Herniation of the intervertebral disc is a medical disease manifesting as a bulging out of the nucleus pulposus or annulus fibrosis beyond the normal position. Most lumbar disc herniation cases have a favorable natural course. On the other hand, surgical intervention is reserved for patients with severe neurological symptoms or signs, progressive neurological symptoms, cauda equina syndrome, and those who are non-responsive to conservative treatment. Numerous surgical methods have been introduced, ranging from conventional open, microscope assisted, tubular retractor assisted, and endoscopic surgery. Among them, microscopic discectomy is currently the standard method. Biportal endoscopic spinal surgery (BESS) has several merits over other surgical techniques, including separate and free handling of endoscopy and surgical instruments, wide view of the surgical field with small skin incisions, absence of the procedure of removing fog from the endoscope, and lower infection rate by continuous saline irrigation. In addition, existing arthroscopic instruments for the extremities and conventional spinal instruments can be used for this technique and surgery for recurred disc herniation is applicable because delicate surgical procedures are performed under a brightness of 2,700 to 6,700 lux and a magnification of 28 to 35 times. Therefore, due to such advantages, BESS is a novel technique for the surgical treatment of lumbar disc herniation.

Pyramidal cells in the hippocampal CA area were recorded from and filled with neurobiotin in anesthetized rats. The extent of their dendrites and the electropharmacological properties of membrane as well as the effect before and after neurobiotin injection were examined. Pyramidal cells had a high resting membrane potential, a low input resistance, and a large amplitude action potential. A afterhyperpolarization was followed a single action potential. Most pyramidal cells did not display a spontaneous firing. Pyramidal cell displayed weak inward rectification and anodal break excitation in response to negative current injection into the cell. Membrane properties of recorded neurons before and after neurobiotin injection with consecutive current injection were compared. Some properties were significantly increased after labelling(P>0.05); the duration and amplitude of sustained AHP, input resistance, and the number of action potentials for simultaneous intra- and extracellular stimulations. Neurobiotin-filled neurons showed pyramidal morphology. Cells were generally bipolar dendrite processes ramifying in stratum lacunosum-moleculare, radiatum, and oriens.
The author examined the effects of lidocaine on the veratrine-or potassium-induced release of neurotransmitters to determine the possible role of amino acid neurotransmitters in lidocaine-induced convulsion. The examined transmitters were gamma-aminobutyric acid (GABA), aspartic acid, glutamic acid and norepinephrine which are released from the synaptosomes. Furthermore, the effects of lidocaine on the binding to receptors and synaptosomal uptake of the two transmitters, GABA and glutamic acid, were determined in crude synaptic membranes and synaptosomes. In addition, the effects of propranolol, norepinephrine and serotonin on the release of amino acid neurotransmitters were also examined. The veratrine-induced release of GABA was most severely inhibited by lidocaine and propranolol, while norepinephrine and serotonin reduced the release of aspartic acid and glutamic acid more than the GABA release. Generally the potassium-induced release was much more resistant to the lidocaine action than the veratrine-induced release. Among the neurotransmitters examined, the aspartic acid release was most prone to the lidocaine action, while the GABA release was most resistant. Concentrations of lidocaine below 1 mM did not significantly change the GABA and glutamic acid receptor binding and uptake. These results indicate that the blocking of sodium channels by lidocaine can result in the selective depression of the GABA release. This may result in unlimited excitation of the central nervous system.
There are many theory in acupuncture mechanism, so we must know the detail contents. and then we can use the acupuncture as we know. the follow article will be helpful in this part. 1. Spinal cord are role in intermediate part in somatosensorypathway also in acupuncture stumulating tract 2. Acute pain pathway started in laminae I, V of gray colmn, next are the spinothalamic tract(trigeminal spinothalamic tract in above neck part) and then go to the specific thalamic nucleus. but chronic pain in laminae II, III, VI, VII, next are spinoreticular tract(trigeminal spinoreticular tract in the neck part) and finally to the nonspecific thalamic nucleus. 3. Thalamus is very important area in somatosensory stimuation including acupuncture stumulating sensory also as a pain control center. but except this, there are Hypothalamus, Limbic system Cerebral cortex and Cerebellum as intermediator. as we Know hypothalamus is related to the emotional analgesic system with a limbic system. 4. A ${\delta$ fiber has relationship in Acute, sharp and initial pain, contrary this C fiber is related with Chronic, dull and last pain. 5. In Acupuncture mechanism of pain analgesia, there are two theory, one is gate control theory as large fiber another is stimuation produced analgesia as small diameter fier. 6. In DNIC, the stimulation sources are mechanical, thermal, heating, pain and acupuncture stimulation etc. we call these as a Heterotopic Noxious Stimulation. 7. In DNIC, SRD(Subnucleus reticularis dorsalis)is core nucleus in pain imtermediated analgesic mechanism. 8. Takeshige insisted nonacupuncture point dependent analgesic mechanism and acupuncture point dependent analgesic mechanism. and protested that Stimulation acupuncture piing evoke blocking nomacupuncture point analgesic pathway.
The physiological saline solution for animals is known as Ringer's solution which is used for keeping the function of cold blooded vertebrate animals. Primaily the saline solution is used for the purpose of perfusion experiment in frogs. Later the saline solution is applied in several kinds of animals including human being with satisfactory results. However, this saline solution was introduced to silkworm and it was found that the result was not as successful as in the case of other animals and human being. Normally, in the case of silkworm, the physiological saline solution is prepared in order to maintain the normal function of separated organs and tissues. To this end, the saline solution is adjusted to contain the certain amount and strength of ions, osmosis pressure and hydrogen concentration. The most of cases, the physiological saline solution should be prepared so that the constituent of the solution be the same with the blood selium and body fluid. The hydrogen concentration in the ion element of the saline solution is adjustable by adding Na$\^$+/, K$\^$+/, Ca$\^$++/, Mg$\^$++/ which are followed by adding of buffer solution such as NaHCO$_3$and NaH$_2$PO$_4$. Determination of optimum concentration of cation in the physiological saline solution, and the optimum mixing rate of more than two kinds of cations are based on the movement of dorsal vessel in the silkworm larvae. The optimum concentration of cations in the solution is prepared by adding NaCl solution which is under zero point. However, this solution was further added with the different concentration of KCl and CaCl$_2$. By dropping the prepared solution on the 5th larvae, the effects of solution was measured. The measurement was done by observation of movement' of dorsal vessel and its time length, and the number of pulses. According to the experiment, it was found that when only NaCl solution was applied, the number of pulses is increased for a moment, and the pulse stopped after one hour or so. When KCl solution was added the time of pulse was prolonged and in the contrast, the number of pulses was slow down. If KCl and CaCl$_2$solutions are added the time of pulse was further prolonged. Even though the adding of KCl and CaCl$_2$are found to be effectible, the correlation between the concentration of solution and the movement of dorsal vessel was not observed. However, it was same in the case of adding Ca$\^$++/ or K$\^$+/. It was found that when Mg$\^$++/ was added to dorsal vessel the number of the pulses was not decreased although the prolonged time pulse was observed.

Stroke is among the leading causes of death and long-term physical and cognitive disabilities worldwide, affecting an estimated 15 million people annually. The pathophysiological process of stroke is complicated by multiple and coordinated events. The breakdown of the blood-brain barrier (BBB) in people with stroke can significantly contribute to the development of ischemic brain injury. Therefore, BBB disruption is recognized as a hallmark of stroke; thus, it is important to develop novel therapeutic strategies that can protect against BBB dysfunction in ischemic stroke. Traditional medicines are composed of natural products, which represent a promising source of new ingredients for the development of conventional medicines. Indeed, several studies have shown the effectiveness of Korean medicine on stroke, highlighting the value of Korean medicinal treatment for ischemic stroke. This review summarizes the current information and underlying mechanisms regarding the ameliorating effects of the formula, decoction, herbs, and active components of traditional Korean medicine on cerebral ischemia-induced BBB disruption. These traditional medicines were shown to have protective effects on the BBB in many cellular and animal ischemia models of stroke, and experiments in various animal species, such as mice and rats. In addition, they showed brain-protective effects by protecting the BBB through the regulation of tight junction proteins and matrix metalloproteinase-9, reducing edema, neuroinflammation, and neuronal cell death. We hope that this review will help promote further investigation into the neuroprotective effects of traditional Korean medicines and stimulate the performance of clinical trials on Korean herbal medicine-derived drugs in patients with stroke.

This study evaluated the in vitro antioxidant activities of ethanolic Polygonum multiflorum (P. multiflorum) extracts and their cytoprotective effects on H2O2-induced HT22 and SK-N-MC cells. Among ethanolic extracts of P. multiflorum, the 40% ethanolic extract of P. multiflorum exhibited high total phenolics and flavonoid contents, with 105.68 mg of GAE/g and 28.92 mg of RE/g, respectively. The 40% ethanolic extract of P. multiflorum showed a high 2,2'-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid) diammonium salt (ABTS) radical scavenging activity and malondialdehyde (MDA) inhibitory effect. The 40% ethanolic extract of P. multiflorum also showed efficient inhibitory activity against α-glucosidase and acetylcholinesterase. Moreover, the 40% ethanolic extract of P. multiflorum reduced oxidative stress and increased cell viability in H2O2-induced HT22 and SK-N-MC cells as determined by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetra-zoliumbromide (MTT) and 2',7'-dichlorodihydrofluorescein diacetate (DCF-DA) assay. High-performance liquid chromatography (HPLC) identified 2,3,5,4'-tetrahydroxystilbene-2-O-beta-D-glucoside (TSG) as the bioactive compound in the 40% ethanolic extract of P. multiflorum.

With increasing interest in health promotion and quality of life, growing attention has been focused on the beneficial effects of urban green area. However, very few evidence-based approaches have been conducted on the health-related benefits of urban greenery. Therefore, this study examined the health-related benefits of green areas using physiological and psychological indices to obtain evidence-based data on these benefits. Twenty male university students were selected as subjects. Data were collected when participants viewed landscapes in a green area or an urban area for fifteen minutes. This research was reviewed and approved by the Ethics Committee of School of Medicine, Chungnam National University. Physiological data in the green area revealed significantly decreased heart rates, significantly increased high-frequency value of heart rate variability, an index of parasympathetic activity, and reduced salivary cortisol concentration, a stress hormone, compared to the urban area. Psychological tests showed the green area significantly reduced the negative mood state and psychological symptoms, and significantly increased the positive mood state. Our data provided evidence for the health-related benefits of green areas, and the findings of this study support that green areas can play a critical role in health promotion for urban residents, by positively affecting autonomic nervous and endocrinal activities.
Essential oils from Fennel fruit(Foeniculum vulgare Mill), Olibanum resin(Boswellia carteii Birew) and Needl Juniperus stem(Juniperus rigida Sieb.) were extracted by a supercritical fluid extraction system(SFE) and biological activity of each essential oils were observed. SFE technique was applied for the isolation and purification of nonpolar biologically active essential oils from each samples. The quantitative analysis of essential oils was carried out by gas chromatography-mass spectrometer(GC/MS). About 60% of the growth of AGS and A549 cells were inhibited by adding 1.0g/l of the crude essential oils and below 40% was observed by the control. Cytotoxicity on human normal lung cell(HEL299) was scored as $15{\sim}18%$ for the crude essential oils and 12% for control, respectively. It meant that the essential oils were more effective than the control in anti-mutagenecity tested by CHO V79 cells. The effect of the essential oils on the growth of nerve cells, PC12 was observed as follows: The viable cell density was about two times higher than control.
Objectives: Deficit schizophrenia (DS) constitutes a disease separate from non-deficit schizophrenia (NDS). The aim of the current study was to compare the quantitative EEG and low resolution electromagnetic tomography (LORETA) imaging between DS and NDS. Methods: This study was performed by 32 channels EEG for 42 schizophrenia patients who we categorized into DS and NDS using proxy instrument deficit syndrome (PDS). We performed the absolute power spectral analyses for delta, theta, alpha, low beta and high beta activities. We compared power spectrum between two groups using Independent t-test. Partial correlation test was performed with clinical parameters. Standardized LORETA (sLORETA) was used for comparison of cortical activity, and statistical nonparametric mapping (SnPM) was applied for the statistical analysis. Results: DS showed significantly increased delta and theta absolute power in fontal and parietal region compared with NDS (p<0.05). Power spectrum showed significant correlation with 'anergia' and 'hostility/suspiciousness' subscale of brief psychiatric rating scale (BPRS)(p<0.05). sLORETA found out the source region (anterior cingulate cortex/limbic part) that delta activity was significantly increased in DS (p=0.042). Conclusions: DS showed different cortical activity compared with NDS. Our results may suggest QEEG and LORETA could be the marker in differentiating between DS and NDS.
