Full mouth rehabilitation of mandibular edentulous patient using implant hybrid prosthesis (하악 무치악 환자에서 임플란트 하이브리드형 보철물을 이용한 전악 수복 증례)

  • In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. Implant hybrid prostheses place 4 to 6 implants between the mental foramens, and cantilevers are used to replace the posterior teeth at both ends. The 85 - year - old female patient visited our clinic with complete edentulism in the maxilla and mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable complete denture in the maxilla and implant hybrid prosthesis in the mandible.

  • This is a case report of a 12.5-year-old girl who presented with moderate to severe anterior dental crowding and rotations. Treatment involved no extraction, but expansion of both the maxillary and the mandibular arches. Maxillary expansion was assisted by rapid palatal expansion despite the fact that this patient did not present with posterior crossbite. Crowding and rotations in both arches were corrected and good occlusal function and improved facial esthetic were achieved, with acceptable overbite and overjet. The application of rapid maxillary expansion in cases with no posterior crossbite, which has increased in recent years, calls for re-evaluation of the diagnostic basis and indications for the use of this technique.

  • A major cause of missing permanent incisors is congenital abscence and extraction because of trauma and pathologic condition. The request for restoration of missing or spaced anterior teeth is common in dental practice. Problems, such as the tilting, drifting, and rotation of teeth adjacent to the space, complicate the restoration of apperance, and a normally simple restorative dental procedure may become difficult. There are two primary treatment alternatives to improving a dentition's irregular and spaced apperance-closing the space by orthodontic means or providing a prosthesis to disguise the space. The treatment choice depends on many variables, but, as a general rule, patients with a normal overbite, overjet, and buccal relationship are better treated by maintaining the sapce and providing a prosthesis, either fixed or removable. This case report presents two cases : Traumatic loss of maxillary right and left central incisors, Extraction of malformed mandibular right central inciosr. The loss of central incisor space was regained by the fixed-removable and fixed orthodontic appliance, and then Maryland bridge was cemented.

  • Crouzon syndrome Is one of the craniofacial syndromes characterized by craniosynostosis, midfacial hypolpasia and ocular proptosis. Distraction osteogenesis is becoming Important technique to treat craniofacial dyplaisa. It has nuy advantages compared with standard orthognathic surgery. Maxillary distraction osteogenesis after Le Fort III osteotomy with the RED system presents successful maxillary protraction to Crouzon syndrome patient with severe maxillary deficiency. It also allows remarkable improvement of facial esthetics and respiratory functions.

  • Bell's palsy is an acute-onset unilateral peripheral facial neuropathy. For patients with sequelae of facial paresis, the successful rehabilitation of fully edentulous arches is challenging. This case report described the treatment procedures and clinical considerations to fabricate complete dentures of a patient who showed unilateral displacement of mandible, unilateral chewing pattern and parafunctional jaw movement due to sequelae of Bell's palsy. Gothic arch tracing was used to record reproducible centric relation and lingualized occlusion was performed to provide freedom to move between centric relation and the patient's habitual functional area in fabricating satisfactory dentures in terms of function and esthetics.

  • Impaction with a severely dilacerated root is seldom reported, especially in the maxillary incisor. It is probably because of the high clinical difficulty associated with bringing the dilacerated tooth into proper position, and the high chance of failure due to ankylosis, external root resolution, and root exposure after orthodontic traction. Even the successful cases may need periodontal surgery to improve the unesthetic gingival shape. However, it has previously been reported that an impacted maxillary central incisor was successfully treated by proper crown exposure and orthodontic traction. This article presents a case of an invertedly impacted maxillary right central incisor with a developing dilacerated root, which was aligned into proper position after orthodontic traction composed of two stages of a closed eruption technique.

  • Loss of teeth may not only imply impaired oral function and loss of alveolar bone but is also often accompanied by reduced self-confidence. This results in a larger problem with the fully edentulous patient. The patient introduced in this study showed multiple missing teeth and mobility of remaining teeth and wanted to have fixed dental prosthesis using implants. Remaining teeth were extracted because of periodontally bad prognosis. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using implant hybrid prosthesis in fully edentulous patient.

  • Proper treatment of an impacted tooth is required as it causes functional and esthetic disharmony, as well as it can cause root absorption of adjacent teeth. Treatment options for impacted teeth include periodic observation, surgical exposure, orthodontic traction followed by surgical exposure, tooth transplantation, and extraction. Modified Nance appliance, used for orthodontic traction, is clinically useful because it does not require patient cooperation. Through orthodontic traction combined with surgical exposure of impacted maxillary incisors, canines, and molars using modified Nance appliance, adequate results can be obtained.

A comparative study on the fracture strength of esthetic porcelain crowns (심미도재관의 파절 강도 비교 연구)

  • In this study, We tried to find out the fracture strength of zirconia coping all ceramic crown and collarless porcelain fused gold alloy crown as the compared group. Each of the collarless porcelain fused gold crown and zirconia all ceramic crown has been produced specimen 10 each. And after pasting them on the steel jig, it had load given at porcelain incisal edge by 130$^{\circ}$ and measured fracture strength by Universal Testing Machine and then We have results from that. 1. The average value of fracture strength suggested 950.49 N at collarless porcelain fused gold alloy crown, 656.81 N at zirconia coping all ceramic crown. 2. Fractured pattern showed that the whole of labial part was exposed in collarless porcelain fused gold crown, but porcelain of margin part did not show fracture in advance. In the practice, the concern that collarless might cause fracture strength to be weaken does not matter, we concluded zirconia all ceramic crown made fracture that only a part of labial porcelain was fractured. The combination of zirconia all ceramic crown and turned out to be excellent.

STRESS ANALYSIS OF VARIOUS ESTHETIC RESTORATIONS BY FINITE ELEMENT METHOD (유한 요소법을 이용한 수종 심미 수복물의 응력 분석)

  • The purpose of this study was to analyze the stresses and displacements of various esthetic restorations and abutment teeth. The finite element models of central incisor were divided into four groups according to the types of restoration. Three load cases were applied; 1) 45 degrees on the incisal edge, 2) horizontal force on the labial surface, and 3) 26 degrees diagonally on the lingual surface. Material property, geometry, and load conditions of each model were inputed to the two dimensional finite element program and stresses and displacements were analyzed. Results were as follows; 1. In the cases of porcelain fused gold ann and porcelain laminate venner, stresses were equally distributed in supporting abutment tooth. 2. The metal coping of porcelain fused gold u and collarless porcelain fused gold crown functioned as a good stress distributor. 3. When the horizontal load applied, the highest tensile and compressive stresses were seen in the cervical margin of restoration and the dentin of the abutment tooth. 4. The highest displacement of restoration was seen when load was applied at an mee of 26 degrees diagonally in lingual surface of tooth in centric occlusion. 5. The influence of loading direction on the stresses and displacements in the restoration was greater than that of various design. 6. The possibility of fracture was highest in porcelain jacket crown.
