A study on Establishment and Management of the CCTV in Operating Room (수술실 CCTV 설치 및 운영에 대한 고찰)

  • Recently, medical accidents related to surgical procedures have increased. In addition, the media reported that some of these accidents were involved in health crimes. Patient-advocate groups have called for mandatory establishment and management of CCTV in operating rooms. There is a lot of discussion among the interested parties, so it is necessary to review the relevant laws and regulations. The purpose of this study is to identify the characteristics of CCTV in operating rooms and to review legislations related to establishment and management of the CCTV in operating rooms. Medical institutions use CCTV for management of facilities and patient safety and install it in operating rooms optionally. The Constitution guarantees the privacy and the privacy of correspondence of every citizen, but it can be limited by the law for public welfare. Currently, however, there is no existing law about establishment and management of the CCTV in operating rooms and it can be defect of legal system. Under the current legislations, it is likely that the Self-determination can be violated due to the characteristic of healthcare provider when CCTV is mandatorily installed in operating room. In addition, the regulations on access and leakage of confidential information known by operator are insufficient. So that, the safety of the visual data might be threatened. Furthermore, unless the period and the place of storage of the visual data are clearly defined, it is highly unlikely to meet the original purpose of patient safety and prevention of medical accidents. This study is meaningful as there is few previous study on this topic although the need for legal review about this is growing and several bills are being proposed. It is expected that the results of this study can be utilized as basic data for enactment or amendment of the laws and regulations about establishment and management of CCTV in operating rooms.

  • 최근 대리수술 (무면허의료행위)과 같이 환자의 안전을 위협하는 사건들이 언론에 보도되고 있다. 대리수술 방지를 위한 수술실 감시카메라 장치 도입 등의 대안이 등장하고 있지만, 의료계의 거센 반발로 인해 시행되기에는 현실적인 어려움이 있다. 하지만 대리 수술과 같은 사건이 빈번하 발생함에 따라 의사에 대한 사회적 신뢰도가 추락하고 있다. 본 논문에서는 근거리 무선 통신 장치인 비콘(Beacon)과 생체인식 중 안잔하고 신뢰할 수 있는 홍채인식을 결합한 의료진 신분 확인 시스템을 제안한다. 이 시스템은 홍채인식을 통해 사용자 인증을 수행함으로써 1차적인 신분확인을 하고 비콘을 통해 의료진이 수술실에 있다는 것을 증명한다. 또한 무작위 주기로 홍채인증을 수행하여 의료진이 초기 인증만 수행하고 수술실을 떠나는 경우를 방지함으로써 집도의에 대한 환자의 신뢰를 보장한다.

The Competence and Satisfaction on Inventory Management of the Operating Room Nurses (수술실 간호사의 물품관리 업무역량과 업무만족도 연구)

  • The study verified the work capacity (job performance, perception, knowledge) and satisfaction on managing the inventory of operating room nurses working in a more than 700-bed hospital. The purpose of this study was to provide basic data of education for work capacity and satisfaction and quality insurance by analyzing the scores between the three different operating rooms and the correlation between the work capacity and satisfaction by investigating the related factors. This study presented a structured and self-administered questionnaire to 181 nurses who had been working in the operating room more than six months. The mean and standard deviation of the job performance, perception, knowledge, and satisfaction were 4.2(${\pm}0.56$), 3.4(${\pm}0.76$), 3.5(${\pm}0.40$), and 3.4(${\pm}0.55$), respectively. The work capacity and satisfaction of each group did not show a statistically significant difference. The correlations between the job performance and knowledge, knowledge and perception, perception and satisfaction were positive (r=.627, p<0.01), (r=.663, p<0.01) and (r=.485, p<0.01), respectively. Among the factors related to the general characteristics of operating room nurses, only age significantly affected their job performance. This study provides basic data on the maintenance and improvement of their competence and satisfaction by being served as a resource for sustainable human resources management and training, and efficient management of the communication channel between hospitals.

  • 문제: 환자가 수술장 내에서 수술을 기다리면서 느끼는 불안감을 최소화할 수 있도록 해야 하지만 수술실의 효율성 및 의료진 편의성 위주로 운영되고 있어 수술실 내 대기시간이 연장되고 있다. 목적: 수술장 내 대기시간을 단축시키기 위해 수술환자의 이동경로에 따른 지연요인과 문제점을 파악하고 개선하여 환자가 수술을 기다리면서 느끼는 불안감을 최소화하고자 한다. 의료기관: 부산시에 소재한 481병상의 종합병원 수술실 질 향상 활동: 수술환자의 대기시간 지연요인과 문제점을 파악하고 개선을 통해 질 향상을 도모하였다. 개선효과: 대기시간 수행율에서 수술장 도착까지의 수행율이 개선전 95%에서 개선후98%로, 수술방 입실까지의 수행율이 개선전 88%에서 개선후 94%로, 마취시작까지의 수행율이 개선전 93%에서 개선후 96%로 수행율이 높아졌다.

A Study on Perception and Nursing Activity for Patient Safety of Operating Room Nurses (수술실 간호사의 환자안전의식과 안전관리활동)

  • This study aimed to investigate the awareness and performance of safety in operating room nurses. There were 161 operating room nurses from eight general hospitals included for analysis in this study. The data were analyzed with t-test, one-way ANOVA, correlation analysis, and Scheffe's test. The results show that their perception of patient safety was 3.33 out of a total of 5 points. Moreover, the score of safety management activity for patients in the operating room was 4.28 out of 5 points. As the perception of patient safety in operating room nurses was significantly correlated with their safety management activities, nurses with higher awareness of patient safety were found to have higher degree of performance of safety management activities. In conclusion, to improve safety management activities, open discussion about patient safety should be encouraged and periodic education regarding patient safety should be implemented for operating room nurses to increase the perception of patient safety.

Effect of Career Commitment and Professionalism of Perioperative Nurses on the Organizational Commitment (수술실 간호사의 경력몰입과 전문직관이 조직몰입에 미치는 영향)

  • This study examined the career commitment and professionalism of perioperative nurses and their impact on the organizational commitment. The subjects were 315 perioperative nurses who were attending the annual conference of the Korean Association of Perioperative Nurses. The data was analyzed by descriptive statistics, ANOVA, and stepwise multiple linear regression using the SPSS/PC 18.0 program. The average level of organizational commitment of the nurses was $3.04{\pm}.42$, whereas the level of career commitment and professionalism were $2.89{\pm}0.67$ and $3.04{\pm}0.29$, respectively. Both factors were found to be influencing factors on the organizational commitment. These variables explained 37.3% of the variance of organizational commitment, and the career commitment (${\beta}$=0.395) was a more influential factor than the professionalism (${\beta}$=0.298).

  • This study aimed to identify the components of the nurses' exposure experiences and clarify the semantic structure of nurses by analyzing the participants' experiences of exposure to radiation. The participants in this study were nine nurses who worked for at least six months in the operating room of a City General Hospital. The data were collected using in-depth interviews and analyzed using the phenomenological method by Giorgi's (2004). As a result, six components were derived: "worrying about the disease by repeated radiation exposures", "exhausting physically and mentally," "trying to protect themselves against radiation exposure", "feeling of internal confusion and conflict", "demanding improvement of work environment", and "accepting and adapting to reality". In conclusion, nurses participating in radiation exposure surgery are concerned about the possibility of disease, experience physical and mental fatigue, and then consider changing their occupation. On the other hand, they try to accept and adapt to reality by considering the possibility of a job change or trying to protect themselves from the harsh environment. To develop a radiologically safe environment, systematic and effective resolutions must be secured at the organizational level.

Convergence study on Relationship among Verbal violence experience, Verbal violence impact and Burnout in Operating Room Nurses (수술실 간호사의 언어폭력경험, 언어폭력충격, 및 소진간의 상관성에 대한 융합연구)

  • The purpose of this study was to evaluate the relationship among verbal violence experience, verbal violence impact and burnout of operating room nurses. The data were collected by structured self-reporting questionnaires from 202 operating room nurses and were analyzed with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and multiple regression. The level of verbal violence experience and verbal violence impact was 1.96 and 1.67. The level of burnout was 3.08. Verbal violence impact and burnout have a significant positive association with verbal violence experience(r=.39, p<.001; r=.41, p<.001). Verbal violence impact was positively associated with burnout(r=.29, p<.001). Factors influencing burnout were verbal violence experience, verbal violence impact and position(staff nurse) which explained 30% of the variance(F=9.15, p<.001). These findings indicate that verbal violence experiences of operating room nurses have influence on stability and productivity in personal, social aspect and suggest developing the verbal violence prevention program in hospital.

Transverse Colon Cancer with Duodenal Fistula (십이지장루를 동반한 횡행결장암 1예)

  • A 57-year-old male visited our hospital due to a growing abdominal mass for 1 month. The patient was diagnosed as transverse colon cancer with duodenal fistula, and then was treated with neoadjuvant concurrent chemoradiation therapy (2 cycles of FOLFOX-4, 3-dimensional conformal radiation therapy: 3,000 cGy in 10 fractions). Despite the improvement of colon cancer and associated inflammation, the symptom of colonic obstruction was aggravated. Thus transverse colon segmentectomy was done. After surgery, he have received adjuvant 12 cycles of FOLFOX-4 chemotherapy. Now, he is currently being followed up in cure state.

Development of Case-based Multimedia Learning Contents for Preventing Malpractice in Operating Room (수술실 간호오류 예방을 위한 사례중심 멀티미디어 학습콘텐츠 개발)

  • Purpose: This study was conducted to develop case-based self-learning multimedia contents for preventing malpractice that frequently occurred among nurses working in the operating room. Methods: Based on the learning needs of operating room nurses, real case reports, and literature reviews, the case-based multimedia learning contents were developed according to the instructional design procedure. The assessment of learning needs was performed by the combination of surveys using structured questionnaire and of interviews for 40 operating room nurses. Results: The learning contents included four learning modules with real malpractice cases from the areas of operating preparation, nursing skills during operation, environmental management of operating room, and patient safety and observation-related. The 80 minute long case-based multimedia learning contents were finally developed after content validity tests from clinical experts. Each module contained photos, sounds and flash animation with voice recording on the contents of nursing error cases and standardized protocols. Conclusion: The developed multimedia learning contents based on real error cases in this study can be utilized as an educational hands-on training materials for nurses to prevent malpractice in the operating room.
