Adequacy of Medical Manpower and Medical Fee for Newborn Nursery Care (신생아실 의료인력의 적정성 및 신생아관리료의 타당성 분석)

  • To assess the adequacy of medical manpower and medical fee for the newborn nursery care, the author visited 20 out of 24 hospitals with the pediatric training program in Youngnam area between July 29 and August 14, 1991. Total number of newborn, both normal and sick, admission and discharge in 1-30 June 1991 was obtained from the logbook of nursery. Head nurse and staff pediatrician of the nursery were interviewed to get the current staffing for the nursery and their subjective opinion on the adequacy of nursery manpower and the difficulties in recruiting manpower. Average medical fee charged for the maternity and normal newborn nursery care was obtained from the division of self-audit of medical insurance claim of each hospital. Average minimum requirement of nursing care time for one normal newborn per day was 179.5 (${\pm}58.6$) minutes; 202.3(${\pm}50.7$) minutes for the university hospitals and 164.2(${\pm}60.5$) minutes for the general hospitals. The ratio of minimum requirement of nursing care time and available nursing time was 1.42 on the average. Taking the additional requirement of nursing care for the sick newborns into consideration, the ratio was 2.06. The numbers of R. N. and A. N. in the nurserys of study hospitals were 31%, and 17%, respectively, of the nursing manpower for the nursery recommended by the American Academy of Pediatrics. These findings indicate that the nursing manpower in newborn nursery is in severe shortage. Ninety percent of the head nurses and 85% of the staff pediatrician stated that the newborn nursery is short of R.N. and 75% of them said that the nurse's aide is also short. Major reason for not recruiting R.N. was the financial constraint of hospital. For the recruitment of nurse's aide, short supply was the second most important reason next to the financial constraint. However, limit of quarter in T.O. was the mar reason for the national university hospitals. Average total medical fee for the maternity and newborn nursery cares of a normal vaginal delivery who stayed two nights and three days at hospital was 219,430won. Out of the total medical fee, 20,323won(9.3%) was for the newborn nursery care. In case of C-section delivery who stayed six nights and seven days, total medical fee was 732,578won and out of the total fee 76,937won (12.0%) was for the newborn care. Cost for a newborn care per day by cost accounting was 16,141won for the tertiary care hospitals and 14,576won for the all other hopitals. The ratio of cost and the fee schedule of the medical insurance for a newborn care per day was 5.0 for the tertiary care hospitals and 4.9 for the all other hospitals. Considering the current wage level of the medical personnel, capital investment for the hospital facilities and equipments, and the cost for hospital maintenance, it is hard to expect adequate quality care in the newborn nursery under the current medical insurance fee schedule.

The Relation Between the Occupational Stress and Fatigue Level of Neonatal Nurses (신생아실 간호사의 직무 스트레스와 피로도와의 관계)

  • Purpose: The purpose of this study was to investigate occupational stress and fatigue level of neonatal nurses and to assess relationship between the two of them. Methods: Subjects were 109 neonatal nurses working at 2 university hospitals and 9 women's hospitals in D city, and the data were analyzed using descriptive statistics and binary logistic regression analysis by the SPSS/WIN 15.0. Results: The mean score of nurses' occupational stress level was 3.22, and the highest was 'night shift' among the 7 subcategories, and fatigue level was 62.0, and the highest was physical fatigue in configuration field. In logistic regression analyses, 'heavy workload' and 'inadequate physical environment' led to most occupational stress. Conclusion: In addition to significantly higher levels of occupational stress of neonatal nurses, the cumulative fatigue also resulted in decreased quality of nursing, job satisfaction and nursing productivity; therefore, appropriate number of nurses and improvement of work environment is necessary. Furthermore, to reduce the occupational stress and fatigue, we should try to develop adequate clinical guidelines and intervention strategies and apply them in neonatal care unit.

The Effect of a Mother-Child Shared Room on Self-Confidence in Infant Care and the Practice of Breastfeeding by Delivering Individual Education to the Mother-Child in the Shared Room (모자동실의 개별교육이 영아 돌보기 자신감과 모유수유 실천에 미치는 효과)

  • The aim of this study was to evaluate the effects of a mother-child shared room on self-confidence in infant care and the practice of breastfeeding by delivering individual education to the mother-child in the shared room. The participants were 50 mothers in the mother-child shared room group and 50 mothers in the mother-child in separate rooms group, all of whom delivered children. The self-confidence in infant care was found to be higher in the mother-child shared room group than the mother-child in separate rooms group. It is necessary to conduct multilateral research into the influential factors on the practice of breastfeeding apart from a variable, namely, the mother-child shared room.

The Effects of the Nurses' Knowledge to Breast-Feeding on the Nursing Activities for Breast-Feeding (간호사의 모유수유에 대한 지식정도가 모유수유 간호활동에 미치는 영향 - 산과외래, 분만실, 산과병실 및 신생아실 간호사를 중심으로)

  • This study was attempted to identify the knowledge to breast-feeding and the performance of nursing activities for breast-feeding and to test 'how does the degree of knowledge to breast-feeding influence the nursing activities for breast-feeding?' The subjects were 180 nurses working in delivery rooms, nursery, obstetrics & pediatrics wards or OPD of obstetrics & gynecology of 8 general hospitals in Pusan as of August 3 through 13, 1996. The results are abstracted as follows. 1) Subject nurses' age, 25-29 was 45.6%(the major), education levels graduates from junior college were 95.0%, unmarried status was 62.2%, 76.5% of married nurses had children, 39.7% in-service education for Breast-Feeding, 33.7% did nursing activities for breast-feeding actively, the reason for inadequate activities for breast-feeding was 'too much other tasks.' 2) The degree of knowledge to breast-feeding ; mean score was 13.54, the degree of performance of nursing activities for breast-feeding : 92.38±20.93 points out of possible 145 points (3.19±.74 out of possible 5 points) moaned that it was a low level. 3) The hypothesis 'the nurses who have higher degrees of knowledge to breast-feeding will show higher degrees of performance of nursing activities for breast-feeding than the nurses who have lower degrees of knowledge to breast-feeding' was tested by t-test(t=-.01, P=.9888), but rejected because it turned out statistically not significant at the level of P<.05. Above results suggested the degrees of knowledge to breast-feeding and the degrees of performance of nursing activities for breast-feeding were generally low and the degree of knowledge didn't influence the nursing activities. Researchers believe that the education for breast-feeding by the nurses need to be performed systemically & practically and new method of breast-feeding education program need to include hospital managers as well as nurses related mothers and their family. In audition, researchers propose the introduction of &lactation specialist system&, for the specialist can change the attitude of feeding-mothers positively with their specialty and authority.

Shortening length of hospital stay and elevating parental confidence in infant care by prematurity discharge training method improvement (미숙아의 퇴원교육방법 개선을 통한 재원일수 단축 및 보호자의 양육자신감 향상)

  • 문제: 장기 입원 치료를 받은 미숙아의 퇴원교육을 퇴원당일 짧은 시간에 시행함으로써 간호사는 제한된 인력으로 교육시간을 길게 할애하지 못하고, 표준화된 지침의 부재로 인해 퇴원교육을 누락하는 경우가 있으며 보호자는 교육내용을 충분히 이해할 시간이 부족하여 퇴원에 대한 필요 이상의 두려움을 가지고 있었다. 목적: 미숙아에 대한 퇴원교육을 표준화된 지침에 따라 단계적이고 체계적인 방법으로 개선함으로써 불필요한 퇴원지연을 예방하여 재원일수 단축시키고 보호자의 양육 자신감을 향상시킨다. 의료기관: 부산광역시에 소재한 대학병원의 신생아실 질 향상 활동: 미숙아의 퇴원을 지연시키는 원인을 분석하고 개선된 미숙아 퇴원교육 방법을 적용한 후 미숙아 재원일수와 보호자의 양육자신감을 측정하였다. 개선효과: 개선 활동 후 미숙아의 재원일수가 11.33일에서 8.28일로 3.05일 단축되었고 보호자 양육자신감 점수는 68점에서 76.25점으로 8.25점 향상되었다. 교훈: 퇴원을 앞둔 미숙아들이 좀 더 개방적으로 가족과 함께 지내면서 퇴원준비를 할 수 있는 여건과 미숙아의 성장과 발달에 대한 보호자들의 이해를 돕기 위한 인터넷강좌나 공개 강좌 등이 필요하다. 보호자들의 양육 자신감 향상과 정보교환을 위해 자조조직을 활성화하고 보다 질 높은 서비스를 위해서는 현실적으로 간호사의 인원 충원이 필요하다.

A Content Analysis of the Advocacy Concept Perceived by Nurses at Nursery Room (옹호개념에 관한 신생아실 간호사의 인지 내용 분석)

  • This descriptive study was done to detect the possibilities of the development of the advocacy concept in nursing. The subjects consisted of 3 nurses who had agreed to participate to the study, working at nursery room in a general hospital. Data was collected from May 22 to June 13 in 1995 through tape recorded intensive-interview, and written down, then content analyzed qualitatively related to the infant advocacy. 1. Derivation fo the meaning about advocacy concept 1) The situations in need of infant advocacy were 12 categories : malpractice, overcharing, negligence, unnecessary services, and services without consent, and so on. 2) Fourteen categories of advocacy activities percived by nurses were derived from participants' statements. Protesting infant or his /her families against their counterpart, providing informations to families, cooperating with medical staffs for her patient, then calling medical staff not so as to be maleficient to her patient in its rank. 3) The expected result of advocacy activities perceived by nurses was respectively positive to her patient or families, but negative to nurse. 4) The feelings of nurses in the sitution of advocacy were expressed in to concern, comprehension, regret, powerlessness, charity, desire, and so on. 5) Nurses perceived that advocacy activities could be influenced positively by factors related to nurses' qualification and negatively by factors related to doctor's overdo and nurse's underdo. 2. Categorization of the meaning and their relationships In case of antecedental situation in need of infant advocacy, nurse perceives her patients need the advocacy to get a benefit through nurse's information, intervention speaking, building cooperations. The expected factors to influence advocacy activities perceived by nurses, are the power imbalance between medical staffs, the nurse's qualification, and the nurse's feeling from the situation. The above results suggest that the infant care situation will be recommendable field work place for concept development of advocacy with hybride model when it involves infant's families.

Nursery Room Nurses′ Role Performance for Maternal Role Attainment of Mothers at Early Postpartum Period (산욕초기 어머니 역할획득을 위한 신생아실 간호사 역할수행에 관한 연구)

  • The early postpartum period is crucial toward in recovery from childbirth and attainment of the maternal role. Maternal role attainment is a complex social and cognitive process of stimulus-response accomplished by learning. Helping for maternal role attainment is one of nursing goals in the early postpartum period. Based on King's conceptual framework for nursing, this study was planned as descriptive correlation study to determine the significant differences of the degree of nursery room nurses' role performance according to several variables of personal, interpersonal, and working system of nurses in nursery room. The purpose of this study was to contribute to the planning of nursing care to help maternal role attainment of the early postpartum period of mothers and to the development of relevant nursing theory. The data were collected from Feb. 3 to 28 by questionnaires with 273 nurses in nursery room. The instruments for this study were consisted of four parts : 21 questions for rot performance of nurse. 37 questions for personal system of nurse including 31 questions for role perception of nurse : 65 questions for interpersonal system including 63 questions for job stress of nurses , 18 questions for working system of nurse. The toos to measure role performance and role perception, and job stress of nurse were tested for internal reliability. Cronbach's Alphas were 0.9612, 0.9058, and 0.9649. The data were analysed by using in S.A.S. computerized program and included percentage, t-test, ANOVA Pearson Correlation Coefficient, and Duncan multiple range test. The conclusions obtained from this study are summerized as follows : 1. The mean score of the items of role performance was 2.12(SD=0.55) in Likert's 4 points scale. 2. The degree of role performance was significantly different according to role perception(p=0.0001), age (p=0.006), educational background(p=0.002) , and certificate of midwife (p=0.03) among variables of personal system of subjects. 3. The degree of role performance was significantly different according to job stress (p=0.0001) and numbers of children(p=0.006) among variables of interpersonal system of subjects. 4. The degree of role performance was significantly different according to having opportunities for baby(p=0.03), the degree of flexibility to bring baby to mother's room(p=0.046), the scope of visitor for baby(p=0.016) , the degree of flexibility of visiting for baby (p=0.049) , the degree of participation of nurse in establishing visiting rules(p=0.017), existence and/or nonexistance of rules for breast feeding(p=0.010) , existence and/or nonexistance of education for breast feeding (p=0.009), existence and/or nonexistance of breast feeding room(p=0.013) , concert methods for breast feeding (p=0.003), working place (p=0.0001), and career(p=0.019) among variables of personal system of subjects.

A Comparative Study on the Perception of the Impediment Factors between Mothers and Nurses in Nursery (모유수유 저해요인에 대한 산모와 신생아실 간호사간의 인식정도 비교 연구)

  • The condition of nutrition during infancy will greatly affect infants' physical, emotional growth, especially breast feeding is important in their growth and development, and emotional stability, too. Despite such advantages of breast-feeding, its rate has continued to fall year after year in Korea : the 95% rate in 1960's has fallen to 25.4% in 1990. It is known that such a downfall of breast-feeding rate is associated with various factors. The purpose of the study is to examine mother's and nurses' in nursery perception of the impediment factors of breast-feeding : to compare those between of breast-feeding : to compare those between two groups : to provide fundamental data for developing strategies for increasing breast-feeding. The subjects were 45 new mothers from one hospital and the same number of nurses sampled from 3 university hospitals, in Seoul. The data were collected for 11 days from April 12 to April 23, 1994 and a questionnaire was developed based on the interview with 14 nurses and 10 mothers and the literature reviews. Liker's Five-Point Scale was used as measurement. The Data were analyzed using SPSS / PC and descriptive statistics, t-test, ANOVA. The results of study are as follows : 1) Most new mothers get the information about breast-feeding from their mothers, books or newspaper, and relatives in rank than professionals(nurses or doctors). 2) The impediment factors the breast-feeding are categorized as mothers, hospital system, medical personnels and social factors. The most frequently mentioned impediment factors from mothers are 'Difficulty by operational wounds'(3.13), 'Lack of will for breast-feeding'(3.47), 'Insufficient rest and sleep'(3.52) and 'Opposition from husband or his family members'(4.77)in rank. On the other hand those factors from nurses are 'Inadequate nipple condition'(2.37), 'Decreasing milk secretion given medicine after operation'(2.63), 'Mothers knowledge deficit by poorly prepared education'(2.79) and 'Mothers abhorance of breast-feeing'(3.87) in rank. 3) As for the hospital system, the highly perceived impediment factors by mothers are 'Seperation of baby from mother after birth'(2.78), 'Lack of space for breast-feeding in the hospital'(2.93), 'Lack of facility for informing the time for breast-feeding'(3.18) and 'Because of babys' preference artificial nipple by being accustomed to it in hospital(3.97), in rank. Meanwhile, those from nurses are "Seperation of baby from mother after birth",(1.92), "Inconsistency between hospital's nursing time and mother's breast secretion time" scretion time(2.97), "Lack of space for breast-feeding in the hospital"(3.39), and "Lack of facility for informing the time for breast-feeding"(3.74) 4) As for the medical personnels, the highestly perceived Impediment factor from mothers in "Lack of professional nurses for breast-feeding"(2.96), and the lowestly perceived one is "Doctors' reluctance"(4.75). Nurses perceived same as mothers, too. 5) As for the social factors, the highestly perceived impediment factor by mothers and nurses is "Inconvience of social activities"(2.83) and the lowestly ones are "The sense of self-sacrifice"(4.22) by mothers, and "The sense of old fashioned"(4.13) by nurses. 6) The difference of the perception of impediment factors between mothers and nurses is statistically significant only in mother factor.
