  • 치경부에 발생된 병소 부위(cervical lesion)는 "우식성 병소(caries lesion)"와 "비우식성 병소(non-caries lesion)"로 분류할 수 있다. 우식성 병소는 구강 내 세균들의 활동에 기인하여 발생된 병소로서 경조직 손상 부위의 수복과 함께 우식이 재발하지 않도록 체계적으로 대응하는 것이 필요하다. 비우식성 병소는 산성 물질, 저작력 및 이물질에 의한 마모 작용과 같은 다양한 원인에 의해 발생될 수 있으며, 수복 치료에 앞서 병소에 발생 원인을 규명하는 것이 중요하다. 먼저 병소 유발 원인에 대한 분석과 이에 대한 적절한 조치를 취한 후에 수복 치료를 시행하는 것이 바람직하다. 심미적인 요구가 높은 치경부 병소의 수복을 위하여 사용될 수 있는 재료는 "불소를 방출하는 재료"와 "불소를 방출하지 않는 재료"로 나뉘어지는데, 세균의 활동에 의해 형성된 우식성 병소의 수복에는 불소를 방출하는 재료를 사용하는 것이 추천된다. 우수한 심미성과 보다 강력한 유지력이 요구되는 경우에는 복합 레진계 수복 재료들을 사용하여 수복하게 되는데, 이 경우에는 "접착 술식(bonding procedure)"이 중요한 역할을 수행하게 된다. "접착 술식"은 수복물의 일차적인 유지 뿐 아니라 지각과민을 해소하고 2차 우식을 방지하며, 궁극적으로 수복물의 수명을 좌우하는 결정적인 역할을 수행한다. 자연 치아를 최대한 보존하고자 하는 "최소침습(minimum intervention)" 개념에 바탕을 두고 세심하게 형성된 와동에 각각의 증례에 최적화된 수복 재료를 선택하여 수복치료를 시행한다면 보다 심미적이고 환자가 편안한 치경부 수복물을 완성할 수 있을 것이다.

Dental application of glass-ceramic materials for aesthetic restoration (심미수복을 위한 글라스-세라믹 재료의 치과 응용)

  • Porcelain is the first ceramic material to be introduced into dentistry. Porcelain jacket crown was introduced by Dr. Charles H Land in 1886, which was an excellent aesthetic dental restoration but has not been widely used due to high firing shrinkage and low tensile strength. Then metal-ceramic system, which combines the esthetic properties of ceramics and the mechanical properties of metals, was introduced and nowadays it is still used in dental clinical field. However, the metal-ceramic system has shown some problems, such as increased lightness by reflection of light at opaque layer, shadow beneath the gingival line due to the block-out of light by metal coping, exposure of metal in margin part, bond failure between metal and porcelain, oxidation of metal coping during firing the porcelain, etc. Recently, along with the advance of fabrication methods of dental ceramics, the all-ceramic restorations with high esthetic and mechanical properties has increased and gradually replaced metal-ceramic restorations. Especially, CAD/CAM technology has opened a new era in fabricating the dental ceramic restorations. This overview will take a look at the past, present and future possibility of the dental ceramic materials.

SURFACE ROUGHNESS OF ESTHETIC RESTORATIVE MATERIALS BY POLISHING SYSTEMS (연마기구에 따른 심미 치아 수복재료의 표면 거칠기에 관한 비교 연구)

  • Proper finishing and polishing of tooth restorations enhance the esthetics and the longevity of the restored tooth. The aims of this study were to identify an appropriate polishing system for each esthetic restorative material(Z250, Heliomolar, Dyract AP, Fuji II LC), and to compare the efficiency of polishing systems(Enhance, Sof-Lex, Composite). The control group remains untouched. The results were as follows: 1. There was no significant difference of surface roughness among the materials, while a roughness value of Z250 was the lowest of all. 2. The smoothest surface was produced by Mylar sheet on all materials. The polishing procedures, however, increased a roughness value. 3. The smoothest surfaces were produced by Sof-Lex, and there was significant difference of surface roughness between Sof-Lex and Enhance systems. 4. The smoother surfaces on the control group showed many scratches after the polishing procedures in the SEM findings.

Development of dental zirconia (치과용 지르코니아의 발전)

  • Since the end of the 1990s, zirconia, which has been applied to dental prostheses, has been used in the form of coping or framework. The prostheses made in this way have been successfully used in the anterior teeth because they have no metal components as a structure. But in the posterior teeth, its use was limited due to the fracture or chipping of the veneering porcelain during the function. Later, as a solution to this shortcoming, a monolithic zirconia restoration was proposed in which a strong coping material was used in the form of a complete prosthesis. But, this resulted in some unesthetic results due to the characteristics of zirconia, which has excellent resistance to strong forces but is very white and opaque. However, now, due to technological advances in increasing the translucency of zirconia, it is possible to produce strong and esthetic zirconia restorations not only in the posterior region but also in the anterior region. In this article, the structural characteristics of various types of zirconia materials that have been developed so far, were discussed, and where the zirconia products actually in use belong to was explained.

  • Restoration of missing oral tissues by using implant therapy is one of viable options in prosthodontic treatment. The purpose of oral rehabilitation are function, esthetics and phonetics. Recently, with the increasing esthetic demand of the patient, dentists and laboratory technicians are more concern ed about natural appearance of implant prostheses. There are so many factors which affect final esthetic results of treatment such a shape, color, proportion, gingival contour etc. To provide natural and vivid looking artificial prostheses to a patient, the careful diagnosis, treatment plan and laboratory Support is crucial. Several cases will be reviewed and discussed in terms of material, gingival shape, contour, speech and connection mode etc. to fabricate natural looking implant prostheses.

THE EFFECT OF 1.23% APF GEL ON THE ESTHETIC RESTORATIVE MATERIALS (APF Gel이 심미 수복재료의 표면에 미치는 영향)

  • Topical fluoride application for children is a widely performed procedure in the field of Pediatric Dentistry for its dental caries prevention effects However, it is recently recognized as having some unwanted effects on several esthetic restorative materials, the author immersed glass ionomer cement, compomer and composite resin specimens in APF gel and measured the surface roughness and also, examined the specimens under the scanning eletron microscope. The followings are the results: 1. In the specimens of glass ionomer cement and compomer, APF gel 4 minute immersion group and pH cycling group show statistically significant increased surface roughness than artificial saliva immersion group (p<0.01). 2. There was no statistically significant surface roughness for composite resin in all group (p>0.05). 3. When the specimens were examined under scanning electron microscope, the surface change were in the order of glass ionomer cement, compomer, composite resin and also in the order of pH cycling group, APF gel 4 minutes immersion group, artificial saliva immersion group.
