Histological Examination of Engineered Mesenchymal Stem Cells Improve Bladder Function in Rat (랫드에서 방광기능 향상의 엔지니어링 중간엽 줄기세포의 조직학적 소견)

  • This study was undertaken to examine the effects and to investigate the relevant mechanisms of overexpressing stromal cell-derived factor-1 (SDF-1) produced by engineered mesenchymal stem cells, in a neurogenic bladder (NB) rat model. Sprague-Dawley (SD) rats (N=48) were randomly divided into 4 groups comprising 12 rats each: control group, Injury group, Injury+imMSC group, and Injury+SDF-1 eMSC group. Rats in the Injury+imMSC group were treated with imMSCs, whereas the Injury+SDF-1 eMSC group were administered SDF-1 eMSCs. After 4-weeks therapy, the bladder and pelvic nerve (PN) tissues were examined by subjecting to Masson's trichrome staining and immunofluorescence. Administration of SDF-1 eMSC resulted in improved smooth muscle content in the bladder tissue, significantly increased β-III tubulin expression of the PN, and enhanced SDF-1 expression (P<0.05). The bladder wall repair can be attributed to the overexpression of SDF-1 by SDF-1 eMSCs. Significantly increased SDF-1 expression was obtained in the Injury+SDF-1 eMSC group (P<0.05). The crushed PN also showed significant recovery in the Injury+SDF-1 eMSC group (P<0.05). In conclusion, our results indicate that SDF-1 eMSCs express more SDF-1 in vivo, thereby facilitating the repair of injured nerve and recovery of NB in rats.

  • The literature review about bladder management method given to maintain and improve health of neurogenic bladder patients was done. Because the study of neurogenic bladder patients in nursing field is not enough, I tried to find report the study tendency through literature review, 1. There are five types of neurogenic bladder such as uninhibited neurogenic bladder, reflex neurogenic bladder, autonomous neurogenic bladder, sensory paralytic neurogenic bladder, and motor paralytic neurogenic bladder. 2. The accurate assessment of neurogenic bladder is done mainly through urodynamics and especially cystometrogram and urethrogram are used. 3. As the study of therapeutic management, the effect of Desmopressin, bladder auto-augmentation, incision of external urethral sphincter muscle, subarachnoid block and pudendal never block using phenol was studied. 4. For the study of general management, the effect of bladder training progam, intermittent catheterization and infection control has been studied but there has not been any obvious study in nursing field. Reviewed the study condition, it is necessary to develope bladder training program in order to increase life quality of neurogenic bladder patients.

Transplantation of Brain-Derived Neurotrophic Factor-Expressing Mesenchymal Stem Cells Improves Lower Urinary Tract Symptoms in a Rat Model (뇌유래신경영양인자 발현 중간엽 줄기세포의 하부요로증상 개선 효과)

  • This study aimed to explore the effects of brain-derived neurotrophic factor (BDNF), produced by engineered immortalized mesenchymal stem cells (imMSC), on lower urinary tract symptoms (LUTS) in a rat model with neurogenic bladder (NB). Forty-eight Sprague-Dawley (SD) rats were randomly divided into the following groups: Sham control, LUTS, LUTS+imMSC (treated with immortalized MSC), and LUTS+BDNF-eMSC (treated with BDNF-expressing MSC) groups. LUTS was induced by a crush injury to the major pelvic ganglion (MPG). Bladder function was tested under anesthesia, and bladder tissue strips were collected thereafter for contractility test and western blot analysis. Western blot results showed that the expression of both Angiopoietin 1 (Ang 1) and platelet-derived growth factor (PDGF) increased with MSC injection. The effect of treatment with BDNF-eMSC on LUTS was also evaluated, and the results were found to be better than those with imMSC (P<0.05). BDNF-eMSC prevented fibrosis in the bladder tissue and significantly reduced caspase-3 levels. In conclusion, high expression of BDNF in vivo resulted in recovery of bladder function and contractility, along with the inhibition of apoptosis in a rat model.

A Study of Occurrence of Secondary Complications and Chronic Diseases due to Aging of Spinal Cord Injury (척수손상환자의 고령화에 따른 2차합병증 발생과 만성질환발병에 관한 연구)

  • The purpose of this study is to provide basic data for effective management and prevention of chronic diseases and secondary complications as elderly spinal cord injuries(SCI). The subjects were 200 spinal cord injuries admitted to S hospital from April, 2013 to April, 2018. We investigated the occurrence of chronic diseases and secondary complication through medical records. The results showed that SCI were affected chronic diseases and secondary complications over 50years. The prevalence of chronic diseases and secondary complications over 50 years of age had the odds ratio 11.8 times higher in hypertension and 6.7 times diabetes mellitus. Secondary complications had the odds ratio Osteoporosis 7.5 times, Pneumonia 5.2 times, and central pain 0.4 times. We suggest that continuous management and service of chronic diseases and secondary complications of elderly SCI are necessary. It will be necessary to expand the target population and to study various characteristics including.

Classification of Congenital Urinary Tract Anomalies Diagnosed by Antenatal Ultrasonogram (산전 초음파로 발견된 선천성 요로계 기형의 분류)

  • Purpose : A study was done to assess the incidence and classification of congenital urinary tract anomalies detected by antenatal ultrasonogram. Methods : We reviewed 558 cases of urinary tract anomaly which were detected by antenatal ultrasonogram and postnatally confirmed between June 1989 and May 2002. We investigated the incidence and classified congenital urinary tract anomalies by review of medical records, antenatal and postnatal radiologic studies retrospectively. Results : In 558 cases of congenital urinary tract anomalies, 292 cases of hydronephrosis were found and the most common. Another anomalies were composed of 65 cases of multicystic dysplastic kidney, 32 cases of hydroureteronephrosis, 31 cases of duplication of kidney, 25 cases of renal agenesis, 21 cases of simple renal cyst, 20 cases of polycystic disease, 13 cases of ureterocele, 11 cases of renal hypoplasia, 10 cases of horseshoe kidney, 9 cases of vesicoureteral reflux, 8 cases of posterior urethral valve, 7 cases of bladder diverticulum, 6 casts of megaureter, 5 cases of ectopia, 2 cases of megacystis, and 1 case of medullary cystic disease. In 82 of the 558 cases, there were two or more combined urinary tract anomalies. Associated diseases other than urinary tract were observed in 13 cases, of which the congenital heart disease was the most common. Conclusion : The congenital urinary tract anomaly is frequently found and diverse during the antenatal ultrasonography. The multicenter study is needed to investigate precise incidence and distribution of each anomalies in general population.

Clinical Report of Baliao Electroacupuncture Treatment on a Stroke Patient Suffering from a Neurogenic Overactive Bladder (뇌경색을 동반한 신경인성 과민성 방광 환자의 배뇨장애 팔료혈 전침 치료 1례)

  • Objective: The purpose of this case study is to report the effectiveness of Baliao acupoints electro-acupuncture treatment on a neurogenic overactive bladder induced by stroke. Method: A patient suffering from middle cerebral artery infarction was diagnosed with a neurogenic overactive bladder and treated by Korean medicine, including electro-acupuncture on Baliao acupoints six times per week. The evaluation was performed by monitoring urinary frequency and using the Korean versions of the Overactive Bladder Symptom Score (OABSS) and Urinary Incontinence Quality of Life (I-QoL). Results: After 27 days of electro-acupuncture treatment, the OABSS score improved from 6 to 3. The I-QoL total score improved from 60 to 87. The patient also showed a decrease in nocturia frequency. Conclusion: This case suggests that electro-acupuncture on Baliao acupoints can be effective for overactive bladders due to supra-pontine lesions.

  • Objectives : This report intended to estimate effects taken by using Dong-Qi Acupuncture Therapy on a patient with neurogenic bladder induced by transverse myelitis(712, L1 level). Methods : From 6th May, 2005 to 30th June, 2005. The patient received Dong-Qi Acupuncture Therapy. Results : 1. Since being hospitalized, the patient's voiding has improved from intermittent catheterization to independent urination state. And the volume of residual urine has decreased from 150cc-350cc to 0cc-l0cc. 2. The urodynamic findings showed that first sensation, maximal detrusor pressure, compliance and maximal bladder capacity changed from 456ml to 195mL, from 29cmH2O to 31cmH2O, from none to 25.31mL/cmH2O, from under 650mL to 590mL, respectively. Conclusion : We considered Dong-Qi Acupuncture Therapy had a useful effect on Neurogenic Bladder.

  • The following case studies illustrate the application of oriental medical treatment of patients diagnosed with neurogenical bladder. The outstanding symptoms of two cases were general weakening and voiding difficulty, urinary retention and lack of urine sense. In one case, neurogenical bladder was caused by an operation on the femur, and in the other case, by diabetes and anemia. Western medical treatment didnot have any effect in these cases, but after oriental medical treatment, self-voiding was possible. These resu1t suggest that oriental treatment should be considered for patients with neurogenical bladder.

  • We treated a 51 year-old female patient of neurogenic bladder. The patient complained of urinary incontinence, lack of urine sense, loss of right leg sense etc. The symptoms developed right after spinal stenosis surgery. The symptoms didn't show any improvement continually. But after admission and treatment in our hospital, symptoms were improved from severe to mild and the score of Assesment of Bladder function got better. We continued to prescribe the herbal medicine over 8 weeks and could observe that symptoms were improved. This result suggests that oriental medical therapy is useful enough to treat the neurogenic bladder. Therefore further study of the approach and application of this treatment for neurogenic bladder would be necessary.

A Case Report of Complex Korean Medical Treatment for Cervical Spinal Cord Injury and Neurogenic Bladder (한의복합치료 후 호전된 경부척수손상 환자의 사지마비 및 신경인성 방광: 증례보고)

  • A 57-year-old male patient was diagnosed as Incomplete cervical spinal cord injury and Neurogenic bladder after falling accident and suffered from tetraplegia and urinary retention. The patient was hospitalized and treated with Complex Korean Medical treatment such as acupuncture, electro-acupuncture, bee venom pharmacopuncture and herbal medication for 13 weeks. International Standard for Neurological Classification of Spinal Cord Injury (ISNCSCI) was adopted to evaluate the functional recovery and Korean version of Modified Barthel Index (K-MBI) was adopted to evaluate the recovery of activities of daily living (ADL). After treatment, Total scores of ISNCSCI and K-MBI were improved. Also patient's micturition reflex was recovered and symptoms of neurogenic bladder were improved. This result shows that Korean Medical treatment may be an effective treatment option for spinal cord injury and neurogenic bladder patients. Further clinical studies are needed to clarify the effect of Korean Medical treatment on spinal cord injury.
