A Convergence Work Experience of Nursing Students under Preceptorship (간호대학생의 프리셉터십 융복합 실습 경험)

  • This study is to understand the work experiences of nursing students under preceptorship in 3+1 year program, and suggests effective practice to prevent stopping nursing profession. Four focus group interviews were made with 23 nursing students in 3+1 year program. Data were analyzed through the grounded theory of Corbin and Strauss' (2008). Through comparative analysis, central phenomenon of was 'looking for lighthouse'. The causal condition was 'opportunity to learning practice'. Contextual conditions were 'mood of being a true nurse', 'getting vivid information about clinical field', and 'lack of students perception about preceptorship'. Intervention conditions were 'work environment', 'proactive learning' and 'attachment to the hospital'. Action/interaction strategies were 'adjustment to diverse demands of patients and guardians', 'feeling as if it were one's own affair', and 'looking as much as knowing'. Consequence were 'pride' and 'itinerary of learning as a new graduate nurse'. This study is meaningful because it gives useful basis to nursing students in the process of 'looking for lighthouse'.

업종별 산업간호사의 보건관리업무 분석

  • 본 연구는 업종별 산업간호사의 보건관리업무의 내용과 수행정도를 측정하고 업종별 보건관리업무 수행정도를 비교 분석코저 한다. 연구방법은 산업간호사회에 등록된 산업장중 서울, 인천, 마산, 창원, 부산, 울산 지역에 있는 130개 산업장을 임의로 선정하여 산업간호사를 대상으로 설문조사하였다. 자료분석은 반도와 백분율, 업종간의 보건관리업무 내용별 수행정도는 일원분산분석과 다중검증 비교로 유의성을 검증하였다. 연구결과는 다음과 같다. 1) 업종별로 산업간호사의 업무수행에 유의한 차이를 보인 보건관리내용은 서비스업에 있어서 근로자의 건강사정에 대한 수행정도가 제조업과 기타제조업에서 보다 높게 나타났다.(F=4.23, P=.0167) 반면에 근로자의 건강진단과 계속관리, 작업환경 관리업무 수행정도는 제조업과 기타제조업에서 서비스업보다 높게 나타났다.(F=9.78, P=.0001 ; F=5.366. P=.005) 2) 업종간에 유의한 차이가 없는 보건관리내용은 산업장진단 또는 건강문제분석, 보건교육, 통상증상에 대한 투약, 기록과 보고, 건강관리실 운영 등이었으며 업무수행정도는 가끔 또는 필요시하는 정도로 나타났다. 반면에 조사, 연구 참여에 대한 수행정도는 세 업종 모두 가장 낮은 것으로 나타났다. 이러한 연구결과들은 산업장 간호사의 신규교육이나 직무교육과정운영이 업종별로 운영되어야 함을 시사해 준다고 볼 수 있으며 또한 산업간호사의 직무 중 보건 교육, 통상질환에 대한 투약, 기록 및 보고, 건강관리실운영등은 업종간에 유의한 차이가 없으나, 수행정도는 낮은것으로 나타나 이에 대한 실제적인 직무교육이 강화되어야 할 것으로 생각된다. 특히 보건교육도 업종별로 차이가 있으므로 다양한 보건교육교재의 개발이 필요하다고 생각된다.

Nursing Students' First Clinical Experiences of Death

  • Purpose: This study was conducted to comprehensively investigate nursing students' experience of their first encounter with death of a patient during clinical practice. Methods: This study took place from January 27 through March 6, 2012 with eight female senior nursing students enrolled at Pusan National University located in Y city who have experienced patient death. We collected their experience of their first death encounter during their clinical rotation by asking, "What is your first experience of patient's death during the clinical practice?" Husserl's phenomenological approach was applied in this study. Results: In this study, 17 themes, 15 clusters of themes and eight categories were derived. The categories included "Desire to avoid the reality of death", "Powerlessness", "Anticipation for recovery shifted to fear of death", "Various interpretations of death", "Limitations in their nursing practice", "Resentment of lack of nurses", "Longing to better understand death", and "Motivation for inner growth". Conclusion: Through their first encounter with death of a patient, nursing students experienced various emotions and viewed their role as hospice caregiver by projecting themselves as fully trained nurses in future. Participants considered terminal care as a part of nursing care. The result of this study indicates the need to include education of death in the nursing school curriculum.

A Development and Effects of Simulation-based Education Program on Emergency Airway Management (시뮬레이션 기반 응급기도관리 교육 프로그램 개발 및 효과)

  • Purpose: After developing and imparting knowledge of a simulation-based emergency airway management education program for nursing students, this study identified the effects of the education by evaluating emergency airway management knowledge, Clinical Performance Ability, self-efficacy, and critical thinking disposition. Method: The participants were 30 nursing students. Data were collected from October 14 to November 11, 2017, and analyzed using IBM SPSS Version 22.0. Results: The simulation-based nursing education program was developed and applied based on the ADDIE model involving five stages: analysis, design, development, implementation and evaluation. Comparing the pre-and post-education results, we observed statistically significant improvement when considering emergency airway management knowledge (t=-9.98, p<0.001), Clinical Performance Ability (t=-23.90, p<0.001), self-efficacy (t=-16.77, p<0.001), and critical thinking disposition (t=-5.04, p<0.001). Conclusions: Simulation-based emergency airway management training program is an effective educational program that enhances the emergency airway management knowledge, Clinical Performance Ability, self-efficacy, and critical thinking disposition of nursing students. We believe that the program developed in this study contributes towards improvement of patient nursing quality by enhancing the ability of nursing students to cope with emergencies in practice. Furthermore, it can be applied for educating new nursing students, and contribute to the development of nursing practices.

Study on an Unplanned Extubation Prevention Program (UEPP) for Intubated Patients in Intensive Care Unit (비계획적 발관 예방을 위한 교육프로그램 적용효과)

  • Unplanned extubation prevention program (UEPP) was developed and applied to intubated patients hospitalized in a medical intensive care unit (ICU). The number of unplanned extubation was 29 during the preliminary investigation and 23 cases during the prevention intervention program. The general features of the patients and nurses who experienced UE did not have any significant differences between the two periods, with only the number of new nurses in the ICU being significantly different (p=.039). Although the number of new nursing staff in the ICU was high, the mean rate of unplanned extubation, which is the number of UE days per 1000 intubation days, was 8.8 during the prevention intervention program, which was significantly lower than 12.3, the mean rate of UE during the preliminary investigation (t=-2.333, p=.040). The results provide a prevention program that could contribute to the safety of patients by actively preventing unplanned extubation in intubated patients in ICU.

Development of a CD Program Applied Logotherapy for Psycho.Spiritual Care of Late Adolescents with Terminal Cancer (청소년 후기 말기 암 환자의 정서적.영적 돌봄을 위한 의미요법 CD 프로그램 개발)

  • Purpose: The purpose of this study was to develop a CD program of applied logotherapy for psycho spiritual care of late adolescents with terminal cancer. Methods: Keller & Song's ARCS theory and a model for developing learning materials was applied to develop this program composed four distinct phases: planning, designing, developing, and evaluation stages. Results: This program was entitled 'Finding meaning in my life' and consisted of 5 sessions and its educational contents were made up as follows: "First Secret" is 'learning three natures of the human mind', "Second Secret" is 'learning creative values first method to find meaning of life', "Third Secret" is 'learning experiential value as second method to find meaning of life', "Fourth Secret" is 'learning attitudinal value as third method to find meaning of life', and "Fifth Secret" is 'Becoming the master of my life'. The sub-menu was made up of 'Beginning', 'Opening mind', 'Learning'. 'Laughing Song', 'Experiencing'. Conclusion: This CD program applied logotherapy with flash animation technique as an emotional and spiritual nursing intervention program for easier and more scientific application in pediatric oncology and hospice care area.

The Comparison between Nursing Graduates' Performance and Clinical Nurses' Performance of Clinical Competency (간호학생과 신규간호사의 실무수행능력 비교)

  • The purpose of this study was to investigate the performance of clinical competency in nursing graduates and clinical nurses. The total of 234 subjects returned the questionnaire with 95% of response rates. The subjects of the study constituted of 195 nursing graduates and 39 clinical nurses. Self report questionnaires were used to measure the clinical competence of nursing graduates and clinical nurses. This instrument had four dimensions of competency : client and health need, nursing process, professional role, and nursing interventions. The data were analyzed by utilizing SPSSWIN and the results were as follows. 1) The mean score of the nursing intervention dimension was the most with 3.82 compared to professional role dimension(3.06), nursing process(3.03), client and health need dimension(2.94) in nursing graduates. 2) The mean score of the nursing intervention dimension was the most with 3.04 compared to client and health need dimension(2.82), professional role dimension(2.81), nursing process(2.77) in clinical nurses. And all of these dimensions' scores were lower than the nursing graduates' scores. 3) The mean scores of nursing process (t=3.76, p<.001) and professional role dimensions(t=3.53, p<.001) in nursing graduates were significantly higher than clinical nurses' scores. Our suggestions based on the results of this study is : 1. It is recommended to repeat the same designed study in large sample of clinical nurses for further study.

The Development and Simulation of Training Cost Estimating Model for the Operation of the Nurse Residency Program (신규간호사 교육 프로그램(Nurse Residency Program) 운영을 위한 교육비용 산출 모형 개발 및 모의 적용)

  • Purpose: This study aims to develop a cost model for NRP (Nursing Residency Program) operation and ultimately provide evidence for financial factors for NRP operation in the future by simulating a cost model. Methodology: This study developed a model for the NRP education cost calculation model based on the review of Hansen's model, which has systematically reported on the development and operation of NRP, and discussions with nursing education experts at a university-affiliated hospital. With the simulation, it was intended to predict nurses' supply and demand in the long term and to calculate changes in long-term education costs. Findings: Firstly, turnover model, term model, cost model necessary for calculating a model for the NRP education cost calculation model was set up. Secondly, the simulation showed the following results; 1) the proportion of newly graduated nurses less than 5 years of working decreases gradually over time, which will make the composition of nurses more balanced. 2) In the first year of the partial introduction of NRP, the cost of training new nurses was about 2.1 times higher than before. After the introduction, the training cost in the 13th year began to be lesser than before the introduction, and in the 25th year, it decreased by 28.1% compared to before the introduction. Practical Implications: Firstly, NRP would be an effective way to solve the higher turnover and frequent departure of new nurses and the imbalance of nurses' composition. Secondly, although the costs of NRP are incurred in the early stages, in the end, NRP training costs are reduced compared to before the introduction of NRP. It is necessary to systematically understand the contribution effect of NRP by analyzing the economic value of NRP considering financial and non-monetary returns in the future and providing a basis for decision-making related to NRP implementation.

The Effects of Simulation Education for New Nurses on Emergency Management Using Low-fidelity Simulator (저충실도 시뮬레이터를 활용한 신규간호사의 응급상황관리 시뮬레이션 교육의 효과)

  • Purpose: This study focuses on investigating the effectiveness of simulation education on emergency management using a low-fidelity simulator as related to clinical skill performance, self-confidence, knowledge, learning satisfaction, and critical thinking disposition in new nurses. Methods: A pre-post test experimental design of nonequivalent control group was applied. Fifty-five new nurses were recruited, 28 nurses for the experimental group and 27 nurses for the control group. A simulation education for emergency management comprising knowledge lecture, team learning, skill education, team simulation, and debriefing was developed and implemented from Feb. 14 to 27, 2015. Data were analyzed with percentage, average, and standard deviation, chi-square, and t-test using SPSS. Results: The experimental group showed significantly higher knowledge (t=5.81, p<.001), clinical skill performance (t=10.08, p<.001), self-confidence (t=-6.24, p<.001), critical thinking disposition (t=2.42, p=.019), and learning satisfaction (t=4.21, p<.001) for emergency management compared with the control group who had traditional lecture education. Conclusion: The results indicate that a simulation education using a low-fidelity simulator is an efficient teaching method for new nurses to deepen their clinical skill performance, self-confidence, knowledge, learning satisfaction, and critical thinking disposition in learning emergency management.

The influence of new nurses' knowledge, nursing performance, and educational needs of chemotherapy medication on chemotherapy medication errors (신규간호사의 항암 투약 간호 지식, 수행도 및 교육 요구도가 항암 투약 오류에 미치는 영향)

  • Purpose: This study aimed to identify the factors affecting the chemotherapy medication errors made by new nurses and to use the results as basic data for the development of a chemotherapy medication nursing education program for new nurses. Methods: This cross-sectional study was conducted with 189 new nurses working at a general hospital and a tertiary general hospital in Korea. The data collection period was from January 11 to February 7, 2021. The data collected during this study were analyzed using the IBM SPSS statistics version 25.0 program. Data analysis included descriptive statistics, independent t-test, ANOVA, and logistic regression analysis. Results: One factor influencing chemotherapy medication errors was new nurses' educational needs (odds ratio=.18, p=.005). As educational needs increased, the probability of making errors in medication was reduced by .18. Conclusion: It is necessary to develop a chemotherapy medication education program tailored to the educational needs of new nurses by considering the education period, method, and content, with a focus on the content with high demand from new nurses.
