Carpal Tunnel Syndrome in Stroke Patients According to the Degree of Spasticity in Median Nerve Cross-Sectional Area and Nerve Conduction Velocity and Comparison of Upper Extremity Function (수근관증후군 뇌졸중 환자에서 경직정도에 따른 정중신경 단면적과 신경전도속도 및 상지기능의 비교)

  • The purpose of this study were carpal tunnel syndrome in stroke patients according to the degree of spasticity in the median nerve cross-sectional area, nerve conduction velocity, and to evaluate differences in upper extremity function. The subjects of this study was in adult patients with stroke 42 patients from 21 patients CTS group and 21 patients Non-CTS group were selected. Measurement of median nerve-cross sectional area, nerve conduction velocity, GST, FMAS, CTS-FSS was measured. The study results were each group between the unaffected side and the affected side CTS and Non-CTS group in each grade between groups unaffected side(p<.001), and affected side(p<.001) median nerve-cross sectional area, median motor and sensory nerve onset latency, there was a statistically significant difference. CTS and Non-CTS group between groups in each grade GST(p<.05), FMAS(p<.05), CTS-FSS(p<.001), there was a statistically significant difference. In this study, the carpal tunnel pathokinesiology ever presented by the contents of upper extremity functional training in stroke patients is one of the information that you need to consider when presented.

Median Nerve Block for Treatment of Carpal Tunnel Syndrome -Report of 5 cases- (수근관 증후군 환자에서의 정중 신경차단 -5예 보고-)

  • Carpal tunnel syndrome is the most common compressive neuropathy of the upper extremity. Clinical manifestations include pain as well as motor and sensory dysfunction in the distribution of the median nerve. As nonoperative treatment, median nerve block is performed to relieve pain for carpal tunnel syndrome. We have experienced 5 such cases. Our results support median nerve block as an excellent treatment for carpal tunnel syndrome.

  • Turner syndrome (TS) is a chromosomal disorder caused by monosomy of the X chromosome, with complete or partial absence of the second sex chromosome. Anomalies of root morphology have been found to occur more often in patients with TS, which make endodontic treatment challenging and requires special handling. The patients with TS may also have systematic problems such as cardiac or renal malformations, so in treating these patients it is important for clinicians not only to be aware of the characteristic intraoral findings, but also to make the patients have regular dental check-ups to prevent oral complications in advance. An 12-year-old girl who had been diagnosed with TS at the age of 10 years was referred due to discomfort in the bilateral mandibular premolar regions. Dens evaginatus and taurodontism were detected in all the mandibular premolars characteristically. The bilateral mandibular first premolars had three roots and the bilateral mandibular second premolars had periapical lesion with two roots. Due to the complexity of the root canal anatomy, root canal treatment were completed with a dental microscope to ensure adequate visualization. After 2 years of regular follow-up examinations, there were no clinical sign or symptom associated with the teeth, and no periapical lesion, was found. This case report describes the characteristic oral features and dental management of TS patients.

Endoscopic Carpal Tunnel Release Using Single Portal Technique (단일 입구를 이용한 내시경적 수근관 감압술)

  • Purpose : Endoscopic carpal tunnel release technique was developed and has being used to decrease postoperative morbidity and complications. The purpose of this study was to evaluate the clinical results and clinical usefulness of endoscopic carpal tunnel release using single portal technique. Methods and Materials : 18 carpal tunnel syndrome patients who were diagnosed by means of clinical symptoms, physical examination, and electrodiagnostic study had endoscopic carpal tunnel release using single portal technique with about 1 cm oblique wrist incision on 30 hands. And then they were followed-up and reviewed in the same way. Late results of operation were analysed by grading system according to patient's own assessments of relief of symptoms at the final fellow-up. The follow-up period ranged 6 to 13 months from surgery. Results : There were postoperative improvements with respect to clinical symptoms, physical examination, and electrodiagnostic study. 23 of 30 hands$(76.7\%)$ had complete resolution of symptoms. 27 hands$(90\%)$ were able to return to normal activities and work within 6 weeks, and 30 hands$(100\%)$ returned within 8 weeks. In grip strength study, 29 hands$(96.6\%)$ regained preoperative strength in 6 months. 12 of 22 hands$(55\%)$ had improvement with respect to thenar atrophy within 6 months. Late results were as follows . 23 hands$(76.7\%)$ was graded as excellent, 6 hands$(20\%)$ graded as good and 1 hand$(3.3\%)$ graded as fair, and there was no poor result. Conclusion : We think that endoscopic carpal tunnel release with single portal technique is technically safe and simple, if the surgeon takes step to stay within the safety zone based on local anatomy and selects an appropriate patient and that endoscopic carpal tunnel release does have advantages over open release. We agree that the surgeon must be prepared to perform an open technique, if technical difficulties arise, difficulty in introducing the device into the carpal tunnel is encountered, or the transverse fibers of the transverse carpal ligaments are not clearly seen.

Six Cases of Carpal Tunnel Syndrome with Taping Therapy for Carpal Tunnel Expansion (수근관 공간확장테이핑치료를 실시한 수근관증후군 환자 6례 보고)

  • Objectives : The purpose of this study is to evaluate the korean medical treatment for carpal tunnel syndrome, especially taping therapy for carpal space expansion. Methods : Six patients were treated with acupuncture, moxibustion and kinesio taping therapy. To evaluate the pain and numbness of the palm and finger, visual analogue scale(VAS) and Phalen's test were used. Results : After about 10 to 20 times treatment, the pain and numbness of almost cases were remarkably removed. Also the VAS of post-treatment was reduced by 1.33(means). And the time being required to response to Phalen's test was signally prolonged, as 5 cases took over one minute. Conclusions : Korean medical treatment including expansion taping of carpal space as kinesio correction therapy is remarkably effective for carpal tunnel syndrome. But further studies are required to concretely prove the effectiveness of correction taping on other compressive neuropathy.

  • Carpal tunnel syndrome, carpal tunnel becomes part of a narrower median nerve pain occurs pressed. How a stable wrist stretch for the prevention of carpal tunnel syndrome is required, it should life guides to fit the user is provided. In this research, users themselves recognize the risk of carpal tunnel syndrome, to be able to stretch, it implements the application. Application, based on the usage time set by the user, executes the alarm and lock. At this time, the user may utilize arduino and inertia sensor, it is possible to wrist stretch, to analyze usage patterns, it is possible to provide a medical information and life guides to suit the user. Data using Bluetooth 4.0 communication has been to meet the runaway. In order to evaluate the mounted system, targeting four subjects, the time and the touch frequency of usage of the 3-day smartphone measured, as a result of the questionnaire, it reduces the frequency of use of smartphones, a positive it was possible to prove the effect.
