Clinical and Electrophysiological Features of HNPP Patients with 17p11.2 Deletion (염색체 17p11.2 유전자 결손을 동반한 유전성 압박마비 편향 신경병증의 임상적, 전기생리학적 특성)

  • Objectives : Although the diagnosis of hereditary neuropathy with liability to pressure palsies (HNPP) is important for correct prognostic evaluation and genetic counseling, the diagnosis is frequently missed or delayed. Our main aim on undertaking this study was to characterize the electrodiagnostic features of HNPP. Material and Methods : Clinical, electrophysiologic and molecular studies were performed on Korean HNPP patients with 17p11.2 deletion. The results of electrophysiologic studies were compared with those of Charcot-Marie-Tooth disease type 1A (CMT1A) patients carrying 17p11.2 duplication. Results : Eight HNPP (50 motor, 39 sensory nerves) and six CMT1A (28 motor, 16 sensory nerves) patients were included. The slowing of sensory conduction in nearly all nerves and the distal accentuation of motor conduction abnormalities are the main features of background polyneuropathy in HNPP. In contrast to CMT1A, where severity of nerve conduction slowing was not different among nerve groups, HNPP sensory nerve conduction was more slowed in the median and ulnar nerves than in the sural nerve (p<0.01), and DML was more prolonged in the median nerve than in the other motor nerves (p<0.01). TLIs were significantly lower in HNPP than in the normal control and CMT1A patients for the median and ulnar nerves (p<0.01), and were also significantly reduced for the peroneal nerve (p<0.05) compared with those of the normal controls. Conclusion : The distribution and severity of the background electrophysiologic abnormalities are closely related to the topography of common entrapment or compression sites, which suggests the possible pathogenetic role of subclinical pressure injury at these sites in the development of the distinct background polyneuropathy in HNPP.

  • Background : Guillain-$Barr{\acute{e}}$ syndrome(GBS) is a neurologically emergent condition, leading to respiratory insufficiency without an early and appropriate treatment. Thus, the treatment of GBS requires early diagnosis but it is difficult due to the low sensitivity of laboratory tools in the initial stage. Hoffman reflex (H-reflex) and its facilitation by Jendrassik maneuver (JM) are sensitive tools evaluating the central circuit of motor system on the spinal cord level. The aim of this study is to test whether the change of H-reflex and F-wave under the JM is able to detect the early stage of GBS and whether GBS involves the central nervous system (CNS). Material and Methods : All 7 GBS patients who showed normal or nearly normal nerve conduction study were included. The facilitation of H-reflex and changes of F-wave were calculated by measuring the percent difference of H-reflex or F-wave amplitude under JM compared to basal H-reflex of F-wave amplitude. The changes of F-wave and H-reflex in the GBS patients were compared with them of 8 healthy controls. Results : The F-wave amplitudes of both healthy controls and GBS patients did not changed under the influence of JM ($102.4{\pm}24.9%$, $108.7{\pm}29.0%$ respectively). However, the facilitation of H-wave by JM in the GBS patients was absent ($98.8{\pm}5.8%$), even though the H-reflex amplitude I the healthy controls increased under the influence of JM ($124.8{\pm}12.1%$). Conclusions : The loss of H-reflex facilitation in GBS implies that this phenomenon might be the most early change detected in the electrophysiological study and support the early diagnosis, and that GBS might include lesion in the spinal motor CNS.

  • Myelin-associated glycoprotein (MAG) has been known to have a crucial role to the formation of myelin sheath during initial stage of myelination. In the present study, we investigated the aging-related expressional changes of MAG in the rat cerebrum. MAG expression was markedly decreased in cerebral cortex by aging. In the adult rat cerebrum, MAG-positive rolls were process-bearing cells with large nucleus, and extensively distributed. However, in the aged rat brain, MAG-positive cells showed small and round morphology with little cytoplasm and few processes. MAG was co-expressed with galatocerebroside, but not with Iba-1, or GFAP. These results suggest that the expressional change of MAG-positive cells is associated with degeneration of oligodendrocyte-myelin system by aging, and that MAG is likely to be a reliable marker for the mature oligodendrocytes in the aged rat brain.

Comparison of Physicochemical Properties of Topsoil from Forest Development and Non-Development Area (산지개발지역과 비개발지역 표토의 이화학적 특성 비교)

  • This study was carried out to evaluate the physicochemical properties of topsoil from forest development area. The results of physicochemical properties of topsoil from forest development area shown on the average loamy sand~sandy clay loam in soil texture, 5.3~7.1 in pH, 0.02~0.18 dS/m in EC, 0.7~1.8% in OM, 0.03~0.11% in T-N, 11~15 $cmol^+/kg$ in CEC, 0.02~0.04 $cmol^+/kg$ in $K^+$, 4.51~8.18 $cmol^+/kg$ in $Ca^{2+}$, 0.93~2.77 $cmol^+/kg$ in $Mg^{2+}$, 6~49 mg/kg in available phosphate. And the results of physicochemical properties of topsoil from forest non-development area shown on the average sandy loam~sandy clay loam in soil texture, 4.4~5.3 in pH, 0.03~0.05 dS/m in EC, 3.1~4.6% in OM, 0.13~0.23% in T-N, 14~18 $cmol^+/kg$ in CEC, 0.02~0.04 $cmol^+/kg$ in $K^+$, 0.78~3.82 $cmol^+/kg$ in $Ca^{2+}$, 0.29~1.31 $cmol^+/kg$ in $Mg^{2+}$, 3~31 mg/kg in Av. $P_2O_5$. On the other hand, forest development area of topsoil sand content higher than 8~18% sand content than the forest non-development area. This trend is thought to be the absence of topsoil management development projects. Consequently, the results suggested a high potential of recycling of the topsoil from forest non-development area for planting soil. Therefore, in construction of the conservation and management of topsoil from forest non-development area is very important.

Tension Stiffening of Reinforced High Performance Fiber Reinforced Cementitious Composites (HPFRCC) (철근 보강 고성능 섬유보강 콘크리트의 인장 강성)

  • To overcome weak and brittle tensile characteristics of concrete, many studies have been conducted on fiber reinforced concrete (FRC). Recently, high performance fiber reinforced cementitious composites (HPFRCC), which shows strain hardening behavior, has been actively investigated. However, most of the studies focused on the material behavior of HPFRCC itself. Only a few studies have been conducted on the tensile behavior of HPFRCC with steel reinforcement. Therefore, a tension stiffening test for HPFRCC members has been conducted in this study in order to investigate the effect of a reinforcing bar on the tensile behavior of HPFRCC. Tensile stress-strain relationship of HPFRCC has been derived from the tests. The HPFRCC resisted tensile stress continuously from the first cracking to the yield of reinforcing bar. Through the comparison with the tensile behavior of HPFRCC members without a reinforcement, it was shown the tensile strength and capacity of HPFRCC were reduced due to the combined effect of the high shrinkage of HPFRCC, restraining effect of steel reinforcement, and the strain hardening behavior of HPFRCC. It is expected that the tension stiffening test results can be useful for an application of HPFRCC with steel reinforcement as structural members.

  • This study aimed to research and analyze the real condition of topsoil management of river development field as a significant case among domestic development fields for topsoil preservation. Through survey with experts, we understood the real condition and problems of topsoil management during river development. In order to verify this, we analyzed the characteristics of soil before and after development focusing on the rivers recently completed as an ecological river restoration project, supervised by Ministry of Environment among domestic river improvement projects. The study results are like below. First, experts preferred collecting and reusing topsoil as the best method to maintain and improve soil for plant growth. Second, realistically collecting and reusing topsoil is not fully conducted due to economical issues and inconvenience in construction. In the soil condition, third, the contents of elements necessary for plant growth like organic matter and total nitrogen declined overall after development.

Morphometric Study on Regeneration of Vascularized Nerve Graft (혈행화 신경이식 후 신경재생에 대한 형태계측학적 연구)

  • Adequate vascularization is pivotally essential for a successful nerve graft. Theoretically, the immediate vascularization will inhibit fibroblast infiltration and stimulate nerve cell regeneration. In this study, histomorphological and electrophysiological studies were performed to determine if vascularized grafts are functionally superior. In rat model, a 4cm segment of the sciatic nerve was obtained and placed as a non vascularized graft on one side, and as a vascularized graft connected to the inferior gluteal vessels on the opposite side. To determine the compound action potential of the gastrocnemius muscle, electromyography was done after 2, 3 and 4 months. Histomorphologically, the distribution of myelinated nerve fibers and Schwann cell were evaluated after toluidine blue staining, The following resutls were obtained: 1. The electrophysiological studies showed no difference between the nonvascularized and vascularized grafts. 2. Two and three months after grafting, myelinated nerve fibers were more abundant in the vascularized proximal, middle and distal areas in all nerve fibers of varying diameters. 3. In the post-nonvascularized graft 2-month group, a few myelinated nerve fibers were present in the proximal and middle areas, but none distally. In the post-vascularized graft 2 month group, myelinated nerve fibers ranging $2-8{\mu}m$ were present in all three areas. 4. In the post-nonvascularized graft 3 month group, a few myelinated nerve fibers ranging in $2-6{\mu}m$ were present in all three areas, but in the post-vascularized graft 3 month group, many myelinated nerve fibers ranging in $2-10{\mu}m$ were present in all three areas. 5. In the post-graft 4-month group, more myelinated nerve fibers were present in all three areas of the vascularized grafts. However, nerve fibers of less than $2{\mu}m$ in diameter were more abundant in the non vascularized grafts. 6. Schwann cells were more abundant in the proximal, middle and distal areas of the post-vascularized 2, 3 and 4-month grafts. Based on these findings, the immediate restoration of circulation in vascularized nerve grafts allows for the increased number of surviving Schwann cells, rapid healing of the axon and myelin sheath changes which occur during Wallerian degeneration, and thus is able to stimulate a morphologically optimal regeneration.

The Clinical Effect of Sparfloxacin for the Treatment of Acute Respiratory Infection (호흡기 감염 질환에서 Sparfloxacin의 임상효과)

  • Sparfloxacin is a new synthetic quinolone antimicrobial developed at the Research Laboratories of Dainippon Pharmaceutical Co, Ltd. To evaluate the efficacy and safety of sparfloxacin in acute pulmonary infection, we administered sparfloxacina(100mg) twice in a day to 30 patients who had signs and symptoms of acute pulmonary infectious diseases regardless of their underlying lung disease for 7 days. The results were as follows: 1) A total 30 patients were enrolled in the trial. Among them, 24 cases(80%) had underlying lung problems such as chronic obstructive pulmonary disease(36.4%), bronchiectasis(36.4%), bronchial asthma(3.3%), or lung cancer(3.3%). 2) In 26 cases(86.6%), we observed effective improvement, and 4 cases(13.4%) show mildly effective improvement of symptoms and signs of respiratory infection. 3) In 23 cases(73.4%), we observed bacteriological eradication in culture or decreased the number of bacteria by Gram stain which found dominantly in previous Gram stain. 4) The significant side effect was not noted. The above results suggested that sparfloxacin was effective as a first line therapy in patients with acute respiratory infection.

  • Objectives : This study is planned to classify correlation between thoracic kyphosis angle and prevalence of cervical intervertebral disc. Methods : We Measured the thoracic kyphosis angel of the 110 men and 179 women patients with neck pain in 00 Korean Medicine Hospital. We use Cobb's angle method and Thoracic cage dimension method for measuring the thoracic kyphosis. And We use Magentic Resonance Imaging(MRI) for classifying the patient who has cervical intervertebral disc or not. Results : 1. There was statistical difference on the thoracic cage dimension with gender(P<0.001), while there was no statistical difference on Cobb's angle with gender(P=0.882). 2. Age and thoracic cage dimension(r=0.383) is statistically more correlative than age and cobb's angle(r=-0.59). Conclusions : 1. Thoracic cage dimension and gender, Thoracic cage dimension and age were statistically concerned on patient who has neck pain, while Cobb's angle had no statistical correlation with age and gender. 2. There was no statistical difference on the Cobb's angle and Thoracic cage Dimension between patient who has one or more cervical disc herniation(Protrusion disc, Extrusion disc) segment and patient who didn't have cervical disc herniation(Protrusion disc, Extrusion disc) segment.

Correlation Analysis Between Lumbar Scoliosis of X-ray and HIVD of L-spine MRI in LBP Patients who Visit Korean Medicine Hospital (한방병원에 요통으로 내원한 환자에서 X-ray상 요추측만과 MRI상 HIVD와의 연관성 분석)

  • Objectives : This study is planned to classify Correlation between Cobbs Angle of Lumbar scoliosis and prevalence of Lumbar Intervertebral Disc. Methods : We Measured the lumbar scoliosis angle of the 114men and 91 women patients with lumbar pain in Bucheon Jaseng Korean Medicine Hospital. We use Cobb's angle method for measuring the lumbar scoliosis. And We use Magentic Resonance Imaging(MRI) for classifying the patient who has lumbar intervertebral disc or not. Results : 1.There was no statistical relation between the Cobbs angle and gender(P>0.05) 2.There was no statistical relation between the Cobbs angle and age(P>0.05) 3.There was significant relation between the direction of lumbar scoliosis and the direction of disc herniation(P<0.05) 4.There was no statistical relation between on the Cobbs angle and disc herniation. The more severe of lumbar scoliosis has not tendency of disc herniation. 5.There was no statistical relation between lumbar scoliosis Cobbs Angle on HIVD of L-spine patient and direction of disc herniation on horizontal plane. Conclusions : The direction of disc herniation has tendency of the opposite direction of lumbar scoliosis. When disc herniation, opposite side bending broad intervertebral foramen and reduce pressure.
