Superimposition: a simple method to minimize occlusal adjustment of monolithic restoration (디지털 장비의 중첩기능을 이용하여 단일체 수복물의 교합조정을 최소화한 증례)

  • The development of translucent zirconia enabled clinicians to choose a monolithic zirconia crown as one treatment modality in the posterior dentition. Careful occlusal adjustments are recommended for monolithic zirconia crowns because grinding zirconia inevitably causes phase transformation, which may deteriorate mechanical properties. intraoral scanners enable the clinician to scan and superimpose a complete tooth structure before preparation onto the prepared abutment. This technique helps to reproduce the original tooth form and occlusion of the patient. In this case report, prostheses were fabricated for patients with cracked or fractured tooth by applying intraoral scanner, Computer aided design-computer aided manufacturing (CAD-CAM) and monolithic zirconia crown to reproduce the occlusion of original tooth and to minimize occlusal adjustment. The clinical results were satisfactory in both esthetic and functional aspects.

  • Various materials and restorative options have been introduced recently with growing interest in esthetic dental treatment in modern society. Zirconia is especially known for its biocompatibility as well as remarkable toughness and resistance to wear, but it is limited in its use for esthetically focused treatment in anterior region for its white opacity. Lately the development of different kinds of zirconium blocks, such as colored block, clear block, and multi-layered block, allowed more extensive use of zirconia as a treatment option. This report describes a case, in which a prosthetic crown maxillary anterior region was stained with a combination of various coloring liquids before sintering to reproduce natural-looking color scheme in final restoration. The case was reported as the utilization of coloring liquid on monolithic zirconia crown could achieve esthetically satisfying prosthesis for both dentist and patient.

Full mouth rehabilitation of a patient with tooth wear and insufficient restorative space due to loss of posterior teeth support: a case report (구치부 지지 상실로 인해 치아 마모와 부족한 수복 공간을 보이는 환자에서의 완전구강회복 증례)

  • To rehabilitate a patient with inappropriate occlusion due to posterior support loss, it is necessary to achieve an appropriate occlusal vertical dimension and create prostheses that demonstrate stable occlusion at the centric relation for full mouth restoration. This case shows full mouth rehabilitation in a patient with missing posterior teeth and tooth wear, achieved through implant-supported prostheses and zirconia full-veneer crowns. To assess adaptation to the increased occlusal vertical dimension, an occlusal splint and temporary restorations were secured. By fabricating the definitive restorations based on the anterior guidance of the temporary restorations, stable occlusal rehabilitation was successfully achieved.

Complete mouth rehabilitation, using jaw motion tracking and double scan technique in a patient with osteoarthrosis: a case report (골관절증 환자에서 하악 운동 추적 장치와 이중 스캔 기법을 활용한 완전 구강 회복 증례)

  • This is a case report of complete mouth rehabilitation in a patient with generalized attrition and loss of posterior support. After analyzing the condition of the temporomandibular joint, multiple implants were placed to restore collapsed occlusion. Fixture/abutment level intraoral scanning was done instead of using conventional impression materials which entail multiple bite registration for cross-mounting. A 'jaw motion tracking'device, 'digital face-bow transfer', and 'double scan technique' which enables duplicating temporary restoration to definitive restoration were used to fabricate definitive prostheses. By using various digital techniques, complete mouth rehabilitation was done with minimal chair time in a patient with unstable occlusion.

Clinical outcomes of implant supported fixed-hybrid prostheses in the fully edentulous arches (완전무치악 환자에서 고정성 임플란트 하이브리드 수복물의 임상성적)

  • Purpose: The aim of this study was to evaluate clinical outcomes of implant supported fixed-hybrid prostheses (FHP) in the fully edentulous arches. Materials and methods: Patients in this retrospective study were restored with fixed-hybrid prostheses supported by 4 to 6 implants and functioned more than 1 year of loading. Outcome measures were marginal bone change of implant related with sex, anatomical location (maxilla vs. mandible), opposing teeth, loading time of patients, tilting of posterior implant by Mann- Whitney U test and cantilever length of superstructure by regression analysis, and complication rates. Significance level was set P<.05. Results: A total number of 84 implants (16 restorations) placed in 16 patients were observed for 28 months and mean marginal bone loss was $0.53{\pm}0.39mm$. There were no differences of marginal bone loss according to sex, anatomical location (maxilla vs. mandible), opposing teeth, loading time of patients (P>.05), and cantilever length was not significantly related with a marginal bone loss of implant next to cantilever (P>.05). Complication was shown in 11 patients and veneer fracture and dislodging of artificial teeth were most prevalent. Conclusion: Within the limitations of this study, although marginal bone loss of FHP was very little, complication rates were high. Irrespective of tilting of most posterior implants, marginal bone loss of most posterior implants next to cantilever was less than those of the other implants positioned anteriorly. Cantilever length (<17 mm) did not affect a marginal bone loss of most posterior implants.

ESTIMATION OF RELATION BETWEEN TECHNIQUES OF DYE PENETRATION FOR MICROLEA-KAGE AND SEM EVALUATION FOR MARGINAL ADAPTATION OF THE RESTORATION (수복물의 미세누출도와 변연적합도 측정에 사용되는 방법 사이의 상관 관계 평가)

  • The purpose of this study was to estimate the relation between techniques used for microleakage from dye penetration and for marginal adaptation from SEM evaluation of the restoration. Using high speed #330 bur class V cavities ($4{\times}3{\times}1.5 mm$ around CEJ) were prepared on the buccal surface of 20 extracted human molars. Six dimples as reference points for SEM and dye penetration evaluation were made with 1/2 round bur. Cavity was bulk filled with microhybrid composite resin (Esthet X) and all-in-one adhesive (Xeno III). Teeth were stored in saline solution for one day, after then, they were finished and polished using Sof-Lex system. Fifty percent silver nitrate dye solution was used for the evaluation of microleakage and resin replica was used for marginal adaptation. All of these were done after 1000 times thermocycling between 5 and $55^{\circ}C$. Vertical sections were made through three dimples of restoration to obtain samples for the evaluation of dye penetration and inner marginal adaptation. Outer adaptational estimation was done with an intact restoration before sectioning. Dye penetration was determined in three degrees and percentage of outer and inner leaky margin was estimated from SEM image The data were analysed statistically: Spearman's rho test were used to check relationships between two methods. The result were as follows : 1. There were significant relationships between degree of dye penetration and inner and outer marginal adaptations each (p < 0.01). 2. However, there was no significant relationship between the results or inner and outer marginal adaptation. Within the results of this study, relationship between the percentage of marginal adaptation and microleakage shows significant relationship. However, inner and outer marginal adaptation did not show any significant relationship mutually.

Functional and esthetic improvement through reconstruction of anterior guidance using the modified Dahl principle and copy-milled technique of CAD/CAM system: A case report (적절한 전방 유도 재현을 위해 수정된 Dahl 원리 및 CAD/CAM 복제 기법을 이용하여 전치부의 기능 및 심미성을 개선한 보철 수복 증례)

  • The anterior guidance is one of the important factors in prosthodontic treatment of anterior teeth. The lingual surface shape of anterior restorations is so critical that small errors of treatment procedure can cause discomfort of the patient and disharmony of the dentition. If the anterior restorations are achieved harmonious anterior guidance through the fabrication and adjustment of provisional restorations, it is important to accurately reproduce the lingual surface shape of provisional restorations as the final prosthesis. In this case report, it was used the modified Dahl principle and copy-milled technique of computer-aided design/computer-aided manufacturing (CAD/CAM) system for systematic diagnosis and treatment. Therefore, we tried to reconstruct the restoration shape more precisely by setting the appropriate anterior guidance and superimposing the digital image of the abutment teeth and the provisional restorations. Thus, by promoting functional and esthetic recovery, this case report demonstrates satisfying results to both the patients and dentist.

Influence of varying cement types and abutment heights on pull-off force of zirconia restorations (시멘트의 종류 및 임플란트 지대주 높이가 지르코니아 수복물의 제거력에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate Ti-base abutment's three different heights and three different cement types on the pull-off force of zirconia-based restorations. Materials and Methods: A total of 90 fixture lab analogs were embedded in auto polymerizing resin bloack. 90 Ti-base abutments heights of 3 mm, 5 mm, 7 mm were scanned and zirconia restoration were prepared from scanned files. Zirconia restoration were cemented with three different types of cements (temporary, semi-permanent, permanent) following manufacturer's instructions. All 90 specimens were placed and tested in a universal testing machine for pull-out testing. Retention was measured by recording the force at load drop. Statistical analysis was performed using Kruskal-Wallis test for detecting whether there are any statistical significance along cement types or abutment heights. After that, Mann-Whitney test was used for figuring out differences regarding abutment height and the comparison between 3 cements. Results: Temp bond showed significantly lower pull-off force compared to Fujicem regardless of any abutment height. However, there were significant differences between Cem-implant and Fujicem in abutment height of 3 mm and 7 mm, but there was no significant difference in 5 mm. Temp bond and Cem-implant had significant differences only in abutment height of 5 mm. Conclusion: Although Ti-base abutment height did not influenced zirconia restorations' retentiveness, cement types showed significant differences.

EFFECT OF THE ADDITIONAL ETCHING PROCEDURE ON PUSH-OUT BOND STRENGTH OF ONE-STEP RESIN CEMENT (부가적 부식 과정이 단일 접착 과정 레진 시멘트의 접착 강도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of additional etching procedure prior to Maxcem resin cement application in indirect restoration cementation using push-out bonding strength. One hundred and two extracted human molars were used to make indirect resin restorations of gold inlay and Synfony. These restorations were cemented using Maxcem and Variolink II. Additional etching procedures were done for one group with Maxcem. Three groups have 17 specimens in both restoration types. Push-out bond strength was measured using multi-purpose tester and calculated for bonding strength per sqaure-millimeter area. The mean bonding strength values were compared using SPSS 12.0K program for one-way ANOVA and Scheffe's Test with 95% significance. Under the condition of this study, the additional etching procedure prior to usage of Maxcem resulted in reduced bond strength for both of restoration types.

Effect of Bleaching on Restorative Materials and Restorations-Review (치아미백제가 수복재와 수복물에 미치는 영향에 관한 고찰)

  • Bleaching of vital and nonvital teeth is becoming a more commonly used treatment in the dental office and at home. To improve appearance and remove discoloration, the teeth of specific patients are treated with a variety of bleaching agents. The typical bleaching agents contains carbamide or hydrogen peroxide as the active component. The purpose of the review article was to summarize and discuss the available information concerning the effect of peroxide releasing bleaching agent on dental restorative materials and restorations. Information from all original scientific full papers or reviews listed in PubMed or ISI Web Science were included in the review. Bleaching may exert a negative influence on restorations and restorative materials. Advice is provided based on the current literature to minimize the impact of bleaching treatment on restorative materials and restorations.
