Factors Associated with Cognitive Function in Breast Cancer Patients Complaining Cognitive Decline (인지 저하를 호소하는 유방암 환자들의 인지 기능 관련 요인)

  • Objectives : Cognitive complaints are reported frequently after breast cancer treatments. The causes of cognitive decline are multifactorial, a result of the effect of cancer itself, chemotherapy, and psychological factors such as depression and anxiety. However, cognitive decline does not always correlate with neuropsychological test performance. The purpose of this study was to examine the relationship of subjective cognitive decline with objective measurement and to explore associated factors of cognitive function in breast cancer survivors. Methods : We included 29 breast cancer survivors who complain cognitive decline at least 6 months after treatment and 20 age-matched healthy controls. Neuropsychological tests were performed in all participants. Multivariable regression analysis evaluated associations between neuropsychological test scores and psychological distress including depression and anxiety, also considering age, education, and comorbidity. Results : There were no statistically significant differences in neuropsychological test performances. However, the breast cancer survivors showed a significantly higher depression(p=0.002) and anxiety(p<0.001) than the healthy controls did. Among the cancer survivors, poorer executive function was strongly associated with higher depression(${\beta}=-0.336$, p=0.001) and anxiety(${\beta}=-0.273$, p=0.009), after controlling for age, education, and comorbidity. In addition, poorer attention was also significantly related with depression(${\beta}=-0.375$, p=0.023) and anxiety (${\beta}=-0.404$, p=0.013). Conclusions : The results of this study showed the discrepancies between subjective complaints and objective measures of cognitive function in breast cancer survivors. It suggests that subjective cognitive decline could be indicators of psychological distress such as depression and anxiety.

Effects of a Mindfulness-Based Mind-Body Intervention Program using Marine Resources on the Improvement of Sleep Quality and Mood Symptoms in Korean Female Emotional Labor Workers : A Pilot Study (해양자원을 활용한 심신치유기법이 여성 감정노동자들의 수면, 우울 및 기분 증상 개선에 미치는 효과 : 예비 연구)

  • Objectives: The purpose of this pilot study was to examine the effects of a mindfulness-based Body-Mind Intervention Program using marine resources on the improvement of quality of sleep, mood symptoms, and cognitive function in Korean female emotional labor workers. Methods: Twelve female workers who experienced excess emotional labor participated at the training camp program for five days in Danghangpo-ocean park, Goseung-gun, Gyeongnam Province in South Korea. Participant sleep quality, mood symptoms, and cognitive functioning before, after, and 1.5 months later were evaluated and analyzed. Results: After participating in the marine resource program, participants reported significantly decreased sleep latency. Global sleep quality, cognitive functions (attention, flexibility, and inhibition control), and mood states, including depression, tension, anger, fatigue, were also improved. These effects were generally maintained after 1. 5 months (PSQI t = 2.63, p = 0.02 ; HAM-D t = 5.92, p < 0.001). Conclusion: A Body-Mind Intervention Program using marine resources was effective in relaxing emotion-related tension and improving cognitive function. To advance this pilot study, it is necessary to carry out further research to investigate the use of marine resources in mental health interventions.

Identification of major risk factors association with respiratory diseases by data mining (데이터마이닝 모형을 활용한 호흡기질환의 주요인 선별)

  • Data mining is to clarify pattern or correlation of mass data of complicated structure and to predict the diverse outcomes. This technique is used in the fields of finance, telecommunication, circulation, medicine and so on. In this paper, we selected risk factors of respiratory diseases in the field of medicine. The data we used was divided into respiratory diseases group and health group from the Gyeongsangbuk-do database of Community Health Survey conducted in 2012. In order to select major risk factors, we applied data mining techniques such as neural network, logistic regression, Bayesian network, C5.0 and CART. We divided total data into training and testing data, and applied model which was designed by training data to testing data. By the comparison of prediction accuracy, CART was identified as best model. Depression, smoking and stress were proved as the major risk factors of respiratory disease.

A Study on the Development of ICT Curriculum for Private Security Workers (민간경비 종사자의 ICT직무교육과정 개발에 관한 연구)

  • This study developed the ICT curriculum focusing on apartment security guards and analyzed the actual application results. We developed the educational contents and it designed ICT job courseware for elder education by considering and that designed in consideration of the computer attitudes and physical and cognitive characteristics of the elderly. The educational program was applied to the experimental group and the satisfaction and the educational effect were analyzed through the pre and post test results. As a result of the analysis, the satisfaction level of the education program was higher than the average of 4.00 points. The elderly informal education program was examined by using the qualitative assessment test tool before and after the application of the education program. Internet self-efficacy of experiment group, improvement of internet utilization ability, usage of internet use, personal information willingness, life correction, elimination of conflict among generations, self-confidence, and internet stress increased statistically significantly. These results suggest that the "Private Expense ICT Job Training" program developed in this study can be effective for elderly informatization and can be applied to other elderly education programs and contribute to promotion.

Evaluation of Health Impact of Heat Waves using Bio-Climatic impact Assessment System (BioCAS) at Building scale over the Seoul City Area (생명기후분석시스템(BioCAS)을 이용한 폭염 건강위험의 검증 - 서울시 건물규모를 중심으로 -)

  • The Bio-Climatic impact Assessment System, BioCAS was utilized to produce analysis maps of daily maximum perceived temperature ($PT_{max}$) and excess mortality ($r_{EM}$) over the entire Seoul area on a heat wave event. The spatial resolution was 25 m and the Aug. 5, 2012 was the selected heat event date. The analyzed results were evaluated by comparing with observed health impact data - mortality and morbidity - during heat waves in 2004-2013 and 2006-2011,respectively. They were aggregated for 25 districts in Seoul. Spatial resolution of the comparison was equalized to district to match the lower data resolution of mortality and morbidity. Spatial maximum, minimum, average, and total of $PT_{max}$ and $r_{EM}$ were generated and correlated to the health impact data of mortality and morbidity. Correlation results show that the spatial averages of $PT_{max}$ and $r_{EM}$ were not able to explain the observed health impact. Instead, spatial minimum and maximum of $PT_{max}$ were correlated with mortality (r=0.53) and morbidity (r=0.42),respectively. Spatial maximum of $PT_{max}$, determined by building density, affected increasing morbidity at daytime by heat-related diseases such as sunstroke, whereas spatial minimum, determined by vegetation, affected decreasing mortality at nighttime by reducing heat stress. On the other hand, spatial maximum of $r_{EM}$ was correlated with morbidity (r=0.52) but not with mortality. It may have been affected by the limit of district-level irregularity such as difference in base-line heat vulnerability due to the age structure of the population. Areal distribution of the heat impact by local building and vegetation, such as spatial maximum and minimum, was more important than spatial mean. Such high resolution analyses are able to produce quantitative results in health impact and can also be used for economic analyses of localized urban development.

  • 한국건강관리협회는 1998년부터 국민건강증진기금의 지원으로 금연사업을 수행하고 있다. 본 연구에서는 협회에서 실시한 '07년 민간단체 금연클리닉 사업을 대상으로 사업의 효과성을 분석하여 사업의 과학적 근거 확보 및 향후 효과적인 사업 수행방안 마련 등 사업의 발전방안을 모색하고자 실시되었다. 대상은 2007년 민간단체 금연클리닉에 참여한 군 경, 중소업체 종사자, 운송업체 종사자, 국방부지정부대 참여자, 학교/공공기관 종사자 중 설문조사에 성실하게 응답한 3,170명이다. 민간단체 금연클리닉 사업 효과성 평가 설계는 구조화된 평가도구를 활용하여 금연클리닉에 참여한 집단만을 대상으로 참여 전(사전조사)과 금연클리닉 종료 후(사후조사)의 조사결과를 비교하는 단일 전후조사 설계(one-group pretest posttest design)방법을 사용하였다. 연구결과 민간단체 금연클리닉 참여자의 하루 평균 흡연량은 15.96개비, 평균 흡연시작연령은 18.59세, 평균 총 흡연기간은 146.39개월로 나타났으며, 참여자의 니코틴 의존도는 평균 3.81점으로 조사되었다. 또한 참여자의 84.5%가 의지로 금연 시도하였으나 실패한 것으로 나타나 이에 대한 개입이 필요함을 알 수 있다. 참여자들의 상황별 흡연욕구는 높은 수준이었으며, 특히 술자리, 스트레스상황 등 특정 상황에서 흡연욕구가 높게 나타났다. 금연관련 경험요인은 5점 기준 2.84점으로 조사되었으며, 인지적 과정이 행동적 과정에 비해 높게 나타났다. 마지막으로 민간단체 금연클리닉의 전반적인 만족도는 높았으나 상담환경에 대한 만족도가 상대적으로 낮았음을 감안할 때, 환경적인 부분에 대한 개선이 필요할 것으로 보인다. 더불어 금연클리닉의 효과나 만족도 부분에서 대상자의 특성에 따른 차이가 발생하므로 참여 대상자의 특성을 감안한 맞춤형 프로그램 개발이 필요할 것이다.

Understanding Assessment for Feeding Disorders in Autistic Spectrum Disorders: A Literature Review (자폐 스펙트럼 장애 섭식장애 평가의 이해: 문헌 고찰)

  • Objective : Children with autism spectrum disorder (ASD) commonly suffer from feeding disorders. Major feeding problems include mealtime behavior problems, picky eating, and a lack of food variety can lead to nutritional problems, developmental and social limitations, and stress for the caregivers. A review of the latest literature was conducted to gain an in-depth understanding of assessment tools for feeding disorders in children with ASD. Method : This study analyzed assessments to identify feeding problems in ASD based on previous studies searched through keywords such as ASD, ASD feeding problem, and ASD feeding evaluation. Results : The ASD feeding disorder assessment was divided into direct and indirect assessments. Indirect assessment, in which caregivers measure a child's situation using questionnaires, is mainly used. The assessment of feeding disorders in children with ASD was divided into 1) mealtime behavior, 2) sensory processing, 3) food consumption, and 4) others. Conclusion : As the main feeding disorder characteristics of children with ASD are very diverse, a comprehensive evaluation is necessary but is still limited. Swallowing rehabilitation experts, such as occupational therapists, should apply comprehensive assessment tools based on a basic understanding of the feeding problems, behaviors, and sensations in ASD.

Prediction of Sleep Disturbances in Korean Rural Elderly through Longitudinal Follow Up (추적 관찰을 통한 한국 농촌 노인의 수면 장애 예측)

  • Objectives: Sleep disturbance is a very rapidly growing disease with aging. The purpose of this study was to investigate the prevalence of sleep disturbances and its predictive factors in a three-year cohort study of people aged 60 years and over in Korea. Methods: In 2012 and 2014, we obtained data from a survey of the Korean Social Life, Health, and Aging Project. We asked participants if they had been diagnosed with stroke, myocardial infarction, angina pectoris, arthritis, pulmonary tuberculosis, asthma, cataract, glaucoma, hepatitis B, urinary incontinence, prostate hypertrophy, cancer, osteoporosis, hypertension, diabetes, hyperlipidemia, or metabolic syndrome. Cognitive function was assessed using the Mini-Mental State Examination for dementia screening in 2012, and depression was assessed using the Center for Epidemiologic Studies Depression Scale in 2012 and 2014. In 2015, a structured clinical interview for Axis I psychiatric disorders was administered to 235 people, and sleep disturbance was assessed using the Pittsburgh Sleep Quality Index. The perceived stress scale and the State-trait Anger Expression Inventory were also administered. Logistic regression analysis was used to predict sleep disturbance by gender, age, education, depression score, number of coexisting diseases in 2012 and 2014, current anger score, and perceived stress score. Results: Twenty-seven percent of the participants had sleep disturbances. Logistic regression analysis showed that the number of medical diseases three years ago, the depression score one year ago, and the current perceived stress significantly predicted sleep disturbances. Conclusion: Comorbid medical disease three years previous and depressive symptoms evaluated one year previous were predictive of current sleep disturbances. Further studies are needed to determine whether treatment of medical disease and depressive symptoms can improve sleep disturbances.

Gender Differences in Behavioral Psychological Symptoms of Dementia in Patients with Alzheimer's Disease (알츠하이머병 환자에서 행동심리증상의 성별차이)

  • Objectives : Behavioral and psychological symptoms of dementia(BPSD) represent significant clinical problems, resulting in functional decline, caregiver distress, institutionalization and increased mortality. A recent study showed gender differences have important role in the development of BPSD, but relationship between BPSD and gender has never been studied in Korea. This study was designed to examine whether patients with Alzheimer's disease show gender differences in behavioral and psychological symptoms of dementia(BPSD). Methods : Ninety-eight subjects with Alzheimer's disease were included in this study. We carried out history taking and cognitive assessment for the diagnosis of Alzheimer's disease based on DSM-IV. Cognitive impairment and BPSD were measured using the Mini Mental State Examination(MMSE), Global Deterioration Scale (GDS), Clinical Dementia Rating(CDR) and the Korean Neuropsychiatric Inventory(K-NPI). Independent samples t-test was used to examine the differences across gender in BPSD. Correlation analysis between MMSE, CDR, GDS and NPI was performed using Pearson's correlation coefficient. Results : There were no significant gender differences between the gender in BPSD. We found statistically significant negative correlations between MMSE with NPI total score, and with scores of several sub-domains such as hallucination. Conclusions : This study showed that gender differences in BPSD are not significant. Further research is necessary to identify whether BPSD affect gender differences or individual differences.

Anti-amnesic and Neuroprotective Effects of Artemisia argyi H. (Seomae mugwort) Extracts (섬애쑥 추출물의 뇌 신경세포 보호효과에 의한 학습 및 기억능력 개선 효과)

  • The anti-amnesic effect of Artemisia argyi H against trimethyltin (TMT)-induced learning and memory impairment and its neuroprotective effect against $H_2O_2$-inducedoxidative stress were investigated. Cognitive behavior was examined by Y-maze and passive avoidance test for 4 weeks, which showed improved cognitive functions in mice treated with the extract. In vitro neuroprotective effects against $H_2O_2$-induced oxidative stress were examined using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium-bromide and lactate dehydrogenase (LDH) assays. A. argyi H. extract showed protective effects against $H_2O_2$-induced neurotoxicity; moreover, LDH release into the medium was inhibited. Finally, high-performance liquid chromatography (HPLC) analysis showed that eupatilin and jaceosidin were the major phenolic compounds in A. argyi H. extract. These results suggest that A. argyi H. could be a good source of functional substances to prevent neurodegenerative diseases.
