Extraction of an Infected Permanent Pacemaker Lead Using Cardiopulmonary Bypass - 2 case reports - (체외 순환을 이용한 감염된 영구 심박동기 도선의 제거 - 2예 보고 -)

  • Implanting a pacemaker is the most often used intervention for treating bradycardia. The most commonly used pacemaker is the intracardiac pacemaker, yet it can have many complications. An infected pacemaker can spread to systemic infection and the condition of the patient can quickly get worse, so if an infected pacemaker is suspected, then the pacemaker must be removed. Apart from the use of interventional methods such as a loop or a weight, we can take a more aggressive approach by using extracorporeal circulation for removal of the pacemaker. We report here on two cases in which extracorporeal circulation was used to remove the infected pacemakers.

  • A 63-year-old man who had a permanent pacemaker placed 17 years ago presented to us with local inflammatory signs on his chest. The skin was opened to remove the permanent pacemaker. However, the pacing wire was not easily removed. So, the permanent pacemaker was removed from the chest with leaving the remnant wire inside. However, the local infection recurred because of the remnant wire. Right thoracotomy was performed and cardiopulmonary bypass was started. After total circulatory arrest, the right atrium was opened and the wire was completely removed.

  • A 52-year-old female presented with pain and swelling owing to recurrent inflammation on a generator pocket. She had undergone a permanent pacemaker implantation (DDD type) 7 years previously. We planned to insert a new pacemaker after removal of the previous generator and wires through a surgical approach. However, she had a history of the left modified radical mastectomy (MRM) with radiation therapy for breast cancer. For this patient, it would be difficult to care for the postoperative wound if we approached via the median sternotomy. Therefore, we decided to use a right atrial approach via a right thoracotomy. We removed the previous pacing wires through an atriotomy and inserted a new pacemaker using epicardial pacing leads without cardiopulmonary bypass.

  • A 56-year-old female with a permanent unipolar pacing(VVI) system underwent elective coronary bypass surgery and mitral valve replacement. Because the unipolar sensing is subject to considerable nonmyocardial electrical noise, the pacing function may be disturbed by the use of electrocautery. Temporary atrial and ventricular bipolar epicardial leads and external generator were used for maintaining A-V sequential pacing during the use of electrocautery. Before aortic cross-clamp was released after cardiac operation, regular cardiac rhythm could spontaneously be resumed with an integrated cardioplegic strategy, avoiding the use of defibrillator

  • Fungal infection of intracardiac pacing wire is very rare. We experienced a case of patient with functioning transvenous pacemaker lead, inserted 3 years previously, which was completely encased in a large thrombus infected with aspergillus. The lung biopsy also confirmed aspergillus infection.

Study on Recipients' Experience of Adapting to Permanent Pacemakers (영구적 인공심박동기 이식자의 적응 경험)

  • Purpose : This study develops a substantive theory on the recipients' experience of adapting to a permanent pacemaker. Method : The study used the grounded theory method developed by Strauss and Corbin (1998). The participants of the study were 13 adults who had received a permanent pacemaker during the previous year. The study addressed the research question "what is the experience of adaptation in people who had received permanent pacemakers?" From October 2016 to March 2017, data were collected from the participants through in-depth interviews. Results : The core category indicating the essence of the adaptation experience was shown to be "accepting the pacemaker as part of my body and living in line with it." Conclusions : In nursing practice, the results of this study will assist nurses in improving their communications with and developing guidelines or interventions for their clients who have received permanent pacemakers. In the field of nursing education, this study is expected to provide a framework to understand the experiences of future nurses and other healthcare workers working with permanent pacemaker recipients.

  • After Transcatheter Aortic Valve Replacement (TAVR), comprehensive management of complications, including the need for Permanent Pacemaker Implantation (PPI), is crucial, increasing the demand for accurate prediction models. Departing from traditional image-based methods, this study developed an optimal PPI prediction model based on ECG data using the XGBoost algorithm. Focusing on ECG signals like DeltaPR and DeltaQRS as key indicators, the model effectively identifies the correlation between conduction disorders and PPI needs, achieving superior performance with an AUC of 0.91. Validated using data from two hospitals, it demonstrated a high similarity rate of 95.28% in predicting PPI from ECG characteristics. This confirms the model's effective applicability across diverse hospital data, establishing a significant advancement in the development of reliable and practical PPI prediction models with reduced dependence on human intervention and costly medical imaging.

Thirteen-year Experience of Permanent Epicardial Pacing in Children (소아연령군에서의 영구 심외막 심박 조율 13년 경험)

  • Background: We investigated the longevity, thresholds of epicardial pacemaker and causes of reoperation in the pediatric patients who underwent epicardial pacemaker implantation performed during the last 13 years Material and Method: 121 operations were performed in 83 patients from January 1989 to July 2002. We analyzed the stimulation threshold, resistance, R-wave and P-wave, and sensitivity of pacemaker lead at initial implantation. Longevity and causes of reoperations were investigated. Result At implantation, epicardial ventricular mean stimula-tion threshold was 1.2$\pm$0.1 (0.1∼5) mV, mean resistance was 519.1$\pm$18.1 (319∼778) Ohm, and mean R-wave sensitivity was 8.9$\pm$0.7 (4∼20) mV, and mean P wave sensivity was 2.5$\pm$0.7 (0.4∼12) mV. The mean longe-vity of pacemaker generator was 64.7$\pm$3.7 (2∼196) months. The reoperation free rate was 94.6% for 1 year, 93.6% for 2 years, 80.8% for 5years, 63.7% for 7 years, and 45.5% for 10 years. The causes of reoperation were battery waste in 26 cases and lead malfunction in 9 cases. There was no postoperative death related to pacemaker malfunction. Conclusion: in the childrens, average longevity of epicardial pacemaker was within accep-table range. 19.1% of the patients required pacemaker related reoperation. However, recent developments, including steroid eluting lead, 6.7% of the patients required pacemaker related reoperation, look promising in expansion of pacemaker life span.
