A case of fatal pneumococcal 19A meningoencephalitis despite administration of seven-valent pneumococcal conjugate vaccines (7가 페구균백신 접종에도 치명적인 폐구균 19A 수막염 1예)

  • Streptococcus pneumoniae is a major cause of serious invasive diseases in children, especially in young infants, but seven- valent pneumococcal conjugate vaccine (PCV7) is believed to prevent invasive pneumococcal pneumonia and meningitis in young children. However, recently, the incidence of non-PCV7 serotype has increased after PCV7 vaccination. A 14-month- old female patient presented at our emergency room with mental change and lethargy. Three days previously, she had developed fever and vomiting. After being admitted, she rapidly progressed to coma and brain death despite prompt and extensive supportive treatment. She expired 20 days after admission with a final diagnosis of pneumococcal 19A (non-PCV7 serotype) meningoencephalitis despite having received PCV7 ($Prevenar^{(R)}$) vaccinations on three occasions. The author reports this first fatality due to pneumococcal 19A meningoencephalitis in Korea and provides a brief review of the literature.

  • In Korea, rural groundwater development faces new challenge, which have not been experienced so far. The problem is a groundwater depletion by the water curtain cultivation (WCC) during winter season. This study investigates the groundwater depletion using three-dimensional finite difference groundwater flow program, MODFLOW to verify the water budget of the shallow aquifer of Cheongweon area. Interdisciplinary research, which has become a worldwide trend, has been adopted in studying groundwater modeling in field scale. In particular, the method of groundwater recharge estimation adopted precise modeling techniques, SWAT to groundwater flow modeling. Based on qualified field data, the model calibrated and validated its reliability. The objective of this study is to simulate various stream-aquifer interactions according to groundwater pumping with artificial boundaries, such as weirs and drainage system. We also analyzed a seasonal variation of cumulative water budget of the site to quantify the groundwater depletion and recovery in the pumping field.

  • Neurosyphilis is a rare infection of the brain and spinal cord caused by a spirochete named Treponema pallidum. We describe the magnetic resonance imaging of a 53-year-old man with syphilis who manifested as both meningovascular, and spinal meningomyelitic types, which involved the optic, trigeminal, facial and vestibulocochlear nerves, both middle and left posterior cerebral arteries, thoracic spinal cord and meninges of the lumbar spine. This case report suggests that neurosyphilis should be considered as a possible diagnosis in patients showing complex brain and spinal imaging features. These features include enhancing meningeal lesions with multiple cranial nerve involvement, stenoses in large to medium size cerebral arteries, and intramedullary and meningeal lesions of spine.

A Study on Performance of Water Curtain Nozzles for Protection of Wooden Cultural Properties from Forest Fire (산불로부터 목조문화재 보호를 위한 수막노즐의 성능에 관한 연구)

  • This study suggests the water curtain nozzles as the way to protect important wooden cultural properties from an adjacent building fire or a forest fire. They are designed to block off the pyrolysis of timbers which occurs at $200{\sim}250^{\circ}C$ by forming a water curtain with the flow of water that spouts over a certain pressure from the bottom. The existing water curtain nozzles installed at the following sites were examined: NakSan-sa (Temple) in Gangwon-do (Province) and in Muwisa (Temple) in Jeollanam-do (South Province). As a way to improve and complement the system, this study designed nozzles with covers in order not to disrupt the landscape. Connected pipes are elevated and jet water when they are in use. Possible ways to install the connected elevating pipes to jet water effectively were investigated.

A Case of Deficiency of the Seventh Component of Complement with Recurrence of Meningococcal Meningitis and Septicemia (C7 결핍증과 연관된 재발성 수막구균 혈증 1례)

  • The complement system is important in the generation of the normal inflammatory response and in host defense against systemic infection. Therefore, inherited or acquired deficiency of complement is associated with an increased frequency of infection. As a major effector of the complement cascade, the membrane attack complex is responsible for direct complement dependent serum bactericidal activity. Especially late complement component deficiency has a markedly increased risk of meningococcal infection and is subject to recurrent infection. We experienced a patient who had recurrent meningococcal meningitis and septicemia. The patient was 13-years old boy and he had a recurrent episode after 20 months. At second admission, we examined complement level and C7 deficiency was confirmed. He was treated without complication. We report a case of deficiency of C7 with recurrent meningococcal meningitis and septicemia.

Heat Transfer in the Passive Containment Cooling System (수동형 격납용기 냉각계통에서의 열전달)

  • The objective of this work is to obtain the experimental data for the heat transfer processes occurring both on the inside and outside surfaces of containment steel wall with dry and wet outer surface conditions in the passive containment cooling system. The test model represented a 60$^{\circ}$ section of a containment vessel based on the AP 600 geometry. Major linear dimensions of the test model ore reduced tv a factor of ten. To simulate the decay heat a steam generator heated by electricity was placed in the test model. The maximum heat flux was 8.91 kW/$m^2$. Two types of tests were performed. The one was the tort on the natural convection of air without water film flow. The other was the evaporative heat transfer test with the falling water film flow and natural air draft. no test result shooed that the heat transfer capability by the natural convection from the containment to the air without oater film flow was limited at about 1.48 kW/$m^2$ heat flux. It was found that the heat removal capability was remarkably enhanced in the tests with the waster film flow and air draft. The obtained heat transfer data ore compared with the existing correlations.
