  • 백내장은 수정체가 흐려져 빛을 투과시키지 못하면서 시력장애를 일으키는 질환이며, 더욱이 뚜렷한 예방책도 없다. 따라서 백내장은 가능한 한 발병 초기에 발견하는 것이 중요하다. 적절한 시기에 수술을 받아야 떨어진 시력을 거의 회복할 수 있게 되기 때문이다. 하지만, 약물요법은 이미 진행된 백내장을 원래 상태로 회복시키지 못하고, 단지 진행을 더디게 하는 일종의 보조요법이라는 한계가 있다. 따라서 백내장을 확실히 극복하기 위해선 수술을 받아야 한다.

  • 백내장 수술에서 인공수정체 삽입영역을 표시하고 제어하는 것은 매우 중요하다. 수술시 발생하는 삽입영역 제어의 오차는 시력의 저하를 가져온다. 이를 위해 디지털 이미지 프로세싱을 이용한 많은 추적방법이 연구 중에 있다. 그 중에서 템플릿 매칭은 영상과 템플릿 영역의 상관관계만을 따지며 추적하기 때문에 추적 할 객체의 크기나 각도 그리고 배경의 변화가 발생하면 정확히 추적을 하지 못하는 경우가 발생한다. 따라서 본 논문에서는 이러한 문제점을 객체의 특징점을 이용하여 매칭시키는 방법인 SURF를 이용하여 해결하고자 한다.

Clinical Study of Cataract Surgery in Diabetic Retinopathy (당뇨망막증 환자의 백내장 수술에 대한 임상적 고찰)

  • Extracapsular cataract extraction with posterior chamber intraocular lens in 24 eyes of 24 diabetics, with and without diabetic retinopathy, were followed up postoperatively for an average of 14 months to determine the incidence of progression of diabetic retinopathy, the final visual acuity and factors predictive of progression of retinopathy and final visual acuity. Overall, retinopathy progressed in 52% of operated-on eyes. Cataract extraction was highly associated with progression of diabetic retinopathy. Women had a significantly increased risk of progression of retinopathy in the operated-on eye compared to men. Visual acuity improved in 22 of 24 orerated-on eyes : however, only 11 eyes achieved a visual acuity of 0.5 or better and only 7 eyes achieved a visual acuity of 0.7 or better. Patients treated with oral hypoglycemic agents had a worse visual prognosis than those treated with insulin. The prognosis of patients with diabetic retinopathy about to undergo cataract surgery, even extracapsular cataract extraction with placement of a posterior chamber lens, is guarded.

Formation of Gametophytes and Development of Zygotic Embryo in Dicentra spectabilis (금낭화(Dicentra spectabilis)의 배우체 형성 및 접합자배의 발달)

  • This study was carried out to obtain the basic informations on the characteristics of gametophytes formation and embryo developement in Dicentra spectabilis. Microspore mother cells developed from archesporial cells, start meiosis when flower bud length reaches around 1 mm, formed tetrahedral type tetrad. The 4 microspores were separated. They were developed to male gametophytes, respectively. Megaspore mother cells were observed when flower bud length was $4{\sim}5\;mm$. The developemental type of megaspore was polygonum and embryo sac was amphitropous. Three large and distinctive antipodals did not degenerated and remained after embryo sac was developed. When the male and female gametophytes was fully developed, the length of stamen and style was very similar or stamen was shorter about 0.5 mm than that of style. This result indicates that self-fertilization can be occurred in this species. After fertilization, developing zygotic embryos showed various stages of development from globular to cotyledonary embryos, and zygotic embryo in seed scattering time seemed to have an early cotyledonary stage.

Refractive Error Shift in Nuclear, Cortical, and Subcapsular Cataract (핵, 피질 그리고 낭 백내장의 굴절이상 전환)

  • The researchers have studied on the effect of three main morphological types(nuclear, cortical, and subcapsular cataract) of age related cataract on refractive error. We also identified that spherical and cylinderical shift in each type of morphological cataract accoding to their locations and effect of intraocular pressure before and after cataract surgery. Nuclear cataract showed myopic shift while cortical cataract showed hyperopic shift, and subcapsular cataract showed not significant changes on both direction. Age related cataract subjects(120 eyes) only were recruited from sun-cheon area. We use optic section of slit lamp biomicroscope to identify the anatomical location of cataract, and optimal refractive correction determined by objective(retinoscope) and subjective refraction then spherical changes were calculated from the spherical eqivalent value. Intra-ocular pressure were measured by auto-tonometer before and 7 days after cataract has been replaced by IOL. The change in cylindrical power, usually A-P diameter decreased according to increases of age, A-P diameter increased because intra-ocular pressure rises by progression of cataract and this convertion with the rule astigmatism to against the rule astigmatism and this increases more by intra-ocular pressure.

Intraocular Pressure Changes in the Uneventful Extra-capsular Cataract Extraction and Extra-capsular Cataract Extraction with Vitreous Loss followed by the Anterior Chamber Lens Implantation (계획적 수정체낭외적출술과 합병증이 동반된 수정체낭외적출술후 전방렌즈의 삽입술시 안압의 변화)

  • These days the posterior chamber lens implantation following the extracapsular cataract extraction has become the standard procedure except those patients who can not take it in one reason or another. In past, however, the anterior chamber lens implantation was popular for sometimes. Even now in those patient complicated by vitreous loss, the anterior chamber lens implantation following anterior vitrectomy is indicated in some cases. We have become interested in the difference of the intraocular pressure change in the patients who underwent the uneventful extra-capsular cataract extraction followed by anterior chamber lens implantation in routine manner and in those whose extra-capsular cataract extraction were complicated by vitreous loss and eventually needed the anterior vitrectomy before the anterior chamber lens implantation. We reviewed the medical records of 15 cases of the former group and 11 cases of the latter one in 1986 through 1988. There was some difference in the postoperative pressure in the two groups, but it was not statistically significant.

Clinical Effects of an Improved Pump Reaction Rate and Automatic Occlusion Sensing System in Phacoemulsification (수정체유화장치의 초음파 출력속도 및 자동막힘감지 기능 향상의 술 후 임상결과 비교)

  • Purpose: A recently introduced phacoemulsification system, the WhiteStar $Signature^{(R)}$ PRO, has demonstrated improved nucleus followability and cutting efficiency via an improved pump regulator with a higher reaction response and an automatic occlusion sensing system. In this study, we compared various phacoemulsification parameters between the new system and an older version of the device. Methods: A total of 80 eyes of 68 patients with cataracts who had undergone phacoemulsification by a single surgeon were included in this study. Forty eyes of patients underwent phacoemulsification using the older $Signature^{(R)}$ system (WhiteStar); these patients were classified as the control group. Another 40 eyes of patients underwent phacoemulsification with the newer enhanced system, the WhiteStar $Signature^{(R)}$ PRO; these patients were assigned to the experimental group. During the operation, operative parameters, including the effective phaco time (parameter of effective phaco time with a specific coefficient for the transversal movement expressed in seconds, EFX), ultrasound time (seconds [s]), effective phacoemulsification time (EPT, s), average phacoemulsification power (AVG, %), and balanced salt solution usage, were measured to determine the performance enhancement offered by the updated system. Central corneal thickness was measured before and after surgery to compare corneal edema. The relationships between the two groups were analyzed using an independent t-test. Results: The Signature $PRO^{(R)}$ system showed a lower EFX (p < 0.001), a shorter EPT (p < 0.001), and a smaller AVG (p < 0.001). Postoperative corneal thickness did not differ significantly between the two groups. Conclusions: Comparing the efficacy of the improved reaction speed of the device and automatic occlusion sensing system in performing phacoemulsification, the updated Signature $PRO^{(R)}$ system demonstrated superior followability and cutting efficiency regardless of nuclear cataract hardness.
