Therapeutic compliance and its related factors in pediatrics patients (소아 환자의 치료 순응도 및 이에 영향을 미치는 요인)

  • Purpose : This study was conducted to investigate treatment compliance and related factors in pediatric patients. Methods : Three hundred and fifty-five patients diagnosed with various acute diseases at a teaching hospital or clinic in October 2003 were enrolled. Data were analyzed using the Health Belief Model, which includes items on self-efficacy and family assistance. Results : The study found that 62.9% of pediatric patients adhered faithfully to agreed-upon hospital revisits, 41.6% complied with dose timings instructions, 65.8% precisely took medication, and 27.2% complied with all of these requirements. According to ${\chi}^2$ test analysis, the factors found to be related to therapeutic compliance (the taking of medicines requested) were; susceptibility, severity, benefit, barriers, mother's self-efficacy, and family assistance (P<.05). Multiple logistic analysis and path analysis showed that susceptibility, severity, barriers, and mother's self-efficacy were related to therapeutic compliance (P<.05). Moreover, mother's self-efficacy was identified as the most important factor. Conclusion : To improve therapeutic compliance among pediatric patients, parental education is necessary, and a health care professional must take a thorough history of how the medication was taken before it is assumed that treatment failure is attributable to the medication prescribed. Furthermore, the type of device recommended for dosing should be determined by clinicians. In addition, it is important that pediatric medications be discussed in relation to their palatability and internal acceptability.

Nutritional Status of Intensive Care Unit Patients According to the Referral to the Nutrition Support Team and Compliance with the Recommendations (영양집중지원팀 자문 의뢰 및 순응 여부에 따른 중환자실 환자의 영양상태 비교)

  • Objectives: This study aimed to examine the effectiveness of the intervention of the nutrition support team (NST) on the nutritional status of critically ill patients. Methods: The medical records of 176 adult patients who were admitted to the intensive care unit and received enteral or parenteral nutrition for more than 7 days were retrospectively analyzed. The patients were classified into the NST and non-NST groups according to whether they were referred to the NST or not. The NST group was further classified into the compliance and non-compliance groups depending on their compliance with the NST recommendations. Results: The NST referral rate was 56.8%, and the rate of compliance with the NST recommendations was 47.0%. Significantly higher energy and protein were provided to the NST and the compliance groups than to the non-NST and the non-compliance groups. The proportion of patients who reached the target calories after the initiation of enteral nutrition was significantly higher in the NST and the compliance groups than in the non-NST and the non-compliance groups. The serum albumin and hemoglobin levels significantly decreased in every group, but the changes were significantly lower in the compliance group. The nutritional status at discharge from the intensive care unit compared to the status at admission was significantly worse in the NST, non-NST, and non-compliance groups. However, the status was maintained in the compliance group. The length of stay in the intensive care unit was significantly shorter in the compliance group. Conclusions: Compliance with the NST recommendations was found to provide more calories and protein and prevent the deterioration of the nutritional status of critically ill patients. Therefore, effective communication between medical staff and the NST from the early stages of admission to the intensive care unit is needed to improve referrals to the NST and compliance with the recommendations.

Robust Tracking and Human-Compliance Control Using Integral SMC and DOB (적분슬라이딩모드와 DOB를 이용한 강인추종 및 인간순응 로봇제어)

  • The robot control with safety consideration is required since robots and human work together in the same space more frequently in these days. For safety, robots must have compliance to human force and robust tracking performance with high impednace for the nonhuman disturbances. The novel idea is proposed to achieve the compliance and high impedance with one controller structure. For the compliance, the ISMC(Integral Sliding Mode Control) and HDOB(Human Disturbance Observer) The human force is identified by using the human band pass filter and its output is sent to the sliding surface. The sliding mode dynamic is affected by human disturbance and the compliance for human is achieved. The disturbances besides human frequencies are decoupled by the ISMC and the robust tracking is achieved. The additional LDOB(Low Frequency Disturbance Observer) decreases the maxim nonlinear gain and leads low chattering. The introduction of human disturbance into the sliding mode dynamic is the main novel idea of this paper.

A Study on Compliance of Hypertensive Patients Registered at Community Health Practitioner Post (보건진료소에 등록된 고혈압 환자의 순응도 연구)

  • Objectives: This study was to evaluate the compliance of hypertensive patients and its related factors registered at Community Health Practitioner Post(CHCP). Methods: 304 patients were interviewed by trained nursing students during one month(June~July 2004). The questionnaire included general charactristics, knowledge of hypertension, health education experience, constructs of Health Belief Model, self efficacy and so on. Compliance group was defined "having regularly medication and good life style". Good life style included regular exercise, non-smoking, little alcohol, low salt diet, weight control. Results: In compliance group 90.3% of man and 93.3% of woman were regularly taking hypertensive medicine, and 45.2% of man and 56.4% of woman were having good life style (compliance group). In both man and woman, the group of higher education were more compliance group, but were statistically significant were in man(p<0.05). In woman, the compliance group have significantly higher score in knowledge of hypertension(p(0.05). The compliance group have significantly higher self-efficacy score in both man and woman (p<0.05). In Health Belief Model, susceptibility and benefit were statistically significant in man, seriousness, benefit and barrier in woman(p<0.05). In multiple logistic regression analysis, education level and self efficacy in man and knowledge of hypertension, self-efficacy and benefit in woman were significant variables (p<0.05). Conclusions: It is very important to evaluate and modify life-style adding to having regularly medication in hypertensive patients registered at CHCP. To this, health education programs about benefit to compliance and the methods to improve self-efficacy should be developed for this patients.

Review and Suggestion for Waste Transfer Stations in Seoul (서울시 생활폐기물 적환장 운영실태 분석 및 제언)

  • This study was conducted to review Waste Transfer Stations(WTSs) in Seoul, especially focusing on three aspects, main roles, neighbor friendly and regulation compliance, and to draw out recommendation for improvement. The 46 units of all 65 WTSs treated large amount of waste on a day shipping-out schedule, the other 19 WTSs treated small amounts of wastes which were transported after 3~30 days stock. About 57% of WTSs are located in residential or commercial area. Other 57% of WTSs were open without walls. 79% of WTSs were placed alone. Just 17% of WTSs were permitted as environmental infrastructures by urban planning law, and 31% of WTSs were operated by private waste haulers who were permitted by waste management law. Most WTSs in Seoul seemed to go well with on-going policy demand, such as regionalizing waste treatment facilities, expanding waste recycling, and so on. However lots of WTSs did not implement sufficient environmental protection measures, and partly not compliant with regulations related to urban infrastructures. In the future, the issue on sealing facilities and integrating functions of waste management facilities could be considered in order to fit in urban environment.

A Study on the Relationship between Individual Patient Behavior, Medical Care Level and Therapeutic Compliance: Community Health Survey (개인 건강행태 및 지역보건의료 수준과 치료순응의 관계: 지역사회 건강조사 바탕으로)

  • Objective: This study was conducted in order to investigate the effects of individual patient behavior regarding the health and medical care level on their therapeutic compliance for people diagnosed with high blood pressure by a physician, in order to evaluate the impact of treatment compliance. In this study, the therapeutic compliance was defined as the hypertension drug cure rate. The current blood pressure controls were persons about 30 years of age diagnosed with high blood pressure by their doctor one month fraction of the people who take more than 20 days (30 days). Methods: The data was analyzed by using the X2-test for different comparisons of the therapeutic compliance in the individual characteristics and using a two-stage multilevel logistic regression to identify the community variance of the related index of high blood pressure therapeutic compliance using the data from 229 adults over the age of 19 in a community health survey conducted in 2010. The Spss 18.0 statistical program was used with HLM 7 (hierarchical linear model) Results: It was found that the rate of therapeutic compliance was affected by the individual health behavior and health and medical care levels. Relationship between Depression, Self-efficacy and Treatment Adherence in Peritoneal Dialysis Patients (복막투석환자의 우울, 자기효능감, 치료순응도의 관련성 연구)

  • Purpose: The purpose of this study was to identify the relationships beween depression, self-efficacy and, treatment adherence in peritoneal dialysis patients. Method: Participants were 139 in a hospital located B city Korea. The data were examined using descriptive statistics, t-test, ANOVA, and Pearson's correlation with SPSS 25.0. Results: Self-efficacy had positive correlation with medication(r=.28 p=.001), fluid restriction(r=.23, p=.008), dietary restriction(r=.38, p<.001) in adherence. Depression had a negative correlation with medication(r=-.17 p=.046), dietary restriction(r=-.17, p=.043) Conclusion: Nursing starategies to strengthen the treatment adherence of patients with peritoneal dialysis need to focus on enhacing self-efficacy and alleviating depression.

A STUDY ON CHANGES OF AIRWAY, TONGUE, AND HYOID POSIT10N FOLLOWING ORTHOGNATHIC SURGERY (하악후방이동수술후 기도, 혀 및 설골의 위치변화에 관한 연구)

  • This study was conducted to investigate changes of airway size, tongue and hyoid position following orthognathic surgery in mandibular prognathism, and how they are adapted to new environment in time dependent manner. 37 patients, who had recieved orthognathic surgery, were selected for this study. lateral cephalogram of each patient was taken at preoperation, immediate postoperation, and over 6 month after operation, and were traced and analyzed The findings of this study were as follows : 1. The size of airway was not changed at PNS and Epiglottis level after operation, but it was changed slightly at 2nd cervical vertebra level. 2. The hyoid was moved inferoposteriorly at immediate postoperation, and then it shifted toward preoperative position, but it remained slightly inferoposterior position. The distance from hyoid to genial tubercle decreased continuously. 3. The position of tongue was moved inferoposteriorly at immediate postoperatioa and then it shifted toward preoperation position, but the root of the tongue remained inferoposteriorly. 4. The distance between tongue and hyoid was increased at immediate postoperation and slightly decreased during follow-up period. 5. The change of the mandibular position was not significantly correlated with changes of airway size, hyoid position, tongue morphology and tongue position.

Study on Health Behavior of Hypertensive Patients and Compliance for Treatment of Antihypertensive Medication (고혈압 환자들의 순응도와 건강행태의 관계)

  • Objectives: To estimate the prevalence rate of hypertension, the changes of health behavior, and compliance for the drug treatment after diagnosed as hypertension. Methods: 7,030 persons who live in Cheonan City of Chungnam Province were selected by the cluster sampling method, and 5,372 persons were surveyed by questionnaire and health examination. This data is analyzed by Chi-square test on each variable. Results: 49.8%- of men and 38.8%- of women had been diagnosed as hypertension, and the prevalence rate of hypertension was significantly increased with aging in both gender. The prevalence rate tended to decrease in highly educated women group. Unemployed persons or obese persons showed relatively higher prevalence rate. The prevalence rate of hypertension increased in groups with higher total cholesterol levels over 240 mg/dl, and groups with glucose level over 200 mg/dl. 53.1%- of male patients and 66.6%- of female patients showed compliance for antihypertensive treatment. Compliance for treatment was higher in aged group or lower educated group in both gender. Among men, proportion of compliant subjects was higher in unemployed group(49.3%-), and lower in labor or primary industry than the others but among women, there was not any significant difference. And men with compliance for treatment had higher monthly income than the others, but women did not show any. Conclusion : This population had a high prevalence rate of hypertension which may lead to cardiovascular disease. Therefore health education programs and distribution of information must be emphasized in order to increase compliance to treatment and encourage the change of health behavior to promote health.
