Tuberculous Tenosynovitis of Flexor Tendon of the Hand (수부 굴곡건에 발생한 결핵성건초염)

  • Purpose: The sheath of tendon is uncommon site of tuberculous involvement as compared to other parts of the body. Especially, tuberculous tenosynovitis affecting flexor tendon of the hand is a rare condition. In recent years, furthermore, the incidence of tuberculosis is increasing in our country. Tuberculous tenosynovitis is a chronic, slowly destructive disease, which is difficult to diagnosis before operation, but can be definitively diagnosed by the pathologic microscopic examination. Early detection and surgical excision combined with antituberculous medication is important. We report a rare case of tuberculous tenosynovitis of the flexor tendon of the hand. Method: A 12-year-old woman presented with a painless, nontender mass on palmar side between distal interphalangeal joint and proximal area of metacarpophalangeal joint of the left third finger. We had surgical excision of the involved flexor tendon sheath and studied histopathologically. Result: The histopathological findings were chronic granulomatous inflammation with caseating necrosis consistent with tuberculosis. We started antituberculous medication. Conclusion: Tuberculous tenosynovitis is a rare condition, especially involving on the flexor tendon of the hand. But because of increasing tendency of tuberculosis, it is important to differentiate it from other tumors of the hand.

  • The epithelioid sarcoma is a rare high grade soft tissue sarcoma that affects young usually male, adults or adolescents. The most common localization is the hand and the forearm, followed by the leg and the foot. In most cases, the sarcoma grows slowly, involves the dermis, subcutis, or deeper soft tissues in the distal extremities. Epithelioid sarcoma is a slowly growing tumor with a high propensity for local recurrences and lymph node metastases. This neoplasm is likely to be confused with a variety of benign and malignant conditions. The treatment consists of wide surgical excision, chemotherapy and radiotherapy. We report the cases of 16-year-old girl with an epithelioid sarcoma on the lower extremity.

Cardiac Hemangioma of the Anterior Mitral Valve Leaflet - A case report - (승모판막 전엽에 발생한 심장혈관종 - 1예 보고 -)

  • A hemangioma at a cardiac valve leaflet is a rare finding. There are only five reported cases in the medical literature and no prior case reported in Korea. A previously healthy 45-year-old woman presented with an acute episode of numbness in the left side of the face and hand. Although no definite abnormalities were found on the neurological examination and testing the echocardiography revealed an echogenic mass in the anterior mitral valve leaflet. The tumor was excised en bloc from the leaflet and the defect was repaired with an autologous pericardial patch. Pathology examination confirmed the mass to be a hemangioma of the valve leaflet. Here we report this case and review the medical literature.

The Fate of Neglected Vascular Injury of the Hand in Acute Hand Injuries (급성 수부 손상에서 간과된 혈관성 손상을 가진 수부의 운명)

  • In acute hand injury, there are sometimes hattened neglected or overlooked vascular injury by primary operators. The authors evaluated the final results and prognosis after secondary revascularization. In eight cases, the authors performed secondary revascularization after prolonged warm ischemia. Five fingers in five cases among them were successfully survived and three cases finally necrosed. The mean warm ischemic time was 56.1 hours. In revascularization procedures, end-to-end artery anastomosis was possible in six cases. In two cases, vein graft was needed to anastomose digital artery, which resulted in complete survival of the fingers. In all three cases, revision amputation of the fingers was done. In acute complex hand injury, the importance of evaluation of the vascular injury can not be overemphasized. The necessity of the early secondary revascularization as well as serious consequence caused by misdiagnosis of vascular injury should be aware.

The Clinical Comparison of Ganglions in Hand and Foot (수부와 족부 결절종의 비교)

  • Purpose: To compare clinical characteristics of ganglions in hand & Foot. Materials and Methods: Seventeen cases of ganglions located in foot and fifty-five cases in hand. Excised from Mar.1988 to Apr.2003, were included in the study. The clinical characteristics and recurrence ratio were evaluated Results: The mean size of 2.2 cm in hand and 2.5 cm in foot. The most common area of ganglions are dorsum of foot and wrist. The cosmetic problem of palpable mass is the primary chief complaint of ganglions on hand and the pain is that of foot. The recurrence was found in 5 cases in hand and 4 cases in foot. The recurrence was related to incomplete excision of ganglion in foot and the large size of ganglion and incomplete excision of ganglion in hand. Conclusion: recurrence ratio in the cases of foot is higher than that of hand. The ganglions in foot and hand need to treated by meticulous surgical excision to prevent the recurrence.

  • Anthropometric data of the hand is one of the important issues for the product design related with hand and manual operation in terms of user-centered design. Direct measurement, which traditionally uses anthropometric measuring tool, has a few disadvantages in cumulation of measuring time, possible errors in raw data, doubtful reliability between inspectors. Newly introduced 3D scanning technology overcomes those disadvantages; however, another disadvantages, such as, hight cost and immovability, have been introduced. In this study, photogrammetric approach was introduced for the measurement of anthropometric data in hand. In order to minimize the possibility of error during direct and indirect measurement, a simulated object was used and effectiveness of photogrametric measurement was confirmed for the collection of anthropometric data in hand.

  • Beau first described transverse depressions of the fingernails after an acute febrile illness in 1846. It was the result of the temporary arrest of nail matrix formation. The transient interference of nail growth rate is usually induced by disability caused by severe systemic disease. Such as Raynaud disease, myocardial infarction, pulmonary embolism, acute renal failure, psoriasis and dysmenorrhea were reported to be the cause of Beau's line. The trauma caused either in a hand or in a forearm was also reported to be the cause of Beau's line. We experienced patients with Beau's line, two cases secondary to fingertip injury and one case due to fracture in radius. They had Beau's lines in all fingers of traumatized hand and nails of opposite hand were normal. During the follow-up, the growth rate of traumatized fingernails were normal and there was no complication.

  • Pigmented villonodular synovitis(PVNS) is a benign proliferative lesion, involving synovial tissue in joints, tendon sheaths, and bursae. Pigmented villonodular synovitis is a rare and usually monoarticular condition and primarily affects the knee joint and hand. Polyarticular PVNS appears in less than 1% of all case and its occurrence in the shoulder is rare(<2%). We present a 64-year-old male who had pigmented villonodular synovitis of both shoulder joints, which was treated by arthroscopic total synovectomy.

Cold hypersensitivity in the Hands, Feet and Abdomen according to Sasang Constitution (사상체질에 따른 수부, 족부, 복부의 냉증)

  • Objectives This study was aimed to examine cold hypersensitivity tendency in the hands, feet and abdomen according to Sasang constitution among Koreans. Methods Multi stage stratified sampling was conducted for random selection of 1,100 participants aged ${\geq}19years$, based on sex, age, region in Oct.2010. Cold hypersensitivity was measured using a questionnaire to score the extent of cold hypersensitivity in the hands, feet, abdomen. Subjects' constitution was diagnosed by Korea Sasang Constitutional Diagnostic Questionnaire (KS-15). The cold hypersensitivity scores according to Sasang constitution were analyzed using logistic regression and one way ANOVA. Results On the one way ANOVA, Soeumin group's cold hypersensitivity in the hands and feet scores were significantly higher than those of Taeeumin and Soyangin groups. However, there was no significant difference in the abdominal cold hypersensitivity scores by constitution. The regression analysis, adjusted for age, sex, and sociodemographic variables, showed that Soeumin group had significantly higher cold hypersensitivity effects of the hands (${\beta}=0.105$, p = 0.002) and feet (${\beta}=0.099$, p = 0.003) than Taeeumin group. Conclusions Soeumin group significantly had higher cold hypersensitivity scores in the hands and feet than Taeumin and Soyangin group.

  • We can sec the case of hand injury patient in the industry field. Hand injury has more particularity than other injury cases. Because it is densely a lot of muscle, tendon, bone, vessels, and in progressing speedily to fibrosis, adhesion, stiffness than other joints. If it has baud injury, it is important to the physical therapy in early stage after operation. Otherwise, it is difficult to recover the function. Hand malfunction intimately has to do with the return to work and the grade of disability. There are many different hand injury cases but, we want to investigate several cases. : fracture. joint injury, tendon injury, and want to looking for treatment tine and method about these. So, we'll understand hand injury of industry disaster. and acknowledge of the importance of physical therapy in the early stage through these.
