The Liquid and Viral Barrier Properties of Reusable and Disposable Surgical Gowns (수술가운의 방수성과 방균성에 관한 연구)

  • 최근에 치명적인 바이러스, 특히 에이즈를 이르키는 Human Immunodificiency Virus (HIV), 또는 간에 심각한 병을 이르키는 Hepatitis B Virus (HBV)와 같은 무서운 바이러스가 환자의 피나 분비물을 통하여 의사에게 전염되는 사례가 늘어감에 따라 이제는 환자의 피나 분비물의 침투를 막기 위해 방수성과 방균성을 가진 수술복 착용이 절대적으로 필요하게 되었다. 따라서 본 연구의 목적은 1. 수술가운을 만드는 5가지 대표적인 직물의 방수성과 방균성을 측정하고 2. 세탁시 표백제의 사용유무가 그 직물의 방수성과 방균성에 미치는 영향을 알아보고 3. 반복세탁이 그 직물의 방수성과 방균성에 미치는 영향을 알아 보는데 있다. 이 실험을 위해 3가지 재사용 수술가운과 (Gore-tex$^R$, membrane reinforced; Compel$^{TM}$, fabric reinforced; Acep$^{TM}$, non-reinforced) 두 가지 일회용 가운 (Evolution$^R$ gown, fabric reinforced; Evolution$^R$ Specialty, film reinforced)이 사용되었다. 표백제를 사용한 세탁이 직물의 방수성과 방균성에 미치는 영향을 알아보기 위해 재사용 가운을 세탁, 건조, 살균을 하였다. 세탁과 살균의 빈도수는 1, 20, 40, 50, 60, 70, 80번 이였다. 방수성 측정방법으로 1. Impact Penetration test (AATCC 42-1985), 2. Elbow Lean test, 3. Synthetic Blood Resistance test (ASTM F 23. 40. 01)가 사용되었고 방균성 측정방법으로 Viral Resistance test (ASTM F 23. 40. 02)가 사용되었다. 실험 결과는 다음과 같다. 1. Gore-Tex$^R$ 가운과 Evolution$^R$ Specialty 가운은 방수성과 방균성의 성질을 가졌다. 그러나 표백제를 사용하여 70번 세탁한 Gore-Tex$^R$ 가운은 membrane의 구조가 파괴되어서 방균성을 상실했다. 2. Evolution$^R$ 가운, Acep$^{TM}$ 가운과 Compel$^{TM}$ 가운은 오직 Impact Penetration test만 통과했다. 즉 이 직물들은 큰 압력의 가함이 없는 splash resistance만 가지고있다. 그러나 Acep$^{TM}$ 가운과 Compel$^{TM}$ 가운은 20번과 60번 세탁 후 각각 그들의 splash resistance 마져도 상실했다.

  • 모든 의복은 최소한의 기능성이 요구되고 신체를 지지하고 신체의 움직임을 허용해야한다. 의복은 이브닝 드레스와 같은 미적인 의복과 미적인 것과 기능적인 요구를 함께 수용하는 비즈니스 수트, 수술가운과 같은 기능성 의복으로 분류된다. 특히 수술가운은 한정된 장소에서 특수한 활동을 가능케하면서 의료진을 보호해야하는 기능을 가져야한다.

STUDY ON CHILDREN'S PREFERENCE TOWARD ATTIRE OF DENTIST AND DENTAL HYGIENIST (어린이가 선호하는 치과 의료진의 복장에 대한 연구)

  • The purpose of this study was to find a method for improving children's preferences during dental treatment in relation to dentist's attire. For this study, 650 children asked to participate in a survey about the attire of the dentists. The results of the survey were as follows: 1. The survey found that gowns were preferred to ordinary clothes for both male and female dentists, 67.7 to 87.6% for male and 59.2 to 75.9% for female dentists. 2. For male dentists, the typical long white gown was the most preferred style, followed by a long gown, and a short jacket type, and then a short operating gown. 3. For female dentist, the typical long white gown was also most preferred, followed by a long gown, a short jacket type, and then a short operating gown. 4. For dental hygienists, the most preferred attire style was a two-piece wear with pants, followed by one-piece, skirt suit, shirts with cartoon characters, and apron. However, the difference among these attires was insignificant 5. White was the preferred color for gown. 6. Children prefer gown with no-pattern.

  • Surgical operating gowns are worn by doctors and nurses in operating theater to address dual function of preventing transfer microorganisms like bacteria and body fluids from the operating staffs to patients. and also from patients to staff. They must allow necessary mobility without rubbing and must resist tearing. These gown must be designed to fit a diversity of body shapes and sizes with a limited range of sizes as hospitals will stock limited quantities. Also they must proper in various operating situations. In order to suggest improving design features of surgical gowns, it has been done to analyze current situation of medical uniform manufacturing and wearing. Surgical gown were made with plain fabric of cotton 100% and had a little size system without sexual division. Many doctors were unsatisfied with surgical gown size and wearing comfort. Surgical operating gowns need continuos research for new material and design as surgical situation, and new sizing system.

A Study of Disposable Medical Clothes (일회용(一回用) 의료용(醫療用) 의복(衣服)에 대(對)한 연구(硏究))

  • The purpose of this study was to provide the basic informations of disposable medical clothes and develop new disposable medical clothes which are not caused environmental pollutions. The staffs of Yuhan-Kimberly and doctors, nurses and personnel of four general hospitals in Seoul were interviewed individually in order to collect useful data of disposable clothes. Also the samples of disposable medical clothes were examined. The materials, designs, size of potential market and actual consumption conditions of disposable medical clothes were found out. New disposable medical clothes were made after having considered all the aspects examined above. Two kind of medical gowns and caps, a kind of medical apron, three kinds of patient robes and a kind of patient cap were made of Korean traditional paper with specific functions instead of nonwoven polypropylene fabric.

  • The situation of wearing of surgery gown and their awareness were surveyed to clinicians and nurses in operation rooms of major university hospitals in the country to develop and manufacture surgery gowns for medical personnels. The survey result was quantified to be used as a base data for design of surgery gowns with freshness and convenience as well as safety to wearers. In the inspection of surgery gowns focused on materials (cotton raw material and disposable nonwoven) and dissatisfaction, there was no significance in the activity and freshness with p-0.745 and p-0.367, respectively, however, there was a significance in safety and management with p-0.011 and 0.049, respectively: disposable nonwoven gown showed higher satisfaction level in safety and management compared to cotton gowns.

Improving the Fading of Reusable Surgical Gown by Repeated Severe Laundering and Sterilization Condition (반복되는 고온 세탁 및 멸균 환경에 대한 Reusable 수술가운 원단의 퇴색 저항성 개선 연구)

  • The purpose of this study is to minimize fading fabrics of surgical gown by repeated severe laundering and sterilization condition. The study showed that the best conditions were reduction cleaning using sulphinic acid derivatives or glucose organic chemicals at $98^{\circ}C$ alkaline solutions. In these conditions, color difference values(dE) were below 1.0 that means unrecognizable color change by repeated laundering and sterilization. If it treated with only laundering, reduction cleaning conditions may adjust over $80^{\circ}C$ alkaline solution. In conclusion, it is needed to select the high-washing fastness dye and reduction cleaning using sulphinic acid derivatives or glucose organic chemicals at $98^{\circ}C$ alkaline solutions for removal unfixed dyes.

A Study on Radiation Exposure Dose of Patients and Operator during Percutaneous Vertebroplasty (경피적 추체 성형술 시행 시 환자와 시술자의 방사선 피폭선량에 관한 연구)

  • Percutaneous vertebroplasty (PVP) is increasingly used to treat osteoporotic vertebral fractures, myeloma and osteolytic vertebral metastases. The purpose of this study was to measure the absorbed radiation exposure dose and time during PVP and to assess the possibility of deterministic radiation effects to the operator and patient. The radiation dose and time measure by three pain physicians performed consecutive procedures using the twenty case PVP. Patient's dosimeter placed at the anteroposterior(AP) side was treatment of the vertebra body located in the upper level 2-3 and lateral(LAT) side was flank proximal to C-arm tube of back. Operator's dosimeter placed at the apron outside of upper sternum (thyroid), left chest, lower extremity and apron inside of left chest. Results: Radiation exposure times were $3.6{\pm}0.71min$. Measurements on the Patient radiation dose were AP $121.4{\pm}48.1{\mu}Sv$, LAT side $614.7{\pm}177.1{\mu}Sv$. Operator radiation dose were outside of the lead apron upper sternum $33.7{\pm}7.3{\mu}Sv$, outside of the lead apron chest $49.2{\pm}15.0{\mu}Sv$, outside of the lead apron lower extremity $12.8{\pm}3.8{\mu}Sv$ and inside of the lead apron chest $4.2{\pm}1.4{\mu}Sv$. To escape from the danger of radiation first long distance from the c-arm tube second exposure time reduced second lead apron used fluoroscopy during PVP is more safety patient and operation from the radiation exposure.
