목적 : 본 연구는 손목과 손가락 관절가동범위 측정에 대한 3차원 동작분석 프로그램의 신뢰도를 알아보고자 하였다. 연구방법 : 연구 대상자는 정상인으로 손목과 손가락의 움직임에 제한이 없는 자, 본 연구의 목적을 이해하고 참여에 동의한 자 50명을 대상으로 하였다. 측각기와 3차원 동작분석 프로그램을 사용하여 총 11항목의 손목과 손가락 관절가동범위를 각각 1회씩 측정하였다. 3차원 동작분석 프로그램의 신뢰도를 측정하기 위해 측정값의 일치도와 불일치 양상을 비교하였다. 결과 : 측정값의 일치도를 분석한 결과 44개의 항목 중 38개 측정항목에서 매우 높은 일치도를 보였으며, 측정값의 불일치 양상을 분석한 결과 3가지 항목에서 불일치 양상이 나타났다. 결론 : 본 연구 결과를 통해 본 연구에서 사용한 3차원 동작분석 프로그램을 통해 관절가동범위 측정이 가능함을 알 수 있었다. 또한 3차원 동작분석 프로그램의 측정의 편리성, 정확성의 장점을 기반으로 다양한 임상 분야에서 활용되기를 기대한다.

Objectives The purpose of this study is to develop a case study protocol to complete final version of Korean medicine clinical pathway (CP) by applying Korean medicine CP based on carpal tunnel syndrome Korean medicine clinical practice guidelines developed by clinical experts to clinical field. Methods After applying previously developed CP, 10-point Likert scale questionnaires on satisfaction of CP, appropriateness and improvement on out patient department (OPD) treatments are carried out in the 2^nd week of OPD treatment. An open-ended questionnaire is also carried out to ask if there is any requirement to be added or improved among CP interventions. Results Final version of CP is going to be completed based on the questionnaire, excluding articles of which average or median value is less than 5 from 10-point Likert scale. Conclusions This evidence-based case study protocol is expected to contribute development of carpal tunnel syndrome clinical pathway.

Objectives This study aims to develop Korean Medicine Clinical Pathway (CP) based on Clinical Practice Guideline of carpal tunnel syndrome to improve quality of treatment and reduce medical cost to maximize the quality of patient management. Methods A draft version of CP for carpal tunnel syndrome is developed by expert agreement and a prospective case study was carried out based on the draft CP. Twenty experts working at various medical institution answered validity verification survey of developed CP. Fifteen patients enrolled in the prospective case study answered survey on demand and satisfaction. Qualification and adjustment process of the draft CP was conducted based on results of both surveys. Results Final version of CP for carpal tunnel syndrome is confirmed after qualification and adjustment on the draft version. Conclusions CP for carpal tunnel syndrome will provide patients with standardized, high-quality Korean medicine treatment and also reduce financial burden of health insurance by reducing medical cost.

Objectives We conducted an online survey to understand the current status of Korean Medicine clinical practice related to carpal tunnel syndrome (CTS) to develop the CTS Korean Medicine Clinical Practice Guidelines (CPG). Methods A thoroughly reviewed questionnaire was distributed by e-mail to 21,719 Korean Medicine doctors between March 26 and April 30, 2021. All raw data were arranged and analyzed systematically. Results The response rate among the doctors was 4%. Of the respondents, 82.1% responded that it is necessary to develop the CTS Korean Medicine CPG. We also obtained data related to the specific diagnosis, treatment, and prognosis management methods used at various treatment sites for patients with CTS. Conclusions This online survey will be helpful in developing the CTS CPG, which will contribute to the standardization of guidelines and improvements in the reliability of Korean Medicine. Our findings will provide valuable data and resources for future clinical studies on CTS.

This study was performed to report the effectiveness of direct moxibustion on a patient with intersection syndrome. The patient was diagnosed as intersection syndrome and treated with acupuncture, direct moxibustion and physical therapy. We have evaluated curative efficacy with measuring changes of visual analogue scale before and after treatment. After treatment, visual analogue scale decreased. These results suggest that direct moxibustion was effective on patient with intersection syndrome.

본 증례에서는 넘어진 후 상완골 골절 진단 하에 수술하였으나 뒤늦게 원위 요척관절 탈구로 진단되어 1차 수술 후 정복 소실되어 Sauve-Kapandji 수술받은 환자를 대상으로 한방재활치료를 약 3주간 시행하였다. 관절가동추나, 침 치료, 한약물 치료 등 한방재활치료 결과 우측 팔꿈치, 손목, 손가락 관절 가동범위, 수술부위 통증, 근력, 상지 기능 등에서 의미 있는 호전을 보였다. 본 연구는 단일 증례 보고라는 한계가 있으나, 한방재활치료가 수술 후 환자의 전반적인 경과에 효과적으로 작용하였기에 보고하는 바이다.

본 연구의 목적은 가상현실 훈련이 아급성 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과를 규명하는 것이다. 아급성기 뇌졸중 환자 18명이 본 연구에 참여하였다. 모든 대상자는 실험군 9명과 대조군 9명으로 배정되었다. 두 군은 하루 30분, 주 5회, 4주 동안 보편적인 작업치료를 받았다. 실험군은 가상현실 훈련을 회기 마다 30분 더 받았으며, 대조군은 보편적인 작업치료를 회기 마다 30분 더 받았다. 결과 측정은 중재 전과 후에 Fugl-Meyer 평가와 한국판 수정바델지수에 의해 수행되었다. 연구 결과, 실험군은 중재 후 Fugl-Meyer 평가 점수와 한국판 수정바델지수 점수에서 유의한 향상을 보였다(p<.05). 대조군은 Fugl-Meyer 평가의 어깨/팔꿈치/전완, 손목, 손 하위항목과 한국판 수정바델지수에서 유의한 향상을 보였다(p<.05). 중재 후 실험군은 대조군보다 Fugl-Meyer 평가의 총점, 손목과 손하위항목에서 유의하게 더 큰 향상을 보였다(p<.05). 이러한 결과들은 가상현실 훈련이 아급성기 뇌졸중 환자의 상지기능 향상에 긍정적인 효과를 나타낼 수 있음을 시사한다.

본 연구에서는 생체 임피던스를 이용하여 3채널 상지(upper arm) 운동 분석 시스템을 제작하였다. 각 채널은 상지의 외전(supination)과 내전(pronation), 팔꿈치(elbow)의 폄과 굽힘, 손목(wrist)의 평과 굽힘에 따른 각 관절의 임피던스 변화를 얻을 수 있도록 설계되었다. 상체 임피던스는 전극의 부착위치에 의존하므로 정밀한 감지를 위해 상지 운동에 대한 임피던스의 변화와 각도계(goniometer)의 각도 변화와의 상관관계를 통하여 상관 관계가 높고, 변화량이 가장 크고 다른 관절의 움직임에 따른 영향이 최소인 곳을 찾아 이를 최적의 전극 위치로 정하였다. 그리고, 선정된 최적의 전극 위치에서 임피던스 변화를 얻어 상지 운동을 분석해 보았다. 최적의 전극 위치에서 손목과 팔꿈치의 각도 변화와 임피던스 변화의 상관 계수는 각각 0.94, -0.97로 아주 높은 상관 관계를 보였다. 또한 이전의 다른 연구에서 임피던스 방법으로 구현해 본적이 없는 팔의 회전(rotation)도 분석이 가능하였다. 이 시스템은 피검자의 동작에 거의 제한을 주지 않고, 가벼우며, 장시간 측정이 용이할 뿐만 아니라 시스템 구성이 영상 분석기에 비해서 단순하다는 장점을 가지고 있다. 본 연구에서 제안하는 방법은 재활과 생체역학, 로봇의 원격 제어, 그리고 가상 현실에서의 동작 구현 등에 활용 할 수 있을 것으로 사료된다.

본 연구는 자전거에 의한 통증 및 부상의 원인과 종류 자전거피팅을 통한 통증 및 부상을 개선하고 재활방법을 제안하는 것을 목적으로 한다. 자전거에 의한 통증 및 부상은 크게 무릎, 발목부위 통증, 손목, 어깨, 목, 허리부위 통증, 엉덩이 통증으로 나눌 수 있다. 이러한 통증 및 부상의 원인은 잘못된 자전거피팅과 자세에서 비롯된다. 이러한 통증 및 부상을 개선하고 재발을 방지하기 위해서는 적절한 자전거피팅과 재활운동이 필요하다. 통증 및 부상은 운동성 통증과 고정된 자세에 의한 통증으로 나뉘며 운동성 통증의 경우 염증치료와 함께 재활운동이 필요하고 고정된 자세에 의한 통증은 정확한 신체의 특징을 이해하고 신체의 특징에 맞는 자전거 자세를 학습하고 연습해야 한다. 이런 방법을 통해 자전거 통증 및 부상의 재발을 예방하고 나아가 안전한 자전거 문화를 정립할 수 있는 환경을 조성하는데 이바지할 수 있다.

Objectives The purpose of this study is to investigate the research trend of Korean traditional medicine for musculoskeletal disorders related to wrist and hand under Korean healthcare facilities. Methods We collected data from search engines for research including foreign and domestic online database using the keywords ligament tendon, wrist sprain, tendinitis, trigger finger, carpal tunnel, ganglion cyst, de Quervain's and Raynaud. Results A total of 45 articles were about ligament tendon, wrist sprain, tendinitis, tenosynovitis, trigger finger, carpal tunnel syndrome, ganglion cyst, de Quervain's tenosynovitis and Raynaud disease. Out of 45 articles, 20 articles were about carpal tunnel syndrome and the most commonly used measurement was visual analog scale. Conclusions This study shows the research trend of musculoskeletal disorders related with the wrist and hand. Through the collected data, the treatment methods and specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in musculoskeletal disorders related with the wrist and hand.
