Association between Antipsychotic-Induced Restless Legs Syndrome and Glutathione S-Transferase Gst-M1, Gst-T1 and Gst-P1 Gene Polymorphisms (Glutathione S-Transferase (GST) 유전자 다형성과 항정신병약물로 유발된 하지불안증후군의 연관 연구)

  • Objectives: The pathophysiology of restless legs syndrome (RLS) has not been fully elucidated. Oxidative stress might play a role in the development of RLS and other antipsychotic-induced side effects such as tardive dyskinesia. In the present study, we investigated whether the glutathione S-transferase (GST) gene polymorphisms are associated with antipsychotic-induced RLS in schizophrenia. Methods: We assessed antipsychotic-induced RLS symptoms in 190 Korean schizophrenic patients using the diagnostic criteria of the International Restless Legs Syndrome Study Group. The GST-M1, GST-T1 and GST-P1 loci were analyzed using PCR-based methods. Results: We divided the subjects into 2 groups: those with RLS symptoms (n = 96) and those without RLS symptoms (n = 94). There were no significant differences in the distributions of the GST-M1 genotypes (${\chi}^2=3.56$, p = 0.059), GST-T1 (${\chi}^2=0.51$, p = 0.476) and GST-P1 (${\chi}^2=0.57$, p = 0.821) between the 2 groups. Comparison of the RLS score among genotypes of the GST-M1 (t = -1.54, p = 0.125), GST-T1 (t = -0.02, p = 0.985) and GST-P1 (F = 0.58, p = 0.560) revealed no significant difference. Conclusion: These data suggest that GST gene polymorphisms do not confer increased susceptibility to RLS symptoms in schizophrenic patients. Future studies are necessary to evaluate the possible influences of other candidate genes involved in the reactive oxygen species system.

Heart Rate Variability and Lipid Profile in Patients with Major Depressive Disorder (주요우울장애 환자에서의 심박변이도와 혈중 지질 농도와의 연관성)

  • Objectives: The analysis of heart rate variability (HRV) is a useful non-invasive tool to investigate the autonomic nerve function. Previous studies on the relationship between HRV and depression have been reported controversial results. Similarly, the correlation between the serum lipids and depression is debatable. The purpose of this study is to examine the relationship between heart rate variability, lipid profile and depression. Methods: A total of 42 patients with major depressive disorder (MDD) and 32 age and sex-matched normal subjects who had no previous history of major medical and mental illnesses were recruited for this study. A structured-interview was used to assess the general characteristics and psychiatric illness. HRV measures were assessed by time-domain and frequency-domain analyses. Psychological symptoms were measured using the Hamilton rating scale for anxiety (HAM-A), Hamilton rating scale for depression (HAM-D). In addition, the evaluation for lipid profile was performed by blood test. Results: In serum lipid profile test, MDD group showed higher cholesterol ($197.68{\pm}42.94$ mg/dL vs. $176.85{\pm}34.68$ mg/dL, p=0.044), TG ($139.45{\pm}92.54$ mg/dL vs. $91.4{\pm}65.68$ mg/dL, p=0.018), LDL ($130.03{\pm}33.18$ vs. $106.62{\pm}27.08$, p=0.004) level than normal control group. In HRV time domain analyses, the standard deviation of the NN interval (SDNN) was decreased in MDD group than normal control group, but was not significant ($32.82{\pm}14.33$ ms vs. $40.36{\pm}21.40$ms, p=0.078). ApEn (Approximate Entrophy) was significantly increased in MDD group than normal control group ($1.13{\pm}0.11$ vs. $0.91{\pm}0.18$, p<0.001). ApEn was correlated with LDL level (r=0.277, p=0.028), HAM-D scores (r=0.534, p<0.001) and HAM-A scores (r=0.470, p<0.001). Conclusions: MDD patients showed increased ApEn, one of the HRV measurement. And this ApEn was correlated with LDL, HAM-D and HAM-A scores. In this study, the analysis of ApEn would be a useful test of MDD.

Thiobarbituric Acid Reactive Substances Levels in Brain Tissue of Aldh2 Knockout Mice Following Ethanol Exposure for 8 Weeks (Aldh2 knockout 마우스에서 8주간 에탄올 노출에 따른 뇌조직의 thiobarbituric acid reactive substances 농도)

  • Excessive alcohol consumption causes various degenerative brain diseases including Alzheimer's disease and Parkinson's disease. Absorbed ethanol is metabolized to acetaldehyde and acetic acid by alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH). Acetaldehyde is well known as a toxicant through generation of reactive oxygen species (ROS). Therefore, ALDH2 activity may play important roles in the pathogenesis of alcohol-induced brain diseases. In this study, we demonstrated the effects of ALDH2 enzyme activity on lipid peroxidation in brain tissues and urine of mice exposed to ethanol for 8 weeks. Five male, 8-week old Aldh2 (+/+) and Aldh2 (-/-) mice (C57BL/6J strain) in each group were exposed to ethanol for 8 weeks (2 g/kg wt./day) using gavage, and those in the control group received 0.9% saline alone. Thiobarbituric acid reactive substances (TBARS) level, a marker for lipid peroxidation, was measured in whole brain tissue and urine by high performance liquid chromatography. As a result, chronic ethanol treatment did not show any statistical change on the TBARS level of brain tissue in both Aldh2 (+/+) mice and in Aldh2 (-/-) mice. However, following ethanol exposure for 8 weeks in Aldh2 (-/-) mice, the urinary TBARS levels were significantly increased to more than double compared to the pretreatment group. This result was not observed in Aldh2 (+/+) mice. These results suggest that although ALDH2 enzyme activity plays a role in the generation of ROS in the whole body, it does not seem to be important in the pathogenesis of alcohol induced degenerative brain diseases.

Effects of Conversion of Infertility Treatment on Semen Quality (불임시술의 전환이 정맥상태에 미치는 영향)

  • Objective: To investigate whether semen parameters in infertile couples who undergone intrauterine insemination (IUI) change in the subsequent IUI cycle and the subsequent in vitro fertilization (IVF) cycle. Methods: Fifty-three infertile couples who had failed to become pregnant after the first IUI cycle with computer-assisted semen analysis (CASA) were included. After the first IUI, thirty-eight couples underwent the second IUI (Group 1), and fifteen underwent IVF-ET procedure (Group 2). All semen parameters including semen volume, concentration, motility and total motile sperm count were analyzed in the second IUI or IVF-ET procedure for comparison with the result of first IUI. Results: There were no significant differences in husband age, interval between the first and second procedure and cause of infertility. In Group 1, only sperm motility at the time of the latter IUI was significantly decreased when compared to the former IUI irrespective of the first semen parameters. In Group 2, sperm concentration, motility and total motile sperm count at the time of subsequent IVF were lower than the former IUI. By sub-analyses of Group 2, in the group of optimal semen parameter at IUI cycle, sperm concentration and total motile sperm count at the time of subsequent IVF were lower than the former IUI, while in the group of suboptimal semen parameter at IUI cycle, only sperm motility at the time of subsequent IVF were lower than the former IUI. Conclusion: The semen parameters in couples converted to IVF cycle were more adversely affected than those remained in IUI cycle. Further study on psychological stress should be necessary to explain the reason.

Quantification of Isoflavone Malonylglucosides in Soybean Seed during Germination (콩 발아 중 isoflavone malonylglucosides의 함량 변이)

  • Soybean produces three major types of isoflavones, daidzein, genistein, and glycitein aglycones and their glucosides and malonylglucosides. It has been known that malonylated glucosides are rapidly converted to their corresponding aglycones due to the unstable thermolabile glucoside malonates; therefore, the analytical study of malonylated glucosides has been insufficient. In this study, we analyzed the malonylglucoside content in soybean seeds. Isoflavone analysis of three soybean cultivars revealed that 81.5~90.0% of the total isoflavones were malonylglucosides, whereas aglycones were rarely detected. Moreover, the total isoflavone content increased during a 5-day germination period where growth regulators and coumaric acid treatments tended to yield higher isoflavone content than the normal germination treatment, however the differences were not significant; notably, the isoflavone accumulation trend continued with additional germination days. The content of malonylglucoside was higher than that of other isoflavones, which was 83.7~86.6% of the total isoflavone content in seeds with a 3-day germination period. Furthermore, isoflavones were significantly accumulated in the hypocotyl of seedlings with a 5-day germination period. The content of isoflavone in the hypocotyl of the Pungsannamul-kong was 10,240 ug/g when treated with coumaric acid, which was considerably higher than that of other cultivars and treatments. Additionally, soybean seeds heated at $60^{\circ}C$ for 1 hour produced higher isoflavone content than non-heated soybean seeds. Our results show that it is possible to increase the isoflavone content in soybean seeds through various treatments.

Risk Factors for the Failure of Non-operative Reduction of Intussusceptions (장중첩증에서 비수술적 정복의 실패 위험인자)

  • Purpose: Intussusceptions are one of the most common causes of intestinal obstruction in infants and young children. Although it is easily treated by non-operative reduction using barium, water or air, this treatment is very stressful for young patients and may cause bowel perforation, peritonitis and shock. In this study, we identified the risk factors associated with the failure of non-operative reduction, to identify a group of children that would benefit from the procedure and those who would not. Methods: We reviewed the medical records of patients with intussusception who were treated at the Korea University Medical Center Ansan hospital from March 1998 to July 2006. Three hundred fourteen children with intussusception were identified. Among them, non-operative reductions were performed in three hundred. Clinical and radiological variables were compared according to the failure or success of the non-operative reduction. Results: Non-operative reductions were successful in 243 (81%) and failed in 57 (19%). The group that had failed procedures had a younger age (12.3${\pm}$17.2 months vs. 18.0${\pm}$15.8 months, p=0.03), longer symptom duration before reduction (33.6${\pm}$29.0 hr vs. 21.5${\pm}$20.3 hr, p<0.01), more vomiting and lethargy (p<0.01), but less abdominal pain and irritability (p<0.01), compared with the group that had a successful procedure. Logistic regression analysis showed that the factors associated with the failure of non-operative reductions were a younger age, less than 6 months of age (odds ratio: 2.5, 95% confidence interval: 1.2~5.2, p=0.01), duration of symptoms, longer than 24 hrs before reduction (odds ratio: 2.1, 95% confidence interval: 1.2~4.2, p=0.03), bloody stool (odds ratio: 4.8, 95% confidence interval: 1.9~12.2, p<0.01), lethargy (odds ratio: 3.4, 95% confidence interval: 1.1~10.4, p=0.04), and abdominal pain or irritability (odds ratio: 0.2, 95% confidence interval: 0.1~0.4, p<0.01). Conclusion: For children with intussusception, an age younger than 6 months, and duration of symptoms more than 24 hrs before reduction, as well as the presence of bloody stools, lethargy and abdominal pain or irritability were variables associated with failure of a non-operative reduction. Knowledge of these variables should be considered in making clinical decisions for therapeutic interventions.

식품영양과 콜레스테롤

  • 콜레스테롤은 성호르몬, 스테로이드 호르몬, 비타민 D, 담즙산 및 다른 체내화합물에 존재하는 생명에 필수적인 물질이며, 특히 세포막에는 이의 안정적인 기능수행을 위해 일정량의 콜레스테롤이 반드시 존재해야 한다. 이러한 중요성 때문에 우리의 몸안에는 완전히 음식물의 섭취에 의한 공급에 의존하지 않고서도 필요에 따라 체조직의 온전한 기능수행을 위해 즉시 이용될 수 있는 충분한 양의 콜레스테롤이 존재한다. 따라서 체내에서 자체적으로 합성되는 콜레스테롤의 양은 음식물을 통해 흡수되는 양보다 일반적으로 훨씬 높다. 매일 우리의 체내에서는 약 1,500mg 정도의 콜레스테롤이 합성되는데, 주로 간세포에서 만들어지며, 소장세포에서도 일부 합성이 이루어진다. 식단(食單)의 구성에 따라 물론 차이는 있으나, 우리가 매일 음식물을 통해 섭취하는 콜레스테롤의 양은 평균 300~700mg 정도이다 .이중 소장을 통해 흡수될 수 있는 양은 50% 정도에 불과하여 대부분 식사를 통해 흡수되는 콜레스테롤의 양은 실제적인 의미에서 그리 큰 비중을 차지하지 않는다. 대부분의 건강한 사람은 혈액내 콜레스테롤의 수준이 항상 정상적인 범위내에서 유지될 수 있도록 기능적인 체내 Feed-back 기작을 유지하고 있다. 음식물을 통한 식이(食餌)콜레스테롤의 섭취량이 많을 경우에는 이 물질의 장관(腸管)내 흡수율이 낮아지고, 체내 콜레스테롤의 합성량 역시 저하된다. 이러한 방법으로 대부분의 건강한 사람은 음식물을 통한 많은 양의 식이 콜레스테롤 섭취에 대해 효율적으로 보상하는 체내기작을 갖게 되어 혈중 콜레스테롤 수준이 필요 이상 높아지는 것을 막게 된다. 물로 s이러한 보상작용(補償作用)은 사람에 따라 일정하지는 않다. 최근의 연구결과에 의하면, 건강한 사람들의 약 60%정도는 하루에 3개 정도의 달걀에 포함되어 있는 양 정도의 콜레스테롤을 추가적으로 보상시킬 수 있다고 하는데, 이는 달걀 이외의 음식물에서도 섭취되는 콜레스테롤 양을 감안할 때 하루 총 1,000~1,500mg에 해당하는 양이다. 뿐만 아니라 이중 일부 사람들은 일반적인 식단 하에서 6개의 달걀을 추가하여 섭취하여도 혈중 콜레스테롤 수준이 높아지지 않는다고 한다. 여러 나라에서 아직도 일률적으로 권장되는, 음식물을 통한 일당 콜레스테롤 섭취량을 최고 300mg으로 제한해야 한다는 것은 건강인에 있어서는 앞에서 언급한 바와 같은 생리적인 피이드-백 기작으로 말미암아 혈중 콜레스테롤 수준을 저하시키지 못하거나 미미한 정도에 불과하다. 혈중 콜레스테롤 수준이 정상적인 범위인 180~240 mg/dl에 해당하는 대부분의 건강인에게도 콜레스테롤이 많이 함유된 달걀이나 기타 축산물을 이용한 식품의 지속적인 섭취를 적극 피하도록 권장하는 것은 국민보건상 별로 큰 위미가 없다고 생각한다. 왜냐하면 이로 말미암아 국민 건강상 문제점이 크게 향상되었다는 연구보고는 아직 발견되지 않고 있기 때문이다. 더욱이 동물성 콜레스테롤 다량 함유식품인 달걀, 우유, 유제품 및 육류 등의 섭취를 꺼리게 되면 이들 식품들이 영양생리학상 매우 중요한 양질의 영양소를 많이 함유하고 있기 때문에 여러 중요한 필수 영양소의 공급상태를 뚜렷히 감소시키게 된다. 병적으로 혈중 콜레스테롤의 수준이 높은 사람은 관상성 심장병의 발병 내지는 심장경색에 의한 사망에 대한 통계학적 위험성이 증가된다. 고콜레스테롤 혈증(청, 중년층의 경우 260mg /dl 이상) 환자중 많은 사람들은 대부분 음식물을 통한 다량의 콜레스테롤 섭취에 의해 혈중 콜레스테롤 수준이 높아진 것이 아니고, 주로 유전적인 콜레스테롤 대사 결함에 그 원인이 있다고 한다. 이런 환자들의 경우에는 콜레스테롤 및 지방함량이 낮은 음식물만 계속적으로 섭취한다고해서 혈중 콜레스테롤의 수준을 만족할만한 정도까지 낮출 수 있는 것은 아니기 때문에 주로 의학적 약물투여에 의한 치료법을 적용한다. 식이성 고콜레스테롤 증상을 보이는 사람들에 있어서는 음식물의 섭취방법을 조절해 줄 필요가 있는데, 가장 효과적인 식이요법은 체중을 정상적인 수준으로 조절하는데 있다. 이러한 환자의 경우는 지방을 통한 열량 섭취량을 총열량의 30% 정도 수준까지 감소시키는 것이 좋으며, 팔미틴산의 함량이 높은 지방의 섭취는 가능하면 삼가는 것이 좋다. 단순 및 고도 불포화지방산의 함량이 높은 지방의 섭취는 혈중 콜레스테롤의 수준을 한 포인트 정도 낮추는데 추가적으로 기여할 수 있다. 최근의 연구에 의하면, 단순 불포화 지방산인 올레인산을 많이 함유하고 있는 올리브 기름이 특히 혈중 콜레스테롤 수준에 대한 유리한 효과를 나타낸다고 하는데, 이는 오로지 혈액내 LDL-Cholesterol을 감소시키는 작용에 기인한다. 이에 반해 고도 불포화지방산인 리놀산은 저밀도지단백질 부분 외에도 심장 보호성(心臟 保護性) HDL-부분까지도 저하시키게 된다. 따라서 특수하게 리놀산을 강화시켜 제조한 마아가린이나 이와 유사한 기름을 이용한 리놀산 강화식품의 섭취는 이러한 이유 외에도 일정한 부작용이 알려져 있어 더 이상 강하게 추천되지 않고 있다. 만일 이러한 식이조절에도 불구하고 혈중 콜레스테롤 수준이 원하는 정도까지 떨어지지 않을 경우, 물론 콜레스테롤의 섭취를 최저로 제한할 수 있는 방법을 꾸준히 시도해 봐야 하나 일반적으로 미미한 정도의 효과만을 가져온다. 유전 및 식이와 관계되는 사항 외에도 추가적으로 고려해야 할 점은, 혈중 콜레스테롤 수준은 나이, 성별, 스포츠, 흡연, 스트레스 및 음주 등과 같은 요인에도 결정적인 영향을 받는다는 사실이다. 물론 현재까지 과도하게 증가된 혈중 콜레스테롤 수준을 저하시킴으로써 전체 국민의 사망률을 현저하게 낮추고 평균수명을 연장시켰다는 증거는 발견되지 않고 있다. 왜냐하면 단독 식이 요법을 통한 해당 연구들이 현재까지 뚜렷한 긍정적인 결과를 제시하지 못하고 있기 때문이다. 의약품의 투여로 심장경색 및 이로 인한 사망을 감소시킬 수 있었다는 보고는 여러 연구결과들에서 보여주고 있으나, 그대신 다른 질병 및 암에 의한 사망률 역시 동시에 증가되었다고 한다. 이러한 이유로 말미암아 콜레스테롤 저하요법에 있어서 본래의 치료목적 이외의 위험성에도 항상 주의해야할 필요성이 존재한다는 사실을 주지해야 한다.

A Study on the Relationship Between Radial Growth and Climate Factors by Regions in Korean Pine (Pinus koraiensis) (지역별 잣나무의 연륜생장과 기후인자와의 관계 연구)

  • This study was carried out to analyze the relationship between major climatic factors (monthly temperature, precipitation) and radial growth of Korean Pine (Pinus koraiensis) in Inje-gun(Mt. Seorak), Sanchung-gun and Namwon-si(Mt. Jiri). Radial growth measurements were crossdated and correlated with climate variables. The climatic factors used were monthly mean temperature and monthly precipitation from August of previous year to September of the current year. In Inje-gun, radial growth was negatively correlated with monthly mean temperature in April, May, June, and August of the current year and was positively correlated with precipitation in August, October of the previous year and in April, August of the current year. Sanchung-gun showed a negative correlation with monthly mean temperature in September of the previous year and August of the current year but positive correlation in January of the current year. In contrast, precipitation in September of the previous year and August of the current year showed a positive influence. The results suggest that high temperature together with low precipitation may thus cause water stress and thereby limit radial growth in this region. In Namwon-si, the climategrowth relationships show that radial growth was negatively correlated with monthly mean temperature in August, October of the previous year and June of the current year. But August of the previous year and May, August of the current year in precipitation was positively correlated with radial growth. In the study areas, precipitation plays an important role for the radial growth of Kroean Pine (Pinus koraiensis). The results suggest that precipitation is important in the radial growth because water deficit in trees would take place in response to high temperature in the study area. Also there was some differences with study areas in the effects of both climatic variables probably as a result of different stand structure and geographical conditions and micro-climate.

Health Conditions and Health Behaviors of Merchants at Traditional Markets (전통시장 상인의 건강상태와 건강행태)

  • This cross-sectional descriptive study was conducted to determine the health conditions and health behaviors of merchants at traditional markets and compare them with members of the general population[ED highlight - do you mean "members of the general population"?]. The data were collected from 307 merchants at N traditional market in C City from 1-20 June 2015 at D Health Center of C City after physical measurement and blood collection and then analyzed using SPSS WIN 21.0. The male, female, and overall smoking rates of merchants at the traditional market were 34.2%, 3.5% and 11.1%, the annual drinking rates were 68.4%, 38.5%, and 45.9%, and the rates of subjects under stress and the rates of experiencing depression were 23.7%, 28.5%, and 27.4% and 5.3%, 6.1%, and 5.9%, respectively, which were all relatively lower than members of the general population[ED highlight - please ensure that my changes here are in keeping with your intended meaning.]. The male, female and overall rates of medium-level physical activities and walking were 32.9%, 19.9%, and 23.1% and 85.5%, 79.2%, and 80.8%, respectively, to have relatively good health behaviors, and the mental health is relatively fine than members of the general population[ED highlight - please clarify this, I cannot infer your intended meaning.].   • Purpose: The purpose of this study was to examine the prevalence of food insecurity in Korean elderly and to analyze the health status as well as food and nutrient intakes according to food insecurity status. Methods: A total of 939 elderly subjects (over 65 years old) were used in our analysis from the fifth 2010 Korean National Health and Nutrition Examination Survey (KNHANES V-1). The variables consisted of general characteristics, physical and mental health, nutrient intake, rate of deficient intake of energy and nutrients compared with Dietary Reference Intakes for Koreans (KDRIs) and food quality and diversity according to the status of food insecurity. Food insecurity status was measured using a self-reported food security questionnaire on the dietary situation in the previous year, and participants were classified according to three groups: food secure group, mildly food insecure group, and moderately/severely food insecure group. Results: The proportion of the food insecure group was approximately 67% and the food insecure group had lower income and educational status than the food secure group. Food insecurity was associated with worse physical and mental health status after adjusting potentially confounding variables. The results showed that food insecurity in Korean elderly significantly affected mental health (including stress cognition, depression experience, and suicide thoughts) which exceeded stages of physical health. In addition, food insecurity showed significant association with low nutrient intake and high rate of deficient intakes of energy and nutrients compared with KDRIs, and a reduction of dietary quality and diversity was indicated in the food insecure group. Conclusion: This study concludes that the prevalence of food insecurity may affect the physical and mental health as well as dietary intake of the elderly Korean population. Therefore, food insecurity should be considered as an important public health issue in Korea.
