The objective of this study was to provide guidelines for the diagnosis and successful treatment of orthodontic treatment with upper premolar extraction only Methods: The sample group consisted of 40 patients (20 with upper premolar extraction only, 20 with both upper and lower premolar extractions) who showed an overjet of more than 7 mm and were finished successfully. Lateral cephalographs were taken before and after orthodontic treatment. Landmarks showing the position of the upper and lower incisors and the position of the upper and lower lip were determined and angular measurement of these values were obtained for statistical analysis (Mann-Whitney test). Results: At pre-treatment, the position of the lower incisor was less labially inclined and the convexity of the lower lip was smaller in the upper premolar extraction only group than in the upper/lower premolar extraction group. At post-treatment, there was no difference in all measurements except for the position of the lower incisor. A comparison of changes between pre- and post-treatment showed that the retraction of upper & lower incisors and the decrease in convexity of the lower lip were greater in the upper/lower premolar extraction group. Conclusions: Treatment by upper premolar extraction can be of benefit for patients whose lower incisor proclination and lower lip protrusion are not excessive.
Purpose : Prostate specific antigen (PSA) is a useful tumor marker, which is widely used as a diagnostic index and predictor of both treatment and follow-up result in prostate cancer. A prospective analysis was carried out to obtain the period of PSA normalization and the half life of PSA and to analyze the factors influencing the period of PSA normalization. The PSA level was checked before and serially after radical radiotherapy. Materials and Method : Twen쇼 patients with clinically localized prostate cancer who underwent radical external beam radiotherapy were enrolled in this study. Accrual period was from April 1993 to May 1998. Median follow-up period was 20 months. Radiotherapy was given to whole pelvis followed by a boost to prostate. Dose range for the whole pelvis was from 45 Gy to 50 Gy and boost dose to prostate, from 14 Gy to 20 Gy. The post-irradiation PSA normal value was under 3.0 ng/ml. The physical examination and serum PSA level evaluation were performed at 3 month interval in the first one year, and then at every 4 to 6 months. Results : PSA value was normalized in nineteen patients (95%) within 12 months. The mean period of PSA normalization was 5.3 (${\pm}$2.7) months. The half life of PSA Of the nonfailing patients was 2.1 (${\pm}$0.9) month. The nadir PSA level Of the nonfailing Patients waS 0.8 (${\pm}$0.5) ng/ml. The period of PSA normalization had the positive correlation with pretreatment PSA level (R$^{2}$=0.468). The nadir PSA level had no definite positive correlation with the pretreatment PSA level (R$^{2}$=0.075). The half life of serum PSA level also had no definite correlation with pretreatment PSA level (R$^{2}$=0.029). Conclusion :The PSA level was mostly normalized within 8 months (85%). If it has not normalized within 12 months, we should consider the residual disease in prostate or distant metastasis. In 2 patients, the PSA level increased 6 months or 20 months before clinical disease was detected. So the serum PSA level can be used as early diagnostic indicator of treatment failure.
Purpose: This study is to investigate if the improvement of visual sensory (VS) by amblyopia treatment affects the ocular functions in refractive errors, accommodative errors and phoria at distance and near. Methods: 10 subjects (17 eyes, mean age of $10.7{\pm}2.9$ years) who treated amblyopia completely, were participated for this study. Refractive errors, accommodative errors, and distance and near phoria were compared between before and after treatments of amblyopia. Refractive errors and accommodative errors at 40 cm were measured using openfield auto-refractor (NVision-5001, Shin Nippon, Japan) and using monocular estimated method (MEM) respectively. Phoria was determined at 3 m for distance and at 40 cm for near using Howell phoria card, cover test or Maddox rod. Results: Mean corrected visual acuity (CVA) significantly increased from $0.46{\pm}0.11$ (decimal notation) for before amblyopia treatment to a level of $1.03{\pm}0.13$ for after amblyopia treatment (p < 0.001). For spherical refractive error, hyperopia significantly decreased from $+2.29{\pm}0.86D$ to a level of $+1.1{\pm}2.38D$ (p < 0.05) but astigmatism did not significantly change; $-1.80{\pm}1.41D$ for before treatment and $-1.65{\pm}1.30D$D for after treatment (p > 0.05). Accommodative error significantly decreased from accommodative lag of $+1.1{\pm}0.75D$ to a level of $+0.5{\pm}0.59D$ (accommodative lag) (p < 0.05). Distance phoria significantly changed from eso $2.9{\pm}6.17PD$ (prism diopters) to a level of eso $0.2{\pm}3.49PD$ (p < 0.05), and near phoria also significantly changed from eso $0.4{\pm}2.32PD$ to level of exo $2{\pm}4.9PD$ (p < 0.05). There was a high correlation (r = 0.88, p < 0.001) between improvement of visual acuity and decrease of accommodative lag. Conclusions: Hyperopic refractive error decreased with improvement of CVA or VS by amblyopia treatment. And the improvement of VS by amblyopia treatment also improved accommodative error, and changed phoria coupled with accommodation.

Purpose : This study is to evaluate the efficacy of small bowel displacement system(SBDS) in post-operative pelvic radiation therapy(RT) of rectal cancer patients by measurement of small bowel volume included in the radiation fields receiving therapeutic dose. Materials and Method : Ten consecutive new rectal cancer patients referred to the department of Radiation Oncology of Samsung Medical Center in May of 1997 were included in this study. All patients were asked to drink $Castrographin^(R)$ before simulation and were laid prone for conventional simulation and CT scans with and without SBDS. The volume of opacified small bowel on CT scans, which was to be included in the radiation fields receiving therapeutic dose, was measured using Picture archiving and communication system (PACS). Results : The average small bowel volumes with and without SBDS were 176.0ml(5.2-415.6ml) and 185.1ml(54.5-434.2ml), respectively The changes of small bowel volume with SBDS compared to those without SBDS were more than $10\%$ decrease in three, less than 10% decrease in two, less than $10\%$ increase in three, and more than $10\%$ increase in two patients. Conclusion : No significant advantage of using SBDS in post-operative pelvic RT for rectal cancer patients has been shown by small bowel volume measurement using CT scan considering additional effort and time needed for simulation and treatment setup.
The purpose of this paper is to explore the characteristics of a digital culture, its negative effects, especially automatism of perception, a poetic strategy against automatism of perception and the utility of poetry including the therapeutic function of poetry. The new technologies offer us various experience, great opportunities and rosy future by freeing us from boring and laborious tasks, and difficult decisions. Through the excessive and habitual use of new technologies, we may become too dependent on machines, losing our thinking abilities, sensibility, perception and creativity. Therefore, we need poetry to recover the sensation of life and make us feel things. Reading poetry allows us to look into someone else's soul and cultivate the ability to empathize. As a healing fountain, poetry leads us to a way for a change of heart, a recovery of psychic health and universal love through perfect communion in ourselves and in society. Furthermore we need a systematic and in-depth study of digital poetry as well as traditional poetry.

Purpose: In radiation therapy, precise calculation of dose toward malignant tumors or normal tissue would be a critical factor in determining whether the treatment would be successful. The Radiation Treatment Planning (RTP) system is one of most effective methods to make it effective to the correction of dose due to CT number through converting linear attenuation coefficient to density of the inhomogeneous tissue by means of CT based reconstruction. Materials and Methods: In this study, we carried out the measurement of CT number and calculation of mass density by using RTP system and the homemade inhomogeneous tissue Phantom and the values were obtained with reference to water. Moreover, we intended to investigate the effectiveness and accuracy for the correction of inhomogeneous tissue by the CT number through comparing the measured dose (nC) and calculated dose (Percentage Depth Dose, PDD) used CT image during radiation exposure with RTP. Results: The difference in mass density between the calculated tissue equivalent material and the true value was ranged from $0.005g/cm^3\;to\;0.069g/cm^3$. A relative error between PDD of RTP and calculated dose obtained by radiation therapy of machine ranged from -2.8 to +1.06%(effective range within 3%). Conclusion: In conclusion, we confirmed the effectiveness of correction for the inhomogeneous tissues through CT images. These results would be one of good information on the basic outline of Quality Assurance (QA) in RTP system.
Objective: To identify clinical, salivary, and bacterial changes during orthodontic treatment with follow-up to 24 months. Methods: In 30 patients, clinical (decayed, missing, and filled surfaces [DMFS], O'Leary's plaque index, and plaque pH), salivary (unstimulated and stimulated saliva, buffer capacity, pH, and occult blood), and bacterial (Streptococcus mutans and Lactobacillus) markers were evaluated. A questionnaire was employed to evaluate their hygienic-dietary habits. Data were analyzed by ANOVA, logistic regression and Spearman's correlation. Results: DMFS increased significantly, whereas the plaque index diminished, plaque pH was more acidic (p = 0.23), and unstimulated salivary flow showed significant differences during the treatment (p = 0.013). Stimulated saliva flow increased in females after the placement of appliances; buffer capacity was diminished in males during the therapy; salivary pH remained at basal values. Bacterial levels and occult blood increased to high-risk levels and were not statistically significant different between genders (p > 0.05). Two major relationships were confirmed: initial plaque with use of dental aids (r = 0.429; p = 0.018) and final DMFS with unstimulated salivary flow (r = -0.372; p = 0.043). Conclusions: The increase in retentive surfaces increased the bacterial levels, plaque pH became acidified, and gingival damage was greater. Buffer capacity was altered but maintained a healthy salivary pH during the treatment.
This study confirmed the correlation between the change of the cooling water in internal cooling system and the size of the ablation site of bovine liver in RF heat treatment equipment. The bovine liver was resection with $4{\times}4{\times}4cm^3$ in 2cm cool-tip and with $6{\times}6{\times}6cm^3$ in 3cm cool-tip for the experiments. Area and perimeter of the ablation site for bovine liver were measured by Freehand techniques of MRI. It showed area and perimeter decreased during cool-tip temperature rises 6 and 12 minutes ablation using a 2cm and 3cm cool-tip. The correlation of cool-tip temperature and area and perimeter was statistically significant the result are shown(p=.000). The measurements of area and perimeter were more accurate with MRI in actual measurements and MRI for ablation range. The statistical results using Paired sample T-test was also significant(p=.038). The ablation range of bovine liver decreased according as cooling water temperature increases in RF heat treatment equipment for reason of carbonization occurred due to does not accurately pass the RF energy. Therefore, it is considered the effect of RF heat treatment would be increased if the temperature of the cooling water consistently maintain the low temperature in order to reduce the generation of carbide at RF heat treatment and RF energy is delivered accurately.
This paper will evaluate the usefulness of 3D target of CBCT by comparing human body's posture and position when simulated treatment is being carried out as well as human phantom posture and position using CBCT which is applying OBI. From the Rando Phantom which is located in the datum point moved in parallel and rotationary direction using CBCT. Then the mean and standard deviation difference on images location difference that are acquired were compared with real the Rando Phantom' moved distance. To make a plan of simulated treatment with the same procedure of real radiation therapy, we are going to setup the Rando Phantom. With an assumption that the position is set in accurate place, we measured the setup errors accroding to the change of the translation and rotation. Tests are repeated 10 times to get the standard deviation of the error values. The variability in couch shift after positioning equivalent to average residual error showed lateral $0.2{\pm}0.2$mm, longitudinal $0.4{\pm}0.3$mm, vertical $-0.4{\pm}0.1$mm. The average rotation erroes target localization after simulated $0.4{\pm}0.2$ mm, $0.3{\pm}0.3$ mm, and $0.3{\pm}0.4$ mm. The detection error by rotation is $0{\sim}0.6^{\circ}$ CBCT 3D/3D matching using the Rando Phantom minimized the errors by realizing accurate matching during simulated treatment and patient caring.

Purpose: To analyze differences in the dose uniformity for the computed breast radiation therapy planning with tangential beam between conventional RT using wedge filter and FiF-IMRT using multileaf collimator based onsizes and volumes of breasts. Materials and Methods: Thirty breast cancer patients were classified according to the sizes and volumes of the breasts using Eclipse treatment planning system ($Varian^{TM}$, USA, V8.0). Conformity Index and Homogeneity Index were computed along with Dose Volume Histogram. Results: No differencein CI (${\pm}1.2%$) was observed. However, lower mean HI (1.67%) in FiF-IMRT was observed compared to that of the conventional RT. Statically significant (P<0.01) correlation was identified between the values of ${\Delta}HI$ (%) and physical parameters such as breast volumes and separations. Conclusion: Increase in breast volume and separation improves the dose uniformities in computed radiation therapy planning for FiF-IMRT. Physical dimension of the breast should be considered to optimize the compured radiation therapy planning.
