED). The categories of advanced life support include advanced cardiovascular life support (ACLS) for adult and pediatric advanced life support (PALS) for children. In the treatment of dental care for children, which is extremely difficult to deal with, and for a variety of reasons, the use of sedation is considered to treat the children who are unlikely to cooperate with dentistry. This is why there is an increasing possibility of an emergency situation involving cardiac arrest. PALS includes the BLS, and it presents a systematic algorithm to treat respiratory failure, shock and cardiogenic cardiac arrest. In order to manage emergency situations in the pediatric dental clinic, respiratory support is most important. Therefore, mastering professional PALS, which includes respiratory care and core cases, particularly upper airway obstruction and respiratory depression caused by a respiratory control problem, would be highly desirable for a physician who treats pediatric dental patients. Regular training and renewal training every two years is necessary to be able to immediately implement professional skills in emergency situations.
